Mongolian Journal of Health Sciences 2026;91(1):116-123
Background:
Numerous studies have shown that air pollution is a serious public health threat and risk worldwide. Mongolia is the world leader in outdoor and indoor air pollution, and the morbidity and mortality caused by it are increasing every year. People spend most of their lives indoors, and it is necessary to regularly assess and study indoor air pollution.
Aim:
To study the concentration of PM2.5 particles in the indoor air of households in Ulaanbaatar and Darkhan cities in relation to some symptoms of respiratory diseases.
Materials and Methods:
The study was conducted using an analytical cohort design from October 2023 to July 2025, and the 4 districts of the central Ulaanbaatar city and Darkhan cities were included in the study. A total of 240 households were included in the study, including 48 households living in ger and house, 24 households living in apartments, and 48 households living in ger and house, and 24 households living in apartments from Ulaanbaatar city. A direct measurement device (PurpleAir) was installed in the households for 3-4 weeks to measure PM2.5 particles in the indoor air for 24 hours. After measuring PM2.5 particles in the indoor air of the household, data were collected through questionnaires from the participants aged 18-60 who participated in the study and underwent health examinations. The results were processed using STATA-19.0 and R programs.
Result:
213-239 households were selected for the study, including households living in 17-26 apartments, 42-58 houses, and 38-50 houses. The 24-hour average concentration of PM2.5 particles in the ambient air of households in Ulaanbaatar city was measured relatively higher in winter and summer than in households in Darkhan city. Among the districts of Ulaanbaatar city, the highest concentration of PM2.5 particles was measured in Songinokhairkhan district, and the lowest concentration was measured in Bayanzurkh district. Considering the cities by heating type, households with stoves are more polluted than households with centralized heating. In Darkhan city, the lowest concentration was measured in apartments (22.78-31.53 μg/m³), houses (44.53-68.17 μg/m³), and the highest concentration was measured in traditional felt houses (houses) (63.20-68.25 μg/m³). In Ulaanbaatar, PM2.5 concentrations were relatively high in all types of housing, including apartments (41.61-58.37 μg/m³), houses (79.22-92.50 μg/m³), and traditional felt dwellings (76.69-86.31 μg/m³). In the summer, PM2.5 concentrations in Darkhan were 3.10-10.03 μg/m³ in apartments, 7.67-14.37 μg/m³ in houses, and 7.98-17.90 μg/m³ in traditional felt dwellings (houses). The concentration of PM2.5 particles in the ambient air of the settlement is statistically significantly associated with shortness of breath, fatigue, weakness, muscle pain, nausea, palpitations, sneezing, runny nose, conjunctivitis, pain in the eye socket, tearing of the eyes, headache, fever, nasal congestion, cough, loss of appetite, sore throat, phlegm, wheezing, and chest pain.
Conclusion
The 24-hour average concentration of PM2.5 particles in indoor air in households in Ulaanbaatar and Darkhan cities is higher than the Mongolian standard during winter and is statistically significantly associated with some symptoms of respiratory diseases.

Mongolian Journal of Health Sciences 2026;92(2):33-39
Background:
Lumbar spine stability is maintained by three distinct yet interconnected systems that control intersegmental stability. Lumbar spine instability is a primary cause of low back pain and a significant contributor to disability. Globally, the “Lumbar Spine Instability Questionnaire (LSIQ)” is widely utilized in clinical practice. This validated tool is effective not only for patients with confirmed lumbar instability but also for individuals experiencing general low back pain.
Aim:
To determine the Mongolian version of the LSIQ and evaluate its reliability.
Materials and Methods:
The translation process commenced after obtaining permission from the original copyright holder of the LSIQ. Following Beaton’s five-stage guidelines for cross-cultural adaptation of self-report measures, a preliminary Mongolian version was developed. Ethical approval was granted by the Research Ethics Committee of the Mongolian National University of Medical Sciences (MNUMS) on November 21, 2025 (Decision No. 25-26/03-01). A total of 68 participants were initially recruited, with 5 excluded based on exclusion criteria. The final 63 participants with non-specific low back pain completed the Mongolian LSIQ. Statistical analysis was performed using SPSS-27.0.
Result:
Internal consistency of the Mongolian LSIQ, assessed using Cronbach’s alpha, was alpha=0.85, indicating good reliability. Test-retest reliability, evaluated over a 24-hour interval using the Intraclass Correlation Coefficient (ICC), yielded ICC=0.999 (95% CI:0.999–1.000, p<0.001), demonstrating excellent stability and reliability of the instrument.
Conclusion
The Mongolian version of the Lumbar Spine Instability Questionnaire (M-LSIQ) is clear in content and possesses high internal consistency and reliability. The results indicate that the LSIQ is a reliable and stable tool for assessing lumbar instability in the Mongolian clinical setting, meeting the necessary diagnostic standards for effective evaluation.

Mongolian Journal of Health Sciences 2026;95(5):12-17
Background:
Sarcopenia is a multifactorial geriatric syndrome characterized by age-related loss of muscle mass, muscle strength, and physical performance. In recent years, neurotrophic and neuromuscular biomarkers have been investigated as potential diagnostic indicators of sarcopenia; however, evidence regarding their sex-specific differences remains limited.
Aim:
To evaluate the sex-specific associations of serum BDNF, GDNF, and CAF22 levels with sarcopenia and muscle-related parameters among older adults.
Materials and Methods:
This analytical cross-sectional study included adults aged 60 years and older. Sarcopenia was defined according to the Asian Working Group for Sarcopenia (AWGS 2019) criteria. Serum BDNF, GDNF, and CAF22 levels were measured using ELISA. Group differences were analyzed using independent t-tests, correlations were assessed using Pearson correlation analysis, and diagnostic performance for sarcopenia was evaluated using receiver operating characteristic (ROC) analysis. Multivariable logistic regression analysis was also performed.
Results:
No statistically significant differences were observed in serum BDNF, GDNF, and CAF22 levels between sarcopenic and non-sarcopenic participants in either the male or female groups (all p>0.05). ROC analysis demonstrated AUC values ranging from 0.540 to 0.542 in males and from 0.508 to 0.530 in females for BDNF, GDNF, and CAF22. Multivariable logistic regression analysis revealed that age (OR=1.12, 95% CI: 1.04–1.21, p=0.003) and lower fat-free mass index (OR=0.78, 95% CI: 0.66–0.91, p=0.001) were significantly associated with sarcopenia, whereas BDNF, GDNF, and CAF22 showed no independent association with sarcopenia.
Conclusions
1. No statistically significant sex-specific differences were observed in serum BDNF, GDNF, and CAF22 levels between sarcopenic and non-sarcopenic older adults (all p>0.05). 2. CAF22 levels were weakly negatively correlated with muscle mass and fat-free mass index in males (p<0.05), whereas BDNF levels showed weak positive correlations with handgrip strength and fat-free mass index in females (p<0.05). CAF22 levels also demonstrated a weak positive correlation with age (p<0.05). 3. ROC and multivariable logistic regression analyses indicated limited diagnostic utility of BDNF, GDNF, and CAF22 for sarcopenia (AUC=0.508–0.542), and none of these biomarkers showed an independent association with sarcopenia (all p>0.05). In contrast, older age (OR=1.12, p=0.003) and lower fat-free mass index (OR=0.78, p=0.001) were significantly associated with sarcopenia.

Mongolian Journal of Health Sciences 2026;95(5):50-56
Background:
According to the 2024 national health statistics of Mongolia, colorectal cancer accounts for 5.1% of all newly diagnosed cancers, with 74.1% detected at an advanced stage. Although population-based screening programs have recently been implemented, no studies have established a risk factor–based scoring system with an optimal cut-off value or evaluated its clinical utility in resource-limited settings in Mongolia.
Aim:
To develop a modified APCS (mAPCS) scoring system and evaluate its clinical applicability for colorectal cancer screening.
Materials and Methods:
A hospital-based prospective cross-sectional study was conducted between December 2024 and February 2026 at the outpatient clinics of the Mongolian National University of Medical Sciences and the Mongolia–Japan Hospital. A total of 447 eligible participants were enrolled. Anthropometric measurements (height and weight) were obtained, and six categories of questionnaires (demographic data, APCS risk score, behavioral factors, medical history, and medication use) were administered. Eleven participants declined colonoscopy; therefore, 436 participants underwent colonoscopic examination, with histopathological evaluation performed when indicated.
Results:
A total of 436 participants were included, of whom 282 (64.7%) were female, with a mean age of 54.8±11.5 years. Colorectal adenomas (advanced and non-advanced) and colorectal cancer were detected in 199 (45.6%) participants. Among them, 43 (21.6%) had advanced colorectal neoplasia. Multivariable regression analysis identified age ≥55 years (OR 4.8), BMI ≥28 kg/m² (OR 3.6), and a family history of colorectal cancer (OR 3.1) as independent risk factors for advanced neoplasia. The optimal cut-off value for the mAPCS score was 4, with an overall diagnostic accuracy of 41.7% (sensitivity 97.7%, specificity 35.6%).
Conclusions
1. Age ≥55 years, BMI ≥28 kg/m², and a family history of colorectal cancer were identified as independent risk factors for advanced colorectal neoplasia. At a cut-off value of ≥4, the mAPCS scoring system demonstrated high sensitivity (97.7%), low specificity (35.6%), high negative predictive value (99.2%), and low positive predictive value (14.2%). 2. Therefore, the mAPCS scoring system may serve as a useful tool for risk stratification and efficient allocation of limited resources in colorectal cancer screening.

Mongolian Journal of Health Sciences 2026;95(5):77-81
Background:
Inflammatory bowel disease (IBD) is a chronic immune-related condition caused by interactions between genetics, immune dysfunction, gut microbiota changes, and environmental factors, often presenting as persistent or recurring diarrhea. Clinically, it commonly presents with chronic diarrhea and other gastrointestinal symptoms. The global prevalence of IBD is increasing, currently affecting approximately 0.5% of the population, with projections suggesting a rise to nearly 1% by 2030. Fecal calprotectin is a neutrophil-derived S100 calcium-binding protein that is elevated in intestinal mucosal inflammation and serves as a reliable noninvasive biomarker of inflammatory activity. Due to its high stability and close association with intestinal inflammatory activity, fecal calprotectin has been established as a dependable, non-invasive marker for identifying intestinal inflammation. In clinical practice, it is extensively used to distinguish IBD from functional gastrointestinal disorders, especially irritable bowel syndrome, among patients presenting with chronic diarrhea. However, its diagnostic utility in Mongolia remains underexplored, and this study aims to determine its sensitivity and specificity in inflammatory bowel disease and compare its diagnostic performance with other inflammatory markers.
Aim:
To determine the sensitivity and specificity of fecal calprotectin in inflammatory bowel disease and to compare it with other inflammatory markers.
Materials and Methods:
A total of 109 participants aged 18-87 years with chronic diarrhea, who attended the gastroenterology outpatient clinics of the Mongolia–Japan Hospital and Intermed Hospital between December 2024 and December 2025 and provided informed consent, were enrolled in this study. Data were collected using structured questionnaires, and laboratory investigations included complete blood count, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and measurement of fecal calprotectin levels from stool samples. Based on fecal calprotectin results, selected participants underwent colonoscopic examination as clinically indicated.
Results:
Among the 109 participants included in the study, 17.4% were diagnosed with inflammatory bowel disease (IBD), with a mean age of 47.41±15.52 years. Fecal calprotectin levels were significantly higher in the IBD group compared to the control group (p<0.001). Fecal calprotectin demonstrated a weak positive correlation with C-reactive protein (CRP) and leukocyte count (p=0.042, p=0.06), while no significant correlation was observed with erythrocyte sedimentation rate (ESR). At a cutoff value of 250 µg/g, fecal calprotectin showed a sensitivity of 94.4%, specificity of 69.2%, positive predictive value (PPV) of 37.8%, and negative predictive value (NPV) of 98.4%. Receiver operating characteristic (ROC) curve analysis identified an optimal cutoff value of approximately 400 µg/g (AUC=0.919, Youden index=0.724), at which the sensitivity remained 94.4% and specificity increased to 78.0%. In contrast, CRP and ESR demonstrated comparatively lower diagnostic performance, with AUC values of 0.640 and 0.644, respectively.
Conclusion
This study demonstrates that fecal calprotectin has superior diagnostic performance compared to conventional inflammatory markers in differentiating inflammatory bowel disease (IBD). An optimal cutoff value of approximately 400 µg/g yielded the highest sensitivity and specificity. Therefore, fecal calprotectin may serve as a valuable tool in clinical practice for both screening and diagnostic confirmation.

Mongolian Journal of Health Sciences 2026;94(4):32-36
Background:
As of 2024, approximately 39 million people worldwide are living with HIV, and approximately 1.3 million new infections are registered annually. In terms of human resources, there is an insufficient supply of doctors and specialists specializing in infectious diseases, and in some regions the workload per doctor is 1.5–2 times higher than the national average.
Aim:
To assess the current status of the structure and accessibility of human resources for STD, HIV, and AIDS care and services.
Materials and Methods:
The study was conducted using a cross-sectional design using quantitative and qualitative research methods. The “SARA-Service Availability and Readiness” standard questionnaire developed by WHO was used to assess the availability, type, location, and accessibility of STD, HIV, and AIDS care and services. The results of the study were processed using Microsoft Excel.
Results:
The current status of human resources providing STD, HIV, and AIDS care and services nationwide. A total of 56 STD, HIV, and AIDS doctors, 37 nurses, 10 junior doctors, 3 counselors, 16 senior laboratory doctors, and 33 laboratory technicians are working. The doctor-nurse ratio is 1:0.66. The average number of doctors and nurses working in the field of STDs, HIV, and AIDS is 0.21 per 10,000 population. The average number of doctors, junior doctors, and nurses providing STDs, HIV, and AIDS care and services per 10,000 population is 0.20. Provinces with a number of doctors, junior doctors, and nurses providing STDs, HIV, and AIDS care and services per 10,000 population is less than 0.10, which are provinces that do not have a qualified senior doctor or no doctor at all, that have a doctor but are not qualified but work alongside other doctors, and that do not have junior doctors or nurses. Six out of 21 provinces, or 29 percent, fall into the category of less than 0.10. The national STD, HIV, and AIDS care and services access index is 54.8.
Conclusion
When calculating the index of access to STD, HIV, and AIDS care and services based on indicators such as human resource adequacy and availability of clinics, access to care and services is poor.

Mongolian Medical Sciences 2026;217(3):11-20
Background:
Pulmonary embolism (PE) is a potentially life-threatening condition. Although advances
in diagnostic and risk-stratification tools have reduced acute mortality, few studies have
evaluated its impact on quality of life after diagnosis.
Aim:
To assess health-related quality of life in patients with acute PE during hospitalization and
three months after diagnosis using the PEmb-QoL questionnaire.
Materials and Methods:
This prospective longitudinal cohort study included 83 patients with acute PE who completed
the PEmb-QoL questionnaire during hospitalization and at three-month follow-up. Six
domains were assessed: symptom frequency, activities of daily living, work-related problems,
social limitations, symptom intensity, and emotional complaints. Scores were transformed
to a 0–100 scale, with higher scores indicating poorer quality of life. Statistical analyses
were performed using SPSS 26.0. Paired scores were compared using the Wilcoxon signed
rank test, and Spearman's correlation was used to assess associations with clinical severity
indices. A linear mixed-effects model including PESI, sPESI, and age assessed whether
hospitalization PEmb-QoL scores predicted three-month outcomes. The study protocol was
approved by the Research Ethics Committee of the Mongolian National University of Medical
Sciences (Approval No. 24/-25/10-1, June 12, 2025), and written informed consent was
obtained from all participants prior to enrollment.
Results:
Mean age was 62.06 ± 17.5 years, and 46.4% of participants were male. Mean scores
improved across all six domains at three months. Significant improvements were observed
in symptom frequency, work-related problems, social limitations, symptom intensity, and emotional complaints (all p<0.05). Activities of daily living did not change significantly
(p=0.332). Hospitalization PEmb-QoL scores significantly predicted three-month scores
(p<0.001), independent of PESI, sPESI, and age.
Conclusion
Quality of life improved within three months after acute PE across most PEmb-QoL domains.
Scores obtained during hospitalization predicted three-month outcomes. Early PEmb-QoL
assessment may help identify patients at risk of persistent quality-of-life impairment and
guide follow-up and rehabilitation.

Mongolian Journal of Health Sciences 2026;97(7):60-65
Background:
A child’s physical growth is fundamentally dependent on hormonal regulation, metabolism, and oxygen supply, within which bone mineralization specifically calcium (Ca²⁺) levels acts as a primary factor determining bone mineral density (BMD), muscle function, and hormonal activity. In cardiovascular disease (CVD), multiple physiological processes are disrupted at various levels, including decreased blood circulation and oxygen supply, increased energy expenditure, and reduced absorption of nutrients and micronutrients. In this context, research linking calcium levels in 6–7-year-old children with electrocardiographic (ECG) changes to physical growth has been scarce, which provided the rationale for conducting this study.
Aim:
To investigate the association between calcium levels and physical growth among 6–7-year-old children with electrocardiographic (ECG) abnormalities detected through early screening.
Materials and Methods:
This study was conducted using a case-control design. The case group included 143 children aged 6–7 with ECG changes, and the control group included 153 children without ECG changes, totaling 296 voluntary participants. Serum total calcium and albumin levels in both groups were measured using a fully automated biochemical analyzer. Statistical analysis was performed using SPSSv27, applying independent samples t-tests, ANOVA, and correlation analysis to compare means between the two groups.
Results:
The control group’s serum total calcium was 2.65±0.15 mmol/L, corrected calcium was 2.58±0.13 mmol/L, and the case group’s serum total calcium was 2.64±0.18 mmol/L, corrected calcium was 2.59±0.17 mmol/L, with no statistically significant difference observed between the study groups (p>0.05). When comparing total calcium in case-group children with normal height (2.66±0.20) versus those with stunted growth (2.61±0.13), a statistically significant difference was found (p<0.05). Correlation analysis between total and corrected calcium levels and physical growth indicators across study groups showed no statistically significant association. When height measurements were divided into four quartiles (Q1–Q4) and compared against total and corrected serum calcium, no statistically significant differences were found between the groups.
Conclusions
Comparison of physical growth and development Z-scores between children with and without ECG abnormalities revealed no statistically significant differences. However, total calcium levels tended to be lower among children with normal growth compared with those with growth retardation in the case group (p < 0.05). No statistically significant differences were observed in serum total or albumin-corrected calcium levels between the study groups (p > 0.05). When height measurements in the study groups were categorized into quartiles, no statistically significant differences in total or albumin-corrected calcium levels were observed between the groups. This suggests that calcium levels alone may not be a decisive determinant of physical growth in children of the same age.

Background:
According to the World Health Organization (WHO), 99 percent of the world’s population is exposed to air
that exceeds WHO recommendations, with low- and middle-income countries being the most affected. The main causes
of indoor air pollution include human activities such as fuel burning, cooking, cleaning, and smoking; housing characteristics such as walls, floors, ceilings, and furniture; ventilation; and outdoor air pollution.
Aim :
To assess PM1 and PM10 concentrations in 120 selected households in Ulaanbaatar.
Materials and Methods :
Indoor PM1 and PM10 concentrations were measured using Purple Air real-time sensors in
randomly selected Ulaanbaatar households between October 2023 and January 2024. Supplementary data on factors affecting the PM2.5 concentration were collected via questionnaires. Each measurement was taken in 10-minute intervals,
yielding 51,309 data for analysis.
Results :
PM1 concentrations were measured at 55.5±53.2 μg/m³ in gers, 54.9 ± 46.7 μg/m³ in houses, and 31.6±40.1 μg//m³ in apartments (p<0.001) and measuring PM10 concentrations were 110.6±108.6 μg/m³ in gers, 110.6±96.7 μg/m³ in
houses, and 62.2±83.0 μg/m³ in apartments (p<0.001) When considering the concentration of PM1, PM10 by heating
type, PM1 was 55.3±50.1 μg/m³ and PM10 was 110.6±103.0 μg/m³ in households with stoves and furnaces, and PM1
was 31.6±40.1 μg/m³ and PM10 was 62.2±83.0 μg/m³ in households connected to the central heating system (p<0.001).
Regarding the months of measurement, the highest concentration was observed in December 2023, at 77.1±94.1 μg/m³. The highest concentrations for both PM₁ and PM₁₀ were recorded in January 2024, at PM₁: 64.8±55.1 μg/m³, PM₁₀:
131.4±116.0 μg/m³.
Conclusion
1. Indoor PM10 concentrations in residential environments in Ulaanbaatar city were within the MNS4585:2016 Air
Quality Standard, however, it was exceeded the WHO air quality guidelines, indicating an excessive risk of increasing morbidity and mortality among the population.
2. Indoor PM1 and PM10 concentrations in residential environments in Ulaanbaatar varies depending on location, type
of housing, type of heating, and month of measurement.

Mongolian Journal of Health Sciences 2025;86(2):36-41
Background:
Obesity, especially central obesity, is a risk factor for non-communicable chronic diseases such as dyslipidemia,
type 2 diabetes mellitus (T2DM), cardiovascular diseases (CVD), and metabolic syndrome (MetS).
Aim:
Study the association between the subcutaneous fat area (SFA) and visceral fat area (VFA) with lipid metabolism
parameters in adults with MetS.
Materials and Methods:
Data from 1511 participants who visited the ‘NURA Mongolia’ Ai Health screening center
between September 2023 and February 2024, including general information, DEXA (Dual X-ray Absorptiometry), and
biochemical analysis results, were used. Metabolic syndrome (MeS) was assessed based on the harmonizing criteria 2009
(≥3 criteria). VFA and SFA were categorized into four groups using quartiles (Q1-Q4). Statistical analysis was performed
using SPSS v26, including T-tests, multiple logistic regression (OR, 95% CI), and ROC (AUC) analysis.
Results:
The average age of the participants was 30.5±3.9 years, with a BMI of 25.1 kg/m², and 49.5% were male. The
group with MetS (n=531) had significantly higher levels of VFA and SFA compared to the group that rated their health as
relatively healthy and had no clinical diagnosis (n=979) (control group) (p<0.0001), with males showing higher VFA and
females showing higher SFA (p<0.0001). The Q4 group for VFA had a significant association with MetS in males (4.611,
95% CI=2.394–9.591) and females (2.253, 95% CI=1.097-3.912) (p<0.001). Logistic regression analysis showed that increased
VFA was more strongly associated with MetS in males (β=0.325, p<0.0001) and females (β=0.338, p<0.003) than
BMI. The AUC for predicting MetS was 0.790 (95% CI=0.750-0.831) for VFA and 0.401 (95% CI=0.351-0.451) for SFA,
with all results being statistically significant (p<0.001). VFA had a higher predictive value compared to other markers.
Conclusion
In healthy men with metabolic syndrome, VFA is more prominently defined, while SFA is higher in healthy
women. Since VFA is a better predictor of metabolic syndrome than SFA, it increases the risk of diseases such as cardiovascular
diseases and type 2 diabetes in men, whereas SFA in women serves as a protective factor.
