1.Complications Following SEMS Insertion for Hemobilia Following Impacted Biliary Basket Removal

There have been rare reports of damage to the blood vessels surrounding the bile duct during the gallstone removal process, causing hemobilia. In cases of severe hemobilia with unstable vital signs, insertion of a fully covered self-expanding metallic stent (SEMS) may be considered as salvage therapy. This case reports a patient with common bile duct stones and acute cholangitis who developed massive hemobilia during attempts to resolve basket device incarceration by performing mechanical lithotripsy. Endoscopic hemostasis was successfully achieved by insertion of a SEMS. However, during follow-up, the stent distally dislocated, resulting in rebleeding with a pseudoaneurysm in the surrounding blood vessels. Ultimately, hemostasis failed despite vascular intervention and additional hemostatic procedures, leading to death. Even if a stent is inserted in an appropriate location, hemostasis may not be achieved well, and if the stent migrates, hemobilia may worsen, so close monitoring is required to determine whether bleeding occurs again.
2.Radiologic evolution of biopsy-proven acute interstitial nephritis: a multimodal imaging case report

This report presents radiologic changes after clinical improvement in a patient with acute interstitial nephritis (AIN). A 45-year-old female patient was referred for decreased renal function. Eight months prior, she had undergone hysterectomy and received chemotherapy. At the start of chemotherapy, her baseline creatinine level was 0.55 mg/dL, which rose to 1.46 mg/dL. Multiple imaging modalities performed when decreased renal function was observed revealed bilateral renal enlargement with increased cortical attenuation on computed tomography (CT), cortical hyperechogenicity on ultrasonography, and diffusion restriction in the renal cortices on magnetic resonance imaging. A renal biopsy was performed, and AIN was diagnosed. Follow-up laboratory tests showed that kidney function had improved to normal levels, and CT at that time showed a reduction in the size of both kidneys. Radiologic changes can serve as clues for the diagnosis of AIN. This is the first report to confirm radiological changes after the clinical improvement of AIN, thereby providing novel information about the course of AIN.
3.Dietary isoflavone intake among breast cancer survivors and cancer-free women

BACKGROUND/OBJECTIVES:
Isoflavones are estrogen-like compounds found in plants and their health effects remain equivocal. We investigated dietary isoflavone intake and its associated factors in Korean breast cancer survivors, with a comparison to cancer-free women.
SUBJECTS/METHODS:
The usual dietary intake of breast cancer survivors (n = 981, mean age 52 yrs) in 9 hospitals between 2012 and 2019 was assessed using 3-day food records or food frequency questionnaires (FFQs). They were age-matched to 2,943 cancer-free women who completed FFQs as part of a nationwide study conducted between 2012 and 2016. We used the flavonoid database of common Korean foods and the Phenol-Explorer database to estimate isoflavone intake. The contribution of each food or food group to the total isoflavone intake was calculated. The adjusted least-squares means of dietary isoflavone intake according to lifestyle and clinical factors were calculated using generalized linear models.
RESULTS:
Breast cancer survivors had a higher mean dietary isoflavone intake (23.59 mg/day) than cancer-free women (17.81 mg/day). Major food sources, including tofu, soybeans, and doenjang, contributed to over 70% of the isoflavone intake in both groups. When we estimated dietary isoflavone intake according to lifestyle characteristics, isoflavone intake increased with higher scores of adherence to the American Cancer Society dietary guidelines but decreased with increasing body mass index in both groups. Among cancer-free women, dietary isoflavone intake was higher among those who had never smoked and among dietary supplement users. Among breast cancer survivors, dietary isoflavone intakes did not vary with clinical characteristics, including time since surgery and estrogen receptor status.
CONCLUSION
Breast cancer survivors were more likely to consume isoflavones than agematched cancer-free women. Dietary isoflavone intake was associated with healthy lifestyle characteristics in women both with and without breast cancer. Further research is needed to understand the role of the higher isoflavone intake among breast cancer survivors compared to cancer-free women on their prognosis.
4.Food insecurity and its associated characteristics of the elderly in Seoul:analysis of the data from the Seoul Food Survey 2023

BACKGROUND/OBJECTIVES:
This study examined the socio-demographic and dietary characteristics of the elderly in Seoul according to their food insecurity status and analyzed the characteristics associated with food insecurity.
SUBJECTS/METHODS:
The data from 513 elderly individuals aged 65 or older were analyzed using the raw data from the Seoul Food Survey 2023. The subjects were classified into food-secure or food-insecure groups. The socio-demographic and dietary characteristics of each group were analyzed. Logistic regression analysis was conducted to identify the characteristics associated with food insecurity among the elderly in Seoul.
RESULTS:
The proportion of the food-insecure group was 56.9% among the total elderly in Seoul. The main reasons for experiencing food insecurity were environmental (38.0%), followed by physical (37.5%) and financial (24.5%). The food-insecure group had a higher frequency of skipping breakfast and eating alone than the food-secure group while having lower dietary literacy and food environment satisfaction. Logistic regression analysis showed that the elderly living alone were approximately four times (odds ratio [OR], 3.59; 95% confidence interval [CI], 1.52–8.51) more likely to suffer food insecurity than those living with others. Higher digital food literacy (OR, 0.63; 95% CI, 0.41–0.97) and food environment satisfaction (OR, 0.38; 95% CI, 0.20–0.74) were associated with a lower likelihood of food insecurity.
CONCLUSION
These results suggest that policies aimed at improving food security for the elderly in Seoul should prioritize those living alone. Educational programs designed to enhance the ability to purchase online food, use self-service kiosks in restaurants, and use food delivery apps would effectively decrease food insecurity among the elderly in Seoul.Furthermore, policies aimed at improving the food environment may also help reduce food insecurity among this population.
5.Comparison of Suicide Completion Rate in Korean Students Pre- and Post-COVID-19 Pandemics

Objective:
This study compared the incidence of suicide cases among Korean students before and after the coronavirus disease-2019 (COVID-19) pandemic.
Methods:
Aggregated case reports of all known suicide victims attending elementary, middle, and high school in South Korea from 2017 to 2022 were analyzed. These reports, compiled by teachers under the directive of the South Korean Department of Education, surveyed the circumstances surrounding each suicide and identified associated risk factors posthumously.
Results:
The completed suicide rate was 2.37 per 100,000 students pre-COVID-19 (2017–2019), significantly increasing to 3.37 per 100,000 students post-COVID-19 (2020–2022; odds ratio [OR] 1.42, p<0.001). The rate began to rise approximately 6 months into the pandemic and continued to worsen throughout 2021 and 2022. Fair attendance significantly increased in 2020 (74.83%, p=0.003) during the initial stage of school closure measures compared to 2019 (58.27%) among students prior to suicide completion. However, as social distancing measures continued, “absence due to medical reasons” showed a significant increase in 2022 (23.56%) compared to 2019 (13.67%, p=0.025) and 2020 (9.52%, p<0.001) among these students. Among those who completed suicide, teachers reported increased signs of depression and anxiety posthumously compared to pre-COVID-19 periods.
Conclusion
The incidence of completed suicide among students was significantly higher in the post-COVID-19 years compared to pre-COVID-19. Attendance records and teachers’ reports indicated a deterioration in mental health among these students before suicide completion, highlighting the need to consider mental health impacts when implementing future quarantine policies.
6.Development and Validation of an Online Intervention Program to Augment Psychological Resilience: A Randomized Waitlist-Controlled Trial

Objective:
Resilience is the ability to bounce back from stressful events using individual and environmental resources. The online intervention program, MoVemEnT, which includes mindfulness training and emotion regulation, was developed to enhance resilience among young adults.
Methods:
Persons without current medical or psychiatric diseases, aged 20–40 years, were recruited. Ninety-six participants were randomly assigned to either the intervention group or a waiting list control (WLC) group, with 45 participants per group completing the program.
Results:
Results demonstrated significant improvements in the intervention group in measures of emotion regulation (Difficulties in Emotion Regulation Scale, DERS) and clinical symptoms of depression (Center for Epidemiological Studies-Depression Scale, CES-D) and anxiety (Generalized Anxiety Disorder-7) compared to the WLC group (all p<0.05). Correlation analyses indicated that improvements in DERS scores were associated with enhanced resilience, as measured by the Korean Resilience Quotient-53 (KRQ-53) and reduced CES-D scores. Reduction in DERS scores can reduce depressive symptoms (CES-D) through direct and indirect pathways via an increase in KRQ-53, according to mediation analyses.
Conclusion
Our findings reported positive effects of the MoVemEnT program, which includes short videos and brief homework assignments, on resilience in young adults. This study suggests that online intervention programs could offer an opportunity to simultaneously improve general mental health among a large population and may help reduce the burden of face-to-face psychological interventions.
7.Factors Associated with Subjective Health Status among People with Spinal Cord Injury: An Application of the International Classification of Functioning, Disability and Health Model

Purpose:
This study aimed to identify factors influencing subjective health status among individuals with spinal cord injury (SCI) using the International Classification of Functioning, Disability and Health (ICF) model.
Methods:
This study analyzed secondary data from the 2021 Survey on the Status and Needs of Individuals with SCI conducted by the Korea Spinal Cord Injury Association. A hierarchical multiple regression analysis was conducted in four steps. Independent variables included sociodemographic factors, satisfaction with medical services (environmental factor), the need for weight control, the need for nutritional education, the need for dietary management, and disability acceptance (personal factors) along with exercise and activity of daily living (activity and participation factors). These factors were added sequentially at each step based on the ICF model.
Results:
A total of 513 participants were analyzed in this study, with an average subjective health status of 5.51±2.39. Higher satisfaction with medical services (β=.27, p<.001), frequent exercise (β=.19, p=.008), and a greater level of activities of daily living (β=.02, p<.001) were significantly associated with improved subjective health status. Conversely, a lower need for nutritional education (β=-.25, p=.042) was negatively associated.
Conclusion
This study highlighted satisfaction with medical services, nutrition education needs, exercise frequency, and activities of daily living as important factors. These findings provide insight into how the health status of individuals with SCI can be enhanced through targeted interventions and support from healthcare providers.
8.Deformation analysis of 3D-printed three-unit fixed partial dentures according to various cleaning strategies after DLP 3D printing:a geometric library matching approach

PURPOSE:
Three-dimensional (3D) printing must go through a post-processing procedure. This study aimed to evaluate the effects of different cleaning methods on 3D printed three-unit fixed partial dentures using 3D analysis.
MATERIALS AND METHODS:
A three-unit fixed partial denture was designed and printed using two different printers. The evaluation was taken regarding cleaning method with ethanol (ETH) or isopropanol (IPA), and combining ultrasonic (US) technique for the following groups: group 1, ETH; group 2, ETH+US; group 3, IPA; group 4, IPA+US; and group 5, US. The 3D evaluation was conducted by calculating the mean distance between two specific points and analyzing various angular deviations, utilizing the geometry-embedded library. In addition, visual analysis was performed by creating sectional planes in different points of view. Normality test and independent t-test were performed to compare results between the two printers used in this study. In addition, one-way ANOVA was performed to compare cleaning method groups.
RESULTS:
Mean distance results showed statistical differences between printers for groups IPA, US, and IPA+US, whereas no significant differences were found among cleaning methods. Angular measurements were taken at the bucco-lingual, proximal, and occlusal planes to assess deviations in the internal surfaces. Overall results showed a tendency of shrinkage of the printed three-unit fixed partial denture.
CONCLUSION
Ultrasonic treatment may impact the washing efficiency when used with other cleaning solutions regarding angular and longitudinal deformation. Adequate selection of post-processing methods is crucial to decrease morphological deformities in 3D printed materials.
10.Novel non-invasive and quantitative assessment of the renal function of transplanted kidneys using Doppler ultrasonography with the vascular index of superb microvascular imaging

Purpose:
This study assessed the reproducibility and clinical value of the vascular index (VI), derived from superb microvascular imaging (SMI) using Doppler ultrasonography, for evaluating renal function in transplanted kidneys.
Methods:
This retrospective study included 63 renal transplant patients who underwent grayscale and Doppler ultrasonography with SMI from January 2022 to February 2023. The VI of the transplanted kidneys was measured using three methods (VIbox, VIF1, VIF2). The VI was compared across chronic kidney disease (CKD) groups categorized by estimated glomerular filtration rate (eGFR) and Kidney Disease: Improving Global Outcomes (KDIGO) CKD risk groups based on eGFR and albuminuria. The correlation between VI and renal function was evaluated. Univariate and multivariate linear regression analyses were used to identify predictors of eGFR.
Results:
Significant differences in VI were observed among CKD groups based on eGFR (VIbox, P=0.001; VIF1, P<0.001; VIF2, P<0.001) and KDIGO CKD groups based on eGFR and albuminuria (VIbox, P=0.039; VIF1, P=0.001; VIF2, P<0.001). VIF1 and VIF2 demonstrated moderate/high correlations with eGFR (r=0.627, P<0.001 and r=0.657, P<0.001, respectively) and serum creatinine (r=-0.626, P<0.001 and r=-0.649, P<0.001, respectively). VIbox moderately correlated with eGFR (r=0.445, P<0.001). Multivariate regression identified the urine albumincreatinine ratio (ACR) (adjusted odds ratio [aOR], 1.122; 95% confidence interval [CI], -0.007 to, 0.000; P=0.030) and VIF2 (aOR, 1.114; 95% CI, 0.466 to 1.235; P<0.001) were independently associated with eGFR.
Conclusion
The VI measured by drawing a region of interest along the border of the transplanted kidney in SMI (VIF2) is highly reproducible and correlates well with eGFR. Both VIF2 and ACR independently predict eGFR.
