1.Chemical constituents from Alternaria alternata and their activity of down-regulating TMSB10 expression.

This study investigated the secondary metabolites in the product of rice fermentation with Alternaria alternata and their activity of down-regulating thymosin beta 10(TMSB10) protein expression. The secondary metabolites of A. alternata were separated and purified by various chromatographic separation techniques, including silica gel column chromatography, Sephadex LH-20 column chromatography, and semi-preparative high-performance liquid chromatography. Their structures were identified by spectral techniques such as nuclear magnetic resonance spectroscopy(NMR), infrared spectroscopy(IR), and high-resolution electrospray ionization mass spectrometry(HR-ESI-MS) and comparison with literature data. A total of 10 compounds were isolated and identified, including two new compounds(1S,3S)-2,3-dihydro-3,6-dihydroxy-8-methoxy-1-methylcyclopenta[c][2]benzopyran-5(1H)-one(1), 3,3a, 6-trihydroxy-8-methoxy-1-methyl-2,3,3a, 9b-tetrahydrocyclopenta[c]isochromen-5(1H)-one(2), and the eight known compounds are alternariol 9-methyl ether(3), 1-deoxyrubralactone(4),(3aR)-3,3a-dihydro-1,6-dihydroxy-8-methoxy-3a-methylcyclopenta[c][2]benzopyran-2,5-dione(5), altechromone A(6), alternariol(7), altenuene(8), altenusin(9), and 3'-hydroxyalternariol 5-O-methyl ether(10). The effects of these compounds on TMSB10 expression were tested. Compound 7 showed a significant down-regulation effect on TMSB10 expression with an inhibition rate of 40.5%. The results showed that benzopyrone compounds of A. alternata have the activity of down-regulating the expression of TMSB10 protein, providing theoretical basis and research value for the study of non-small cell lung cancer.

2.The relationship between serum CXCL9,NG levels and condition and prognosis of patients with acute ischaemic stroke

Objective To explore the relationship between the levels of serum CXC chemokine ligand 9(CXCL9)and neurogranin(NG)and the condition and prognosis of patients with acute ischemic stroke(AIS).Methods A total of 93 confirmed AIS patients(case group)admitted to Xuzhou First People's Hospi-tal from September 2020 to September 2022 were selected as the research subjects.Another 80 physically healthy individuals with similar gender and age to the case group during the same period were selected as the control group.The case group was divided into the mild group(n=27),the moderate group(n=41),and the severe group(n=25)based on the National Institutes of Health Stroke Scale(NIHSS)score.Three months after treatment,they were divided into the good prognosis group(n=71)and the poor prognosis group(n=22)based on the modified Rankin scale(mRS)score.The levels of serum CXCL9 and NG in each group were detected by enzyme-linked immunosorbent assay,and other clinical data were collected.The correlations be-tween serum CXCL9,NG and NIHSS score and mRS score were analyzed by Pearson product-moment correla-tion analysis.The influencing factors of poor prognosis were analyzed by Logistic regression,and the predic-tive value of poor prognosis was evaluated by the receiver operating characteristic curve.Results Compared with the control group,the levels of serum CXCL9 and NG in the case group increased,and the differences were statistically significant(all P<0.05).Compared with the mild group,the levels of serum CXCL9 and NG in the moderate group and the severe group increased.The levels of serum CXCL9 and NG in the severe group were higher than those in the moderate group,and the differences were statistically significant(all P<0.05).Serum CXCL9 and NG were positively correlated with NIHSS score and mRS score(P<0.05).Com-pared with the good prognosis group,the poor prognosis group had a larger cerebral infarction volume,a more severe condition,and higher levels of serum CXCL9 and NG,the differences were statistically significant(all P<0.05).Elevated serum CXCL9 and NG were independent risk factors for poor prognosis in AIS patients(OR=1.667,1.589,P<0.05).The area under the curve of serum CXCL9,NG and their combination for pre-dicting poor prognosis was 0.828,0.809 and 0.931,respectively,and the predictive value of the combination of the two was greater than that of any single indicator(Z=2.556,2.783,P<0.05).Conclusion Serum CXCL9 and NG are upregulated in AIS patients and are related to the disease condition and prognosis.Early combined detection can assist in clinical assessment of the disease severity and prediction of the risk of poor prognosis.
3.Evaluation of a deep learning-driven centerline extraction algorithm for optimizing the diagnosis of the"gray zone"in noninvasive coronary fractional flow reserve

Objective To evaluate the diagnostic performance of the minimum-cost-path-based CT angiography-derived fractional flow reserve(MCP-FFR)and the deep learning-driven CT angiography-derived fractional flow reserve(DeepCL-FFR),and to particularly explore the potential value of the DeepCL algorithm in improving diagnostic accuracy within the"gray zone."Methods A retrospective analysis was conducted on 151 coronary vessels from 109 patients with coronary artery disease,who were hospitalized at the General Hospital of the People's Liberation Army between January 2020 and June 2021.Pearson correlation and Bland-Altman plots were employed to assess the correlation and agreement of the two CT-FFR methods with invasive FFR.A CT-FFR range of 0.70-0.80 was defined as the diagnostic"gray zone."The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value for detecting hemodynamic abnormalities were calculated and analyzed.The DeLong test was used to compare the areas under the receiver operating characteristic curves(AUC)between the two CT-FFR calculation methods.Results Both CT-FFR methods exhibited a positive correlation with invasive FFR(MCP-FFR:r=0.75,P<0.001;DeepCL-FFR:r=0.86,P<0.001)and showed good agreement(MCP-FFR:mean difference=0.010,P=0.351;DeepCL-FFR:mean difference=-0.003,P=0.772).Both DeepCL-FFR(AUC 0.97,95%CI 0.94-0.99)and MCP-FFR(AUC 0.92,95%CI 0.88-0.97)demonstrated favorable diagnostic performance for detecting hemodynamic abnormalities(P=0.122).In the"gray zone"for hemodynamic abnormality,the diagnostic accuracy of MCP-FFR was 68.8%,whereas DeepCL-FFR increased it to 89.7%.DeepCL-FFR also exhibited superior diagnostic performance(AUC 0.89,95%CI 0.73-0.99)within the"gray zone,"which was significantly higher than that of MCP-FFR(AUC 0.71,95%CI 0.54-0.87)(P<0.001).Conclusions The deep learning-driven coronary centerline extraction algorithm,DeepCL,demonstrates superior diagnostic performance in CT-FFR for detecting hemodynamic abnormalities,particularly by significantly improving diagnostic accuracy in the"gray zone."
4.Detection and clinical significance of serum FGF23 and 25-OH-VD levels in children with osteoporosis

Objective:To analyze the relationship among serum levels of fibroblast growth factor 23 (FGF23), 25-hydroxyvitamin D (25-OH-VD) and osteoporosis in children.Methods:Eighty children with osteoporosis admitted to Affiliated Hosptial of Jining Medieal University from Jun. 2019 to Jun. 2024 were included as the observation group, and 60 healthy children who underwent physical examination during the same period were included as the control group. Serum FGF23 and 25-OH-VD levels were compared between the two groups, and bone mineral density and bone metabolism indexes [CTX-1, osteocalcin (OC) and N-terminal propeptide of type I precollagen (PINP) ] were detected. Pearson correlation was used to analyze the correlation among serum FGF23, 25-OH-VD, bone metabolism indexes and bone mineral density. Logistic regression model was used to analyze the risk factors of osteoporosis in children.Results:Serum FGF23 and CTX-1 levels in the observation group were significantly higher than those in the control group ( t=15.77, 7.56, P < 0.05), while serum 25-OH-VD, OC, PINP and BMD levels were significantly lower ( t=14.09, 2.70, 13.25, 3.63, P < 0.05) ; Pearson correlation analysis showed that BMD was not correlated with age, BMI and OC ( r=-0.07, -0.02, 0.01, P > 0.05), BMD was negatively correlated with FGF23 and CTX-1, and positively correlated with 25-OH-VD and PINP ( r=-0.35, -0.34, 0.41, 0.40, P < 0.05). The stepwise regression model showed that FGF23 and 25-OH-VD were the main factors affecting BMD ( t=-2.40, 9.02, P<0.05). Multifactor Logistic regression model showed that FGF23 and 25-OH-VD were related factors of osteoporosis in children ( OR=3.01,1.16, P<0.05) . Conclusion:Serum FGF23 level is higher and 25-OH-VD level is lower in children with osteoporosis, which is significantly correlated with bone mineral density and is a related factor for osteoporosis in children.
5.Analysis of influencing factors of time prolongation of time up and go test in maintenance hemodialysis patients

Objective To explore the influence factors of timed up and go test(TUG)in patients with maintenance hemodialysis(MHD).Methods Cross-sectional investigation method was used to select patients who underwent regular hemodialysis in Beijing Luhe Hospital,Capital Medical University from June 1,2023 to January 31,2024,with complete data and cooperation.The patients were respectively treated with a simple mental state checklist and 5 times sit to stand test(STS-5)and TUG scale were used to assess cognitive function,lower limb muscle strength,balance and gait function.Grouping was carried out according to the results of TUG.If TUG 12s,it is judged as the TUG normal group;if TUG>12s,it is judged as the TUG prolonged group.Multi-factor Logistic regression was used to analyze the influencing factors in the TUG prolonged group,and the predictive value of influencing factors to TUG prolonged patients was analyzed by receiver operating characteristic(ROC)curve.Results A total of 146 patients with MHD were included in this study,including 86 patients in TUG prolonged group and 60 patients in TUG normal group.Gender,age,combined hypertension,combined diabetes,blood glucose,education level,Minimum Mental State Examination(MMSE)score,C-reactive protein were compared between groups.Serum albumin,serum creatinine,serum sodium,whole parathyroid hormone,low density lipoprotein-cholesterol,serum uric acid,STS-5 results,and single urea clearance index(Kt/V)had statistical differences(P<0.05).Multivariate Logistic regression showed that old age,female,low MMSE score,low Kt/V,and STS-5 duration were independent risk factors for TUG prolonged(P<0.05).ROC curve showed that the area under the curve(AUC)of age,gender,total MMSE score and STS-5 time in predicting TUG prolongation in MHD patients were 0.825,0.678,0.777 and 0.836,respectively(P<0.01),which had high predictive value.The AUC of Kt/V in predicting TUG prolongation was 0.602(P=0.037),has a certain predictive value;AUC of TUG prolongation in MHD patients predicted by the above 5 indicators as a global model was 0.923(P<0.01).Conclusions The age,gender,MMSE score,STS-5 prolongation and Kt/V level of MHD patients are independent risk factors for TUG prolongation,and may be biological indicators for predicting TUG prolongation.

As a serious cardiovascular disease, atherosclerosis (AS) causes chronic inflammation and oxidative stress in the body and poses a threat to human health. Lipoprotein-associated phospholipase A2 (Lp-PLA2) is a member of the phospholipase A2 (PLA2) family, and its elevated levels have been shown to contribute to AS. Lp-PLA2 is closely related to a variety of lipoproteins, and its role in promoting inflammatory responses and oxidative stress in AS is mainly achieved by hydrolyzing oxidized phosphatidylcholine (oxPC) to produce lysophosphatidylcholine (lysoPC). Moreover, macrophage apoptosis within plaque is promoted by localized Lp-PLA2 which also promotes plaque instability. This paper reviews those researches of Chinese medicine in treating AS via reducing Lp-PLA2 levels to guide future experimental studies and clinical applications related to AS.
