Childhood simple obesity has become a significant public health issue in China. Modern medicine primarily relies on lifestyle interventions and often suffers from poor long-term compliance, while pharmacological options are limited and associated with potential adverse effects. Traditional Chinese Medicine (TCM) has a long history in the prevention and management of this condition, demonstrating eight distinct advantages, including systematic theoretical foundation, diversified therapeutic approaches, definite therapeutic efficacy, high safety profile, good patient compliance, comprehensive intervention strategies, emphasis on prevention, and stepwise treatment protocols. Additionally, TCM is characterized by six distinctive features: the use of natural medicinal substances, non-invasive external therapies, integration of medicinal dietetics, simple exercise regimens, precise syndrome differentiation, and diverse dosage forms. By combining internal and external treatments, TCM facilitates individualized regimen adjustment and holistic regulation, demonstrating remarkable effects in improving obesity-related metabolic indicators, regulating constitutional imbalance, and promoting healthy behaviors. However, challenges remain, such as inconsistent operational standards, insufficient high-quality clinical evidence, and a gap between basic research and clinical application. Future efforts should focus on accelerating the standardization of TCM diagnosis and treatment, conducting multicenter randomized controlled trials, and fostering interdisciplinary integration, so as to enhance the scientific validity and international recognition of TCM in the prevention and treatment of childhood obesity.

OBJECTIVE To observe the clinical efficacy of mycophenolate mofetil (MMF) combined with Budesonide enteric capsules in the treatment of high-risk progressive immunoglobulin A nephropathy (IgAN). METHODS A total of 150 adult patients with high-risk progressive IgAN who attended the Department of Nephrology, Chongqing University Three Gorges Hospital, between August 1, 2024 and March 1, 2025 were enrolled in this study. The control group ( n =94) received MMF combined with glucocorticoid, while the observation group ( n =56) received MMF combined with Budesonide enteric capsules. The 24-hour urine protein (24 h UP), estimated glomerular filtration rate (eGFR), and albumin (ALB) levels of patients in both groups were compared at 1, 2, 3, and 6 months post-treatment. The complete response (CR) rate and overall response rate were calculated for both groups at 1, 2, 3, and 6 months post-treatment. Adverse reactions occurring during treatment were compared between the two groups. RESULTS Compared with before treatment, 24 h UP decreased significantly in both groups at different time points after treatment ( P <0.05), and ALB increased significantly ( P <0.05). However, there was no significant change in eGFR ( P >0.05). The 24 h UP in the observation group at 1, 2, and 3 months after treatment was significantly lower than that of the control group ( P <0.05), while the ALB level was significan tly higher ( P <0.05). However, at 6 months after treatment, there was no statistically significant difference in these two indicators between the two groups ( P >0.05). There was no statistically significant difference in eGFR between the two groups at different time points after treatment ( P >0.05). The overall response rates in the observation group at 1 and 2 months after treatment were significantly higher than those in the control group ( P <0.05). There was no statistically significant difference in the overall response rate at the remaining treatment time points and the CR rate at all time points between the two groups ( P >0.05). Patients in the observation group had significantly lower rates of skin abnormalities, elevated blood glucose, and overall adverse reactions compared with the control group ( P <0.05). CONCLUSIONS Compared with MMF combined with glucocorticoids, MMF combined with Budesonide enteric capsules for the treatment of high-risk progressive IgAN patients can reduce proteinuria and improve serum ALB levels more quickly, significantly increase the early overall response rate, and significantly reduce glucocorticoid-related skin adverse reactions, blood glucose elevation, and the overall incidence of adverse reactions, demonstrating a better short-term benefits.

Background/Aims:
Acetyl coenzyme A (acetyl-CoA) is one of the most essential metabolites in cell metabolism but its function and concentration in hepatocellular carcinoma (HCC) remain elusive and controversial.
Methods:
A comprehensive analysis of acetyl-CoA levels and acetyl-CoA synthetase 2 (ACSS2) expression across a range of samples, including patient specimens from both hepatitis B virus (HBV) positive and HBV negative HCC individuals, HBV-transgenic mouse HCC models, and multiple cell lines. Furthermore, to evaluate the functional significance of ACSS2 in HBV-related HCC, we implemented both genetic and pharmacological inhibition strategies targeting ACSS2. Molecular mechanism and mitophagy assessment were revealed by cleavage under target and tagmentation sequencing, RNA sequencing, bioinformatic analyses, transmission electron microscopy and JC-1 staining.
Results:
Our study revealed a distinct metabolic signature of HBV-related HCC, marked by elevated acetyl-CoA, which was driven by ACSS2. ACSS2 was upregulated by the carbohydrate response element-binding protein in HBV-related HCC. Furthermore, ACSS2 improved tumor cell proliferation, an effect that was dependent on its enzymatic activity. Mechanistically, ACSS2-induced acetyl-CoA accumulation activated voltage-dependent anion channels 1 transcription through increased H3K27ac occupancy, which subsequently promoted mitophagy and HBV-related HCC tumorigenesis. Notably, targeting ACSS2 by depletion or inhibition with a catalytic inhibitor significantly suppressed tumor growth.
Conclusions
These findings not only illustrate the interplay between metabolic reprogramming, epigenetic modification, and tumorigenesis in the context of HBV infection, but also highlight ACSS2 as a novel metabolic vulnerability in HBV-related HCC. Therefore, targeting ACSS2 could be a novel strategy against HBV-related HCC.

The development of traditional Chinese medicine (TCM) diagnosis and treatment has undergone multiple paradigms, evolving from sporadic experiential practices to systematic approaches in syndrome differentiation and treatment and further integration of disease and syndrome frameworks. TCM is a vital component of the medical system, valued alongside Western medicine. Treatment based on syndrome differentiation embodies both personalized treatment and holistic approaches; however, the inconsistency and lack of stability in syndrome differentiation limit clinical efficacy. The existing integration of diseases and syndromes primarily relies on patchwork and embedded systems, where the full advantages of synergy between Chinese and Western medicine are not fully realized. Recently, driven by the development of diagnosis and treatment concepts and advances in analytical technology, Western medicine has been rapidly transforming from a traditional biological model to a precision medicine model. TCM faces a similar need to progress beyond traditional syndrome differentiation and disease-syndrome integration toward a more precise diagnosis and treatment paradigm. Unlike the micro-level precision trend of Western medicine, precision diagnosis and treatment in TCM is primarily reflected in data-driven applications that incorporate information at various levels, including precise syndrome differentiation, medication, disease management, and efficacy evaluation. The current priority is to accelerate the development of TCM precision diagnosis and treatment technology platforms and advance discipline construction in this area.

Adipocytic tumors are the most common type of soft tissue tumors, which can be divided into lipomas and liposarcomas.Studies have found that benign lipomas resected from the same site can transform into liposarcomas, especially atypical lipomatous tumors(ALT),suggesting an underlying biological association. In this case, the patient underwent resection of a right neck mass in our hospital in 2017,and the pathology showed fibrolipoma. One year later, the mass recurred at the original site but was not treated.By 2023,the mass had significantly enlarged and extended deeply.MRI showed that the lesion had an unclear boundary with the surrounding fat, suggesting liposarcoma.Puncture and molecular pathology showed MDM2 gene amplification, and the diagnosis of ALT was made, which was confirmed by pathology after complete surgical resection. It can be seen that adipocytic tumors have great differences in biological behavior, and benign lipomas can transform into liposarcomas(especially ALT).Clinically,it is necessary to combine imaging, pathology and molecular detection(such as MDM2/CDK4)for differentiation to formulate treatment plans.

Implementing the reform of outpatient mutual-aid is a crucial measure to improve China's medical security system.Based on the policy diffusion theory,we took 228 prefecture-level cities across the country as the research objects and used the event history method to analyze the diffusion mode and influencing factors of the general outpatient pooling policy for employees'basic medical insurance, and divided into three types:the type of reducing the stock of Medical Savings Accounts(MSA),the welfare increment type I and type II for separate analysis. We found that the diffusion of the general outpatient pooling policy presented an"S"-shaped curve over time, and was mainly horizontally diffused in space, with a neighboring effect and a regional demonstration effect. The type of reducing the stock of MSA policy diffuses horizontally;the welfare increment type I is mainly horizontal diffusion, supplemented by vertical diffusion;the welfare increment type II is not affected by the pressure from higher-level governments, and mainly adopts the horizontal diffusion mode.In terms of the influencing factors of policy diffusion, the diffusion of the general outpatient pooling policy was affected by multiple factors such as policy issues, legitimacy, and government capacity. The diffusion of the type of reducing the stock of MSA policy was not affected by the pressure from higher-level governments, but was influenced by policy issues and government capacity. The diffusion of the welfare increment type I policy was most affected by policy issues, and the welfare increment type II was most affected by government capacity.

Aim To explore the protective effects and underlying molecular mechanisms of periplogenin(Ppg)in preventing acetaminophen(APAP)-induced drug-induced liver injury(DILI).Methods An APAP-induced liver injury model was established in vi-vo and in vitro.Liver/spleen indices were recorded, and serum aspartate aminotransferase(AST)and ala-nine aminotransferase(ALT)levels were measured. The pathological changes in liver tissue were observed using hematoxylin-eosin(HE)staining.The influence of Ppg on inflammatory cytokines(IL-6,TNF-α,IL-1 β)and key proteins within the associated signaling pathways was examined using enzyme-linked immu-nosorbent assay(ELISA),quantitative polymerase chain reaction(qPCR),and Western blot.Additional-ly,glutathione(GSH),malondialdehyde(MDA),and superoxide dismutase(SOD)activity levels were meas-ured.The expression of markers related to the NF-κB signaling pathway was assessed using qPCR,Western blot, and immunofluorescence. Results In vivo exper-iments showed that Ppg improved the liver and spleen index of mice with drug-induced liver injury, and the levels of AST and ALT in the serum decreased signifi-cantly,reducing the pathological damage of liver tis-sue.In vitro experiments showed that Ppg significantly inhibited the expression of IL-6,TNF-α and IL-1 β,and decreased the levels of GSH and MDA and in-creased SOD activity in AML-12 cells.Additionally, the inhibition of the NF-κB signaling pathway using pyrrolidinedithiocarbamate ammonium(PDTC),an NF-κB signaling pathway inhibitor reduced inflammato-ry cytokine expression and significantly decreased p65 nuclear translocation, showing results comparable to those observed in the high-dose Ppg group.Conclu-sions Ppg alleviates APAP-induced liver injury,po-tentially by inhibiting the NF-κB signaling pathway in hepatocytes, thereby reducing the inflammatory re-sponse and protecting liver from APAP-induced dam-age.
