OVERVIEW OF THE ANTENATAL DETECTION OF LYMPHATIC MALFORMATION, ITS ASSOCIATED GENETIC FACTORS, AND THE COMPLICATIONS
Lymphatic malformation (LM) is a dysfunction of the lymphatic system that is associated with genetic diseases. The clinical manifestation is established, but the outcome concerning genetic abnormalities is still not well understood. This review reports the advancement of technology in detecting LM antenatally, its associated genetic factors, and the complications of LM. Articles which were mainly case reports published from 1983 to 2023 were obtained from the search in Ovid MEDLINE and Scopus using the keywords “gene*”, OR, “DNA”, OR “epigenetic*” AND “lymphatic malformation” OR “cystic hygroma”. The advancement of technology over the years has contributed to the various types of genetic investigations conducted on a foetus with LM, including fluorescence in situ hybridization and multiplex ligation-dependent probe amplification. Poor prognosis indicated by the presence of genetic or karyotype abnormality results in opted termination of pregnancy, intrauterine death, or death at early hours of life. The PTPNII, FOXC2, FOXF1, and SRY genes and various chromosome abnormalities are associated with LM. The complications of LM include bone deformity, cardiac and urinary anomalies, and the worst is foetal hydrops. This critical dysfunction warrants future research directions to identify risk factors or biomarkers to prevent future cases of pregnancy with LM.

Limited health literacy is highly associated with poor health status. The concept of health literacy in public health relates to the concern that people with lower health literacy were also less likely to engage in health promoting behaviours. This is an analysis on health literacy and its associated factors among housewives and also a sub analysis of the Malaysian My Body is Fit and Fabulous at Home (MyBFF@home) study involving 328 overweight and obese housewives living in low cost flats in Klang Valley, part of a weight loss programme recruited using convenient sampling. Health literacy was measured using the Malay version of Newest Vital Sign (NVS) test with scores ranging from 0 to 6. Total score of ≤2 indicate limited health literacy and total scores of >2 indicate adequate health literacy. Descriptive analysis, chi-square test and multiple logistic regression were used to analyse the data. The mean(SD) NVS score was 1.07(1.19) with most (87.5%) housewives having limited health literacy. Factors that were significantly associated with limited health literacy were older age of 45 to 59 years old (p = 0.040), primary education and below (p = 0.001) and absence of internet connectivity (p = 0.001). In the final model, absence of internet connectivity (OR 2.61; 95%CI 1.31 to 5.22) was associated with limited health literacy. Majority of the housewives have limited health literacy and providing internet connectivity to housewives may increase their health literacy.

Yolk Sac tumour (YST) of the nasal cavity is extremely rare with only less than five previously reported cases in the English literature. Due to its rarity, the diagnosis is not one that is considered at initial presentation until tissue biopsies are sent for histopathological examination. Serum α-fetoprotein levels aid in diagnosis and in monitoring progression of the tumour. Being a very uncommon tumour of germ cell origin presenting as localized disease in the nasal cavity, prognosis and survival rates are difficult to determine. We are reporting a rare yolk sac tumour case in an infant and the challenges encountered in its management.
