1.ABO*Ael03/O Genotype with ABO Discrepancy: The First Case in Korea.

The Ael subgroup expresses the least amount of A antigens and could only be detected by performing the adsorption-elution test. The frequency of the Ael subgroup is about 0.001% in Koreans, and the Ael02 allele, which originates from A102, is the most frequently identified allele in the Korean population. We report a Korean family with the Ael03 allele identified by molecular genetic analysis. To the best of our knowledge, this is the first such report in Korea to date.

2.Role of Acid and Weakly Acidic Reflux in Gastroesophageal Reflux Disease Off Proton Pump Inhibitor Therapy.

BACKGROUND/AIMS: Available data about reflux patterns and symptom determinants in the gastroesophageal reflux disease (GERD) subtypes off proton pump inhibitor (PPI) therapy are lacking. We aimed to evaluate reflux patterns and determinants of symptom perception in patients with GERD off PPI therapy by impedance-pH monitoring. METHODS: We retrospectively reviewed the impedance-pH data in patients diagnosed as GERD based on results of impedance-pH monitoring, endoscopy and/or typical symptoms. The characteristics of acid and weakly acidic reflux were evaluated. Symptomatic and asymptomatic reflux were compared according to GERD subtypes and individual symptoms. RESULTS: Forty-two patients (22 males, mean age 46 years) were diagnosed as GERD (17 erosive reflux disease, 9 pH(+) non-erosive reflux disease [NERD], 9 hypersensitive esophagus and 7 symptomatic NERD). A total of 1,725 reflux episodes were detected (855 acid [50%], 857 weakly acidic [50%] and 13 weakly alkaline reflux [< 1%]). Acid reflux was more frequently symptomatic and bolus clearance was longer compared with weakly acidic reflux. In terms of globus, weakly acidic reflux was more symptomatic. Symptomatic reflux was more frequently acid and mixed reflux; these associations were more pronounced in erosive reflux disease and symptomatic NERD. The perception of regurgitation was related to acid reflux, while that of globus was more related to weakly acidic reflux. CONCLUSIONS: In patients not taking PPI, acid reflux was more frequently symptomatic and had longer bolus clearance. Symptomatic reflux was more frequently acid and mixed type; however, weakly acidic reflux was associated more with globus. These data suggest a role for impedance-pH data in the evaluation of globus.

3.Characteristics of Intron 6 Nucleotide Sequences of cis-AB in Koreans.

BACKGROUND: The cis-AB is a very rare phenotype in the ABO blood group system. It corresponds to a special ABO allele that encodes glycosyltransferase that is capable of synthesizing both A and B antigens. Until now, the exon 6 and 7 gene sequences of cis-AB alleles are well known. In this study, we report on the intron 6 sequence structure of the cis-AB allele. METHODS: Standard serologic tests for the ABO blood group phenotypes were performed in four cis-AB samples. Allele-separation by cloning and subsequent sequencing was carried out. RESULTS: The results showed that intron 6 of cis-AB is almost identical to the A101 allele except for three single nucleotide polymorphisms at nucleotide positions 163, 179 and 662, where the nucleotides of the A101 replace those of B101. CONCLUSION: The intron 6 sequences of cis-AB in Koreans have both A101 and B101 blood group sequences.

4.Predictive Factors of Level II Lymph Node Metastasis in N1b Papillary Thyroid Carcinoma Patients.

BACKGROUND AND OBJECTIVES: Cervical lymph node (LN) metastases are common in papillary thyroid carcinoma (PTC) and cervical neck dissection (ND) is frequently performed to improve regional control of the disease. However, there is a controversy as to the extent of lateral cervical ND. In the present study, we examined the diagnostic accuracy of ultrasonography (US) or CT and the predictive factors of level II LN metastases in PTC patients with clinically positive lateral neck nodes. SUBJECTS AND METHOD: We retrospectively analyzed 78 patients who underwent thyroidectomy and lateral ND including level II LN between August 1998 to June 2008. To identify predictive factors of cervical node metastases to level II, diverse factors were analyzed. RESULTS: The most common site of metastasis was level IV (83.3% of cases). The accuracy of diagnostic imaging in the detection of level II metastasis revealed sensitivity of 89.4%, and specificity of 93.5%. The results of the univariate analysis showed that the presence of level II metastases was significantly associated with the location of primary tumor (p<0.001), number of metastatic LN (p=0.001), and great size of metastatic LN (p=0.010). In addition, the multivariate analysis showed that the location of primary tumor and number of metastatic LN were an independent risk factor for the presence of level II metastasis. CONCLUSION: Preoperative imaging was accurate for detecting the presence of level II LN metastasis. Patients with upper lobe tumor and multiple metastatic LN have a higher risk of harboring metastatic disease at the level II. We suggest that routine dissection of the level II LN may not be necessary for patients with no evidence of diagnostic imaging and predictive factors of level II LN metastasis.

5.Preimplantation Development and Apoptosis of Mouse Embryos in the Medium Containing Extracellualr Matrix.

OBJECTIVE: To verify the effect of Matrigel, a ECM complex from Engelbreth-Holm-Swarm(EHS) mouse sarcoma on the preimplantation development and apoptosis of mouse fertilized eggs. METHOD: Late Pronucleus stage eggs were cultured through the blastocyst stage in the presence of Matrigel (0.5%, v/v). Characteristics of apoptosis and cell number assessed by Hoecst staining and TUNEL labeling at the blastocyst stage, respectively. RESULTS: Morphological development, number of cells per embryo was significantly increased but rate and number of TUNEL positive nuclei of the embryo were decreased in the presence of Matrigel. CONCLUSION: This result suggested that at low concentration of Matrigel improves both viability and morphological development in the preimplantation mouse embryos.

6.A Clinical Study of Foreign Body Aspiration in Children in Suwon-Kyungki Province.

PURPOSE: Foreign bodies in the airway of infant and children require prompt diagnosis and removal. To evaluate the pattern of clinical characteristics and disease course, we reviewed and analyzed a 6-year experience with pediatric airway foreign bodies in children. METHODS: We retrospectively reviewed the medical records of pediatric foreign body aspiration managed by pediatric and otorhinolaryngologic departments of Ajou University Hospital from November 1994 to October 2000. Nineteen cases of aspiration were collected and we analyzed their age, sex, symptoms, duration between onset of symptoms and diagnosis, initial simple radiographic findings, matierials of aspirations, anatomic location of foreign body, length of hospital day, and complications. RESULTS: The mean age was 2.7 years and male-to-female ratio of 1.8 : 1. In 11 cases, choking episode was identified by parents or patients. The correct diagnosis was made within first 24 hours of aspiration in 4 patients; while in 15 cases, the proper diagnosis was done after 24 hours. The most common presenting symptoms were cough (73.6%) and decreased breath sounds (57.9%) and the most common initial finding of simple radiographs was emphysema, presented in 68.4% of patients. Vegetable substances, particularly peanuts were the most common material of identified foreign body. Nine foreign bodies were in the right bronchus, 8 in the left bronchus and 2 in the subglottic area. The mean duration of admissions were 3.75 days in pateints who were diagnosed within 24 hours, however, 8.61 days in those with a delayed diagnosis (P=0.012). CONCLUSION: Taken together, the majority of accidental foreign body aspiration in airway was found under 3-year of age. Pediatricians and parents should be conscious to check for history of foreign body aspiration who have suspicious clinical features of aspiration, for early diagnosis and proper management without complications and long term morbidity.

7.Expression of Oscillin in Mouse Testis.

PURPOSE: To verify the expression of the egg activator oscillin in mouse testis and adult organs. MATERIALS AND METHODS: Genomic PCR using primers for oscillin was conducted to confirm that the PCR product was derived from cDNA. Total RNA isolated from developing, immature, and mature testis was subjected to RT-PCR and restriction analysis. In situ hybridization of adult testis was performed to localize the oscillin transcript using cRNA probe. RESULTS: Genomic PCR using the primer for RT-PCR revealed no amplification product, suggesting that the oscillin gene consists of at least two exons. The RT-PCR product of oscillin mRAN was detected in testis beginning 2 weeks after birth. Oscillin mRAN was detected in both germ and somatic cells such as Sertoli and Leydig cells by in situ hybridization. The testis showed al high level of oscillin mRAN compared with other adult organs. CONCLUSION: Oscillin is not a testis-specific transcript and therefore may have another function intracellularly as well ad serving as a trigger for egg activation.

8.Zosteriform Atrophoderma of pasini and Pierini.

A 13-year-old girl developed rice to nut-sized, bluish to slate-gray colored, depressed patches in a zosteriform distribution along the right side of her upper back and arm without any symptoms. Microscopically, the lesions showed a normal-appearing epidermis, dermis, and subcutaneous fat tissue except for a thinning of collagen fibers and widening of intervals between them in the lower dermis. There have been no changes to the skin lesions during the last 2 years.

9.The Effect of Free Radical Scavenger, Superoxide Dismutase ( SOD ) in the Early Embryo Culture of Mice.

It is well known that developmental delay or arrest occurs before implantation in mamals, which have undergone in vitro culture. Recently, these phenomena are being attributed to oxygen free radicals, and successful cell culture are being obtained by lowering the oxygen environment of in vitro culture. This is due to the fact that the oxygen concentration in the fallopian tube is around 5%, which is lower than the room air 20% concentraition for in vitro culture. Superoxide dismutase(SOD), which are free radical scavenger, were added to early embryo mice culture and compared the free radical level at each stage of development with that of culture in which free radical scavenging agent was not added. The highest free radical level in control group observed in the late 2-cell stage and was markedly decreased if SOD added to culture media. Blastulation rate, hatching rate, and blastomere count as the parameters of embryo development was significantly increased in the experimental group. This study suggests the possibility of improvement in in-vitro embryo culture with media including free radical scavengers.

10.The Effect of Estradiol on the Osteoporosis Induced by Oophorectomy in the Rat

Post-menopausal osteoporosis is known to be related to estrogen deficiency. The purpose of this experimental study is to investigate whether or not estrogen administration can prevent or retard the process of osteoporosis induced by oophorectomy in rats. Fifty-two Sprague-Dawley rats, aged five months, were separated into three groups. Group 1 (4 rats) was control group that had sham operation without oophorectomy. Goup 2 (32 rats) was another control group that had bilateral oophorectomy alone. Group 3 (16 rats) was experimental group that had estradiol administration from the fourth week through the eighth week after bilateral oophorectomy. In group 1, all the rats were killed at the fourth week after sham operation. In group 2 and group 3, four rats in each group were killed weekly until the eighth week after oophorectomy. All the animals were subjected to radiological, histological studies on the tibia, ilium, skull, and the sixth lumbar vertebra. Also, histomorphometric measurements were 1. Osteoporotic changes were evident from the fifth week after oophorectomy and increased with time on the radiologic examination in group 2. In contrast, osteoporotic changes were evident from the second week after oophorectomy and progressed with time on the histological and histomorphometric studies in group 2. 2. After administration of estradiol, osteoporotic changes which include thinning of the cortex, decrease of the amount of trabeculae were partially restored on the radiological, histological studies in group 3. 3. Histomorphometric study revealed that the trabecular area percent (the proportion of total area of the trabeculae from the whole metaphyseal area), decreased progressively through the eight week after oophorectomy in gorup 2. Whereas the trabecular area percents of group 3 were higher than those of group 2, but still lower than initial value before oophorectomy. Based on the above observations, estradiol administration appeared to delay the progression of the osteoporotic changes induced by oophorectomy in rats.
