1.Factors affecting prognosis of pancreatic cancer patients with carbohydrate antigen 19-9 negative

Objective:To investigate the factors affecting prognosis of pancreatic cancer patients with carbohydrate antigen 19-9 (CA19-9) negative before surgery.Methods:Retrospectively analyzed the clinic pathological data of 158 patients with pancreatic cancer who underwent radical surgery between November 2011 and August 2022, including 84 males, and 74 females, aged (65±9) years old. Patients were divided into two groups according to CA 19-9 levels over a threshold of 37 U/ml. Patients with CA19-9 levels ≤37 U/ml were included in the CA19-9 negative group ( n=34), while patients with CA19-9 levels >37 U/ml were included in the CA19-9 positive group ( n=124). We performed a comparison between the two groups regarding demographic and clinical data, tumor staging, lymphovascular and nerve invasion and so on. Survival data were extracted from outpatient or telephone records during follow-up. The Kaplan-Meier method was used to draw the survival curve, and the survival rate was compared by log-rank test. Multivariate analysis was performed using the Cox regression analysis model. Results:CA19-9 expression was associated with tumor location and nerve invasion (both P<0.001). There was no significant difference in cumulative survival rate ( χ2=0.07, P=0.787) and progression-free survival rate ( χ2=0.13, P=0.721) between two groups. Multivariate analysis showed that lymphovascular invasion ( HR=5.962, 95% CI: 2.189-16.239, P<0.001) was a prognostic factor for overall survival in the CA19-9 negative group. Age ≤60 years old ( HR=0.306, 95% CI: 0.118-0.796, P=0.015) and lymphovascular invasion ( HR=3.096, 95% CI: 1.232-7.780, P=0.016) were prognostic factors for progression-free survival. Conclusion:Age ≤60 years old and lymphovascular invasion are the influencing factors of poor prognosis in pancreatic cancer patients with CA19-9 negative.
2.Bioequivalence and safety evaluation of domestic Nadroparin calcium injection in Chinese healthy volunteers

OBJECTIVE To evaluate the bioequivalence and safety of two k inds of Nadroparin calcium injection in healthy Chinese volunteers by subcutaneous injection. METHODS According to the block randomization method ,24 Chinese healthy adult volunteers were included and divided into TR (test preparation-reference preparation )group and RT (reference preparation-test preparation)group at a ratio of 1∶1. A randomized ,open-labelled,single-dose and two-cycle crossover study was designed ,the fasting subjects of two groups were given test or reference preparation 6 150 AⅩaIU subcutaneously on the first day of each cycle and exchanged in the second cycle ,and the wash-out period was 7 days. The blood samples were collected at different time points before and after administration. The activity of anti-coagulant factor Ⅹa(Anti-Ⅹa)and Anti- Ⅱa in human plasma were determined by chromogenic substrate method ,and the pharmacodynamic parameters were calculated according to the non-atrioventricular model and the bioequivalence was evaluated. The occurrence of adverse events (AEs)was recorded. RESULTS After administration ,the main pharmacodynamic parameters for Anti- Ⅹa activity of test preparation and reference preparation were as follows :t1/2 were(4.87±1.06) and(4.03±1.00)h,tmax were 4.50(2.00,8.00)and 5.50(2.50,8.00)h,Anti-Ⅹamax were(0.66±0.12)and(0.56±0.11)IU/mL;main pharmacodynamic parameters of Anti- Ⅱa activity of two preparations were as follows :t1/2 were(3.64±1.60)and(5.74±7.23)h,tmax were 4.00(2.50,8.00)and 4.00(2.00,8.00)h,Anti-Ⅱamax were both (0.10±0.03)IU/mL. The values of 90%confidence interval of geometric mean ratio of Anti- Ⅹamax,AUEC0-t and AUEC 0-∞ were 110.98%-123.50%,112.11%-121.24%and 111.57%-120.00%, respectively. During experiment ,14 subjects reported 19 cases of mild AEs ,among which hematoma ,purpura and maculopapular rash may be related to drugs ;no serious AEs were observed. CONCLUSIONS The domestic Nadroparin calcium injection is bioequivalent to the reference preparation ,and both of them show good safety.
3.Expression Patterns of Inducible Cre Recombinase Driven by Differential Astrocyte-Specific Promoters in Transgenic Mouse Lines.

Astrocytes are the most abundant cell type in the central nervous system (CNS). They provide trophic support for neurons, modulate synaptic transmission and plasticity, and contribute to neuronal dysfunction. Many transgenic mouse lines have been generated to obtain astrocyte-specific expression of inducible Cre recombinase for functional studies; however, the expression patterns of inducible Cre recombinase in these lines have not been systematically characterized. We generated a new astrocyte-specific Aldh1l1-CreER knock-in mouse line and compared the expression pattern of Cre recombinase between this and five widely-used transgenic lines (hGfap-CreER from The Jackson Laboratory and The Mutant Mouse Resource and Research Center, Glast-CreER, Cx30-CreER, and Fgfr3-iCreER) by crossing with Ai14 mice, which express tdTomato fluorescence following Cre-mediated recombination. In adult Aldh1l1-CreER:Ai14 transgenic mice, tdTomato was detected throughout the CNS, and five novel morphologically-defined types of astrocyte were described. Among the six evaluated lines, the specificity of Cre-mediated recombination was highest when driven by Aldh1l1 and lowest when driven by hGfap; in the latter mice, co-staining between tdTomato and NeuN was observed in the hippocampus and cortex. Notably, evident leakage was noted in Fgfr3-iCreER mice, and the expression level of tdTomato was low in the thalamus when Cre recombinase expression was driven by Glast and in the capsular part of the central amygdaloid nucleus when driven by Cx30. Furthermore, tdTomato was clearly expressed in peripheral organs in four of the lines. Our results emphasize that the astrocyte-specific CreER transgenic lines used in functional studies should be carefully selected.
4.Studies on fingerprints and efficacy-related substance of classical prescription Zhuru Decoction.

Based on fingerprint and network pharmacology,the whole process quality control of Zhuru Decoction was conducted and efficacy-related substances were predicted.The fingerprints of raw materials,decoction pieces and Zhuru Decoction were established,and 25 common peaks were identified,including 9 common chromatographic peaks of 3'-hydroxy puerarin,puerarin,3'-methoxy puerarin,puerarin,aperioside,daidzin,daidzein,liquiritin,glycyrrhizic acid and 6-gingerol, with similarity all greater than 0.95.The main groups of pharmacodynamic substances can be transferred from raw materials,decoction pieces to Zhuru Decoction step by step,with a clear affiliation relationship.Based on the testability and traceability,the active ingredients were screened,and the network relationship of "component-target-pathway" was constructed and analyzed for the nine chemical components screened by network pharmacology.The enriched pathways included energy metabolism,alcoholism,and smooth muscle contraction and relaxation-related pathways.The nine active components of Zhuru Decoction may achieve the effects of clearing heat, alleviating a hangover, harmonizing stomach and stopping vomiting through these signaling pathways.Based on transitive and traceable properties of the above 9 components as well as their close relationship to the efficacy of Zhuru Decoction,these 9 components can be identified as potential efficacy-related substances and provide basis for the overall quality control of Zhuru Decoction.

6. Effects of different softening methods on quality of Glycyrrhizae Radix et Rhizoma

Objective: A method was established to obtain fingerprint and determination of six components in Glycyrrhizae Radix et Rhizoma Pieces (GRRP) based on HPLC-PDA, and samples with four kinds of softening methods (showering moistening, steaming, 70 ℃ decompression steaming, 85 ℃ decompression steaming) were analyzed. Methods: The content of total flavonoids and total saponins was determined by ultraviolet spectrophotometry with liquiritin and glycyrrhizic acid as reference materials. Simultaneous determination of six components of liquiritin, ononin, isoliquiritin, glycyrrhizin, echinatin, glycyrrhizic acid was performed based on HPLC. Changes of the components content in the samples which treated by different softening methods were compared. The similarity evaluation of samples with different softening methods was carried out by the chromatographic fingerprint similarity evaluation system of traditional Chinese medicine, and cluster analysis was also carried out. Results: The results showed that the content of total flavonoids and total saponins in untreated samples was the highest, and the content of total flavonoids and total saponins in samples treated by showering moistening was the lowest. The three treatment methods of atmospheric pressure steaming, steaming decompression at 70 ℃ and steaming decompression at 85 ℃ had little effect on the samples. The content determination showed that the content of isoliquiritin was decreased significantly after softening treatment. The difference among the different softening treatment groups was not significant. The samples with different softening methods of the three batches of samples were grouped together with their raw products. Different softening methods had no significant difference in the composition of the medicinal herbs. Conclusion: The established method can quickly and accurately determine the six components, and in particular, the content of isoglycyrrhizin should be monitored. Combining production efficiency, production cost and quality evaluation, steaming is the most feasible in the production process. This study provided theoretical guidance for the large-scale production of softening, which was conducive to further standardizing the production process of GRRP.
7.Quality research of Puerariae Lobatae Radix from different habitats with UPLC fingerprint and determination of multi-component content.

To establish ultra performance liquid chromatography( UPLC) fingerprint of Puerariae Lobatae Radix from different habitats and simultaneously determine the contents of six isoflavonoids. The UPLC fingerprint analysis and content determination were performed on a Waters ACQUITY UPLC BEH C_(18)( 2. 1 mm×50 mm,1. 7 μm) chromatographic column,with acetonitrile-0. 05% formic acid as mobile phase for gradient elution. The detection wavelength was set at 250 nm; the flow rate was 0. 2 mL·min~(-1); the column temperature was 30 ℃ and the injection volume was 2 μL. Similarity evaluation system for chromatographic fingerprint of traditional Chinese medicine( TCM) was adopted; principal component analysis( PCA) and discriminant analysis by partial least square method( PLS-DA) in Simca-P software were used to identify the differential components in samples from three habitats. The similarity was over 0. 90 in 29 batches of samples,indicating good consistency of the samples. The samples were clustered into 3 categories by PCA and PLS-DA,and six differential components such as puerarin apioside,daidzin,and isoflavoues aglycone were found. The determination results of 6 isoflavones,including 3'-hydroxy puerarin,puerarin,3'-methoxy puerarin,puerarin apioside,daidzin,and isoflavoues aglycone,showed that the content of the same component and the fluctuation range between different components were all different among different habitats. The total content of 6 isoflavones from different regions was Anhui 11. 21% >Henan 10. 97% >Shannxi 9. 38%. The establishment of UPLC fingerprint combined with simultaneous determination of 6 active components provides a more comprehensive reference for quality control and quality evaluation of Puerariae Lobatae Radix.

8.Study on quality of standard decoction of Coptidis Rhizoma based on traditional decoction process.

In order to determine the quality evaluation method for standard decoction of Coptidis Rhizoma,15 batches of standard decoction of Coptidis Rhizoma were prepared by using standardized process. Parameters such as traits,p H value,indicative component content,fingerprint similarity,composition transfer rate and dry extract rate were selected as the indexes for quality evaluation. Similarity evaluation and cluster analysis were performed for HPLC fingerprint of standard decoction,and mathematical model was used to study the correlation between dry extract rate,berberine content,berberine transfer rate in standard decoction and berberine content in decoction pieces. The results showed that the similarity of fingerprints was greater than 0. 99 for these 15 batches of standard decoctions of Coptidis Rhizoma. In cluster analysis,the standard decoctions of Coptidis Rhizoma from 4 producing areas were classified into 3 categories,consistent with the content determination results,indicating that there were quality differences among different producing areas.R2 in three linear regression mathematical models established was all greater than 0. 9,with significant difference. The validation of three batches of data showed that the models had good accuracy. Therefore,this model can be used to predict the quality of standard decoction prepared from different Coptidis Rhizoma pieces. In the standard decoction process established in this study,the integrity of the traditional process was greatly preserved,and the established quality evaluation method could be used to comprehensively examine the quality of the standard decoction,which can provide a demonstration for the related research of water extraction preparation containing Coptidis Rhizoma pieces.

9.Diagnosis of acute posteromedial corner injury of knee joint under arthroscope

Objective To study the performance of knee arthroscopy in patients with acute posteromedial corner injury and to explore the use of arthroscopy to help diagnosis. Methods From April to August 2017, the arthroscopic posteromedial corner after operation of 7 cases of acute knee joint injury patients data were retrospectively studied, treatment are in the same group. They were two man and three women, 37 ~ 59 years of old (average 49.6 years). All patients were selected for preoperative MRI to indicate medial collateral ligament damage. The VAS score was evaluated preoperative. When check-up knee flexion and 30° valgus stress test positive and 0° valgus stress test positive. Intraoperative arthroscopy was performed to confirm the injury of the Posteromedial corner. The medial meniscus was formed or stitched. The medial collateral ligament and Posteromedial corner were repaired. The knee X-ray examination were measured preoperative and in 2 months after surgery, Evaluation of the knee VAS score in 2 months after surgery. Results All patients were followed up for 2 months. The medial meniscus were easily observed and raise easily 5 cases undergoing arthroscopic surgery. All 7 patiens were showed the injury of the joint capsule ligament complex. All of the 7 cases showed the injury of the joint capsule ligament complex. Preoperative and postoperative knee medial clearance for 2 months in knee valgus stress buckling 30° [(9.2 ± 1.3) vs (3.0 ± 1.0) mm]. Preoperative and postoperative knee medial clearance for 2 months in knee valgus stress 0° [(8.8 ± 1.1) vs (2.9 ± 1.0) mm]. There were statistically significant differences (P < 0.05). After 2 months in knee valgus stress buckling is compared with the normal control group 30° [(3.0 ± 1.0) vs (2.9 ± 1.2) mm]. After 2 months in 0° knee valgus stress compared with normal control group [(2.9 ± 1.0) vs (2.8 ± 1.2) mm]. There was no statistical difference (P > 0.05). Preoperative and postoperative knee joint pain VAS score [(5.4 ± 0.3) vs (2.3 ± 1.1)]. There were statistically significant differences (P < 0.05). Conclusion Under arthroscopy detection in patients with acute Posteromedial corner injury. If it is easy to observe posterior horn of medial meniscus and the medial meniscus angle is easy to raise; The medial meniscus posterior horn joint capsule and ligament damage. They were indicated acute Posteromedial corner injury. Timely deal with medial meniscus and repair Posteromedial corner, the stability of the knee joint can be satisfactory.
10.Study on the inverse problem of electrical impedance tomography based on self-diagnosis regularization.

The inverse problem of electrical impedance tomography (EIT) is seriously ill-posed, which restricts the clinical application of EIT. Regularization is an important numerical method to improve the stability of the EIT inverse problem as well as the resolution of the imaging. This paper proposes a self-diagnosis regularization method based on Tikhonov regularization and diagonal weight regularization method (DWRM). Firstly, the ill-posedness of the inverse problem is analyzed by sensitivity. Then, the performance of the self-diagnosis regularization is analyzed through the singular value theory. Finally, some simulated experiments including simulations and flume experiment are carried out and verify that the self-diagnosis regularization has better image quality and anti-noise ability than those of traditional regularization methods. The self-diagnosis regularization method weakens the ill-posedness of inverse problem of EIT and can prompt the practical application of EIT.
