1.Construction and Evaluation of "Constitution-disease-syndrome" Trinity Model for Rodents with Qi Deficiency

The theory of constitution in traditional Chinese medicine (TCM) has emerged as a new discipline in recent years. Constitution plays a vital role in the onset,progression,transformation,and prognosis of diseases. At present,some clinical scholars have adopted a novel diagnostic and treatment model of "constitution differentiation-disease identification-syndrome differentiation",in which constitution is regarded as a core element throughout the diagnostic and therapeutic process. Constitution is closely associated with etiology,onset,pathogenesis,syndrome differentiation,and treatment. Against this background,the construction of animal models based on constitution holds far-reaching significance for advancing clinical research. This paper focuses on the construction and evaluation of rodent models with Qi-deficiency constitution,aiming to explore how to further induce Qi-deficiency syndromes and related disease states on the basis of Qi-deficiency constitution models,thereby developing an integrated animal model that embodies the trinity of "constitution-disease-syndrome". The establishment of this model not only provides a solid experimental foundation for the development of new therapies and drugs in TCM targeting specific constitutions,diseases,and syndromes,but also greatly promotes the modernization and scientific advancement of TCM theory. By comprehensively applying multidisciplinary technologies and methods,the study evaluates the model's validity,reliability,and practicality,with the aim of opening new avenues for future research in TCM and promoting the development of the field.

3.Application Value of 18F-FDG PET/CT Metabolic Parameters in Prognosis of Nasopharyngeal Carcinoma

Objective To investigate the value of 18F-FDG PET/CT metabolic parameters in the prognostic assessment of nasopharyngeal cancer patients. Methods The clinical data and PET/CT metabolic parameters of 185 nasopharyngeal cancer patients were retrospectively analyzed. The collected parameters were SUVmax, MTV, TLG, total metabolic tumor volume (TMTV) and whole-body total lesion glycolysis (WTLG). The ROC curve was used to determine the optimal cut-off values of PET/CT metabolic parameters. Univariate and multivariate Cox regression models were used to screen the independent prognostic factors. Kaplan–Meier curves were used to analyze the survival differences. Results The results of univariate Cox regression analysis showed that age, pathologic type, WTLG, TMTV, MTV, and TLG were closely associated with OS and PFS; and SUVmax was associated with PFS (P<0.05). Multivariate Cox regression analysis results showed that age, TMTV, and WTLG were the independent prognostic factors for OS and PFS (P<0.05). The combination of WTLG with T/N staging (AUC=0.781 and 0.781) and TMTV with T/N staging (AUC=0.800 and 0.790) yielded greater predictive accuracy than that of WTLG and TMTV alone (AUC=0.724 and 0.719) or T/N staging (AUC=0.593 and 0.575). Conclusion TMTV and WTLG are important prognostic predictors of nasopharyngeal carcinoma. TLG and MTV of primary lesions are prognostic factors for patients’ PFS and OS. SUVmax has limited prognostic value. Systemic metabolic indexes (TMTV and WTLG), when combined with T/N staging, can optimize prognostic stratification.
4.Research progress on pentacyclic triterpenoids in medicinal Ilex species and their pharmacological activities.

Traditional Chinese medicine(TCM) capable of clearing heat and removing toxin is most commonly used in clinical practice and has the effect of removing fire-heat and toxin. Studies have shown that most of the Ilex plants have the effect of clearing heat and removing toxin, among which the varieties of I. cornuta, I. pubescens, I. rotunda, I. latifolia, and I. chinensis are most widely used. These plants generally contain triterpenoids and their glycosides, alkaloids, flavonoids, phenylpropanoids, and other chemical components, especially pentacyclic triterpenoids. According to their skeletons, pentacyclic triterpenoids can be divided into the oleanane type, the ursane type, the lupinane type, etc. Among them, ursane-type components are the most abundant, and 136 species have been found so far. These components have been proved to have pharmacological effects such as anti-inflammatory, anti-tumor, hypolipidemic, anti-thrombosis, cardiomyocyte-protective, antibacterial, and hepatoprotective effects. Therefore, this paper systematically reviews the domestic and foreign literature on Ilex plants with a focus on the research progress on pentacyclic triterpenoids and their pharmacological activities, aiming to provide reference for the development of TCM resources with the effect of clearing heat and removing toxin.

5.Study of a deep learning-based artificial intelligence model for automatic measurement and classification of cystocele

Objective:To explore the clinical application value of convolutional neural network(CNN)based on deep learning in the automatic measurement of dynamic pelvic floor ultrasound video parameters and the diagnosis and classification of cystocele.Methods:A retrospective analysis was conducted on dynamic pelvic floor ultrasound videos from 398 postpartum women who underwent examinations at the First People's Hospital of Foshan between June 2020 and June 2022. The lowest point of the posterior bladder wall(PWB),urethral rotation angle(URA),and retrovesical angle(RVA)were manually measured by a senior radiologist(R1)and a junior radiologist(R2),and cystocele was classified according to the Green standard. The CNN model was employed to automatically extract the above parameters and to diagnose and classify cystocele. Using R1 measurements as a reference,intraclass correlation coefficient(ICC)was used to evaluate the consistency between the CNN model and R1,as well as between R2 and R1. The Kappa value was used to assess the agreement between the CNN model,R2,and R1 in the diagnosis and classification of cystocele. Additionally,the time consumption of the three measurement methods was compared.Results:The CNN model showed good consistency with R1 in measuring PWB and URA(ICC = 0.983,0.894),while its consistency in measuring RVA was moderate(ICC = 0.614). The ICC between R2 and R1 in measuring PWB,URA,and RVA was 0.979,0.815,and 0.627,respectively. In the measurement of PWB and URA,the consistency between the CNN model and R1 was superior to that between R2 and R1. For cystocele diagnosis,the Kappa value between the CNN model and R1 was 0.924,which was higher than that between R2 and R1(0.904). In cystocele classification,the Kappa value between the CNN model and R1 was 0.503,also higher than that between R2 and R1(0.426). The CNN model processed a single video in 2.5(0.6)s,significantly faster than R1[59.9(16.9)s]and R2[56.8(11.2)s](all P < 0.001). Conclusions:The CNN model demonstrates high accuracy and efficiency in the measurement,diagnosis,and classification of cystocele,outperforming a junior radiologist and showing potential for clinical application.
6.Study on artificial intelligence-based ultrasound diagnosis and auxiliary decision-making for ovarian tumors

Objective:To apply artificial intelligence(AI)in classifying ovarian tumors on ultrasound images,and compare the diagnostic results of several sonographers with varying seniority levels.Methods:A total of 645 patients diagnosed with adnexal masses via gynecological ultrasound examination at Qilu Hospital of Shandong University from January 2021 to December 2024 were enrolled. Three deep learning architectures,i.e.,Alexnet,Densenet121,and Resnet50 were developed and used to internally test the classification effectiveness of ovarian tumors,while the optimal model was selected for external testing. Two junior sonographers and two senior sonographers were recruited to independently diagnose ovarian tumors in the external test dataset. Subsequently,the benign and malignant results of the model's predictions were disclosed to each sonographer,and their revised diagnoses on the same external test data in combination with the best AI model were recorded.Results:The optimal model achieved an accuracy of 0.941,sensitivity of 0.936,and specificity of 0.944 on the internal test dataset,and maintained robust performance on the external test dataset with accuracy of 0.891,sensitivity of 0.880,and specificity of 0.907. Compared to junior sonographers,the optimal model demonstrated significantly higher sensitivity in discriminating benign from malignant ovarian tumors(0.880 vs. 0.723,0.602;all P<0.05). No statistically significant difference was observed in diagnostic accuracy between the optimal model and senior sonographer 1( P=0.05). With assistance from the optimal model,junior sonographers achieved significant improvements in both sensitivity and specificity(sensitivity:0.723 vs. 0.843,0.602 vs. 0.819;specificity:0.778 vs. 0.833,0.685 vs. 0.741;all P<0.05). Conclusions:The optimal model achieves comparable performance to that of senior sonographers in ovarian tumor classification. With model assistance,the diagnostic performance of junior sonographers is significantly improved.
7.Advancements in the role of iris parameters in implantable collamer lens implantation

Phakic intraocular lens implantation has become one of the important means of correcting refractive errors today. Among them,the implantable collamer lens(ICL)is favored for its wide range of correction, excellent optical quality, and high safety, but the risks of postoperative complications such as glaucoma and anterior subcapsular opacification still exist. Vault is an important indicator for evaluating the safety after ICL implantation, and its ideal state is crucial for preventing complications. Studies have shown that iris morphology has a significant impact on vault. In order to further optimize surgical outcomes and improve surgical safety, this review comprehensively reviews the research progress of iris-related parameters in ICL implantation and discusses the importance of various parameters in preoperative evaluation and postoperative follow-up.
8.Elucidation of the anti-head and neck squamous cell carcinoma mechanism of Phellinus baumii polyphenol based on network pharmacology and experimental verification

Objective To investigate the effects of Phellinus baumii polyphenol(PBP)on head and neck squamous cell carcinoma(HNSCC)and analyze the potential mechanism based on network pharmacology and in vitro experiments.Methods Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP),DrugBank,GeneCards,Comparative Toxicogenomics Database(CTD)and Online Mendelian Inheritance in Man(OMIM)database were used to screen the active components of PBP and potential targets of PBP on HNSCC.The potential target interaction network was constructed using String database,and the core targets were screened by two-step topology analysis.Enrichment analysis of potential targets was performed using the DAVID database.Human HNSCC cell lines SCC-15 and SCC-25 were cultured in vitro using PBP intervention of 0,25,50 mg/L,respectively.The cell proliferation and colony formation ability were detected by cell counting reagent(CCK-8)and colony formation assay.Western blot assay was used to detect the expression of PBP core target protein in 2 cell lines.Results A total of 280 targets were identified for 17 active components of PBP,264 of which were HNSCC-related genes.Two-step topology analysis showed that hypoxia inducible factor 1 subunit alpha(HIF1A),tumor protein p53(TP53),AKT serine/threonine kinase 1(AKT1),signal transducer and activator of transcription 3(STAT3),cyclin A2(CCNA2)and JUN proto-oncogene(JUN)were the core targets.The enrichment results suggested that PBP may play a role in HNSCC through various pathways.In vitro experiment results showed that with the increase of PBP intervention concentration,the proliferation ability and colony formation ability of SCC-15 and SCC-25 cells were significantly decreased(P<0.05),while the protein expression levels of STAT3,AKT1 and CCNA2 were decreased(P<0.05).Conclusion PBP can inhibit the progression of HNSCC by multi-target and multi-pathway.
9.Effect of Early Enteral Nutrition Intervention on Neurological Function,Nutritional Status,and Inflammatory Stress Response in Patients with Severe Traumatic Brain Injury

Objective:To observe the effects of early enteral nutrition(EN)intervention on neurological function,nutritional status,and inflammatory stress response in patients with severe traumatic brain injury.Methods:This study was a single-center prospective study,68 patients with severe traumatic brain injury were divided into control group[received parenteral nutrition(PN)intervention,n=34]and observation group(received early EN intervention,n=34)according to the envelope lottery method.The improvement of neurological function[serum central nervous system specific(S1 00(3)protein,national institutes of health stroke scale(NIHSS)score,myelin basic protein(MBP)],nutritional status[transferrin(TRF),total protein(TP),albumin(ALB)and prealbumin(PA)],inflammatory stress indicators[C-reactive protein(CRP),procalcitonin(PCT),glutathione peroxidase(GSH-PX),malondialdehyde(MDA)],daily living activity ability,quality of life,and incidence of complications between two groups were compared.Results:Compared with the control group after intervention,the observation group had lower NIHSS scores,S100[3 protein,MBP,CRP,PCT and MDA,had higher TRF,TP,ALB,PA,GSH-PX,improved Barthel index and world health organization quality of life assessment scale-100(WHOQOL-100)score(P<0.05).There was no difference in the incidence of complications between the two groups(P>0.05).Conclusion:Early EN intervention in patients with severe traumatic brain injury can effectively improve neurological function,nutritional status,and reduce the body's inflammatory stress response,which is worthy of clinical reference and application.
10.Protective effect of prenatal exercise on myocardial ischemia/reperfusion injury in offsprings of mice

Objective To explore the effect of maternal exercise on blood pressure,cardiac pheno-type and susceptibility to myocardial is chemia/reperfusion(MI/R)injury in adult male offsprings of mice.Methods Pregnant mice were randomly divided into a sedentary group(p-Ctr)and an exercise group(p-EX),each of 12.The exercise group underwent daily 60-minutenon-weight bearing swim-ming from gestational day(GD)1 to 18,6 days a week.Then,their male offsprings at 3 months of age(3M)were selected as the research subjects,namely the Ctr-3M group and the EX-3M group,with 12 mice in each group.The MI/R model was established by ligation of the anterior descending branch of the left coronary artery(LAD)surgery(30 min of ischemia,24 h of reperfusion).Then,the systolic blood pressure(SBP),diastolic blood pressure(DBP),and mean arterial pressure(MAP)were monitored non-invasively via tail artery blood pressure measurement,while the cardiac function was detected by using the small animal ultrasound.Moreover,the cardiac morphology and col-lagen volume fraction(CVF)of myocardium was observed by HE staining and Masson staining,respec-tively.Meanwhile,the cross-sectional area(CSA)of myocardial cells and myocardial infarction area(INF/AAR)was measured using WGA staining and Evans Blue-TTC double staining,accordingly.What's more,the apoptosis index(AI)of myocardial cells and serum cardiac troponin I(cTnI)were de-tected by using TUNEL staining and ELISA,respectively,while the expression levels of Bax and Bcl-2 proteins were determined by using Western blotting.Results 1)From GD11 to GD19,the body weight of females in the p-EX group was always significantly lower than that in the p-Ctr group(GD11,GD13;P<0.05;GD12,GD14~GD19;P<0.01),but there was no significant difference in the litter size and abortion rate(P>0.05).2)There were no significant differences between the EX-3M and Ctr-3M groups in the heart weight(HW),body weight(BW)and heart weight to body weight ratio(HW/BW),SBP,DBP,MAP,myocardial CVF,and CSA of male offsprings(P>0.05).After MI/R surgery,in both p-Ctr and p-EX groups,myocardial transverse striations disappeared and cardiomyocytes were disarranged with degeneration and necrosis.Moreover,CVF of the EX-3M group was significantly lower than the Ctr-3M group after MI/R surgery(P<0.05).3)No significant differenc-es were found between the Ctr-3M and EX-3M groups in the average cardiac ejection fraction(EF),fractional shortening(FS),left ventricle internal diameter in diastole(LVIDd),and left ventricle inter-nal diameter in systole(LVIDs)(P>0.05).However,after the MI/R surgery,the postoperative EF and FS(P<0.01),and LVIDs(P<0.05)in the EX-3M group were significantly higher than the Ctr-3M group,without significant difference in LVIDd(P>0.05).4)After MI/R surgery,the average AI,INF/AAR and serum cTnI level of the EX-3M group were significantly lower than the Ctr-3M group(P<0.01 for all).5)The relative expression level of myocardial Bcl-2 protein after MI/R surgery in the EX-3M group was significantly higher than the Ctr-3M group(P<0.05),while that of myocardial Bax protein(P<0.05)and the Bax/Bcl-2 ratio(P<0.01)were significantly lower than the latter.Conclu-sions Prenatal exercise significantly reduces the susceptibility to myocardial ischemia injury in 3-month-old male mice offspring,improves the contractile function of their heart,and attenuates the degree of cardiomyocyte apoptosis and necrosis,exerting a cardioprotective effect in MI/R injury.
