Objective: To evaluate the feasibility of streamlined radioembolization using yttrium-90 resin microspheres without lung shunt fraction (LSF) assessment in patients with intrahepatic cholangiocarcinoma (ICC). Materials and Methods: This single-center retrospective study included 23 patients with ICC who underwent radioembolization using resin microspheres (SIR-Spheres; SIRTEX, Woburn, MA, USA) without LSF measurement between April 2022 and April 2025.Eligibility criteria, based on prior institutional data, included a target tumor size less than 10 cm, absence of hepatic vein invasion and intratumoral dysmorphic vessels, and an institutional waiting time exceeding one week for macroaggregated albumin scintigraphy. All patients had at least one follow-up imaging study. Radiation activity was prescribed according to tumor location, liver function, and clinical setting, using both single-compartment and multi-compartment dosimetry under the assumptions of a 5% LSF and a tumor-to-normal (TN) ratio of 3. Post-treatment yttrium-90 PET/CT dosimetry was performed in 12 patients. Treatment-related toxicity, tumor response, and local tumor progression-free survival were analyzed. Results: The median administered activity was 1.43 GBq (interquartile range, 0.89–2.15). The median mean absorbed dose to the perfused tissue was 147 Gy, and the median tumor absorbed dose (TAD) was 339 Gy, assuming a TN ratio of 3. Posttreatment PET/CT analysis demonstrated a median TAD of 371 Gy and a median TN ratio of 4.7. No patient developed symptomatic radiation pneumonitis. Best tumor response was partial response in 52% of patients and stable disease in 48%. Local tumor progression-free survival rates at six months, one year, and two years were 95.2%, 81.1%, and 81.1%, respectively. Conclusion Streamlined radioembolization without LSF assessment appears feasible and may represent a practical alternative to conventional multi-step workflows in patients with ICC measuring less than 10 cm.

Their high biocompatibility and intrinsic targeting abilities make them promising candidates for therapeutics, drug delivery, and disease biomarkers. In liver diseases, EVs are essential in liver regeneration, fibrosis modulation, and ischemia-reperfusion injury repair, and EV-derived biomarkers have shown potential for non-invasive disease monitoring, particularly in hepatitis B virus infection, non-alcoholic fatty liver disease, and hepatocellular carcinoma. This review provides a comprehensive overview of EV biology, cellular sources, isolation techniques, and strategies to enhance their therapeutic potential. Furthermore, we discuss the role of EVs in liver regeneration and their clinical application in biomarker discovery. Despite significant advancements in EV-based therapies, challenges such as scalability, standardization, immunogenicity, and regulatory approval remain key hurdles for clinical translation. Future research should focus on optimizing EV bioengineering, refining isolation methods, and addressing regulatory concerns to facilitate successful application of EVs in liver disease management and precision medicine.

Background: Regulatory agencies operate their own advisory committee with external experts to address complex scientific issues in the approval of pharmaceutical products. However, each advisory committee operates very differently. Hence, the authors per-formed a comprehensive gap analysis among the committees operated by Food and Drug Administration (FDA), European Medicines Agency (EMA) and Ministry of Food and Drug Safety (MFDS). Methods: The regulations, guidelines, minutes and reports on advisory committees were retrieved from the websites of FDA, EMA and MFDS. A gap analysis comparing the advisory committeesof each regulatory authority was performed, including the disclosure of information and meeting procedures, and conflict-of-interest policies. Results: Substantial differences were found among the advisory committees in the strictness of conflict-of-interests and the transparency of meeting details. Whereas FDA and EMA disclose the detailed curriculum vitae of each committee member, MFDS does only names and majors. Whereas FDA live-streams each meeting and publishes the transcript of all dialogues by each member, EMA and MFDS release only anonymized summary minutes without live broadcasts. Whereas FDA and EMA require members to disclose their financial interests, MFDS merely requires signing a statement that confirms no conflict-of-interest. Conclusions Compared with Advisory Committee of FDA and Scientific Committee of EMA, the Central Pharmaceutical Affairs Advisory Committee (CPAAC) of MFDS appears to require substantial improvements in the disclosure of conflict-of-interests and the transparency ofmeeting details. This gap analysis will likely serve as a basis for policy discussions to improve the credibility of CPAAC.

Objectives: This study was conducted to systematically summarize trends in research concerning patients with coronavirus disease 2019 (COVID-19) as reported in Korean medical journals. Methods: We performed a literature search of KoreaMed from January 2020 to September 2022. We included only primary studies of patients with COVID-19. Two reviewers screened titles and abstracts, then performed full-text screening, both independently and in duplicate. We first identified the 5 journals with the greatest numbers of eligible publications, then extracted data pertaining to the general characteristics, study population attributes, and research features of papers published in these journals. Results: Our analysis encompassed 142 primary studies. Of these, approximately 41.0% reported a funding source, while 3.5% disclosed a conflict of interest. In 2020, 42.9% of studies included fewer than 10 participants; however, by 2022, the proportion of studies with over 200 participants had increased to 40.6%. The most common design was the cohort study (48.6%), followed by case reports/series (35.2%). Only 3 randomized controlled trials were identified. Studies most frequently focused on prognosis (58.5%), followed by therapy/intervention (20.4%). Regarding the type of intervention/exposure, therapeutic clinical interventions comprised 26.1%, while studies of morbidity accounted for 13.4%. As for the outcomes measured, 50.7% of studies assessed symptoms/clinical status/improvement, and 14.1% evaluated mortality. Conclusions Employing a systematic approach, we examined the characteristics of research involving patients with COVID-19 that was published in Korean medical journals from 2020 onward. Subsequent research should assess not only publication trends over a longer timeframe but also the quality of evidence provided.

We describe the first reported case of grain-free diet-induced dilated cardiomyopathy (DCM) in a dog in Korea. An 11-year-old female dog was referred with abdominal distention, anorexia, and vomiting, having been fed a grain-free diet for > 5 years. Thoracic radiography revealed cardiomegaly and pulmonary edema. Atrial fibrillation was detected using electrocardiography. The dog was tentatively diagnosed with congestive heart failure (CHF) secondary to grain-free diet-induced DCM, and its diet changed to contain grain. Digoxin and diltiazem were prescribed for the atrial fibrillation, and pimobendan, enalapril, and furosemide for CHF. Significant improvements in echocardiographic indices were confirmed after 3 months.

The progress of transplantation has been propelled forward by animal experiments.Animal models have not only provided opportunities to understand complex immune mechanisms in transplantation but also served as a platform to assess therapeutic interventions. While small animals have been instrumental in uncovering new therapeutic concepts related to immunosuppression and immune tolerance, the progression to human trials has largely been driven by studies in large animals. Recent research has begun to explore the potential of porcine organs to address the shortage of available organs. The consistent progress in transplant immunology research can be attributed to a thorough understanding of animal models. This review provides a comprehensive overview of the available animal models, detailing their modifications, strengths, and weaknesses, as well as their historical applications, to aid researchers in selecting the most suitable model for their specific research needs.

Organ transplantation, a critical treatment for end-stage organ failure, has witnessed significant advancements due to the integration of improved surgical techniques, immunosuppressive therapies, and donor-recipient matching. This review explores the progress of organ preservation, focusing on the shift from static cold storage (SCS) to advanced machine perfusion techniques such as hypothermic (HMP) and normothermic machine perfusion (NMP). Although SCS has been the standard approach, its limitations in preserving marginal organs and preventing ischemia-reperfusion injury (IRI) have led to the adoption of HMP and NMP. HMP, which is now the gold standard for high-risk donor kidneys, reduces metabolic activity and improves posttransplant outcomes. NMP allows real-time organ viability assessment and reconditioning, especially for liver transplants. Controlled oxygenated rewarming further minimizes IRI by addressing mitochondrial dysfunction. The review also highlights the potential of cryopreservation for long-term organ storage, despite challenges with ice formation. These advances are crucial for expanding the donor pool, improving transplant success rates, and addressing organ shortages. Continued innovation is necessary to meet the growing demands of transplantation and save more lives.
