1.Community-based Mental Health Promotion: Exploratory Study Using Theory of Planned Behavior in Primary Care of Dum Southwest Papua, Indonesia

Introduction: The treatment gap in mental health services in Indonesia is a critical issue driven by the shortage of professionals, challenging geography, limited medication availability, and stigma around mental health. Despite having limited access and facilities, Dum Primary Care in the District of Sorong Islands is the only primary care that provides mental health services to the community. This study aims to explore community-based mental health promotion using the theory of planned behavior. Methods: This study used qualitative research consist of three stages: developing interview guidelines, which were piloted with local health workers; semi-structured (SSIs) interviews; and focus group discussions (FGDs). A total of 11 participants were engaged in the study, including six professionals, two patients, and three caregivers. Results: Behavioral beliefs influence patients' intentions to recover and seek treatment independently. In subjective norms, patients with mental illness face contrasting family attitudes toward treatment, including support and rejection. Perceived control factors affecting mental health promotion in the community include family socioeconomic status, limited access to care, stigma, and inadequate government funding. The knowledge and skills of health workers are also vital for effectively reaching and supporting patients in promoting mental health. Conclusion: Enhancing collaboration between families and primary care is vital in promoting mental health. By actively engaging families and communities, primary care can improve mental health literacy and reduce the stigma associated with mental health issues. Implementing mental health lay workers can help address the shortage of professionals and improve service delivery in the community.
2.Clinicopathological profile of high-grade differentiated thyroid carcinoma in an Indonesian tertiary hospital

High-grade differentiated thyroid carcinoma (HGDTC) is a recently recognized entity in the 2022 World Health Organization classification, representing a more aggressive subtype of differentiated thyroid carcinoma. Previously, high-grade features such as increased mitotic activity and tumor necrosis were often overlooked, despite being important independent prognostic factors. Although rare, HGDTC carries significant diagnostic, prognostic, and therapeutic implications. Data remain limited in Indonesia. Methods: This retrospective descriptive study reviewed 565 thyroid carcinoma cases diagnosed at Cipto Mangunkusumo Hospital from 2019 to 2024. Eleven cases (1.9%) met HGDTC criteria. Clinicopathological characteristics, histologic subtypes, Ki-67 proliferation index, molecular alterations, treatment modalities, and clinical outcomes were analyzed. Results: Patients had a mean age of 54.6 years, with a female-to-male ratio of 2.7:1. Papillary thyroid carcinoma was the main type (90.9%), with the tall cell subtype predominating. Mean tumor size was 6.4 cm. Lymphatic invasion, vascular invasion, and extrathyroidal extension were present in 54.5%, 18.2%, and 45.5% of cases, respectively. All tumors showed necrosis. Mean mitotic count was 3 per 2 mm². The Ki-67 index ranged from 5% to 45% (median, 14%). BRAFV600E and TERT promoter mutations were detected in 18.2% and 36.4% of cases, respectively, with co-mutations in 18.2%. Six cases (54.5%) had metastases at time of diagnosis. During a mean follow-up of 20.5 months, one patient (9.1%) developed new vertebral metastases and all patients (100%) remained alive. Conclusions: HGDTC presents with more aggressive characteristics and a worse prognosis. Accurate diagnosis, molecular profiling, and long-term monitoring are essential for optimal management.

The magnitude of the problem of stunting is high in low- to middle-income countries, including Indonesia. The government of Indonesia has implemented specific and sensitive intervention programs to reduce this problem. This public health problem requires multidisciplinary management with suitable approaches and programs. In the first 1000 days of life, the quality of life is determined by conception and delivery until two years old.The requirement and fulfillment of nutrients during this period would benefit the growth and development of children. Indonesia is an archipelago country with 100,000 km of coastline and 16,771 islands with great potential for fishing. In coastal areas, optimizing protein consumption from the sea and enhancing educational programs in psychosocial care are appropriate intervention programs that will benefit stunting eradication. The complexity of the stunting problem puts authorities and other stakeholders together to formulate and design appropriate comprehensive intervention programs to improve the quality of life of the next generation.

Changes in cognitive function are associated with increased depression and decreased quality of life (QOL). This study aimed to determine the relationship between the characteristics of geriatric patients and anesthetic management with changes in postoperative cognitive function and QOL of geriatric patients undergoing elective surgery.
Methods:
This prospective observational analytic multicenter study included patients aged ≥60 years who underwent elective surgery in hospitals in Indonesia. We used the whole sampling method and performed follow-up 30 days after surgery. Data were analyzed using bivariate chi-square and multivariate regression tests with a confidence interval of 95% and α=5%.
Results:
Among the 933 geriatric patients included in this study 55.0%, females most (59.8%) received general anesthesia. Factors including age (p<0.001, B=-0.374, odds ratio [OR]=0.688), body mass index (BMI) (p<0.05, B=0.129, OR=1.138), and physical status based on American Society of Anesthesiologists (ASA) classification (p<0.001, B=-0.458, OR=0.633) were significantly associated with postoperative cognitive function. BMI (p<0.001, B=-0.218, OR=1.244), absence of comorbidities (p<0.05, B=-0.312, OR=0.732), regional anesthesia (p<0.05, B=0.606, OR=1.883), and changes in cognitive function (p<0.05, B=0.288, OR=1.334) were significantly correlated with changes in postoperative QOL.
Conclusion
Age, BMI, and ASA physical status were significantly associated with postoperative cognitive function in geriatric patients, whereas BMI, comorbidities, regional anesthesia, and changes in postoperative cognitive function were associated with QOL. These preoperative factors can predict postoperative cognitive function and QOL and may be useful during preoperative planning.
