1.Quality Evaluation of Gegen Qinlian Tablets Based on HPLC Multi-component Quantification Combined with Chemical Pattern Recognition and TOPSIS Analysis

ObjectiveTo establish a high-performance liquid chromatography(HPLC) for the quantitative analysis of multiple components in Gegen Qinlian tablets, and to comprehensively evaluate the quality of samples from different manufacturers by integrating chemical pattern recognition and technique for order preference by similarity to ideal solution(TOPSIS), in order to provide a reference basis for quality evaluation and control of Gegen Qinlian tablets. MethodsHPLC was employed to determine the contents of 10 components in 28 batches of Gegen Qinlian tablets collected from 6 manufacturers, and taking the detection results as variables, SIMCA 14.1 and SPSS 26.0 were employed for cluster analysis(CA), principal component analysis(PCA), and orthogonal partial least squares-discriminant analysis(OPLS-DA) to identify key components affecting the quality. Then, TOPSIS analysis was employed to rank the quality of Gegen Qinlian tablets from the 6 manufacturers and establish a comprehensive quality evaluation method. ResultsA quantitative method for Gegen Qinlian tablets was established. After methodological validation, the method was found to be stable and reliable, and could be used for the quantitative analysis of this preparation. The contents of 3′-hydroxy puerarin, puerarin, 3′-methoxy puerarin, daidzein, coptisine hydrochloride, epiberberine, jatrorrhizine hydrochloride, berberine hydrochloride, palmatine hydrochloride and baicalin in 28 batches of samples were 3.58-7.35, 24.88-42.32, 4.20-9.36, 4.33-7.60, 2.52-6.44, 0.93-4.10, 0.58-3.05, 10.68-22.92, 0.82-4.82, 11.73-60.16 mg·g-1, respectively. Among them, puerarin, berberine hydrochloride and baicalin all met the limit requirements for this preparation specified in the 2025 edition of the Pharmacopoeia of the People's Republic of China. CA and PCA clustered the 28 batches of samples into 5 categories, PCA extracted 2 principal components with a cumulative variance contribution rate of 90.588%, and OPLS-DA screened out 4 differential markers with variable importance in the projection(VIP) values>1.0, namely baicalin, 3′-hydroxy puerarin, coptisine hydrochloride and palmatine hydrochloride, which might be the main components affecting the quality of Gegen Qinlian tablets. TOPSIS analysis showed that the comprehensive score of each evaluation index(Ci) values of different manufacturers were different. Among them, the Ci of manufacturer B was ranked higher, indicating potentially superior quality, while the Ci of manufacturer A was ranked lower, suggesting potentially inferior quality. ConclusionThis study establishes a quantitative method for Gegen Qinlian tablets, and the content uniformity of the same manufacturer is good, while there are differences in the contents of active components among different manufacturers. Through the chemical pattern recognition analysis, it is found that the content differences of Gegen Qinlian tablets may be related to baicalin, 3′-hydroxy puerarin, coptisine hydrochloride and palmatine hydrochloride.
2.Effect of age-friendly social and family care environment on the long-term care services for the disabled elderly people.

OBJECTIVES:
To investigate the effect of age-friendly social and family care environment on the long-term care (LTC) services for the disabled elderly people.
METHODS:
A questionnaire-based survey was conducted among disabled elderly people in three cities of Zhejiang province from June to August 2022, involving 311 subjects from Ningbo city (LTC service insurance pilot site, insured group) and 542 subjects from Hangzhou and Quzhou cities (uninsured group). The service provisions, including ensuring daily activities, preventive healthcare, and satisfying spiritual comfort, were compared among the groups. The family friendly care environment was evaluated with the Family Function Scale and assistance of daily activities, financial support and emotional comfort. The social friendly care environment was measured with the revised WHO recommended age-friendly city environmental framework, including accessibility guarantee environment, information dissemination environment, social participant environment, and life security environment. After controlling for covariates such as sociodemographic, elderly care status, and health risk characteristics, the impact of environment on the effectiveness of service provision of LTC insurance was explored by multiple logistic regression analysis. The mediating and moderating effects were tested to explore the role of age-friendly care environment. A fixed effects model was used to test the service provision effects of LTC insurance policy.
RESULTS:
Disabled elderly with LTC insurance had a higher proportion of their preventive health care and spiritual comfort needs met. Additionally, a multifactorial analysis found a significant positive association between LTC insurance and meeting the spiritual comfort needs. Compared with insured group (Ningbo city), disabled elderly people in Hangzhou urban area (OR=0.45, 95%CI:0.27-0.74, P<0.01) and Quzhou rural area (OR=0.21, 95%CI:0.12-0.37, P<0.01) were more likely to feel unsatisfied with spiritual comfort. The results of mediation analysis showed that the scores of accessibility guarantee environment (OR=1.22, 95%CI:1.02-1.45, P<0.05), information dissemination environment (OR=1.19, 95%CI:1.02-1.39, P<0.05), and social participation environment (OR=1.40, 95%CI:1.17-1.67, P<0.01) in a socially friendly care environment were positively correlated with the satisfaction rate of mental comfort services. The results of the moderation effect analysis indicated that a socially friendly care environment (OR=1.46, 95%CI:1.16-1.84, P<0.01) could compensate for the difference in effectiveness between insured (Ningbo) and uninsured (Hangzhou and Quzhou) areas of LTC insurance. A fixed effect model confirmed the policy chain of LTC insurance policy-social friendly care environment-mental health service satisfaction.
CONCLUSIONS
The implementation of LTC insurance has improved service accessibility, making disabled elderly people feel "seen and valued", and generating psychological and spiritual satisfaction. Accelerating the establishment and improvement of the LTC insurance system requires systematic design, especially emphasizing the supportive role of a socially friendly care environment, and promoting it in urban and rural areas according to the local conditions.

Biosensors have become powerful tools for real-time monitoring of specific small molecules and precise control of gene expression in biological systems. High-throughput sensors for 1, 4-butanediamine biosynthesis can greatly improve the screening efficiency of high-yielding 1, 4-butanediamine strains. However, the strategies for adapting the characteristics of biosensors are still rarely studied, which limits the applicability of 1, 4-butanediamine biosensors. In this paper, we propose the development of a 1, 4-butanediamine biosensor based on the transcriptional regulator PuuR, whose homologous operator puuO is installed in the constitutive promoter PgapA of Escherichia coli to control the expression of the downstream superfolder green fluorescent protein (sfGFP) as the reporter protein. Finally, the biosensor showed a stable linear relationship between the GFP/OD600 value and the concentration of 1, 4-butanediamine when the concentration of 1, 4-butanediamine was 0-50 mmol/L. The promoters with different strengths in the E. coli genome were used to modify the 1, 4-butanediamine biosensor, and the functional properties of the PuuR-based 1, 4-butanediamine biosensor were explored and improved, which laid the groundwork for high-throughput screening of engineered strains highly producing 1, 4-butanediamine.

4.Effect of thalamic nucleus reuniens on spindle waves in the medial prefrontal cortex during non-rapid eye movement sleep

Objective To determine the effect of thalamic nucleus reuniens(RE)on sleep spindles and neuronal activity in the medial prefrontal cortex(mPFC)during non-rapid eye movement(NREM)sleep.Methods Thirteen wild-type C57BL/6 male mice(8~12 weeks old,weighing 22~28 g)were randomly divided into an experimental group(n=7)and a control group(n=6).Optogenetic inhibition combined with multi-channel recording technology was used to specifically inhibit RE projections to mPFC during NREM sleep with yellow light for 20 s,in a 5-min interval between 2 times of light stimuli.The types and distribution of neurons were identified based on the waveform width and firing frequency of neurons.The changes in spindle activity in the mPFC following inhibition,as well as in firing of interneurons and pyramidal neurons were analyzed.Results Morphological data confirmed that neural fibers originating from the RE densely projected to the mPFC,primarily distributed in the deep layers.In the experimental group,light stimulation significantly affected the spindle activity in the mPFC when compared with the period before light stimulation,mainly manifested as a decrease in spindle rate,central frequency,and peak-to-peak amplitude(P<0.05).However,no impacts on spindle duration,number of cycles,symmetry,and power were observed.Meanwhile,there were no significant changes in the spindle activity of the control group.The proportion of interneurons in the mPFC area was 21.9%,while pyramidal neurons accounted for 78.1%.The firing rates of interneurons in the experimental group decreased significantly during light stimulation(P<0.05),whereas there was no significant change in the firing rates of pyramidal neurons.In addition,the firing activities of both interneurons and pyramidal neurons in the control group remained unaffected.Conclusion During NREM sleep,RE inputs may participate in the generation of sleep spindles network oscillations in the mPFC by activating interneurons.
5.Effect and mechanism of N-acetylcysteine on methylnitrosourea-induced precancer ous lesions of gastric cancer

Objective To investigate the effects of N-acetylcysteine(NAC)on methylnitro-sourea(MNNG)-induced precancerous lesions of gastric cancer(PLGC)and analyze its underlying mechanisms.Methods Gastric epithelial cell line GES-1 was cultured in vitro and treated with dif-ferent concentrations of MNNG.The rat PLGC model was established by MNNG induction.The mor-phological changes of the cells were observed by microscope,and the migration and invasion ability of the cells were evaluated by wound healing and transwell tests,respectively.Western blot analysis was conducted to evaluate epithelial-mesenchymal transition(E-cadherin and N-cadherin proteins expres-sion)and related proteins expression of reactive oxygen species(ROS)/phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/nuclear factor-KB(NF-κB)pathway[phosphorylated PI3K(p-PI3K),p-AKT and nuclear p65 protein].ROS levels were detected using DCFH-DA staining.Histopathological changes in gastric tissues were observed using hematoxylin and eosin(HE)stai-ning.Results In vitro experiments demonstrated that NAC treatment alleviated MNNG-induced al-terations in the morphology and growth characteristics of GES-1 cells,inhibiting cell migration and in-vasion.In vitro experiments showed that NAC treatment reduced MNNG-induced lesions and hyper-plasia.NAC inhibited the EMT process(increased E-cadherin expression and decreased N-cadherin expression)and suppressed the ROS/PI3K/AKT/NF-κB pathway(decreased expression of p-PI3K,p-AKT and nuclear p65 protein)both in vitro and in vivo.Activation of the ROS/PI3K/AKT/NF-κB pathway partially reversed the effects of NAC on MNNG-induced cell migration,invasion,epithelial-mesenchymal transition and related proteins in the ROS/PI3K/AKT/NF-κB pathway in GES-1 cells.Conclusion The protective effect of NAC on MNNG-induced PLGC may be associated with the in-activation of the ROS/PI3K/AKT/NF-κB pathway.
6.Construction of domain knowledge graph of dementia care

Objective To construct a domain knowledge graph of dementia care,so as to provide the foundation and guarantee for the next intelligent application based on the knowledge graph.Methods A top-down approach was adopted to construct a domain knowledge graph of dementia care.Firstly,the ontology concept is constructed from the top level,namely the schema layer of knowledge graph.Then,instances are filled,and knowledge extraction is carried out from the existing data sources,and the extracted entities and relationships are filled into the pattern layer ontology database to complete the data layer construction of the knowledge graph.Finally,the"entity relationship entity"triplet data was input into the Neo4j graph database for storage.Results In this study,the personalized care plan set of 1 012 dementia cases was used as the corpus to construct a domain knowledge graph of dementia care.The knowledge graph takes people with dementia as the core,and unfolds,one by one,around basic characteristics,care problems,and care plans in a standardized"entity-relationship-entity"triplet format,forming a large knowledge network,which contains a total of 1 522 specific dementia care knowledge entities and 8 kinds of inter-entity relationships.Conclusion The domain knowledge graph of dementia care constructed in this study clearly and intuitively shows the global pedigree and logical path of knowledge,which provides an efficient and intelligent basic guarantee for the browsing,retrieval and application of dementia care knowledge,so as to realize personalized and intelligent management of people with dementia,break through the bottleneck of lack of professionals,improve the health outcomes of people with dementia,promote the implementation of inclusive pension services,and promote healthy aging.
7.Investigation on the causes of visual disability in Yangpu district of Shanghai from 2019 to 2022

AIM: To analyze the causes of blindness and low vision in patients with visual disability in Yangpu District of Shanghai from 2019 to 2022.METHODS:Cross-sectional study. A total of 1 604 patients who participated in the evaluation of visual disability in Shanghai Yangpu District Kongjiang Hospital, from April 2019 to December 2022 were selected for the study. The grade of visual disability and the main causes of blindness and low vision were determined by trained doctors.RESULTS:A total of 804 patients with visual disabilities were identified, with 87.31% aged 60 and above. The causes of visual disability were high myopic retinopathy(30.47%), age-related macular degeneration(23.26%), glaucoma(17.04%), and diabetic retinopathy(11.07%). Glaucoma(36.96%)is the leading cause of blindness.CONCLUSION: The majority of patients with visual disability are aged 60 years and above. More attention should be paid to the elderly population. Comprehensive prevention, treatment and rehabilitation measures should be applied in different diseases based on classification, so as to early reduce the occurrence of visual disability.
8.Application of logistics intelligent integrated management platform in logistics management of multi-campus hospitals

Objective To strengthen the homogeneous management of logistics of multi-campus hospital and improve the quality and efficiency of logistics service in order to improve the satisfaction of logistics.Methods On the basis of existing serv-ice model,we developed the logistics intelligent integrated management platform,and used the information management method to realize the homogeneous service and standardized management.Results With the logistics intelligent integrated management platform,maintenance time was reduced,and logistics satisfaction was significantly increased.Conclusion The establishment of a one-stop logistics service model based on the logistics intelligent integrated management platform can enhance multi-hospital lo-gistics management and improve both quality and efficiency.
9.Disease distress and its influencing factors in patients with inflammatory bowel disease

Objective To investigate current situation and the influencing factors of disease distress suffered by patients with inflammatory bowel disease hence to provide a foundation for intervention.Methods A total of 240 patients with inflammatory bowel disease who visited or were hospitalized in two Grade III and Class A hospitals from January 2021 to December 2022 were selected as research subjects.General information questionnaire,inflammatory bowel disease distress scale(IBD-DS),social support rating scale(SSRS),general self-efficacy scale(GSES),were used in the survey among the patients.Multiple linear regression were used to explore the influencing factors.Results IBD-DS scores among 235 patients were found from 65 to 93,with an average of 75.8±4.9,at a medium to low level.Pearson correlation analysis and multiple linear regression revealed that marital status,disease duration,SSRS scores,GSES scores and disease activity were the independent risk factors in disease distress,toally explaining 83.6%its variance(P<0.01).Conclusion Patients with inflammatory bowel disease generally suffer a moderate to low level of disease distress.It is necessary to focus on the patients who are unmarried,have a long course of disease,have low social support and self-efficacy,and have high disease activity,and take targeted measures to reduce their disease suffering.
10.Construction and verification of a risk prediction model for postoperative malnutrition in infants with congenital heart disease

Objective To develop a risk prediction model for postoperative malnutrition in children with congenital heart disease(CHD)and to verify it both internally and externally.Methods By a convenience sampling method,300 CHD children treated at a tertiary hospital in Shandong Province from January 2018 to December 2021 were selected as a modeling group,and 129 children from January 2022 to June 2023 were selected as a validation group.Data on patient demographics,disease-specific variables,therapeutic interventions,and nursing care parameters were collected.Single factor and logistic regression were employed to construct a risk prediction model for postoperative malnutrition in CHD children,and the nomogram was drawn and its prediction effect was evaluated.Results The incidence of postoperative malnutrition among CHD children was 33.10%.Logistic regression analysis revealed that risk factors for malnutrition in children included birth weight<2.5 kg,preoperative malnutrition,negative fluid balance 1 week after surgery,and long duration of cardiopulmonary bypass(P<0.05).The area under the receiver operating characteristic curve of the modeling group was 0.933;the sensitivity was 83.30%;the specificity was 90.90%.The Hosmer-Lemeshow test showed that x2=7.765(P=0.457).The AUC of the validation group was 0.918;the sensitivity was 87.20%;the specificity was 90.00%.The Hosmer-Lemeshow test showed that x2=4.947(P=0.763).Calibration curves for both groups indicated good calibration of the model,and the clinical decision curves demonstrated its practical clinical utility.Conclusion The risk prediction model developed in this study exhibits good predictive ability,which can provide a reference for medical staff to early identify high-risk infants for postoperative malnutrition following CHD surgery and to formulate targeted intervention measures.
