Objective To investigate the perioperative management of antithrombotic drugs in patients undergoing non-cardiac surgery. Methods Patients on long-term antithrombotic drugs who underwent non-cardiac surgery in our hospital were included.Through interviews with patients and physicians, perioperative antithrombotic medication regimens were reviewed and compared with the"Multidisciplinary Expert Consensus on Perioperative Management of Antithrombotic Therapy"to evaluate compliance with consensus and analyze influencing factors. Results A total of 372 patients were included in the analysis.Among them,355 patients were on long-term antiplatelet therapy alone, and 17 were on long-term oral anticoagulantion.364(97.8%)discontinued antithrombotic medications prior to surgery.109 patients(29.3%)received low molecular weight heparin(LMWH)bridging therapy.Among the 355 patients on antiplatelet therapy,108(30.4%)had discontinuation durations consistent with the consensus recommendations, while 186(52.4%)discontinued medications for longer periods.Postoperatively, the average hospital stay for antiplatelet therapy patients was 6.64 days, with only 37(10.4%)resuming therapy during hospitalization. The average hospital stay for patients on anticoagulants was 9.94 days, with only 2(11.8%)resuming therapy during hospitalization.Regarding perioperative risk assessment, only 40(10.8%)of patients underwent additional internal medical evaluation for thromboembolic risk after medication discontinuation, with the remainder assessed soly by surgeons.Coronary heart disease was an independent risk factor associated with internal medical evaluation(OR 2.851,95%CI 1.160-7.011,P=0.022).For bleeding risk assessment, surgeons evaluations aligned with the consensus in 68.0%of cases, but surgeons tended to underestimate risk compared to the consensus. Conclusions In this single-center study, perioperative antithrombotic management showed low compliance with expert consensus, characterized by prolonged preoperative medication discontinuation, high rates of LMWH bridging, and low postoperative in-hospital resumption of therapy.A robust multidisciplinary collaboration system should be established to enhance comprehensive patient assessment.

Aim To study the role and mechanism of ML210 in glioma. Methods The cell viability was detected by CCK8 assay. The percentage of dead cells was detected by SYTOXstaining. The role of ferroptosis-signaling pathway in gliomas was detected bygenomics.Cell proliferation was observed by EdU staining and clone formation assay.Cell migration ability was detec-ted by scratch healing assay. The apoptosis was detec-ted by flow cytometry.Cell mitochondrial function was assesses by JC-1 staining. The mechanism of action of ML210 was detected by molecular docking coupled with immunoblotting assay(Western blot). The levels of ROS,MDA were observed by ELISA.Results Compared with the control group,ML210 treatment dose-dependently decreased glioma cell viability,in-hibited cell proliferation, migration, and increased cell apoptosis and mitochondrial dysfunction, which were reversed by ferroptosis antagonists.Gene microarray screening showed that 688 genes of the ferroptosissig-naling pathway were aberrant and 10 signaling path-ways were altered in gliomas.Molecular docking re-sults showed that ML210 binding to GPX4 significantly inhibited the protein expression level of GPX4 and pro-moted the elevation of ROS and MDA levels.Conclu-sions ML210 produces anti-glioma cells via GPX4-mediated ferroptosis pathway.

Objective To analyze the applicability of Berg Balance Scale(BBS)in the balance assessment of Parkinson's disease(PD)patients with balancing instrument test as the standard,so as to formulate targeted intervention programs accordingly. Methods A total of 186 patients with PD admitted to Department of Neurology and Department of Rehabilitation Medicine,Huashan Hospital,Fudan University from Jan 2020 to Sep 2022 were selected as the research objects. The static balance function of the patients was evaluated by balancing instrument. The balance function parameters were analyzed according to BBS and levodopa equivalent dose. Results The drug dose did not affect the score of balance instrument. There was a good correlation between BBS and balance instrument. The higher the scale score, the lower the balance instrument score(r=-0.333,P<0.001).There was a positive correlation between levodopa equivalent dose and item 11 of BBS in subjects with BBS total score of 21-40(r=0.715,P=0.046).There was a significant negative correlation between levodopa equivalent dose and item 6 of BBS in subjects with BBS total score of 51-56(r=-0.300,P=0.001).The number of people who lost the most points in BBS from large to small was item 14 of standing on one leg(134 cases),item 13 of standing without support with one foot in front(122 cases),item 11 of turning 360 degrees(75 cases),item 8 of reaching forward with outstretched arms(70 cases),and item 12 of placing one feet alternately on a step or a stool(56 cases). Conclusion In the absence of balancing instrument,BBS has good clinical applicability in the evaluation of PD patients, and some items can be focused on according to the lost points.

Objective To evaluate the cost-effectiveness of inclisiran injection treatment in patients with atherosclerotic heart disease(ASCVD)in China. Methods From the perspective of China's health system, according to the Markov model, patients with ASCVD were divided into inclisilan injection group and placebo group, and both groups were treated with convention-al lipid-lowering drugs. The study period was 25 years. The rate parameters, cost parameters, and effectiveness parameters were de-rived from the ORION-18 trial and other literature.Effects were expressed as quality-adjusted life-years(QALYs).The incremental cost-effectiveness ratio(ICER)was used to evaluate the economy of inclisilan injection.One-way sensitivity analysis and probabil-ity sensitivity analysis were used to verify the reliability of the results. Results The treatment effect of the inclisilan injection group was higher(10.02 QALYs),and the cost of the placebo group was lower(255 179 yuan). The ICER of the two groups was 137 850 yuan per QALY gained, and using 257 094 yuan per capita by 2022 as the threshold, the treatment would be economically advantageous.Sensitivity analysis supported this result. Conclusion At present, the additional use of inclisilan injection can get a better treatment effect, and it has economic advantages under the threshold of three times GDP per capita in China.

Objective To summarize the magnetic resonance imaging(MRI)features of spinal adnexal tuberculosis in order to improve its early diagnosis. Methods Totally 21 spinal adnexal tuberculosis patients confirmed at some hospital from January 2019 to October 2023 had their clinical data and MRI images analyzed retrospectively to determine the basic clinical characteristics and MRI features. Results Of the 21 patients,8 ones had sudden lower extremity weakness and pyramidal tract signs, and the remaining 13 ones had no significant symptoms of neurologic deficit. The lesion involvement ranged from C4 to L5 vertebrae, with the involvement of lumbar segments in 13 cases, thoracic segments in 6 cases and cervical segments in 2 cases. There were 15 cases that had tuberculosis involving in only a single spinal adnexa and peripheral soft tissue,4 cases in 2 vertebrae and 2 cases in 3 vertebrae. There were 9 cases involving in pedicles,8 cases in vertebral plates and some cases involving in sphenoid process, transverse process or facet joints. The MRI features of spinal adnexal tuberculosis included the tuberculosis-infected bone showing slightly low signals on T1WI while slightly high signals on T2WI,edge enhancement by enhanced scan and edema and abscess of paravertebral soft tissue and intra-and extradural tuberculous abscess displayed clearly by the fat suppression and diffusion weighted imaging sequences of T2WI.Conclusion MRI effectively detects the abnormal signs of bone and soft tissue of spinal adnexal tuberculosis. The MRI findings of spinal adnexal tuberculosis are of characteristics, and MRI can be used as the first choice for imaging examination and differential diagnosis to realize early detection of spinal adnexal tuberculosis.[Chinese Medical Equipment Journal,2025,46(1):55-59]
