1.Clinical trial of thymopeptide enteric-coated capsules combined with human interferon α2b vaginal effervescent tablets in the treatment of patients with high-risk HPV infection-related cervical lesions

Objective To observe the therapeutic effect of thymopeptide enteric-coated capsules combined with human interferon α2b vaginal effervescent tablets in the treatment of patients with high-risk human papillomavirus(HPV)infection-related cervical lesions and the impact on T lymphocyte subsets.Methods The patients with high-risk HPV infection-related cervical lesions were divided into the treatment group and the control group according to the cohort method.The control group was given human interferon α2b vaginal effervescent tablets 5 × 105 U,once daily,for 2 months.On this basis,the treatment group was treated with thymopeptide enteric-coated capsules at a dose of 30 mg·time-1,three times a day,for 2 months.Therapeutic effects,cervical vaginal microecology indicators[pH values of vaginal secretions,Nugent scores and white blood cell count],T lymphocyte subsets,cytokines[interleukin-4(IL-4),IL-6,IL-10,tumor necrosis factor-α(TNF-α)and interferon-γ(IFN-γ)]were compared between the two groups,and the safety were evaluated.Results Seventy cases in the treatment group and 72 cases in the control group.After treatment,the total effective rate of the treatment group was 94.29%(66 cases/70 cases),which was higher than the control group's 80.56%(58 cases/72 cases),the differences were statistically significant(P<0.05).After treatment,pH values of vaginal secretions in treatment group and control group were 4.26±0.19 and 4.38±0.17;Nugent scores were(2.53±0.36)and(2.79±0.40)scores;Blood cell count were 5.12±1.14 and 6.03±1.21;CD3+were(69.32±8.10)%and(66.54±7.03)%;CD4+were(37.25±5.03)%and(34.67±4.82)%;CD4+/CD8+were 1.50±0.35 and 1.42±0.33;serum IL-4 levels were(7.49±1.64)and(9.20±2.18)pg·mL-1;serum IL-6 levels were(20.95±5.03)and(26.64±6.22)pg·mL-1;serum IL-10 levels were(11.72±2.46)and(13.43±2.97)pg·mL-1;serum TNF-α levels were(16.48±3.75)and(13.25±2.03)pg·mL-1;serum IFN-γ levels were(13.35±2.11)and(10.93±2.86)pg·mL-1;the differences were statistically significant(all P<0.05).Adverse drug reactions in the treatment group included nausea,fever,vaginal itching and pain;which in the control group included vaginal itching and pain.The total incidence rates of adverse reactions in the treatment group and the control group were 8.57%(6 cases/70 cases)and 5.56%(4 cases/72 cases),without statistically significant difference(P>0.05).Conclusion Thymopeptide enteric-coated capsules combined with human interferon α2b vaginal effervescent tablets can improve immune function,reduce inflammatory reactions,and improve cervical and vaginal micro-ecological environment in patients with high-risk HPV infection-related cervical lesions.

3.Setup accuracy of two reference surfaces in nasopharyngeal carcinoma surface-guided radiotherapy

Objective To compare the setup accuracies of the surface-guided radiation therapy for nasopharyngeal carcinoma based on volume rendering technique(VRT)reference surface and digital imaging and communications in medicine(DICOM)reference surface.Methods Twenty patients with nasopharyngeal carcinoma who received radiotherapy using the customized AccuCushion with a thermoplastic fixation mask were selected.After the first cone beam CT(CBCT)-based image-guided radiotherapy of each patient was completed,the thermoplastic mask was removed to obtain the VRT reference surface based on optical surface monitoring system(OSMS),during which the patient position was maintained through instructions;the DICOM reference surface was got based on the body surface contour data extracted from the patient's localized CT images.After the completion of the 5th,10th,and 15th CBCT image-guided radiotherapy treatments,the patients'6 dimensions of setup errors were measured based on the 2 reference surfaces,respectively.Dimensional correlation analyses were carried out on the setup errors measured based on the 2 reference surfaces separately using Python's Stats tool to test the independence between the setup errors in each dimension.After independence test,Wilcoxon rank sum test was performed on the data of the setup errors based on each dimension measured with the 2 reference surfaces.Results The two sets of setup errors were independent of each other in all dimensions.The errors based on VRT reference surface were lower than those based on DICOM reference surface in 6 dimensions,while the difference between the 6-D errors of the two groups did not gradually become significant as the treatment progressed.Conclusion In the SGRT of nasopharyngeal carcinoma using a customized cushion combined with a thermoplastic mask,the setup accuracy and consistency based on VRT reference surface are both higher than those based on DICOM reference surface.

Tibetan medicine is an essential part of Chinese medicine and has unique theoretical experience and therapeutic advantages. According to the development principle of inheriting the essence, sticking to the truth, and keeping innovative, the supervision department should give clear and reasonable guidance considering the characteristics of Tibetan medicine, establish a standard system for quality control, clinical verification and evaluation, and accelerate the research and commercialization of new drugs. In view of the needs of drug supply-side reform and the current situation of Tibetan medicine and new pharmaceutical research, we ponder and provide suggestions on the confusion faced by the current supervision of Tibetan drug registration, hoping to contribute to the supervision strategy of Tibetan drug registration and the high-quality development of Tibetan medicine industry.

5.Expression and regulation of XBP1 in mouse uterus during early pregnancy.

The transcription factor X-box binding protein-1 (XBP1) plays a key role in unfolded protein reaction. This study was aimed to investigate the expression pattern and regulation of XBP1 in the mouse uterus during early pregnancy. The methods of immunohistochemistry (IHC) and real time quantitative RT-PCR were used to test XBP1 expression in early pregnancy, artificial decidualization, oestrous cycle and hormone-regulated mouse models. The results showed that XBP1 was spatiotemporally expressed in mouse uterus during early pregnancy. The XBP1 protein was mainly detected in the luminal and glandular epithelia on days 1-4 of pregnancy, and was strongly detected in the decidual area on days 5-8 of pregnancy. Similarly, XBP1 expression was also mainly expressed in decidual cells following artificial decidualization. During the oestrous cycle, Xbp1, Xbp1u, and Xbp1s mRNA was predominantly present in proestrus. In the ovariectomized uterus, the expression of XBP1 in luminal and glandular epithelia was up-regulated after estrogen treatment. These results suggest that XBP1 is associated with embryo implantation and decidualization during early pregnancy in mice, and the expression of XBP1 in luminal and glandular epithelia may be regulated by estrogen.

6.A multicenter retrospective study on the real-world outcomes of autologous vs. allogeneic hematopoietic stem cell transplantation for peripheral T-cell lymphoma in China.

BACKGROUND:
There were few studies on real-world data about autologous hematopoietic stem cell transplantation (auto-HSCT) or allogeneic HSCT (allo-HSCT) in peripheral T-cell lymphoma (PTCL). This study aimed to investigate the clinical outcomes of patients who received auto-HSCT or allo-HSCT in China.
METHODS:
From July 2007 to June 2017, a total of 128 patients who received auto-HSCT (n = 72) or allo-HSCT (n = 56) at eight medical centers across China were included in this study. We retrospectively collected their demographic and clinical data and compared the clinical outcomes between groups.
RESULTS:
Patients receiving allo-HSCT were more likely to be diagnosed with stage III or IV disease (95% vs. 82%, P = 0.027), bone marrow involvement (42% vs. 15%, P = 0.001), chemotherapy-resistant disease (41% vs. 8%, P = 0.001), and progression disease (32% vs. 4%, P < 0.001) at transplantation than those receiving auto-HSCT. With a median follow-up of 30 (2-143) months, 3-year overall survival (OS) and progression-free survival (PFS) in the auto-HSCT group were 70%(48/63) and 59%(42/63), respectively. Three-year OS and PFS for allo-HSCT recipients were 46%(27/54) and 44%(29/54), respectively. There was no difference in relapse rate (34%[17/63] in auto-HSCT vs. 29%[15/54] in allo-HSCT, P = 0.840). Three-year non-relapse mortality rate in auto-HSCT recipients was 6%(4/63) compared with 27%(14/54) for allo-HSCT recipients (P = 0.004). Subanalyses showed that patients with lower prognostic index scores for PTCL (PIT) who received auto-HSCT in an upfront setting had a better outcome than patients with higher PIT scores (3-year OS: 85% vs. 40%, P = 0.003). Patients with complete remission (CR) undergoing auto-HSCT had better survival (3-year OS: 88% vs. 48% in allo-HSCT, P = 0.008). For patients beyond CR, the outcome of patients who received allo-HSCT was similar to that in the atuo-HSCT group (3-year OS: 51% vs. 46%, P = 0.300).
CONCLUSIONS
Our study provided real-world data about auto-HSCT and allo-HSCT in China. Auto-HSCT seemed to be associated with better survival for patients in good condition (lower PIT score and/or better disease control). For patients possessing unfavorable characteristics, the survival of patients receiving allo-HSCT group was similar to that in the auto-HSCT group.

Objective: To explore the efficacy and prognostic factors of hematopoietic stem cell transplantation (HSCT) for the treatment of patients with anaplastic large cell lymphoma (ALCL) . Methods: The clinical records of 33 ALCL patients after HSCT were collected and analyzed retrospectively to evaluate the rates of overall survival (OS) and recurrence after autologous (auto-HSCT) and allogeneic HSCT (allo-HSCT) and the factors influencing prognosis. Results: The median-age of this cohort of 33 ALCL cases at diagnosis was 31 (12-57) years old with a male/female ratio of 23/10, 24 cases (72.7%) were ALK(+) and 9 ones (27.3%) ALK(-). Of them, 25 patients (19 ALK(+) and 6 ALK(-)) underwent auto-HSCT and 8 cases (5 ALK(+) and 3ALK(-)) allo-HSCT with a median follow-up of 18.7 (4.0-150.0) months. Disease states before HSCT were as follows: only 6 patients achieved CR status and received auto-HSCT, 16 patients achieved PR (14 cases by auto-HSCT and 2 ones allo-HSCT) , the rest 11 cases were refractory/relapse (5 cases by auto-HSCT and 6 ones allo-HSCT) . There were 7 cases died of disease progression (5 after auto-HSCT and 2 allo-HSCT) and 5 cases treatment-related mortality (TRM) (2 after auto-HSCT and 3 allo-HSCT) , TRM of two groups were 8.0% and 37.5%, respectively. Both the median progression-free survival (PFS) and OS were 15 months after auto-HSCT, the median PFS and OS after allo-HSCT were 3.7 (1.0-90.0) and 4.6 (1.0-90.0) months, respectively. There was no statistically significant difference in terms of survival curves between the two groups (OS and PFS, P=0.247 and P=0.317) . The 2-year OS rates in auto-HSCT and allo-HSCT groups were 72% and 50%, respectively. The 5-year OS rates in auto-HSCT and allo-HSCT groups were 36% and 25%, respectively. Conclusion: ALCL treated by chemotherapy produced high rates of overall and complete responses. Chemotherapy followed by auto-HSCT remained to be good choice for patients with poor prognostic factors. High-risk patients should be considered more beneficial from allo-HSCT.

Objective The aim of this study was to explore the regulatory effects and mechanism of autophagy on epithelial-to-mesenchymal transition(EMT) in idiopathic pulmonary fibrosis(IPF). Methods The experiment was divided into two group: control group and experimental group(IPF group, autophagy induction group, autophagy inhibition group). A549 cells were cultivated by the conventional method in control group. The A549 cells of the experimental group were induced by TGF-β1(5 ng/mL). Then, no further treatment was given to IPF group. Rapamycin(10 μg/L) or 3-Methyladenine(10mmol/L) was given to autophagy induction group or autophagy inhibition group respectively. The hydroxyproline content of lung tissue was measured, and the mRNA and protein levels of α-SMA, LC3-Ⅱ or LC3-Ⅱ/LC3-Ⅰ, Beclin1, E-cadherin and Vimentin were tested by Realtime PCR and Western blot. 10.The neural encoding of continuous speech - recent advances in EEG and MEG studies.

Speech comprehension is a central cognitive function of the human brain. In cognitive neuroscience, a fundamental question is to understand how neural activity encodes the acoustic properties of a continuous speech stream and resolves multiple levels of linguistic structures at the same time. This paper reviews the recently developed research paradigms that employ electroencephalography (EEG) or magnetoencephalography (MEG) to capture neural tracking of acoustic features or linguistic structures of continuous speech. This review focuses on two questions in speech processing: (1) The encoding of continuously changing acoustic properties of speech; (2) The representation of hierarchical linguistic units, including syllables, words, phrases and sentences. Studies have found that the low-frequency cortical activity tracks the speech envelope. In addition, the cortical activities on different time scales track multiple levels of linguistic units and constitute a representation of hierarchically organized linguistic units. The article reviewed these studies, which provided new insights into the processes of continuous speech in the human brain.
