Objectives: This study aimed to identify predictors of health literacy (HL), focusing on nonlinear relationships and interaction effects in a nationally representative population. Methods: This cross-sectional study analyzed data from 8,630 Korean adults participating in the Korea Health Panel Survey. HL was assessed using the Korean version of the European Health Literacy Survey Questionnaire 16-item (HLS-EU-Q16) and categorized as sufficient or insufficient. An extreme gradient boosting algorithm (XGBoost) was applied, incorporating survey weights. Model performance was evaluated using standard metrics, including the area under the receiver operating characteristic curve (AUC) and Brier score. Shapley additive explanations (SHAP) values were calculated to quantify individual feature importance and identify interaction effects among 69 features. Results: The XGBoost model achieved good discrimination (AUC = 0.840) and calibration (Brier score = 0.161). Age (24.4%) and education level (19.0%) were the most influential predictors. SHAP interaction analysis identified a meaningful interaction between age and education level (mean |interaction value| = 0.078), with interaction plots indicating positive patterns among adults aged 60–80 with lower educational attainment. The contribution of online health information use varied by age, showing a negative association among younger adults but a positive association among older adults. Conclusions HL is shaped by nonlinear and interactive effects across sociodemographic and health-related factors. Explainable machine learning approaches can facilitate the identification of high-priority populations and support the development of tailored, patient-centered educational strategies to improve HL and promote engagement with care.

Background: The exposome encompasses all factors people encounter through life, with the skin constantly exposed. While particulate matter (PM) and sleep deprivation are known to contribute to barrier dysfunction, their combined effects remain unclear. Objective: To evaluate the independent and combined effects of PM exposure and short-term sleep deprivation on skin barrier function. Methods: Forty healthy Korean women (aged 24–58 years) were enrolled in this study. Forearms were divided into 4 sites: control, PM exposure, sleep deprivation, and PM plus sleep deprivation. Parameters such as trans-epidermal water loss (TEWL), hydration, elasticity, roughness, and redness were measured at baseline and post-exposure. RNA sequencing and reverse transcription-polymerase chain reaction were conducted on tape-stripped skin samples. Results: PM exposure significantly increased TEWL (+25.59%, p<0.01), roughness (+21.9%, p<0.01), and redness (+13.7%, p<0.0001) while reducing elasticity (−3.98%, p<0.01). Sleep deprivation modestly reduced elasticity (−1.39%, p<0.05) without affecting other parameters.Combined PM and sleep deprivation did not further exacerbate barrier dysfunction compared to PM alone. RNA sequencing revealed reduced FLG and LORICRIN expression and upregulated endoplasmic reticulum (ER) stress markers (HSP90B1, CANX) in both PM and sleep deprivation conditions. Conclusion PM exposure impaired skin barrier function, while short-term sleep deprivation alone did not significantly affect the barrier, either independently or in combination with PM.However, it was observed that the sleep deprivation-only, while not directly causing barrier damage, induced changes in ER stress-related gene expression in tape-stripped skin samples, like the PM exposure-only. This suggests that such signaling pathways could potentially exacerbate skin barrier deterioration.

Dilated cardiomyopathy（DCM）is the most common type of cardiomyopathy in children.Gene mutation is one of the important causes of DCM in children.Gene mutations encoding sarcomeres，cytoskeleton，nuclear membrane proteins and ion channels lead to hereditary DCM.Hereditary DCM in children has a poor prognosis，and it is easy to be complicated with arrhythmia，has rapid deterioration of cardiac function and high rate of sudden cardiac death.Studies have found that some mutant genes have specific genetic patterns and clinical characteristics，and that the mutant genes have incomplete dominance and age-related dominance.This article discusses the clinical manifestations，auxiliary examinations and diagnosis of hereditary DCM in children through the relationship between gene and phenotype，and divides cardiomyopathy into stage A，stage B，stage C and stage D for treatment，guides follow-up，and suggests life and exercise methods，in order to provide reference for clinical work.

Purpose: This study explored current pediatric palliative care (PPC) in South Korea and suggests future directions based on the perspectives of full-time PPC workers. Methods: A mixed-methods secondary analysis was conducted using survey data from nurses and social workers involved in a government PPC project. The survey assessed services provided by hospitals, nurses, and social workers, and gathered perceptions of challenging tasks, evidence needs, the significance of PPC, and future expectations through open-ended questions. Data were analyzed using frequency and conventional content analyses. Results: Eighteen full-time workers from nine of the ten project hospitals participated. Eight hospitals provided pain and symptom management and all offered counseling and education for patients and parents. Only one provided respite care and legacy-making services. Bereavement care was offered in all hospitals primarily through individual counseling and there were also three self-help groups. Nurses have diverse roles including in practice, education, leadership, research, and consultation. Social workers address psychosocial and economic challenges, rehabilitation and social reintegration. The workers highlighted the challenges in end-of-life care and communication, stressing the need for protocols and further training.PPC was seen as meaningful by patients and families, as well as staff. Conclusion Despite significant progress in PPC in South Korea, quantitative capacity remains limited. Further qualitative improvements in policies, practices, human resource development, and healthcare training are essential. This study provides insights into current PPC practices and their limitations. The development of practical evidence to enhance employment stability warrants further investigation.

Purpose: Notable effectiveness of trastuzumab deruxtecan in patients with human epidermal growth factor receptor 2 (HER2)–low advanced breast cancer (BC) has focused pathologists’ attention. We studied the incidence and clinicopathologic characteristics of HER2-low BC, and the effects of immunohistochemistry (IHC) associated factors on HER2 IHC results. Materials and Methods: The Breast Pathology Study Group of the Korean Society of Pathologists conducted a nationwide study using real-world data on HER2 status generated between January 2022 and December 2022. Information on HER2 IHC protocols at each participating institution was also collected. Results: Total 11,416 patients from 25 institutions included in this study. Of these patients, 40.7% (range, 6.0% to 76.3%) were classified as HER2-zero, 41.7% (range, 10.5% to 69.1%) as HER2-low, and 17.5% (range, 6.7% to 34.0%) as HER2-positive. HER2-low tumors were associated with positive estrogen receptor and progesterone receptor statuses (p < 0.001 and p < 0.001, respectively). Antigen retrieval times (≥ 36 minutes vs. < 36 minutes) and antibody incubation times (≥ 12 minutes vs. < 12 minutes) affected on the frequency of HER2 IHC 1+ BC at institutions using the PATHWAY HER2 (4B5) IHC assay and BenchMark XT or Ultra staining instruments. Furthermore, discordant results between core needle biopsy and subsequent resection specimen HER2 statuses were observed in 24.1% (787/3,259) of the patients. Conclusion The overall incidence of HER2-low BC in South Korea concurs with those reported in previously published studies. Significant inter-institutional differences in HER2 IHC protocols were observed, and it may have impact on HER2-low status. Thus, we recommend standardizing HER2 IHC conditions to ensure precise patient selection for targeted therapy.
