1.Research progress on the effect and mechanism of NLRP3 inflammasome in head and neck squamous cell carcinoma.

The NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome, a high-molecular-weight protein complex in the cytoplasm, is composed of three core components: the sensor protein NLRP3, the adaptor protein apoptosis-associated speck-like protein containing a caspase-recruitment domain (ASC) and the effector protein caspase-1. It plays a critical role in regulating host immune and inflammatory responses. Studies have shown that the NLRP3 inflammasome has increasingly become a focal point in tumor molecular biology field. A growing body of evidence indicates that the increased expression and activation of the NLRP3 inflammasome is closely associated with the pathogenesis of head and neck squamous cell carcinoma (HNSCC) and the tumor microenvironment (TME). It may promote tumor proliferation, invasion, migration, and other biological behaviors through various regulatory mechanisms while influencing tumor immune evasion and therapy resistance, which holds promise as a prognostic biomarker for patients. This review explores the current effect and mechanism of the NLRP3 inflammasome and its signaling pathways in head and neck cancer, providing insights into clinical targeted drug development and molecular immunotherapy.

The Expert Consensus on Clinical Application of Qinbaohong Zhike Oral Liquid in Treatment of Acute Bronchitis and Acute Attack of Chronic Bronchitis (GS/CACM 337-2023) was released by the China Association of Chinese Medicine on December 13th, 2023. This expert consensus was developed by experts in methodology, pharmacy, and Chinese medicine in strict accordance with the development requirements of the China Association of Chinese Medicine (CACM) and based on the latest medical evidence and the clinical medication experience of well-known experts in the fields of respiratory medicine (pulmonary diseases) and pediatrics. This expert consensus defines the application of Qinbaohong Zhike oral liquid in the treatment of cough and excessive sputum caused by phlegm-heat obstructing lung, acute bronchitis, and acute attack of chronic bronchitis from the aspects of applicable populations, efficacy evaluation, usage, dosage, drug combination, and safety. It is expected to guide the rational drug use in medical and health institutions, give full play to the unique value of Qinbaohong Zhike oral liquid, and vigorously promote the inheritance and innovation of Chinese patent medicines.
3.Effect of indocyanine green fluorescence imaging technology in robot-assisted laparoscopic hemi-nephrectomy for duplex kidney in children

Objective:Exploring the efficacy of indocyanine green(ICG)near-infrared fluorescence(NIRF)imaging technology in robotic-assisted laparoscopic partial nephrectomy for children with duplicated kidneys.Methods:A retrospective analysis was conducted on the medical records of 7 children with duplicated kidneys admitted to Guizhou Provincial People's Hospital from December 2023 to September 2024,including 3 males and 4 females. The patients with a median age of 36(2,132)months. All cases of duplicated kidneys were located at the renal pole,with 5 on the left side and 2 on the right side. All 7 patients had a history of worsening hydronephrosis,with 4 having a history of urinary tract infection and 1 accompanied by urinary incontinence(urine leakage outside the normal voiding period). Preoperative diagnosis was confirmed via enhanced CT of the urinary tract,diuretic renal dynamic imaging(SPECT),and voiding cystourethrography. One case had an ectopic ureteral opening. One case had recurrent urinary tract infections following ureteral cyst fenestration surgery,and five cases had terminal ureteral obstruction. All seven cases showed that the functional capacity of the duplicated upper kidney was <10%. All 7 cases underwent robot-assisted laparoscopic repeated nephrectomy of the upper kidney using ICG-NIRF technology. Intraoperative ICG fluorescence imaging was performed in three stages:a catheter was inserted through the lower kidney ureter,and after the first injection of ICG,green fluorescence was observed in the lower kidney ureter. Under fluorescence guidance,the upper and lower ureters were anatomically dissected;The second ICG injection via intravenous administration revealed the vessels supplying the upper kidney,and the blood supply to the upper kidney was blocked;After the third ICG injection via intravenous administration,the normal lower kidney showed fluorescence imaging,while the ischemic upper kidney did not. The upper kidney was resected along the anatomical boundary between the upper and lower kidneys.Results:All 7 surgeries were successfully completed,with an average surgical time of(155.1 ± 22.7)min;Intraoperative blood loss was(12.5 ± 8.7)ml;postoperative hospital stay was(5.2 ± 1.7)days. Postoperative follow-up duration was(9.2 ± 3.5)months,with preoperative concomitant urinary tract infection and post-micturition dribbling symptoms resolved. No cases of ureteral stump effusion or urinary leakage were observed postoperatively. At the 3-month postoperative follow-up SPECT examination,no loss of lower kidney function was detected.Conclusions:The application of ICG-NIRF imaging technology in robotic-assisted laparoscopic heminephrectomy for children with duplicated kidneys is easy to perform and safe. Its main advantage is the ability to clearly identify normal ureters,blood vessels of non-functional kidneys,and the boundary between the upper and lower halves of the kidney,thereby guiding the optimization of surgical strategies.

Objective To integrate relevant evidences on postoperative pulmonary infections in patients with aneurysmal subarachnoid haemorrhage so as to provide references for clinical practice.Methods Domestic and international databases were searched,including BMJ Best Practice,UpToDate,Cochrane Library,the U.S.National Guidelines Library,JBI,NICE,Medlive,Embase,CINAHL,Web of Science,PubMed,CNKI,VIP,Wanfang Data,SinoMed and Yiigle for clinical decisions,expert consensus,clinical guidelines,best evidence summaries,systematic evaluations,Meta-analyses,and randomized controlled trials on pulmonary infections in patients with aneurysmal subarachnoid haemorrhage.Literature was screened according to the inclusion criteria.Qualitative assessment was performed,and evidences were extracted.Results A total of 16 articles were included,comprising 2 clinical decisions,7 expert consensus,3 guidelines,3 systematic reviews and 1 RCT.Finally,30 pieces of evidence were obtained in 7 domains:team management,risk assessment,mis-inhalation management,airway management,nutritional support,oral care and rehabilitation.Conclusion The best evidence for the management of postoperative pulmonary infection in patients with aneurysmal subarachnoid haemorrhage summarised in this study can provide references for clinical interventions.Clinical staff should reasonably apply the evidence according to the specific situations.

7.The prediction effect of modified caries risk assessment tool on children's caries risk

Objective:To explore the efficiency of a modified caries risk assessment tool(MCAT)on caries-risk prediction of children in Nanning.Methods:MCAT was designed based on caries-risk assessment tool(CAT)of American Academy of Pediatric Dentistry(AAPD)and the caries related factors of the primary school children in Guangxi,China.A cohort study was conducted in 332 children aged 3-year-old in kindergarten of Jiangnan District,Nanning,Guangxi.The data were integrated through questionnaires and oral exam-ination.The MCAT data were respectively scored by CAT and Cariogram and the caries-risk of the children was evaluated.The develop-ment of increased average caires incidece over a period of 1.5 years was compared,the predictive capacity of the methods was com-pared.Results:The ranks of caires risk assessed by CAT and Cariogram were significant different(Z=-10.34,P<0.001),and the consistency of the 2 methods was poor(Kappa=0.234,P<0.001).After 1.5 years,there was significant statistical difference of the caries prevalence rate between the low-high and moderate-high risk groups,and dmft increments between the moderate-high risk groups e-valuated by CAT(P<0.05).There was significant difference of the caries prevalence rate between each risk groups and dmft increments between the low-high and moderate-high risk groups evaluated by Cariogram(P<0.05).The AUC of CAT and Cariogram was 0.571 and 0.722 respectively(P<0.001).In the prediction models of caries risk,the fitting of CAT was poor(P<0.001),while that of Cariogram was better(P=1.00).Cariogram analysis showed that the chance of developing new caries lesions of the subjects with moderate and high caries risk was 2.86 and 11.65 times more than those with low caries risk.Conclusion:MCAT com-bined with Cariogram can more efficiently predict new caries de-velopment in 3-year-old children.
8.Effect of radiation on damage of mouse salivary gland tissue based on NLRP3

Objective To study the effects of radiation on the morphology,function,and NLRP3 expression of mouse salivary gland tissue,and provide new ideas to repair radiation-induced damage to salivary gland tissue.Methods We established a mouse model of radiation-induced submandibular gland injury and Recorded the weight of drinking water.The salivary flow rate was assessed,and HE staining was used to observe submandibular gland injury.Immunohistochemistry and Real-time PCR were used to assess expression of NLRP3 and Caspase-1 in the radiation-injured submandibular gland of mice at 1,3,7 and 14 days after radiation exposure.Results Over time,the amount of water consumed by radiation group mice was gradually increased,the salivary flow rate was decreased,and inflammatory cells in the submandibular gland continued to increase.Acinar cells gradually showed lesions,such as nuclear pyknosis and vacuolization.At 7 and 14 days after radiation exposure,expression levels of NLRP3 and Caspase-1 proteins and genes in the radiation group were significantly higher than those in the normal group(P<0.05).Conclusions Radiation damages mouse submandibular gland tissue and activates the NLRP3 inflammasome to increase its expression level.
9.Diagnosis and treatment of testicular torsion in children——analysis of 221 cases

Objective To improve the level of diagnosis and treatment through analyzing the clinical characteristics,treatment and prognosis with testicular torsion in children.Methods We conducted a retrospective analysis of 221 boys with testicular torsion,who received trearment from May 2003 to May 2023 at the Children's Hospital of Chongqing Medical University and Guizhou Provincial People's Hospital.Data analysed included:clinical characteristics,treatment progress and outcome.Results The peak onset age of testicular torsion in children was infancy and prepuberty(71%).The incidence of left side(77%)was higher than that of right side(23%).The first symptoms were mainly scrotal pain and swelling(62%),but some cases mainly manifested lower abdominal pain and inguinal pain(11%).The rate of misdiagnosis and mistreatment of patients in basic hospitals was high.Scrotal color Doppler ultrasonography is the first choice for assistant examination,and mainly showed none or dramatically decrease of blood supply in testis,and abnormal axial direction.The golden time for testicular torsion diagnosis was within 8 hours.The chief pathological manifestation were interstitial hyperemia,edema and seminiferous tubules degeneration within 8 hours,and hemorrhagic necrosis or infarction over 8 hours.Conclusions Children Testicular torsion occurs more frequently in infancy and prepuberty,torsion position in the left side is more than that in right side.The main symptoms are scrotal pain and swelling,as well as lower abdominal or inguinal pain.Testicular pathological lesion is closely associated with the diagnosis time and degree of torsion.
10.Discussion of Hp(3) calibration with two thermoluminescent dosimeters in the same standard X-ray RQR radiation field

Objective To compare Hp(3) calibration with a homemade (A) thermoluminescent dosimeter (TLD) and an imported (B) TLD in a standard X-ray RQR radiation field, to explore the different responses of A and B, and to provide foundation for the calibration of Hp(3). Methods A column mode was selected. Hp(3) calibration was performed using A and B in a standard X-ray RQR radiation field in the Secondary Standard Dosimetry Laboratory, National Institute for Radiological Protection, China Center for Disease Control and Prevention. Angle response, energy response, and linear response were calibrated with RQR4 (60 kV), RQR7 (90 kV), and RQR9 (120 kV), respectively. Results In terms of angle response, the calibration results of A were relatively high, while the calibration results of B were relatively low. In terms of energy response, the calibration results showed a similar pattern to angle response. In terms of linear response, the calibration results of both A and B were satisfactory. Conclusion Both A and B can be used for normal calibration of Hp(3) in a standard X-ray RQR radiation field. However, in actual monitoring, attention should be paid to the energy and angle response values of TLDs.
