Blue rubber bleb nevus syndrome (BRBNS) is a rare disorder characterized by multiple venous malformations on the skin, gastrointestinal system, and, less often, other organ systems. BRBNS lesions commonly manifest as deep-blue, soft, easily compressible, rubbery nodules commonly distributed on the limbs, trunk, and face. One of the serious complications is gastrointestinal bleeding associated with gastrointestinal lesions. Occult bleeding can lead to chronic anemia necessitating life long iron replacement therapy. In severe cases with massive bleeding, blood transfusion may be needed. We report a case of a healthy and active 18-year-old male with multiple dark-blue soft rubbery compressible nodules on the face, trunk, limbs and gluteal areas associated with tenderness on palpation and easy bleeding with minor trauma without other systemic signs and symptoms. Histopathology and radiology findings showed multiple venous malformations on the skin and gluteal muscle tissues. Hematologic and stool tests were normal. Comprehensive evaluation and early detection of complications are very important for favorable outcome of BRBNS patients.
