1.Development and validation of a machine-learning model based on routine laboratory parameters for preoperative prediction of microvascular invasion in patients with hepatocellular carcinoma

Objective:To develop and validate a machine learning (ML) noninvasive model based on routine laboratory parameters to preoperatively predict the microvascular invasion (MVI) in patients with hepatocellular carcinoma (HCC).Methods:A total of 629 HCC patients who underwent hepatectomy at the First Affiliated Hospital of Fujian Medical University between January 2019 and December 2023 were retrospectively enrolled in this study and were divided chronologically into a training set ( n=464) and internal validation set ( n=165). A cohort with 190 HCC patients from Fujian Provincial Hospital were used as an external validation set. Preoperatively demographic features, tumor size and routine laboratory data were collected. All patients were divided into MVI-positive or MVI-negative group. The Boruta algorithm and LASSO regression algorithm were used to screen out related features in the training set. Eight different ML algorithms including multivariate logistic regression, decision tree (DT), random forest (RF), extreme gradient boosting (XGboost), k-nearest neighbor (KNN), support vector machine (SVM), light gradient boosting machine (LGBM) and Naive Bayes were used to construct the prediction models. The predictive performances of these models on training and internal validation sets were evaluated by the receiver operating characteristic (ROC) curve with the area under the curve (AUC). The ML model with the highest AUC values was defined as the optimal model and its performance was further validated in the external validation set. The calibration curve showed that the probability value curve was close to the actual occurrence probability curve, and the DCA showed that it could be applied within the threshold probability range of 0.3-0.8 to obtain net benefits. Results:After screening, eight parameters including α-fetoprotein (AFP), protein induced by vitamin K absence Ⅱ (PIVKA-Ⅱ), tumor size, eosinophil count, neutrophil count, creatinine, ApoA1 and total bilirubin were finally selected for the construction of the preoperative prediction model for MVI in HCC. Among all the tested eight ML algorithms, XGboost obtained the optimal performance with an AUC of 0.820 in training set, an AUC of 0.803 in internal testing set and an AUC of 0.758 in external testing set. Further stratified analysis showed that the AUC for preoperatively predicting MVI by XGboost was 0.817 for HCC patients with positive hepatitis B surface antigen, 0.779 for male patients and 0.790 for elder patients. The calibration curves showed good agreement between observed and predicted values and the decision curve analysis curve showed relatively higher net benefits.Conclusions:We successfully established and verified a novel XGboost model based on eight routine laboratory parameters with relatively high and reliable predictive accuracy to preoperatively predict MVI in HCC.
2.Evaluation of surgical department service capability and operational efficiency based on Boston matrix analysis

Objective:To evaluate the service capability and operational efficiency of surgical departments in a hospital using data analysis models such as entropy weight TOPSIS and Boston matrix analysis, for references for optimizing medical resource allocation, promoting refined management and sustainable development of the hospital.Methods:The operational data of 24 surgical departments (A~X) in a tertiary public hospital in 2023 from its information system were extracted. The number of doctors, actual number of open beds, and average length of stay etc., were served as evaluation indicators. The TOPSIS and rank sum ratio methods were used to evaluate the medical service capabilities, the data envelopment analysis (DEA) was used to analyze operational efficiency, and Boston matrix analysis was used for departmental classification.Results:In terms of medical service capabilities, the top 5 departments were F, S, K, D, and C ( Ci>0.41), all of which were in the excellent category; The bottom three departments were L, N, and G ( Ci<0.04), all of which were in the poor range. From the perspective of operational efficiency, 8 departments had achieved strong DEA effectiveness, while the remaining 16 departments were non DEA effective, resulting in resource redundancy or insufficient output. According to the Boston Matrix analysis, 8 departments (stars) had strong medical service capabilities and high operational efficiency; 6 departments (cash cows) had poor medical service capabilities but high operational efficiency, and 8 departments (dogs) had poor medical service capabilities and operational efficiency; 2 departments (question marks) had strong medical service capabilities but low operational efficiency. Conclusions:This study comprehensively applied multiple data models to objectively and comprehensively evaluate the service capabilities and operational efficiency of surgical departments. Hospitals could develop corresponding resource allocation optimization strategies based on the Boston matrix classification results, combined with the disease characteristics and business scale configuration of each department.
3.Thyroid ultrasound results of soldiers stationed on a certain island

Objective To investigate the thyroid diseases in soldiers(aged 30 years and above)stationed on a certain island,and to clarify the value of ultrasonic diagnosis.Methods Thyroid ultrasonography was performed in 270 soldiers(aged≥30 years)stationed on an island by the high-frequency probe of the Mindray Z6 portable B-mode ultrasound diagnostic instrument.According to the C-TIRADS classification,the proportion of soldiers with thyroid nodules was calculated.Results Thyroid nodules were found in 50 soldiers(18.52%),including 29 cases(10.74%,cystic nodules)of C-TIRADS type 2,15 cases(5.56%,solid or cystic nodules)of C-TIRADS type 3,5 cases(1.85%,solid or cystic nodules)of C-TIRADS type 4a,and 1 case(0.37%,solid nodule)of C-TIRADS type 4b.Conclusion Thyroid ultrasound helps to analyze the causes of thyroid diseases,make grading assessment,and provide suggestions for soldiers stationed on islands.Not only does it clarify the necessity of thyroid ultrasonography in the physical examination,but also provides suggestions to improve the medical environment on islands.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
5.Prognostic Value of Dynamic Monitoring of WT1 Expression Levels for Relapse and Overall Survival in AML Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation During First Complete Remission

Objective:To analyze the predictive role of WT1 expression levels pre-and early post-transplantation on relapse and overall survival(OS)in patients with acute myeloid leukemia(AML)undergoing allogeneic hematopoietic stem cell transplantation(allo-HSCT)during their first complete remission(CR1).Methods:A retrospective analysis was conducted on the clinical data of 107 adult AML patients who underwent allo-HSCT during their CR1 at our center between May 2012 and December 2021.The predictive role of bone marrow WT1 expression levels before transplantation and at 3 and 6 months post-transplantation on relapse and OS was explored in combination with relevant clinical factors.Results:The median follow-up time for the 107 patients was 70(range:11-117)months.Among the patients,15 cases died.Kaplan-Meier survial analysis showed that the 3-year overall survival(OS)rate was 85.0%.20 patients experienced relapse,with a median time to relapse of 8(range:0.5-44)months and a l-year cumulative relapse rate of 13.1%.The overall median value of WT1 before transplantation,3 months after transplantation,and 6 months after transplantation was 0.26%(range:0%-23.64%),with an upper quartile value of 0.74%.No statistically significant differences in WT1 expression levels were observed among the pre-transplantation,3-month post-transplantation,and 6-month post-transplantation time points(P=0.227).Univariate analysis showed that patients with WT1 levels>0.74%at 3 months post-transplantation had a higher 1-year relapse rate(P=0.029)and lower 3-year OS rate(P<0.001)compared to patients with WT1 levels ≤0.74%.Other significant factors affecting 1-year relapse included stem cell source(P=0.041)and chronic graft-versus-host disease(cGVHD)(P=0.013).For 3-year OS,additional influencing factors were genetic high risk(P=0.048)and stem cell source(P=0.016).Multivariate analysis revealed that WT1 level>0.74%at 3 months post-transplantation had a trend to affect 1-year relapse rate(HR=3.309,95%CI:0.958-11.431,P=0.058),while the absence of cGVHD was an independent risk factor for 1-year relapse(HR=3.473,95%CI:0.749-16.100,P=0.037).Only WT1 level>0.74%at 3 months post-transplantation was an independent risk factor for 3-year OS(HR=6.886,95%CI:2.402-19.738,P<0.001).Conclusion:High WT1 expression level at 3 months post-transplantation in AML patients undergoing allo-HSCT during CR1 affects the 1-year relapse rate and 3-year OS,and is an independent risk factor affecting 3-year OS.These findings suggest that dynamic monitoring of WT1 expression levels has certain value in prognostic assessment of AML patients who received allo-HSCT during CR1.
6.Nafamostat Mesylate Alleviates Renal Ischemia-Reperfusion Injury in a Rat Model Through HMGB1 Modulation:An Omics Analysis-Based Study of the Protective Effect and the Mechanisms Involved

Objective To investigate the organ protective role and the underlying mechanism of nafamostat mesylate(NM)in a renal ischemia-reperfusion injury(RIRI)model.Methods A total of 21 healthy male Sprague-Dawley(SD)rats were randomly assigned to 3 groups(n=7 in each group),including the sham operation group(Sham group),the RIRI group,and the NM intervention group(NM group).The RIRI and NM groups underwent ischemia-reperfusion injury(IRI)modeling.The NM group was given an intraperitoneal injection of NM at 0.75 mg/kg before modeling.Venous blood and renal tissue samples were then collected from the rats 24 hours after modeling.The levels of serum creatinine,cystatin C,and serum inflammatory factors were determined using the serum samples.Hematoxylin-eosin(HE)staining and TUNEL stainings were performed on the renal tissues to evaluate the damage of the renal tissues.The localization and expression of HMGB1 were analyzed by immunofluorescence and Western blotting,respectively.Single-cell RNA sequencing of the nuclei was performed to obtain the single-cell transcriptome of the kidneys from the rats in the RIRI and the NM groups and to acquire the RIRI cell profile.The cells were annotated according to the cell marker genes to explore the cell type composition in the disease model and the functional status of immune cells between the groups.Results 1)Compared with those of the Sham group,the levels of cystatin C,creatinine,and inflammatory factors in the RIRI and NM groups were significantly increased,and the expression levels in the NM group were lower than those in the RIRI group(P<0.05).Compared with those of the RIRI group,the tubular injury score and apoptosis rate in the NM group were significantly decreased(P<0.05),but those of both the NM and RIRI groups were higher than those of the sham group.Compared with that in the RIRI group,the expression of HMGB1 in the NM group was significantly decreased(P<0.05),but the expression levels in both the RIRI and NM groups were higher than that in the sham group.Immunofluorescence showed that there was increased cytoplasmic expression of HMGB1 in both the NM and RIRI groups,with the increase being more prominent in the RIRI group.2)A total of 13 major cell populations were identified through the single-nucleus sequencing results.The proportion of tubular cells in the NM group was higher,with the HMGB1 gene being highly expressed in the damaged proximal convoluted tubular cells.The proportion of the polarized Macro3 cell subpopulation in the macrophages in the NM group was lower compared to that in the RIRI group.Conclusion NM may play a protective role in a rat model of RIRI,and its underlying mechanisms may be related to the regulation of the functional abnormalities of HMGB1-mediated macrophages.

Objective To establish a stable,reliable,and clinically relevant porcine model of endotoxin-induced acute respiratory distress syndrome(ARDS).Methods Ten 8-month-old male Bama minipigs were deeply sedated,followed by invasive mechanical ventilation and electrocardiographic monitoring.Lipopolysaccharide(LPS)was intravenously pumped at 600 μg/(kg·h)for 3 hours,then maintained at 15 μg/(kg·h)thereafter.Dynamic monitoring was performed at five time points after LPS injection(LPS 0,1,3,5,and 8 h),including arterial blood gas analysis and chest computed tomography(CT)scans.Pathological examination of lung tissues obtained via bronchoscopic biopsy(HE staining and transmission electron microscopy)was conducted.These indicators were comprehensively used to evaluate the success of the animal model.Results At 5 hours after LPS administration,8 minipigs developed symptoms such as skin cyanosis,elevated body temperature,and respiratory distress.The oxygenation index decreased to<300 mmHg.Chest CT scans showed diffuse pulmonary infiltrates.Histopathology revealed alveolar edema and hyaline membrane formation.Transmission electron microscopy demonstrated disruption of pulmonary blood-air barrier,depletion of lamellar bodies in type Ⅱ pneumocytes,inflammatory cell infiltration,and exudation of plasma proteins and fibrin.Compared with LPS 0 h,at LPS 8 h,the oxygenation index and arterial blood pH were significantly decreased(P<0.001),while blood lactic acid and serum potassium were significantly increased(P<0.05);serum calcium and base excess were significantly decreased(P<0.05),and the lung injury score based on HE-stained lung sections was significantly increased(P<0.01).Conclusion The porcine ARDS model established by continuous LPS injection can dynamically simulate the pathophysiological characteristics and typical pathological manifestations of clinical septic ARDS,making it an effective tool to study the pathogenesis,prevention,and treatment strategies of septic ARDS.
8.Mendelian randomization analysis and molecular mechanism of T-cell exhaustion-related genes in multiple myeloma

Objective To investigate the expression changes of T-cell exhaustion-related genes in multiple myeloma(MM)and their potential causal relationships.Methods A bidirectional summary-level Mendelian randomization(MR)analysis was used to explore the causal relationship between T-cell exhaustion and MM.The eQTL data and genome-wide association study(GWAS)were used to summarize data,and corresponding single nucleotide polymorphisms(SNPs)were extracted as instru-mental variables.Four methods,namely inverse variance weighted(IVW)method,MR Egger,weighted median,and weighted mode were used to assess the reliability of the causal relationship.The robustness of the results was validated using Cochran's Q heterogeneity test and pleiotropy test.In cel-lular models,RNA interference was used to silence key target genes,and phenotypic changes such as myeloma cell viability,colony-forming ability,and apoptosis were observed to experimentally confirm the causal effects revealed by MR.Results The genes PRDM1,ENTPD1,PTPN11,and HLA-B were involved in the T-cell exhaustion process in MM.The presence of the PRDM1 gene(OR=0.998 5,95%CI,0.997 1 to 0.999 8,P=0.024 6)may reduce the risk of MM,whereas ENTPD1(OR=1.000 4,95%CI,1.000 1 to 1.000 7,P=0.015 8),HLA-B(OR=1.000 4,95%CI,1.000 1 to 1.000 8,P=0.012 4),and PTPN11(OR=1.002 5,95%CI,1.001 0 to 1.003 9,P=0.001 2)were associated with an increased risk of MM.Real-time quantitative polymerase chain reaction showed overexpression of PTPN11 in MM cell lines and patients' samples.By assessing cell viabili-ty,colony formation and detecting apoptosis,it was found that inhibiting PTPN11 promoted apopto-sis in MM cell lines.Conclusion A causal relationship exists between T-cell exhaustion and MM.Targeted interventions against specific T-cell exhaustion-related genes may help reduce the incidence of MM.
9.Relationship between Sleep Problems and Temperament among Infants Aged 12~35 Months

Objective To analyze the sleep problems and temperament characteristics of children aged 12-35 months so as to understand the relationship between the sleep problems and temperament in early childhood.Methods 129 toddlers with sleep problems and 129 normal controls were selected.The brief infant sleep questionnaire(BISQ)was used to investigate children's sleep problems and the temperament questionnaire was used to investigate their temperament characteristics.The influence of temperament on sleep problems was analyzed by Logistic regression analysis.Results The temperament types of children aged 12 to 35 months with sleep problems were mainly easy to cultivate and moderately easy to cultivate.There was a statistically significant difference(P<0.05)in the comparison of easily cultivating temperament types between the children with the sleep problems and the control group;Compared with the control group,there were statistically significant differences(P<0.05)in the five dimensions of avoidance,adaptability,emotional essence,persistence,and attention dispersion among children with sleep problems.Compared with the temperament type of easy to cultivate,the proportion of sleep problems in children intermediately easy to cultivate and slow to start was higher(both P<0.05)Conclusion There is a certain relationship between the sleep problems and temperament types in children aged 12 to 35 months.Early understanding of sleep problems and temperament characteristics of toddlers can guide parents to adjust their education methods in time,give parents"individualized"sleep guidance and intervention,reduce the occurrence of long-term development problems and promote the healthy physical and mental development of toddlers.
10.Multimodal MRI features of cerebral small vessel disease combined with type 2 diabetes mellitus

Objective To analyze the imaging features of cerebral small vessel disease in patients with type 2 diabetes mellitus by multimodal MRI.Methods The clinical data of 160 patients with cerebral small vessel disease admitted to our hospital from January to December 2020 were retrospectively analyzed.According to whether they were complicated with type 2 diabetes mellitus,they were divided into the diabetic group and the non-diabetic group,with 80 cases in each group.Both groups underwent multimodal MRI scans.And the severity of lacunar infarction,the severity of subcortical and periventricular white matter lesions,white matter integral and cerebral microbleeds of patients in the two groups were compared.Results The severity of lacunar infarction(χ2=34.076,P=0.001),subcortical white matter lesions(χ2=25.000,P=0.001),periventricular white matter lesions(χ2=22.895,P=0.001)and white matter integral(t=12.370,P=0.001)of patients in the diabetic group were significantly higher than those in the non-diabetic group.No cerebral microbleeds were detected in either group of patients.Conclusion Patients with cerebral small vessel disease and type 2 diabetes mellitus show characteristic multimodal MRI changes.The increase in the number of lacunar infarction lesions and the aggravation of white matter lesions can be used as the characteristic imaging basis for the diagnosis of type 2 diabetes mellitus related cerebral small vessel disease.
