1.Prospective study on risk factors for positive test of COVID-19 antibody in the population of Fuzhou in 2024

OBJECTIVE To explore the risk factors for serum COVID-19 antibodies in the population of Fuzhou to provide scientific basis for prevention and control strategies.METHODS The prospective study and the multi-stage stratified random sampling method were performed to select at least 200 subjects from each of the 12 counties(cit-ies)in Fuzhou.Questionnaires follow-ups and serum collections were conducted in Mar,Jun,Sep,and Dec.2024,respectively.The antibody levels in Dec.were divided into a high protective antibody group(IgG antibody≥50 AU/ml)and a low protective antibody group(IgG antibody≤20 AU/ml).The influencing factors of serum anti-body levels were analyzed by multivariate logistic regression model.RESULTS A total of 2816 subjects were en-rolled.In Mar,1454 subjects with high-protective antibodies were identified.After nine months of follow-up,of the 1454 subjects,344 had high protective antibodies and 369 had low protective antibodies in Dec..There were statistically significant differences in age,presence of typical symptoms after infection,underlying diseases,surgi-cal history,healing time,smoking and drinking histories between the high and low antibody groups(P<0.05).Multivariate logistic regression model showed that older age,underlying diseases,history of surgery,smoking and drinking were risk factors which could reduce antibody levels(P<0.05),while typical symptoms after infec-tion and longer recovery time were protective factors which could increase antibody levels(P<0.05).After the COVID-19 infection in spring of 2024,the serum antibody levels in Fuzhou residents showed the downward trend,especially after Sep.2024,with a more pronounced downward trend.CONCLUSIONS According to the risk factors of COVID-19 antibody levels,prevention and control efforts should be focused on the key population.It is necessa-ry to strengthen health education and carry out vaccination to maintain the levels of immunity to protect population from COVID-19 infection.

OBJECTIVE To explore the risk factors for severe influenza patients in Fuzhou,and to provide reference for the prevention and control of severe influenza.METHODS Information on 412 patients with severe influenza in Fuzhou City with onset dates from Jan.2023 to Jan.2025 was collected from the China Influenza Surveillance In-formation System and the China Disease Prevention and Control Information System.The severe patients were matched 1∶1 with non-severe patients based on gender and age±3 years of the severe patients.Basic infor-mation,vaccination history,clinical symptoms,types of infecting viruses and medical history of both groups of patients were collected to summarize the risk factors for severe illness progression in influenza patients.RESULTS The proportions of obesity,retired personnel,children or students,unemployed individuals and smokers in the se-vere group were all higher than those in the non-severe group,while the vaccination rate(6.07%)was lower than that of the non-severe group(16.50%)(P<0.001).The proportions of typical symptoms of shortness of breath/dyspnea and altered mental status/convulsions in the severe group were 23.30%and 21.60%,respectively,which were higher than those in the non-severe group(P<0.001).The proportions of individuals with a history of chro-nic respiratory diseases and cancer/tumors in the severe group were 26.70%and 19.90%,respectively,which were higher than those in the non-severe group(P<0.001).The proportion of influenza A(H1 N1)in the severe group(58.50%)was higher than that in the non-severe group(39.32%)(P<0.001).Retired personnel,children or students and unemployed individuals were high-risk groups for the severe influenza,while farmers were a low-risk group.Obesity(OR=1.966),unvaccination(OR=3.738),smoking(OR=1.787),typical symptoms of shortness of breath/dyspnea(OR=3.305),altered mental status/convulsions(OR=4.099),history of chronic respiratory diseases(OR=4.820)and history of cancer/tumors(OR=3.269)and infection with influenza A(H1N1)(OR=6.422)and influenza A(H3N2)(OR=4.441)were risk factors for the severe influenza(P<0.05).The recovery time in the severe group was 21(6,33)days,which was longer than that in the non-severe group(P<0.001).CONCLUSIONS Obesity,unvaccination,smoking,typical symptoms of shortness of breath/dyspnea,altered mental status/convulsions,history of chronic respiratory diseases and history of cancer/tumors and infection with influenza A(H1N1)and influenza A(H3N2)are risk factors for severe influenza patients.It is necessary to strengthen influenza prevention and control among the elderly and children,enhance health edu-cation,and continuously promote influenza vaccination among key populations.

6.Effects of Bruner's constructivist theory on critical thinking and practice performance of student nurse interns in internal medicine

Objective:To investigate the effects of Bruner's constructivist theory on the critical thinking and practice performance of nursing students interning in internal medicine.Methods:One hundred and fifty-three undergraduate nursing students practicing in the department of internal medicine of a Beijing grade A tertiary hospital were selected by convenience sampling and divided into control group ( n=76) and experimental group ( n=77) according to the order of rotation time. The experimental group received teaching based on Bruner's constructivist theory, while the control group was given traditional teaching. SPSS 24.0 was used to perform the t test. Results:Compared with the control group, the experimental group showed a significantly higher critical thinking score [(306.13±33.00) vs. (325.03±32.09)], a significantly higher exit theory assessment score [(94.74±3.24) vs. (96.94±1.79)], a significantly higher exit skills assessment score [(86.68±11.95) vs. (90.23±9.17)], and a significantly higher degree of satisfaction with teaching [(180.08±13.35) vs. (187.91±14.50); all P<0.05]. Conclusions:Bruner's constructivist theory can help nurse students improve their critical thinking, theoretical and practical performance, and satisfaction with teaching, which enhances the effects of internal medicine nursing teaching.
7.Analysis on the spectrum characteristics of noise hazards in metal products industry

{L-End}Objective To analyze the spectrum characteristics of noise hazards in the metal products industry. {L-End}Methods A total of six metal product industries were selected as research subjects using stratified sampling method. The noise intensity (A/C-weighted) and noise spectrum (Z-weighted) of workplaces and job positions were detected. The characteristics of the noise spectrum of each job position were analyzed using the difference between equivalent continuous C-weighted sound pressure level (LCeq)-equivalent continuous A-weighted sound pressure level (LAeq), the dominant frequency of the noise spectrum, and cluster analysis methods. {L-End}Results The workplace noise and job position noise of 11 main noise positions in the metal products industry were exceeded national standard, with incidence of 69.2% and 78.1%, respectively. The average of normalization of equivalent continuous A-weighted sound pressure level to a normal 40 hours working week exceeded the national standard in 100.0% of hydraulic workers, welders, and ultrasonic cleaning workers. The result of spectrum analysis showed that the noise in the metal product industry was mainly broadband. The noise of the collision welder position was classified as low-frequency broadband noise, while the noise of the painter position was classified as low-frequency narrowband noise. The noise spectrum characteristics of other positions were similar and classified into one category, all of which were broadband noise. Among them, numerical control lathe workers, welders, threaders, machinists, and cutters were exposed to high-frequency broadband noise, while press workers were exposed to mid-frequency broadband noise, and grinders, hydraulic workers, and ultrasonic cleaning workers were exposed to low-frequency broadband noise. The detection rate of binaural high-frequency hearing threshold improvement among workers was 61.7%, and there was no statistical correlation between the detection rate of binaural high-frequency hearing threshold improvement and the noise level intensity of each spectrum. {L-End}Conclusion The level of noise hazards in the metal products industry is severe. Effective engineering control measures and hearing protection measures should be implemented based on the spectral characteristics of noises.
8.Molecular epidemiological study on HIV-1 patients in Hongkou District, Shanghai

ObjectiveTo determine the characteristics of human immunodeficiency virus type 1 (HIV-1) subtypes among newly confirmed HIV-1 patients in Hongkou District of Shanghai, and provide evidence for establishing HIV-1 subtype database and precise prevention and control strategy against AIDS in Hongkou District of Shanghai. MethodsA total of 119 plasma samples from newly confirmed HIV-1 patients in Hongkou District in 2016 were used to detect HIV-1 RNA pol gene by using reverse transcription PCR. HIV-1 subtypes were determined with Los Alamos 2022 HIV-1 gene database using HIV-BLAST tool, and then characterized by epidemiological data. ResultsIn the 119 samples, a total of 89 pol gene fragments were obtained, which were mainly classified as CRF07_BC (38.2%), CRF01_AE (36.0%), and subtype B (13.5%). Furthermore, two new recombinant subtype CRF55_01B and two unique recombinant subtype URF_0107 were identified (2.3%). ConclusionIn 2016, HIV-1 subtypes in Hongkou District are mainly CRF07_BC and CRF01_AE. In addition, new recombinant subtype CRF55_01B and unique recombinant subtype URF_0107 have appeared in Hongkou District. It warrants a need of enhanced surveillance.
9.Adverse outcomes of hypertensive disorders of pregnancy: development and validation of a predictive model

Objective:To develop and validate a predictive model for adverse outcomes in women with hypertensive disorders of pregnancy (HDP).Methods:We retrospectively analyzed the clinical data of patients diagnosed with HDP and delivered at the First Affiliated Hospital of Soochow University or Sichuan Provincial Maternity and Child Health Care Hospital between May 1, 2011, and April 30, 2019. These patients were categorized as the adverse outcome group or the control group with adverse outcomes within 48 h after admission. Univariate analysis, least absolute shrinkage, selection operator (LASSO), and multivariable logistic regression were employed to analyze factors influencing the adverse outcomes and develop a predictive model. The receiver operating characteristic (ROC) curve and calibration plot was used to assess the predictive performance. Bootstrapping was used for the internal validation and the retrospective dataset of patients with HDP from the First Affiliated Hospital of Soochow University from May 1, 2019, to April 30, 2020, for the external validation. A graphic nomogram was created through R software based on the model.Results:(1) Of the 2 978 HDP patients who were included in the development set, 356 were in the adverse outcome group, accounting for 12.0%; of the 233 patients who were included in the external validation set, 40 presented with adverse outcomes within 48 h after admission, accounting for 17.2%. (2) Nine optimal predictors were identified based on the LASSO regression analysis and multivariable logistic regression, consisting of gestational age on admission, routine prenatal care, number of symptoms, mean arterial pressure, platelet count, fibrinogen, albumin, serum urea, and serum creatinine, based on which the logistic predictive model was established. (3) The ROC curve for this predictive model achieved an area under the curve (AUC) of 0.878 (95% CI: 0.858-0.897), and the ideal cut-off value for predicted probability was 0.136, with a sensitivity of 0.778 (95% CI: 0.731-0.820) and specificity of 0.848(95% CI: 0.834-0.862). The model was well-calibrated as the Hosmer-Lemeshow test showed that P>0.05. The calibration plot of the model had a slope of 1 and an intercept of 0. (4) The model showed good consistency in the internal validation and had an AUC of 0.872 (95% CI: 0.807-0.937) in the external validation. The Hosmer-Lemeshow test showed that the P value was >0.05, and the calibration slope was 1.001. (5) A nomogram was constructed for convenient clinical use. Conclusion:A relatively accurate prediction model for adverse outcomes in HDP patients was established, which could be used as a valuable quantitative tool for assessing HDP-related complications.
10.HIV testing and associated factors among technician school students in Guangzhou

Objective:
To provide data support for AIDS prevention and control by investigating HIV testing among technician school students in Guangzhou.
Methods:
A total of 1 112 students were investigated with a questionnaire about AIDS knowledge, attitude, sexual behavior and HIV testing through a stratified cluster random sampling method in October 2018. A Logistic regression analysis was conducted for influencing factors of students’ HIV testing.
Results:
The HIV testing rate of technician school student was 10.3%. The HIV testing rate (12.1%-24.3%) of male, under 18 years of age, with monthly living expenses over 3 000 yuan and with poor family atmosphere was higher than that of the corresponding group (6.6%-8.4%), the difference were statistically significant (χ2=12.07,7.25,10.73,9.77, P<0.05). Multivariate Logistic regression analysis showed that <18 years of age, willingness to participate in relevant AIDS prevention activities, homosexuality, “support for multiple sexual partners” and having sexual behavior were associated with more HIV testing(P<0.05).
Conclusion
The incidence of sexual behavior of students in technician school is high while the rate of HIV testing is low, especially for male and male homosexuals.The health and education departments should strengthen students’ AIDS counseling and testing services and raise students’ awareness of AIDS risk in order to improve the coverage of students’ HIV testing.
