1.Traditional Chinese medicine fumigation assisted with moxa-wool moxibustion in patients with neurogenic bladder after spinal cord injury and paraplegia:a retrospective cohort study

Objective To explore the efficacy of traditional Chinese medicine(TCM)fumigation assisted with moxa-wool moxibustion in patients with neurogenic bladder after spinal cord injury and paraplegia.Methods A retrospective cohort study was used to collect the general data of 90 patients with neurogenic bladder(NB)after paraplegia with spinal cord injury(SCI),who were admitted to The Affiliated Huai'an No.1 People's Hospital of Nanjing Medcial University from January 2019 to June 2021.According to different treatment profiles,the NB patients were divided into 2 groups,of whom 49 patients were treated with moxa-wool moxibustion(control group)and 41 patients were treated with traditional Chinese medicine fumigation assisted with moxa-wool moxibustion(observation group).After 4 weeks of treatment,the efficacy of the 2 groups of the NB patients was evaluated.The TCM symptom scores and urodynamic changes(including the maximum flow rate[Qmax],bladder leakage point pressure,maximum bladder capacity,and bladder compliance),single voided volume,daily urine frequency and residual urine volume(RUV)before treatment and 4 weeks after treatment were compared between the 2 groups.Adverse reactions were also observed in the process.Results The effective rates of the observation group(87.80%)was significantly higher than that of the control group 67.35%(P<0.05).The main symptoms,secondary symptoms,and total scores of the NB patients in the 2 groups after treatment were all decreased,with the observation group being lower than the control group(P<0.05).After treatment,Qmax,bladder compliance and maximum bladder capacity of the 2 groups of the NB patients were all increased,and the pressure of bladder leakage point was decreased,with various indexes of the observation group all being superior to those of the control group(P<0.05).After treatment,the single voided volume of the NB patients in the 2 groups were all increased,daily urine frequency and RUV were all decreased,with the indexes in the observation group being superior to those of the control group(P<0.05).The incidences of adverse reactions in the observation group and the control group were 7.32%and 14.29%,respectively,without statistical significance(P>0.05).Conclusion Traditional Chinese medicine fumigation assisted with moxa-wool moxibustion can improve the therapeutic effects of NB patients,improve symptoms of traditional Chinese medicine and the function of urodynamics,and displayed good safety in the recovery of bladder function.
2.FFDM differential diagnosis of mastitis,benign hyperplasia and breast cancer

Objective The aim of this study was to investigate FFDM differential diagnosis between breast mastitis,benign hyperplasia and breast cancer. Methods Fifty-nine cases of non-puerperal breast mastitis,sixty-eight cases of benign hyperpla-sia and two hundred and forty cases with non-mass type breast cancer were analyzed retrospectively,which were verified by surgery and pathology by contrast with FFDM signs,pathological types,grouped and statistics processed. The observation indexes of lamellar shadow included shape,density and edge. The observation indexes of linear shadow included direction,form and diameter. Results The FFDM signs in three groups of breast diseases were statistically significant(P<0. 05):the form of linear shadow,accompanied by calcifications,the shape of lamellar shadow,the direction of linear shadow,the distribution of lesion,the sharp angle of shadow edge. χ2 segmentation show that there were significant differences between three groups(P<0. 0125):the shape of lamellar shadow,the direc-tion of linear shadow. Conclusion There have some values for the diagnosis of breast cancer by rigid form and radial distribution of linear shadow,rigid shape and segmental distribution of lamellar shadow,the polymorphic calcifications and the sharp angle sign.
3.Effects of neurophysiological facilitation and external pacing of diaphragm on post-stroke respiratory function

Objective To observe of the effect of neurophysiological facilitation of respiration combined with external diaphragm pacing on the respiration of stroke survivors. Methods Sixty-four stroke survivors were divided randomly into a treatment group and a control group, each of 32. Both groups were given routine drugs, while the treatment group was additionally provided with an external diaphragm pacemaker. Those in the treatment group also received neurophysiological facilitation of respiration six times a week for 3 weeks. Before and after the treatment, ar-terial oxygen partial pressure (PaO2) was measured in both groups along with arterial carbon dioxide partial pressure (PaCO2), arterial oxygen saturation ( O2sat) and C-reactive protein (CRP). Ultrasonography was used to measure diaphragm mobility at the end of expiration and inspiration (Δm) , diaphragm mobility of the end of forced inspiration and expiration ( ΔM), and the difference of diaphragm thickness ( Δd). First second forced expiratory volume ( FEV), and maximum voluntary ventilation (FVC) were also measured. Results After the treatment, the average PaO2, PaCO2, O2sat, CRP, Δm, ΔM, FEV and FVC of the treatment group were all significantly better than before the treatment and better than those of the control group. Conclusion Neurophysiological therapy combined with an external diaphragm pacemaker can significantly improve the respiration of stroke survivors, reducing the risk of lung infection.
4.Research progress of oral mucositis during the treatment of tumor

During the treatment of tumor,especially in head and neck cancer,oral mucositis is one of the common complications. The incidence of oral mucositis have relations with chemotherapy,targeted therapy,radiotherapy dose,radiotherapy technology and risk factors of patient. Severe oral mucositis may bring about treatment interrupted,decrease of tumor control rate,affect quality of life and so on. In general, what is very important during the treatment of tumor is to explore the prevention and treatment of oral mucositis.
5.Long-term survival outcome and failure pattern after intensity-modulated radiotherapy for nasopharyngeal carcinoma

Objective To analyze the 10-year survival outcome and failure patterns for patients with nasopharyngeal carcinoma (NPC) after intensity-modulated radiotherapy (IMRT),aiming to provide reference for optimized treatment for NPC.Methods Clinical data of 866 patients with NPC receiving IMRT from January 2001 to December 2008 were retrospectively analyzed.Survival analysis was performed using the Kaplan-Meier estimator.Univariate analysis was carried out by log-rank test and multivariate analysis was performed using Cox proportional hazards model.Results The median follow-up time was 132 months.The 10-year local recurrence-free survival (LRFS),distant metastasis-free survival (DMFS),progression-free survival (PFS) and disease specific survival (DSS) were 92.0%,83.4%,75.7% and 78.6%,respectively.A total of 210 patients died including 124 patients (59.0%) from distant metastasis,which was the primary cause of death,and 47 (22.3%) from local regional recurrence.Independent negative factors of DSS included age>50 years (P=0.00),LDH ≥ 245 IU/L (P=0.00),Hb< 120 g/L (P=0.01),T2-T4 staging (P=0.00),N1-N3 staging (P=0.00) and GTV-nx>20 cm3(P=0.00).The 10-year LRFS,DMFS and DSS of stage Ⅱ NPC patients did not significantly differ after IMRT alone and chemoradiotherapy (P=0.83,0.22,0.23).For patients with stage Ⅲ NPC,the 10-year LRFS and DSS in the chemoradiotherapy arm were significantly higher than those in the IMRT alone (P=0.01,0.01),whereas no statistical significance was observed in the DMFS between two groups (P=0.14).The overall survival of stage Ⅳa+Ⅳb NPC patients is relatively poor.Conclusions IMRT can improve the long-term survival of NPC patients.Distant metastasis is the primary failure pattern.Patients with stage Ⅰ-Ⅱ NPC can obtain satisfactory survival outcomes after IMRT alone.The addition of chemotherapy can further enhance the LRFS and DSS of stage Ⅲ NPC patients.However,the optimal therapeutic strategy remains to be urgently investigated for stage a+ Ⅳb NPC patients.
6.Curative effects of second-line regimen combined with rituximab in treatment of relapsed or refractory non-Hodgkin lymphoma

Objective To investigate the clinical efficiency,safety and prognostic factors of secondline chemotherapy regimen with gemcitabine combined with rituximab in the treatment of relapsed or refractory B-cell non-Hodgkin lymphoma.Methods A total of 157 patients with relapsed or refractory B-cell nonHodgkin lymphoma were selected from July 2008 to February 2015 in Xi'an Central Hospital.Among them,87 patients were given GEMOX regimen (gemcitabine + oxaliplatin) combined with rituximab,and 70 patients were given GDP program (gemcitabine + cisplatin + dexamethasone) combined with rituximab.The chemotherapy efficacies of the two groups were evaluated.At the same time,the patients were grouped according to whether rituximab was applied or not,and the total objective response rate (ORR) difference was compared.The relevant prognostic factors affecting overall survival (OS) were found.The adverse reactions of patients after treatment were observed.Results The ORR of the GEMOX regimen combined with rituximab group was 65.5%,and the ORR of the GDP regimen combined with rituximab group was 55.7%,but the difference between the two groups was not statistically significant (x2 =1.58,P =0.210).The ORR was 75.2% in 105 patients who had not used rituximab,and the ORR was 32.7% in 52 patients who had previously received rituximab.The difference between the two groups was statistically significant (x2 =29.50,P < 0.001).Univariate analysis showed that middle-high risk or high risk of the lymphoma international prognostic index (IPI) score (x2 =69.21,P <0.001),lactate dehydrogenase (LDH) increased (x2 =16.90,P <0.001),refractory patients (x2 =14.43,P =0.001),large mass (x2 =4.57,P =0.030),and failure to achieve CR or PR after salvage chemotherapy (x2 =50.85,P < 0.001) were risk factors for OS.Cox multivariate analysis showed that middle-high risk or high risk of IPI (HR =2.138,95% CI:1.301-3.512,P =0.001),refractory patients (HR =3.157,95%CI:1.001-10.644,P =0.014),failure to achieve CR or PR after salvage chemotherapy (HR=3.017,95%CI:2.218-7.366,P<0.001),LDH increased (HR =2.236,95% CI:1.797-2.781,P =0.001),large mass (HR =1.792,95% CI:1.255-2.558,P < 0.001) were independent risk factors affecting OS.Adverse reactions to chemotherapy were neutropenia,thrombocytopenia,nausea and vomiting,liver damage and cardiotoxicity,with no treatment-related death.Conclusion The second-line chemotherapy regimen containing gemcitabine combined with rituximab has a better curative effect on relapsed or refractory B-cell non-Hodgkin lymphoma,and the safety is good.Middle-high risk or high risk of IPI,refractory patients,failure to achieve CR or PR after salvage chemotherapy,elevated LDH and large mass were independent risk factors for OS.In patients with relapsed or refractory disease after rituximab treatment,re-application of rituximab was not effective.
7.Effects of the crude and processed Epimedium Sagittatum Maxim. on castrated mice

AIM: To observe the supplementation action of the crude and processed Epimedium Sapittatum Maxim. METHODS: The effects of different extracts of Epimedium Sagittatum Maxim. on the castrated mice were observed. RESULTS: The aqueous and alcohol extracts of the crude and processed Epimedium Sagittatum maxim. showed same marked inhibition on atrophy of castrated mice. CONCLUSION: The crude and processed Epimedium Sagittatum maxim. both have the supplementation action and show no marked difference in action.
