1.Discussion on the Effects of Hedysarum Polybotrys Polysaccharide on Glucose Metabolism of Small Intestinal Smooth Muscle in Diabetic Gastroparesis Rats with Spleen Qi Deficiency Syndrome Based on AMPK/GLUT4 Signaling Pathway

Objective To observe the effects of Hedysarum Polybotrys polysaccharide(HPS)on the glucose metabolism of small intestinal smooth muscle in rats with diabetic gastroparesis(DGP)of spleen qi deficiency syndrome;To explore its mechanism based on AMPK/GLUT4 signaling pathway.Methods Totally 72 male Wistar rats were randomly divided into 12 in the blank group and the remaining 60 rats were assigned to the model group.The DGP spleen qi deficiency syndrome model was replicated by intraperitoneal injection of streptozotocin+irregular feeding with high-fat and high sugar feed combined with swimming exhaustion method.The model rats were randomly divided into model group,metformin group and HPS high-,medium-and low-dosage groups.The metformin group was given 100 mg/kg metformin hydrochloride by gavage,while the HPS high-,medium-and low-dosage groups were given 200,100 and 50 mg/kg HPS by gavage,respectively.The blank group and model group were given purified water by gavage once a day for 8 consecutive weeks.The gastric emptying rate and small intestine propulsion rate in rats were detected,HE staining was used to observe the smooth muscle morphology of gastric antrum and ileal tissue,immunofluorescence staining was used to detect the expression of glucagon like peptide-1(GLP-1)in ileal tissue,ELISA was used to detect the contents of adiponectin(APN),glucagon(Glu)and insulin(INS)in serum,Western blot was used to detect the protein expressions of glucose transporter(GLUT)4,GLUT1,AMP-activated protein kinase(AMPK)and p-AMPK in ileal tissue.Results Compared with the blank group,the model group rats showed significantly increased random blood glucose,significantly decreased body mass,gastric emptying rate and small intestinal propulsion rate(P<0.01);the edema in the submucosal layer,loose arrangement of connective tissue,infiltration of a small number of lymphocytes,granulocytes and macrophages,reduced number of goblet cells in the ileal tissue,shedding of intestinal villous epithelial cells and widening of the gap between the submucosal layer and the lamina propria;the expression of GLP-1 in ileal tissue was significantly decreased(P<0.05),the contents of serum APN and INS were significantly decreased,and the content of Glu significantly increased(P<0.01),the expressions of GLUT4 and p-AMPK/AMPK proteins in ileal tissue were significantly decreased,while the expression of GLUT1 protein significantly increased(P<0.01).Compared with the model group,rats in metformin group and HPS high-dosage group showed a random decrease in blood glucose,an increase in body mass,gastric emptying rate,and small intestine propulsion rate(P<0.05,P<0.01);a more regular arrangement of gastric antral tissue cells,a reduction of shedding cells and edema,the smooth muscle structure of the ileal tissue was relatively intact,with evenly distributed cells and reduced infiltration of inflammatory cells;the expression of GLP-1 in ileal tissue increased,the contents of serum APN and INS increased,and the content of Glu decreased,the expressions of GLUT4 and p-AMPK/AMPK proteins in ileal tissue increased,while the expression of GLUT1 protein decreased(P<0.05,P<0.01).Conclusion HPS may up-regulate GLUT4 and down-regulate GLUT1 expression through activates AMPK,promotes glucose uptake and utilization by small intestinal smooth muscle,and improves glucose metabolism of DGP rats with spleen qi deficiency syndrome.
2.Optimization of Kaempferol Extraction from Alpinia officinarum Hance Using Box-Behnken Response Surface Methodology and Evaluation of Its Antioxidant Activity

Objective To determine the optimal extraction process for kaempferol from Alpinia officinarum Hance and to investigate its antioxidant activity.Methods Based on single-factor experimental data,this study employed the Box-Behnken central composite design and response surface methodology(RSM),combined with high-performance liquid chromatography(HPLC),to plot response surface and contour maps using kaempferol content as the response value.The optimal parameters for ethanol concentration,solid-liquid ratio,extraction time,and extraction temperature were selected.The antioxidant activity of kaempferol was comprehensively evaluated by calculating the radical scavenging rate using the DPPH radical scavenging assays.Results The optimal extraction conditions were determined to be an ethanol concentration of 84%,a solid-liquid ratio of 1∶80,an extraction time of 2 hours,and an extraction temperature of 82℃.Under these conditions,the kaempferol content extracted from Alpinia officinarum Hance reached 0.034 16%.In the DPPH radical scavenging assay,the extract showed a scavenging rate of 91.20%at a concentration of 1 mg/mL.Conclusion The optimized extraction process for kaempferol from Alpinia officinarum Hance was obtained using the Box-Behnken response surface design method,and this process is practical and feasible.The DPPH radical scavenging assay demonstrated that kaempferol from Alpinia officinarum Hance possesses antioxidant activity.

4.Efficacy of interferon combined with Yanfukang Lotion in treating high-risk cervical human papillomavirus infection

Objective:This study aims to investigate the relationship between high-risk human papillomavirus (HR-HPV) infection and the composition of vaginal microecology. It will analyze the efficacy of integrated traditional Chinese and Western medicine intervention in treating HR-HPV infection, as well as the effects of this combined approach on vaginal microecology. Additionally, the study seeks to identify new methods to inhibit the malignant progression of cervical intraepithelial lesions.Methods:A prospective study was conducted involving 100 patients with HR-HPV infection who visited the Department of Gynecology, The Second Affiliated Hospital of Shaanxi University of Chinese Medicine from April to June 2023. The participants were randomly divided into a control group and an observation group, with 50 patients in each group using a random number table method. The control group received interferon treatment, while the observation group was treated with a combination of interferon and Yanfukang Lotion. High-throughput DNA sequencing of vaginal secretions was conducted to compare the differences in pre- and post-treatment vaginal microecological composition and the rate of HR-HPV clearance between the two groups. Results:In the observation group, both the diversity index and the abundance of vaginal microecology showed significant improvements compared with the control group, particularly in the abundance of dominant Lactobacillus species. These differences were statistically significant ( R = 0.124, 0.304, 0.030, all P < 0.05). The rates of HR-HPV clearance and overall effective rate (clearance + effective) in the observation group were 44% (22/50) and 68% (34/50), respectively, which were significantly higher than those in the control group [20% (10/50), 32% (16/50), χ2 = 12.98, both P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (χ2 = 1.76, P > 0.05). Conclusion:The combination of interferon and Yanfukang Lotion effectively treats HR-HPV infection by regulating the composition of vaginal microecology. This approach shows considerable efficacy and good safety, indicating its potential for further clinical application.

6.Bilobed free peroneal artery perforator flap for repairing defects of maxillofacial malignant tumors

Objective:To explore the clinical outcome of bilobed free peroneal artery perforator flap to repair the postoperative defect of maxillofacial malignant tumors.Methods:A total of 47 patients with maxillofacial malignant tumor with postoperative defects in the Affiliated Hospital of Nantong University from January 2018 to December 2021 were included. They were treated with bilobed free peroneal artery perforator skin flap. After the tumor was removed, the left leg was used as donor site. Based on the defect area of maxillofacial region, a bilobed perforator flap was designed and harvested. After surgery, the survival of the flap, recovery of voice, swallowing function, and flap appearance were evaluated. Speech and swallowing function were scored on a scale of 10.Results:A total of 47 patients were included, 27 males and 20 females, aged 43-79 years.There were 19 cases of buccal malignant tumors, 19 cases of tongue malignant tumors, 9 cases of gingival malignant tumors, 40 cases of squamous-cell carcinoma, 5 cases of adenoid cystic carcinoma and 2 cases of mucoepidermoid carcinoma. The area of maxillofacial defects ranges from (54-84) cm 2, the area of the flap ranges from (50-81) cm 2. During the operation, 17 cases were repaired and the mandibular was reconstructed with vascularized fibula flap. All 47 patients were followed up for an average of 12 months. All patients’ postoperative defects were effectively covered, the flaps survived, the appearance was satisfactory, the flap donor area is well restored, and the speech and swallowing functions were good. The scores of speech function and swallowing function were (8.15±1.35) and (7.43±1.19) respectively. Conclusions:The bilobed free peroneal artery perforator skin flap can be applied to repair the defects of maxillofacial malignant tumors. The flap appearance is acceptable, the recovery time is short. The defective skeletal tissue can be repaired at the same time. with concealed the donor site.

8.Predicting fall related injury risk of patients in hospital

Objective:To develop and internally validate a fall related injury risk nomogram in of patients in Patients in hospital.Methods:Patients who fall during the hospital stay from a grade-three general hospital of Zhejiang province were recruited. Data were collected from January 2014 to December 2019. Data was collected after patients fall in hospital, including age, sex, and 18 other predictive factors. The LASSO regression model and multivariable logistic regression analysis were applied to build a predicting model. Discrimination and calibration of the predicting model were assessed using the C-index and calibration plot. Internal validation was assessed using the bootstrapping validation.Results:Of the 243 patients included in the study, 70 patients had fall related injury. The fall related injury probability was 28.81%. Predictors contained in the prediction nomogram included age≥60, dysfunction, combined utilization of special drugs, no-escort, hypocalcemia. The model displayed good discrimination with a C-index of 0.823 (95% confidence interval 0.756-0.875) and good calibration. High C-index value of 0.803 could still be reached in the interval validation.Conclusion:This fall related injury risk nomogram in of patients in Patients in hospital incorporating the age≥60, dysfunction, combined utilization of special drugs, no-escort, hypocalcemia could be conveniently used to predict fall related injury risk of patients in hospital.
9.Kinsenoside attenuates osteoarthritis by repolarizing macrophages through inactivating NF-B/MAPK signaling and protecting chondrocytes.

The objective was to investigate the effect of kinsenoside (Kin) treatments on macrophage polarity and evaluate the resulting protection of chondrocytes to attenuate osteoarthritis (OA) progression. RAW264.7 macrophages were polarized to M1/M2 subtypes then administered with different concentrations of Kin. The polarization transitions were evaluated with quantitative real-time polymerase chain reaction (qRT-PCR), confocal observation and flow cytometry analysis. The mechanism of Kin repolarizing M1 macrophages was evaluated by Western blot. Further, macrophage conditioned medium (CM) and IL-1 were administered to chondrocytes. Micro-CT scanning and histological observations were conducted on anterior cruciate ligament transection (ACLT) mice with or without Kin treatment. We found that Kin repolarized M1 macrophages to the M2 phenotype. Mechanistically, Kin inhibited the phosphorylation of IB, which further reduced the downstream phosphorylation of P65 in nuclear factor-B (NF-B) signaling. Moreover, Kin inhibited mitogen-activated protein kinases (MAPK) signaling molecules p-JNK, p-ERK and p-P38. Additionally, Kin attenuated macrophage CM and IL-1-induced chondrocyte damage. , Kin reduced the infiltration of M1 macrophages, promoted M2 macrophages in the synovium, inhibited subchondral bone destruction and reduced articular cartilage damage induced by ACLT. All the results indicated that Kin is an effective therapeutic candidate for OA treatment.
10.Sonic Hedgehog promotes fibroblast-like synoviocytes proliferation via modulating the mitogen-activated protein kinase/extracellular signal-regulated kinase signaling pathway in rheumatoid arthritis

Objective To study the effect of mitogen-activated protein kinas/extracellular signalregulated kinase (MAPK/ERK) signaling pathway on cell proliferation modulated by Sonic Hedgehog (Shh) signaling in fibroblast-like synoviocytes (FLS) isolated from patients with active rheumatoid arthritis (RA).Methods The synovial tissue were collected by the synovial arthroscopic debridement or arthroscopic synovectomy of RA patients with active disease activity [disease activity score(DAS)28 ≥3.2].The RA-FLS were primarily cultured by the explanted culture,and then were treated with Shh agonist purmorphamine,inhibitor cyclopamine or MAPK/ERK signaling pathway inhibitor U0126,respectively.Western blotting was used to examine the phosphorylation level of ERK 1/2 (p-ERK1/2),which was the critical protein of MAPK/ERK signaling.The cell proliferation activity was detected using cell proliferation and cytotoxicity kit-8 (CCK8),and the cell proliferation rate was detected using a flow cytometry.Analysis of variance and Kruskal-Wallis H(K) test were used for statistical analysis.Results Compared with the control group,purmorphamine transiently increased p-ERK1/2 protein at the concentration of 1 μmol/L,and the peak activations of p-ERK1/2 took place at 15 min (P<0.01).Cyclopamine and U0126 decreased the expression ofp-ERK1/2 protein (P<0.01).After the RA-FLS treated with purmorphmine(1 μmol/L)for 48 hours,the cell proliferation activity was (114±4)% and the percentage of S phase cells was (8.39±0.60)%,which was significantly higher than those of the control group (100±0)% (P<0.01) and (3.29±0.69)% (P<0.01).After treated with cyclopamine (10 μmol/L) for 48 hours,the cell proliferation activity of RA-FLS was (89±1)% (P<0.05) and the percentage of S phase cells was (1.53±0.22)% (P<0.05).When co-treated with purmorphamine (1 μmol/L) and U0126 (10 μmol/L),the cell proliferative activity was (89±2)% (P<0.05) and the percentage of S phase cells was(1.07±0.25)%(P< 0.05).Conclusion Shh may promote proliferation of RA-FLS via modulating MAPK/ERK signaling,which in turn contributes to hyperplasia of synovium and ultimately leading to RA.
