1.Exploring Neonatal NaV1.5 Voltage-Gated Sodium Channel as a Therapeutic Target in Cancer

Voltage-gated sodium channels (VGSCs) play pivotal roles in cancer progression and
have emerged as promising therapeutic targets and biomarkers. VGSCs comprise
multiple subtypes with distinct tissue distributions, influencing tumour characteristics in
different ways. Among these, the tetrodotoxin-sensitive α-subunits and the β1 subunit,
commonly found in breast cancer, have been implicated in metastasis and tumour
aggressiveness. The NaV1.5 channel and its neonatal variant (nNaV1.5) are
overexpressed in aggressive cancers such as breast, prostate, colorectal, and lung cancers,
thereby enhancing their invasive capacity. nNaV1.5 is particularly significant due to its
tumour-specific expression and strong association with poor prognosis, especially
in breast cancer, where it regulates cell proliferation, invasion, and tumour
microenvironment remodelling. This review highlights nNaV1.5 as a critical ion channel
that drives metastasis through ion regulation, extracellular acidification, and cytoskeletal
remodelling. We further evaluate current therapeutic strategies, including siRNA,
monoclonal antibodies, and small-molecule inhibitors, while addressing translational
challenges such as tumour heterogeneity, drug delivery limitations, and off-target
cardiotoxicity due to its similarity with the adult isoform. In addition, we explore the
potential of nNaV1.5 as a biomarker subject to epigenetic regulations by factors
including RE1-silencing transcription factor (REST) and histone deacetylase 2 (HDAC2),
which may facilitate patient stratification and treatment optimization. By integrating
mechanistic insights, therapeutic opportunities, and translational challenges, this review
goes beyond descriptive summaries to provide a framework for advancing nNaV1.5
research from preclinical studies toward clinical application in cancer therapy.

Cost-Effectiveness of Mobile Application Use in Managing Type 2 Diabetes Mellitus: A Systematic Review
Introduction
Type 2 diabetes mellitus (T2DM) is a debilitating condition that imposes a significant economic burden on its management. This study aims to systematically review the published evidence on the cost-effectiveness of mobile health (mHealth) application interventions for T2DM.
Methods
A search strategy was conducted using electronic bibliographic databases, including PubMed, Web of Science and Scopus for published studies. The inclusion criteria included original articles that reporting cost-effectiveness evaluation studies on mHealth application interventions directed at patients diagnosed or at risk of T2DM, English-language articles and published in the year 2016 to 2020.
Results
A total of 6 eligible studies were selected. The cost savings per person for treating T2DM ranged from USD 1,346 to USD 3,781 per year. The major contributor to cost savings was reduced complication management resulting from good glycaemic control. The direct impacts of the intervention include reduced hospitalisation and fewer unplanned clinic visits, while diabetes- related mortality was indirectly reduced usingmHealth.
Conclusions
The mHealth intervention was cost-effective in managing patients with T2DM. Implementation can be extended to other disease management areas to not only reduce total healthcare expenditure but also improve patients’ quality of life.
5.Gastrointestinal Stromal Tumour Masquerading as a Gynaecological Neoplasm

Gastrointestinal stromal tumours (GISTs) are mesenchymal tumours commonly found in the gastrointestinal tract, particularly in the stomach, but they can mimic gynaecological tumours. We report the case of a 56 -year-old postmenopausal woman who presented with a right adnexal pelvic mass. Computed tomography (CT) revealed a large, lobulated pelvic mass with liver lesions. Pan - endoscopy showed normal findings, and her tumour markers were within normal limits. Based on these findings, the gynaecological oncology team proceeded with a laparotomy and tumour debulking, diagnosing the pelvic mass as likely ovarian in origin. Intraoperatively, the mass was found to originate from the stomach and extend posteriorly to the transverse colon. An en -bloc resection with primary anastomosis was performed and histopathology confirmed a high -risk GIST. The patient made a good post -operative recovery and was scheduled for adjuvant chemotherapy at her follow -up visit. This case highlights the importance of considering GIST as a differential diagnosis in postmenopausal women presenting with an abdominopelvic mass that may mimic an ovarian tumour

7.Central Precocious Puberty: Evaluation of Predicted and Final Adult Height in Girls Who Received GnRH Analogs

Introduction:
Central precocious puberty (CPP) is defined as the early occurrence of puberty before the age of 8 years in girls and 9 years
in boys. CPP has many implications for the patient’s physical, emotional, and psychological aspects. The most soughtafter benefit of the treatment is the preservation of the growth potential of final adult height. This study aims to evaluate
the predicted and final adult height in girls with CPP who received treatment in the University Malaya Medical Centre
(UMMC), Malaysia.
Methodology:
In this retrospective longitudinal study, 32 CPP patients who received treatment with GnRH analogs in UMMC between
2010 and 2025 were identified. The demographic and clinical data of the patients were retrieved from the medical record
system and analyzed.
Results:
The study revealed that predicted adult height (PAH) by using bone age closely approximated the actual final adult height
(FAH) with no significant difference observed (p = 0.793), and a strong positive correlation (r = 0.75, p <0.001). The predicted
height based on the genetic potential of mid-parental target height (MPTH) showed a significant underestimation of FAH
(mean difference = -1.89 cm, p <0.01), although it remained a robust predictor (r = 0.75, p <0.01). Across age groups, the
correlation between PAH and FAH was strongest in girls aged 6–8 years old. Notably, girls treated before age 6 exhibited
a significant height gain compared to initial prediction values, underscoring the importance of early intervention.
Conclusion
From this study, we concluded that the FAH outcome in girls with CPP who received treatment with GnRH analogs is
preserved, similar to the PAH calculated by bone age. However, the FAH is significantly lower than the MPTH (genetic
potential). The baseline height SDS appears to be a strong predictor of FAH. The bone age value is the strongest predictor
of FAH. The preservation of adult height potential is more prominent when subjects developed CPP before 6 years and
were treated immediately.

8.Shrapnel in carotid sheath: A rare penetrating neck injury.

Injuries deeper than the platysma are considered as penetrating neck injuries, constituting approximately 5% - 10% of all trauma. Many vital organs are at risk from a penetrating neck injury. These injuries in zone 1 have the highest mortality, because the injuries are close to the vital organs and difficult to access surgically. A 41-year-old male, a car mechanic by profession, presented to the emergency department with a penetrating neck injury on the right side. CT scan demonstrated a metallic foreign body in zone 1 between the right internal jugular vein and the common carotid artery. The patient was asymptomatic, and the foreign body was removed surgically. This case shows a rare presentation of a penetrating neck injury with a foreign body located in zone 1, where no vital internal structure was injured. As of now, no previous case report has been identified on such presentation. Thus, it will provide a valuable addition to the pre-existing literature.

9.Influence of Food-Simulating Liquids on the Microhardness and Surface Roughness of ToothColoured Restorative Materials

The effects of food-simulating liquids (FSLs) on the microhardness and surface roughness: Luna (LN, conventional nanohybrid), Aura Bulk Fill (AB, bulk-fill composite), and Stela Automix (SA, self-cured composite) were evaluated. The materials were exposed to air as control, artificial saliva and three FSLs: 50% ethanol-water solution, 0.02N citric acid, and heptane-over different time intervals to compare their performance. Microhardness and surface roughness were measured at baseline, seven days, and 30 days after immersion. Two-way ANOVA revealed significant interactions between FSLs and materials for both microhardness and surface roughness (p<0.001). All materials showed a decrease in microhardness and an increase in surface roughness over time, except when exposed to air. LN exhibited the highest microhardness and lowest surface roughness, while SA demonstrated the lowest microhardness and the highest surface roughness in all FSLs. SA's surface roughness decreased in citric acid, potentially due to its lower filler content, which may result in a less durable surface. Ethanol and citric acid had the most detrimental effects on the microhardness of all materials, while heptane caused the most significant surface roughness changes in SA. These results highlight the material-specific responses to FSLs, with important implications for material selection and restoration longevity in clinical practice.
10.Perspectives on Colorectal Cancer Screening in A Multiethnic Population in Kuala Lumpur using the Health Belief Model: A Qualitative Study

Colorectal cancer (CRC) carries a significant burden in most world regions. However, its screening uptake remains low. This study aimed to explore awareness and perspectives on CRC screening program in a multiethnic population and their preference for CRC screening decision aid. In-depth interviews were conducted until data saturation was reached. All interviews were audiotaped, transcribed verbatim, translated to English and analysed thematically using hybrid inductive and deductive approaches. 17 informants from three main ethnic groups (Malay, Chinese and Indian) with various levels of risk for developing CRC were recruited. Awareness on CRC screening program was found to be low. Majority of informants never heard of CRC screening program. Among 11 eligible informants, only five experienced CRC screening uptake. Thematic analysis of the transcripts yielded six major themes; knowledge on CRC, screening process, authority’s role, curability, willingness to screening and preference for decision aid, and theywere mapped onto the Health Belief Model. Specific multiethnic perspectives found included preference for traditional medicine coming from all ethnics, and reliance in God coming from Malay informants. Majority preferred short videos as CRC screening decision aid, in the form of animation and live-action screenplay. Exploration of perspective of CRC screening helps in producing impactful decision aids. Future efforts should focus on developing short videos that incorporate population’s perspectives and can be disseminated through electronic media.
