Objective To explore influencing factors of subsequent fracture among elderly osteoporotic patients and provide a basis for improving the management and early intervention after osteoporotic fracture. Methods A total of 14 349 elderly patients with osteoporotic fracture in 594 hospitals across 31 provinces(autonomous regions and municipalities)were selected by convenience sampling method from September to December 2023.A general information questionnaire and a clinical characteristics of fracture questionnaire were used. The univariate analysis and logistic regression were used to clarify the influencing factors and gender differences. Results A total of 11364 valid questionnaires were collected, with a valid questionnaire recovery rate of 79.20%.Gender,age,occupation,education,marital status, family history of osteoporosis, comorbid osteoarthropathies/rheumatism/ophthalmopathies,bone mineral density, risk of fall, risk of fracture grade, reasons and site for first fracture, whether to use walking aid and receive home rehabilitation guidance were associated with the occurrence of subsequent fracture.Gender comparisons found that widowhood, family history of osteoporosis, comorbid rheumatism, vertebral fracture, alcohol consumption, fall or no causative factor triggering the fracture, and educational level, home rehabilitation instruction were associated with the risk of subsequent fracture only in women(P<0.05).Smoking was associated with the risk of subsequent fracture only in men(P<0.05).Conclusion Osteoporotic subsequent fracture in the elderly is affected by multidimensional factors, with significant gender differences. The surveillance of key populations should be strengthened in prevention, control and intervention.

Objective:To assess the relationship between basophil levels and mortality in patients with intra-abdominal infection. Methods:Information on patients with intraperitoneal infection admitted to the intensive care unit were extracted from the MIMIC database.A time-dependent Cox regression model was used to adjust for confounders associated with 28-day mortality.Propensity score matching(PSM)was used to balance the baseline differences be-tween groups with different basophil levels, and a restricted cube chart(RCS)was used to show the relationship between basophil count and 28-day mortality in patients with intra-abdominal infection. Results:A total of 4403 patients with intra-abdominal infection were enrolled in the MIMIC database.Patients with high basophil levels have lower mortality than those with low basophil levels. There was an L-shaped curve between basophil level and 28-day mortality, with a cut-off value of 0.47×109/L.Cox regression analysis showed that basophil levels were an independent protective factor for mortal-ity in patients with intra-abdominal infection after adjusting for potential confounders(HR=0.586,95%CI:0.443-0.769).Protective factors for death at basophil levels remained after PSM adjusted for potential confounders(HR=0.628,95%CI:0.470-0.832).Conclusion:Basophil level is an independent protective factor for mortality in patients with intra-abdominal infection, and basophil levels should be dynamically monitored to better evaluate the prognosis of patients.

Objective:Varicocele(VC)induces male infertility by mediating changes in the testicular microenvironment,in which testicular hypoxia and high-pressure are important pathological conditions. This study aims to compare the mouse spermatogenesis(GC-2spd)cells and Sertoli(TM4)cells of mouse testis after hypoxic modeling and hypoxic and high-pressure combined modeling, and to explore the feasibility of establishing a hypoxic and high-pressure combined cell model. Methods:On the basis of cell hypoxia induced by CoCl2,the complex model of testicular cell hypoxia and high pressure was constructed by changing the osmotic pressure of GC-2 and TM4 cell medium with a high concentration of NaCl solution. After selecting the intervention concentration of CoCl2 by MTT test and detecting the expression level of HIF-1α for the determination of the optimal osmotic pressure conditions of the cell model, the cells were divided into normal group, hypoxia model group and composite model group. And the levels of OS,programmed cell death, inflammatory factors, and the expression levels of pyroptosis-related proteins were compared between the normal group and the groups with different modeling methods. Results:The optimal intervention concentration of CoCl2 in GC-2 and TM4 cells was 150 and 250μmol/L,respectively,and the expression of HIF-1α was the highest in both cells under osmotic pressure of 500 mOsmol/kg(P<0.05).Compared with the normal group, the SOD levels of GC-2 and TM4 cells decreased(all P<0.05),CAT level decreased(all P<0.05),and MDA level increased(all P<0.01),and the OS level of GC-2 and TM4 cells was more obvious than that of the hy-poxia model group(all P<0.05).Compared with the normal group, apoptosis occurred in GC-2 and TM4 cells after composite model-ing(all P<0.05).Compared with the normal group, the mRNA expressions of IL-1β,IL-18,TNF-α and COX-2 in GC-2 and TM4 cells significantly increased(P<0.01)and higher than those in hypoxia model group(P<0.05)and induced pyroptosis(P<0.01).The expression level of GSDMD increased(P<0.05).Conclusion:The cell model with hypoxia and high pressure com-bined modeling can not only induce oxidative stress and apoptosis of cells better than that with hypoxia alone, but also further cause in-flammatory response damage and pyroptosis, which simulates the changes of testis microenvironment mediated by hypoxia and high pressure combined conditions in VC.This cell model can be used for studying the pathogenesis of VC-associated male infertility,evalu-ating drug efficacy, and exploring pharmacological mechanisms.

Purpose To investigate the clinicopathological features, diagnosis, differential diagnosis, treatment, and prognosis of intrathyroid thymic carcinoma(ITC).Methods The clinical manifestations, histological features and immunophenotypes of 11 patients with ITC were retrospectively analyzed.Immunohistochemistry and Sanger sequencing were used for protein and gene detection. The relevant literature is retrieved and the data of this paper is analyzed sta-tistically.Results Among the 11 patients,6 were female and 5 was male, aged from 35 to 75 years old.Tumor inva-sion into peripheral thyroid tissue was observed in 2 cases.Immunohistochemical analysis revealed positive expression for CD5,CD117,p63,p40,CK5/6,CK17 and CKpan, with focal positivity for Syn observed in only 1 case.Genetic testing:1 patient had mutations in the TERT promoter gene(C.-228 C>T).Conclusion ITC is an extremely rare,low-grade malignant tumor.Due to its morphological similarity to other highly malignant thyroid tumors,it is prone to misdiagnosis.Immunohistochemistry and genetic testing play a crucial role in accurate differential diagnosis.

This study investigated the prevalence and potential mechanisms of gentamicin heterore-sistance in Streptococcus isolates from dairy cows.In this study,A total of 39 Streptococcus isola-ted from raw milk were collected, and the minimum inhibitory concentration(MIC)of gentamicin and other drugs on the isolates was determined by micro broth dilution method, and the K-B paper diffusion method, colony analysis profile(PAP),and resistance stability test were used to investigate the heteroresistance characteristics of Streptococcus, and the mechanism of heteroresis-tance was analyzed based on whole genome sequencing and resequencing.Seven suspected heterore-sistance strains were identified by K-B paper diffusion method, accounting for 17.95%(7/39)of the total number of suspected strains.PAP confirmed that the MIC to MNIC ratio of L147,L108 and L174 was greater than 8,and the frequency of resistant subgroups ranged from 1.38×10-5 to 8.18 × 10-5,which was greater than 1 × 10-7,confirming that they were heteroresistance strains.Resistance stability tests revealed that the resistant subpopulations of all three strains were not stably inherited.Whole-genome sequencing revealed mutations in the ribosomal target genes of aminoglycoside antibiotics, rsmA,rsmB and rsmE,compared with the reference genome, which may lead to heteroresistance to gentamicin in Streptococcus. The occurrence of heteroresistance of Streptococcus to gentamicin is high in dairy sources,so more attention should be paid to the occur-rence of heteroresistance when using gentamicin for clinical treatment.
