A patient in their 60s with a basilar tip aneurysm underwent endovascular treatment using a 14-mm Contour device. Fifteen months later, the patient presented with a fatal intraventricular hemorrhage, and imaging revealed device displacement and aneurysm growth. The first extensor compartment of the wrist is a distinctly variable anatomical area. Anatomical variations in this region contribute to the pathophysiology and treatment failure of de Quervain’s disease, which is a kind of tenosynovitis that develops in the first extensor compartment of the wrist. We aim to describe the first extensor compartment morphology, to evaluate the septum frequency, location of the septum, and the number of tendons of abductor pollicis longus (APL) and extensor pollicis brevis muscles (EPB). First extensor compartment of 87 wrists of 45 cadavers were dissected. The presence or absence of septum and number of tendon slips of APL and EPB revealed. The proximal and distal widths of the compartments were measured. Septums were detected in 60.9% (n=53) of the wrists. Incomplete (distal) and complete (proximal) septa were present in 35.6% (n=31) and 25.3% (n=22) of the cases. Only 26.4% of the wrists had a single slip of APL tendon. The Remaining had multiple slips. The median inner width of the proximal and distal compartments in all wrists were calculated as in the order of 9.11±1.14 mm and 8.55±1.12 mm. We believe that understanding the anatomy of the first extensor compartment in the Turkish population would be helpful to surgeons, radiologists, and physiotherapists to diagnose and manage de Quervain’s disease.

The cross-sectional study was conducted among ESRD patients required haemodialysis in Ipoh aimed at determining their level of knowledge, attitude, and practice (KAP) towards drug phosphate binders. A total of 105 patients on phosphate binders’ medication participated in the study. The questionnaires on sociodemographic characteristics and KAP towards phosphate binder medication were used to obtain the data. In this study, most patients were aged 40 to 60 and slightly more women. Patients were generally highly knowledgeable about phosphate binders. Over 90% of respondents were aware of the importance of the medication and the need to follow the regimen. More 70% of the patients have a positive attitude toward medication. In terms of practice, most of patients take the medication daily (90.5%) and always taking it with meals (93.3%). The older patients aged more than 45 years old, married and not working patients had significantly better compliance to medication. In conclusion, most patients in this study have a good knowledge of phosphate binder medications, a high positive attitude and good adherence to a medication. There is still a need for a structured health education method and counselling session to improve the knowledge of patients, change an attitude of a specific group of patients and increase the treatment compliance focusing on a specific ethnic group, young, unmarried, and employed patients.

Aims: Helicobacter pylori is a gastrointestinal bacterium that causes peptic ulcers and stomach cancer in nearly half of the world’s population. Many virulence factors influence the outcome of H. pylori related disorders. The purpose of this study was to see if there was a relationship between H. pylori virulence factors and histological and endoscopic findings in stomach biopsy specimens from Sudanese gastritis patients. Methodology and results: In the period between March 2018 and January 2020, a total of 290 gastric biopsies were taken from patients in Khartoum State hospitals. Histopathology and polymerase chain reaction (PCR) assays were performed on all specimens. Histological investigation revealed H. pylori in 103/290 (35.5%) samples, while PCR revealed H. pylori 16S rRNA positivity in 88/290 (30.3%) samples. Eighty-eight positive PCR specimens were subjected to PCR for genotypic detection of cagA, cagE, vacA, dupA and iceA1 genes. All of strains were vacA positive 100% (88/88) followed by dupA 50.0% (44/88), cagA 40.9% (36/88), cagE gene 38.6% (34/88) and iceA1 gene was detected in only 15.9% (14/88). The vacA s1/m1 68.2% (60/88) was the most prevalent vacA subtype. Conclusion, significance and impact of study Helicobacter pylori virulence genes were widespread and diversified in Sudanese gastritis patients. Helicobacter pylori cagA and iceA1 were significantly in association with gastric mucosa inflammation degree, whereas the dupA gene was found to be associated with the clinical outcomes.

A series of new 2,5-disubstituted-1,3,4-oxadiazole derivatives (5a-j and 6a-j) have been designed and synthesized in four-steps. Sixteen compounds among the twenty compounds are reported for the first time. The compounds were characterized and confirmed by the FTIR, 1D- and 2D-NMR and HRMS analyses, and were tested against Mycobacterium smegmatis and Mycobacterium tuberculosis H37Ra. Compound 5d was the most active against M. smegmatis with MIC value of 25 µM, and exhibited cidal activity with MBC of 68 µM, respectively. The time-kill assay showed the good killing rate at 77% with the combination of isoniazid (INH). In addition, checkboard assay confirmed the interaction of compound 5d was categorised as additive. Docking simulation has been performed to position 5d into the pantothenate synthetase active site with binding free energy value –8.6 kcal mol-1. It also occupied the same active site as that of standard native ligand with similar interactions, which clearly indicate their potential as pantothenate synthetase inhibitor.

This study aims to contribute to the literature on sustainable careers by studying the effect of t-anxiety on the career self-management dimensions which are Personal Mastery (PM), Verbal Persuasion (VP), and Vicarious Learning (VL), and the moderator effect of s-anxiety about layoffs due to COVID 19 pandemic on these relationships. The causal comparison screening approach was adopted. Hypotheses were investigated by the SEM method. The research data were collected online from 365 white-collar employees between July 1, and August 30, 2021, using a simple random sampling approach. The purpose of collecting the data between these dates was that the ban on lying off the employees due to the COVID 19 pandemic has been lifted on June 30, 2021. Empirically, a negative and significant relationship was found between t-anxiety and PM. It was also found that positive and significant relationships exist between t-anxiety and VP and VL. A negative relationship between s-anxiety about layoffs due to COVID 19 pandemic and PM was determined. S-anxiety about layoffs due to COVID 19 pandemic has been found to have moderating negative effect on the relationships between t-anxiety and VP and VL. High levels of anxiety (both for t-anxiety and s-anxiety) affect PM negatively whereas low levels of anxiety strengthen it. PM gets weaker in anxious individuals, while VP and VL get stronger. Because of the s-anxiety about layoffs due to COVID 19 pandemic white-collar professionals are likely to face a career shock, resulting in unsustainable trajectories.
