Introduction: Itch, an uncomfortable sensation leading to the urge to scratch, is often inadequately managed by conventional antihistamine drugs, which can be ineffective in certain pruritic conditions and cause undesirable side effects. Therefore, there is a need to identify new pharmacologically potent antipruritic compounds with fewer side effects. A synthetic curcuminoid analogue, 2,6-bis-(4-hydroxy-3-methoxybenzylidene)-cyclohexanone (BHMC), a derivative of curcumin - a bioactive compound found in turmeric - has demonstrated various pharmacological ac-tivities. Previous studies have shown that BHMC possesses antinociceptive and anti-inflammatory properties. This study aimed to investigate the antipruritic effects of BHMC in mice models of induced pruritus. Materials and Meth-ods: The pruritus in mice was induced using compound 48/80, substance P, histamine, and serotonin to establish an itch-induced mouse model. BHMC was administered intraperitoneally (i.p.) at doses of 0.1, 0.3, and 1.0 mg/kg. Results: BHMC significantly reduced pruriceptive responses in all models tested and notably inhibited compound 48/80 and substance P-induced mast cell degranulation in skin tissues. Conclusions: These findings suggest that BHMC inhibits pruriceptive responses in mice, likely through the inhibition of mast cell degranulation and/or direct antagonism of peripheral histamine and serotonin receptors. This may warrant further exploration of the antipruritic effect of BHMC in clinical trials for the betterment of animal and human health.

Introduction: The 2021 IDF-DAR risk calculator had been previously evaluated in multiple studies and subsequently widely accepted and applied in clinical practice as a practical standardized tool for patient risk stratification. Recently updated, the 2026 IDF-DAR Risk calculator enables a more individualized, evidence-related evaluation of patientrelated and disease-related risk factors, incorporating modern diabetes technologies, including continuous glucose monitoring (CGM), automated insulin delivery (AID) systems, and advanced insulin formulations to enhance risk stratification. This tool allows medical professionals to tailor Ramadan practices based on overall factors toward promoting safe fasting. Methodology: This prospective multicentre observational study recruited adults with Type 1 and Type 2 diabetes attending public hospitals nationwide. People with diabetes (PwD) intending to perform Ramadan fasting were invited to participate and assessed using the 2026 IDF-DAR Risk Calculator in the 6-week pre-Ramadan period between 30th January and 19th March 2026. Results: A total of 458 PwD were evaluated and stratified into low (15.7%), moderate (41%), and high risk (43.3%) categories. Most participants had Type 2 diabetes (83.6%), with 60.3% having a disease duration exceeding 10 years and 43% exhibiting poor glycemic control (hemoglobin A1c >9%). Insulin therapy was used by 76.4% of participants, including two individuals with Type 1 diabetes using AID systems. Most participants reported no recent hypoglycemia (76.4%), 81.0% performed glucose monitoring, and 3.3% used CGM. Severe comorbidities were uncommon, with 1.1% having unstable macrovascular disease and 4.4% advanced chronic kidney disease (estimated glomerular filtration rate <30). Notably, 72.2% received structured Ramadan education. Conclusion Majority of PwD attending tertiary diabetes clinics were in the moderate- to high-risk category and intended to fast despite medical advice against fasting in some cases. Although most participants were on insulin therapy, hypoglycemia was low in the pre-Ramadan period. Integration of modern technologies, advanced insulin therapies, and structured education may support safer fasting practices.

Introduction: Mindful eating is being fully present in the moment of eating and it is critical in preventing poor eating habits. The main objective for this study was to determine the associations of body mass index (BMI) and physical activity level with mindful eating behaviour among undergraduate medical students of Universiti Sains Malaysia (USM), Health Campus, Kubang Kerian, Kelantan. Methods: A cross-sectional study was conducted among 158 students (Years 1-5; 69% females and 31% males) by using a self-administered online questionnaire consisting of socio-demographic data, anthropometric data, short version of the International Physical Activity Questionnaire (IPAQ), and Mindful Eating Questionnaire (MEQ). The associations between BMI and physical activity with MEQ were determined by Spearman’s Correlation and One-Way ANOVA or Kruskal-Wallis Test, respectively. Results: Majority of the students had normal BMI (66.4%, n=105), and 39.9% (n=63) had low physical activity levels. The mean MEQ summary score of the students was considered high (2.82±0.26). There was a significant negative association between MEQ summary score (r=-0.191; p=0.016) and disinhibition subscale score (r=-0.340; p<0.001) with BMI, whereby MEQ summary and disinhibition subscale scores increased as BMI decreased. However, there was no significant association between physical activity level with MEQ score (p>0.05). Conclusion: A student with positive mindful eating behaviour has the potential to lower his/her BMI. However, further research is required to verify this finding.

Introduction: Benzodiazepines (BZDs) are commonly prescribed to psychiatric patients. However, there have been few studies evaluating BZD refusal among patients with psychiatric disorders during the dispensing process. Thus, this study aimed to determine 1) the factors associated with BZD refusal during the dispensing process and to determine 2) the association between BZD refusal and psychiatric medication adherence among patients diagnosed with depression or schizophrenia. Method: This study was conducted at the Specialist Clinic Pharmacy, Jerantut Hospital, Malaysia, from May 2018 to June 2018. BZD refusal status was determined after the dispensing process, and general information on BZD was determined using a questionnaire developed by the researchers. Medication adherence was assessed using the Malaysian Medication Adherence Scale (MALMAS). Results: Overall, 75 patients with psychiatric disorders participate in the study. Participants had been on BZD treatment for a mean of 32.8± 21.6 months. The BZD refusal rate was 38.7%. BZD refusal was significantly associated with several factors. A one-year increase in age resulted in increased odds of BZD refusal by 1.16 times (95% CI 1.05-1.27). Other factors were male gender (OR,9.14; 95% CI, 1.17-71.27), being single (OR,15.07; 95% CI, 1.12-184.28), the diagnosis of schizophrenia (OR,13.45; 95% CI, 1.75-10.33) and not having history of illicit drug use (OR,20.63; 95% CI, 2.49-171.0). Medication adherence was not associated with BZD refusal. Conclusion: BZD refusal was significantly associated with demographic factors such as increased age, male gender and being single and diagnosis factors namely schizophrenia diagnosis and not having history of illicit substance use. Thus, the need for BZD in these groups of patients should be reviewed regularly.

Primary hyperparathyroidism in children and adolescents is rare and often symptomatic at presentation. A 15-year-old boy presented with bilateral genu valgum for two years. Biochemical results were consistent with primary hyperparathyroidism. Calcium levels normalized two months after removal of a left inferior parathyroid adenoma.

Falls is one of the main problems in older adults, which indirectly contributes to deterioration of quality of life, morbidityand mortality. The purpose of this study was to examine the difference in values between Timed Up and Go (TUG) andgait speed tests among community dwelling older adults with high and low risk of falls classified using PPA (PhysiologicalProfile Assessment). This cross-sectional study involved 278 older adults aged between 60 to 88 years, recruited fromsenior citizen club around Klang Valley. 40.6% older adults had high risk of falls and mostly were women (75%). Theresults of the study showed that there was a significant (p < 0.05) difference in TUG and Gait speed tests performancebetween older adults with high and low risk of falls. Declined mobility (longer time of TUG test) and increased age wereidentified as significant (p < 0.05) risk factors for falls in this study. The TUG test reference values obtained in this studymay be useful for early identification, prevention and management of falls among community dwelling older adults.
