1.Comparison of Medial Rectus Recession Versus Lateral Rectus Resection in Divergence Insufficiency Esotropia: A Comparative Analysis

Purpose:
We compared the surgical outcomes of medial rectus recession (MR Rc) and lateral rectus resection (LR Rs) in patients with divergence insufficiency esotropia, characterized by greater esotropia (ET) at distance than at near.
Methods:
This retrospective comparative study included 22 patients who underwent either MR Rc or LR Rs for divergence insufficiency esotropia, defined as comitant ET with >8 prism diopters (PD) difference between distance and near, with persistent diplopia at distance. Surgical success was defined as final deviation <8 PD with diplopia-free status at distance and near at 12 months postoperatively. The mean dose-response ratio (PD/mm) of each surgical technique was analyzed.
Results:
Of the 22 patients, 13 underwent MR Rc and 9 underwent LR Rs. The surgical success rate was higher in the LR Rs group than in the MR Rc group; however, the difference was not statistically significant (69% vs. 100%, p = 0.11). In the MR Rc group, three patients had under-correction and one had over-correction. The mean dose-response ratio for distance was 2.43 ± 0.51 PD/mm at 1 week and 2.15 ± 0.93 PD/mm at 12 months, whereas for near it was 1.89 ± 0.83 and 1.69 ± 1.06 PD/mm, respectively. In the LR Rs group, all patients achieved surgical success. The mean dose-response ratio for distance was 1.96 ± 0.46 and 1.91 ± 0.51 PD/mm at 1 week and 12 months, whereas for near it was 1.23 ± 0.42 and 1.13 ± 0.52 PD/mm, respectively.
Conclusions
Both surgical techniques demonstrated favorable outcomes for divergence insufficiency esotropia. However, the dose-response ratio was more predictable in LR Rs surgery. For MR Rc, augmented surgical approaches may be considered to improve predictability and success rates.
2.A Case of Honeycomb Corneal Epitheliopathy Induced by Netarsudil 0.02% Ophthalmic Solution

Purpose:
To document the clinical presentation and progression of corneal epitheliopathy, with its characteristic honeycomb pattern, associated with topical Netarsudil 0.02% use in a patient with neovascular glaucoma (NVG).Case summary: A 60-year-old man presented with left ocular pain and headache, with an intraocular pressure (IOP) of 37 mmHg in the left eye. After a diagnosis of NVG, he was treated with intravitreal anti-VEGF injection and topical anti-glaucoma medications. On the fifth day after adding Netarsudil, central corneal epithelial edema was noted. By day 7, a diffuse epithelial haze with a honeycomb-like appearance had developed. IOP measured by rebound tonometry was 47 mmHg, while Goldmann applanation tonometry underestimated the value at 22 mmHg, likely due to irregularities at the corneal surface. Netarsudil was discontinued and replaced with latanoprost, leading to rapid improvement. Due to persistent elevated IOP, Ahmed glaucoma valve implantation was performed, resulting in stable IOP and complete normalization of the corneal surface.
Conclusions
While Netarsudil is effective for lowering IOP, it can infrequently lead to reversible corneal epithelial edema characterized by a honeycomb-like pattern, particularly in patients with compromised corneal integrity. Therefore, close monitoring of corneal status is necessary when using Netarsudil in high-risk patients.
3.Final adult height in male patients with central precocious puberty after gonadotropin-releasing hormone agonist treatment

Purpose:
We aimed to compare the final adult height (FAH) of male patients with central precocious puberty (CPP) after treatment with a gonadotropin-releasing hormone agonist (GnRHa). Specifically, we compared FAH with the target height (TH) and the predicted adult height (PAH) before and after GnRHa treatment to quantify height gain and identify predictive factors.
Methods:
We retrospectively reviewed the medical records of 92 male patients with CPP and known FAH after GnRHa treatment at the Department of Pediatrics of Severance Children’s Hospital between January 2000 and June 2024.
Results:
The mean duration of GnRHa treatment was 2.7±1.3 years. A significant 1.1±0.9 years narrowing was observed in the difference between bone age (BA) and chronological age (CA) during treatment (P<0.001). TH was 172.4±3.4 cm. FAH was 173.6±6.4 cm. FAH was greater than TH by 1.2±5.9 cm (P=0.047). PAH before and after treatment was 179.9±8.1 and 181.2±7.4 cm, respectively. PAH was increased by 1.3±4.9 cm (P=0.012) after treatment. As the PAH standard deviation score (SDS) before GnRHa treatment increased, FAH tended to exceed TH. In contrast, higher testosterone levels before treatment are associated with FAH falling below TH. A longer duration of treatment and taller TH are associated with an FAH SDS greater than height SDS before treatment. Conversely, a greater weight SDS, BA–CA difference, and testis size before treatment are associated with FAH SDS being less than height SDS before GnRHa treatment.
Conclusion
GnRHa treatment improved FAH and inhibited bone maturation in male patients with CPP.
4.Minimally Invasive Biportal Endoscopic Resection of a Cervical Laminar Osteoid Osteoma Adjacent to the Facet Joint: A Case Report

Osteoid osteoma is a benign bone-forming tumor that rarely occurs in the cervical spine. Surgical removal is indicated when symptoms persist despite conservative management. Although open excision or radiofrequency ablation has traditionally been performed, these approaches may be associated with extensive tissue disruption or postoperative spinal instability. Recently, minimally invasive techniques, including unilateral biportal endoscopy (UBE), have been introduced for spinal tumor resection. We report the case of a 50-year-old man with persistent left-sided neck pain lasting 6 months. Imaging studies revealed a 15-mm nidus in the left lamina of C3, consistent with osteoid osteoma. The lesion was completely excised using the UBE technique, with preservation of the facet joint and surrounding structures. Histopathologic examination confirmed the diagnosis of osteoid osteoma. The patient’s symptoms improved immediately after surgery, and no perioperative complications occurred. Follow-up computed tomography and magnetic resonance imaging demonstrated complete lesion removal, with no evidence of recurrence at the 7-month follow-up. UBE-assisted resection represents an effective and minimally invasive surgical option for cervical osteoid osteoma. This approach enables adequate visualization, precise lesion excision, and preservation of spinal stability, resulting in favorable clinical and radiologic outcomes.
5.Association between Allergen Sensitization and Open-Angle Glaucoma: An Analysis Using 2010 Korea National Health and Nutrition Examination Survey (KNHANES) Data

Purpose:
We sought to investigate the association between allergen-specific immunoglobulin E (IgE) sensitization and open-angle glaucoma (OAG) using a nationally representative South Korean population.
Methods:
This cross-sectional study used data from the 2010 Korea National Health and Nutrition Examination Survey (KNHANES). Among 8,958 participants, 1,334 subjects with complete ophthalmologic and allergen-specific IgE test results were included. Sensitization to indoor allergens (house dust mites, cockroaches, and dog dander) was assessed using ImmunoCAP assays. Multivariate logistic regression models were used to evaluate the association between allergen sensitization and OAG, adjusting for demographic, metabolic, and lifestyle factors.
Results:
Among the 1,334 participants analyzed, 62 were diagnosed with OAG. Sensitization to cockroach allergens was associated with increased odds of OAG (adjusted odds ratio [aOR], 1.19; 95% confidence interval [CI], 1.02–1.38), whereas sensitization to house dust mites was associated with lower odds (aOR, 0.56; 95% CI, 0.31–1.00). No significant associations were found for total IgE levels or sensitization to dog allergens.
Conclusions
Allergen sensitization is differentially associated with OAG, depending on the allergen type. Cockroach sensitization was associated with greater odds of glaucoma, while house dust mite sensitization demonstrated an inverse association. These findings provide epidemiological evidence for a differential association between specific allergen sensitizations and OAG. Also, it supports the hypothesis of immune-mediated mechanisms in glaucoma pathogenesis and justify further investigation into these inflammatory pathways.
6.Deep Learning-Based Augmented Contrast-Enhancement and Denoising for Reduced-Iodine and Low-Radiation 70-kVp Cerebral CT Angiography: A Prospective Study

Objective:
To evaluate the feasibility of cerebral computed tomography angiography (CTA) obtained with reduced iodine and low radiation at 70 kVp and the effect of deep learning-based augmented contrast enhancement (DL-ACE) and denoising (DL-DN) algorithms on the CTA quality.
Materials and Methods:
In this prospective study, 47 healthy volunteers (male:female, 31:16; mean age ± standard deviation, 57.8 ± 10.9 years) were randomly assigned to one of three CTA protocols: Group A (n = 16; 100 kVp, 40 mL of 350 mgI/mL), Group B (n = 16; 70 kVp, 40 mL of 270 mgI/mL), and Group C (n = 15; 70 kVp, 28 mL of 270 mgI/mL [ultralow iodine]), with an injection rate of 2.5 mL/s for all. Images were reconstructed using filtered back projection (FBP), and images in Groups B and C were additionally reconstructed using DL-ACE and DL-DN. Arterial attenuation, image noise, contrast-to-noise ratio (CNR), and subjective image quality were compared among five image sets.
Results:
Compared with Group A, Groups B and C received 23.7% lower radiation doses. With FBP, arterial attenuation was significantly higher in Groups B (435.8 ± 50.2 Hounsfield units [HU]) and C (391.8 ± 52.1 HU) than in Group A (321.1 ± 47.4 HU) (P < 0.001), while CNR did not differ significantly (Group A, 19.9 ± 4.7; Group B, 20.3 ± 3.8; and Group C, 18.4 ± 4.6) due to higher image noise in Groups B and C. After applying DL-ACE and DL-DN in Groups B and C, arterial attenuation increased by 45.4% and image noise decreased by 34.5%, resulting in significantly higher arterial attenuation, CNR, and subjective image quality compared with Group A (P < 0.001).
Conclusion
Cerebral CTA at 70-kVp using ultralow iodine enhanced arterial attenuation but increased image noise compared with the 100-kVp CTA protocol. DL-ACE and DL-DN significantly increased arterial attenuation and reduced image noise, resulting in higher CNR and better subjective image quality.

Purpose:
The aim of this study is to compare the detection ability of quality of life (QoL) changes and responsiveness of the KOrean QUality of life in Stomach cancer patients Study group (KOQUSS)-40 and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ).
Materials and Methods:
A multicenter prospective observational study was conducted to evaluate QoL changes after various gastrectomies between January 2021 and April 2022. Participants were instructed to complete the KOQUSS-40 and EORTC QLQ-C30/STO22 preoperatively and at 1, 3, 6, and 12 months postoperatively. QoL changes over time and QoL responsiveness were assessed for each questionnaire.
Results:
Data from 491 patients who underwent curative gastrectomy for gastric cancer at 22 institutions were analyzed. The summary scores of the KOQUSS-40 and EORTC QLQ-STO22 showed significant differences between the total and proximal gastrectomy groups (p=0.044 and p=0.038, respectively), but no difference was observed for the EORTC QLQ-C30. Dysphagia on the KOQUSS-40 was significantly different between the total and proximal gastrectomy groups (p=0.031); however, dysphagia on the EORTC QLQ-STO22 did not differ. The responsiveness of the KOQUSS-40 was similar to that of the EORTC QLQ in patients who experienced ≥ 10% body weight loss, but approximately 10% less in patients receiving adjuvant chemotherapy than the EORTC QLQ.
Conclusion
KOQUSS-40 has several advantages over EORTC QLQ-C30/STO22 when comparing QoL between the total and proximal gastrectomy groups. The findings provide information for researchers investigating the QoL of patients who have undergone curative gastrectomy for gastric cancer.
8.Long-term Immunogenicity of the 13-valent Pneumococcal Conjugate Vaccine during Adjuvant Chemotherapy in Patients with Gastric and Colorectal Cancer: A 5-Year Follow-up of a Randomized Controlled Trial

Purpose:
Current guidelines recommend vaccination at least 2 weeks before chemotherapy initiation to optimize the immune response despite limited evidence. Our previous study indicated no differences in short-term immune response for the 13-valent pneumococcal conjugate vaccine (PCV13) according to the vaccination timing. This study aims to investigate the long-term efficacy of PCV13 and clinical factors associated with the respective antibody response.
Materials and Methods:
Patients with gastric or colorectal cancer who received adjuvant chemotherapy were enrolled and divided into two groups: vaccinated 2 weeks before chemotherapy (arm A) and vaccinated concurrently with chemotherapy (arm B). Serum samples were collected before vaccination and in one month, 3 years, and 5 years. Immune responses were measured using enzyme-linked immunosorbent assay and multiplex opsonophagocytosis assay.
Results:
Including 63 patients, both groups showed an initial increase in the geometric mean titers of opsonophagocytic activity and the geometric mean concentrations of serotype-specific IgG levels after one month, followed by a decline at 3 and 5 years, particularly for serotypes 1, 14, 18C, and 19A. Despite the decline, global protection was maintained for 5 years, although global response decreased. The two arms did not show significant differences in immunogenicity nor in factors such as vaccination timing, age, cancer type, or chemotherapy regimen.
Conclusion
Vaccination timing is not a significant factor for the immunogenicity of PCV13 in cancer patients undergoing adjuvant chemotherapy. Global protection against pneumococcal infection was sustained for > 5 years, and global response remained in over half of patients.
9.Exploring Oncologists’ Perspectives on the Early Integration of Specialty Palliative Care in Korea: Challenges, Needs, and Clinical Implications

Purpose:
This study aimed to explore the practices, perceptions, and barriers related to specialty palliative care (SPC) referrals among oncologists in Korea, highlighting the clinical implications of early integration.
Materials and Methods:
A cross-sectional online survey targeting board-certified hemato-oncology specialists was conducted between August 1-25, 2024. The survey assessed referral practices, attitudes toward early SPC integration, referral criteria, barriers, and institutional characteristics.
Results:
A total of 227 oncologists participated (response rate, 36.7%). Among them, 68.7% reported frequent SPC referrals, with higher referral rates observed among younger physicians, those in tertiary hospitals, and institutions with in-house SPC teams (p < 0.001). Although 74.9% supported early SPC integration, referrals were often inconsistently timed, frequently occurring after disease progression or at the discontinuation of chemotherapy. For time-based referrals, the most commonly endorsed triggers were disease progression despite palliative second-line treatment and a prognosis of expected mortality within 6-12 months. Need-based referral triggers such as patient or family requests (96.5%), psychological distress (89.9%), or uncontrolled symptoms (83.3%), were also widely endorsed. The major barriers to early SPC integration included patient and family resistance (70.0%) and limited availability of SPC teams (34.4%).
Conclusion
This study emphasizes the importance of systematic efforts to promote timely SPC integration in Korea, including education to raise patient awareness, improved referral systems, and enhanced infrastructure. The positive attitudes toward early SPC among oncologists reflect a growing recognition of its value, highlighting the need for strategies that align with international standards.

Background:
With the aging population and advancements in medical care worldwide, the number of older patients with end-stage kidney disease continues to rise. This study aimed to identify factors associated with osteoporosis and osteopenia in older patients undergoing incident hemodialysis and assess their impact on mortality.
Methods:
We analyzed a large multicenter retrospective cohort of patients aged ≥70 years undergoing incident hemodialysis to identify factors associated with osteoporosis using logistic regression analysis and to assess the association of death with osteoporosis and osteopenia using Cox multivariable analysis.
Results:
Among 710 patients, 39.0% and 19.6% had osteoporosis and osteopenia, respectively. Osteoporosis was significantly associated with female sex, a history of fractures, and the absence of phosphate binder use. During a median follow-up of 36.8 months, 348 participants (58.8%) died. Mortality rates were the highest in the osteoporosis group (79.8%), followed by the osteopenia (77.2%) and normal bone mineral density (BMD) groups (35.2%). Cox regression analysis revealed that even after adjusting for covariates, the osteoporosis group was significantly associated with a higher mortality risk than the normal BMD group. Osteoporosis at the start of hemodialysis was significantly associated with higher mortality.
Conclusion
We should consider the importance of bone health in patients undergoing incident hemodialysis and pay attention to the use of phosphate binders and fracture prevention.
