1.Exploration on the liver-protective effect of Peipi Shugan Decoction on liver fibrosis in rats based on network pharmacology

Objective:To investigate the liver-protective effect of Peipi Shugan Decoction (PSD) in rats with liver fibrosis through network pharmacology and experimental animal models.Methods:TCMSP was used to retrieve the active components of Peipi Shugan Decoction, GeneCards database was used to obtain the liver fibrosis related targets, and Venny 2.1 was used to obtain the intersection targets. A protein-protein interaction (PPI) network was constructed using the STRING 12.0 database, and Go function and KEGG pathway enrichment analysis were performed through David database. A total of 60 male Sprague-Dawley (SD) rats were divided into two groups: a blank control group ( n=10) and a model establishment group ( n=50) with random number table method. Liver fibrosis models were induced in the model group by intraperitoneal injection of a 40% CCl?- olive oil solution. After successful modeling, the rats were randomly assigned to the following groups ( n=10 per group): model group, colchicine group, and Peipi Shugan Decoction low-, medium-, and high-dosage groups. The colchicine group received colchicine suspension at 0.2 ml/kg via intragastric administration. Peipi Shugan Decoction low-, medium-, and high-dosage groups were administered the decoction at dosages of 2.81, 5.63, and 11.25 g/kg, respectively. The blank control and model groups received an equal volume of normal saline. All treatments were administered once daily for 8 consecutive weeks. HE staining and Masson staining were used to observe the morphological changes of liver tissue and the deposition of collagen fibers. he levels of GPT, GOT and ALP were detected by automatic biochemical analyzer. The expressions of JAK2/STAT1 signaling pathway related proteins, α - smooth muscle actin (α-SMA) and Collagen Ⅰ in rat liver were detected by Western blot. The expression of α-SMA protein in liver tissue was detected by immunohistochemistry. The number and ratio of CD4 + T cells and CD8 + T cells in liver tissue were analyzed by flow cytometry. The levels of IL-6, IL-1β and TNF-α in serum were detected by ELISA. Results:A total of 94 active components were screened out from Peipi Shugan Decoction; the prediction analysis results revealed that this compound formula shared 122 common targets with liver fibrosis diseases; The top five core targets with degree values in the PPI network are AKT1, TNF, IL-6, TP53, and IL-1β. GO enrichment analysis further indicated that Peipi Shugan Decoction mainly achieved anti-liver fibrosis effects by regulating JAK2/STAT1 signaling pathway and other mechanisms. Compared with the model group, the colchicine group and Peipi Shugan Decoction low-, medium-, and high-dosage groups exhibited decreased serum levels of GPT, GOT, ALP, IL-6, TNF-α, and IL-1β ( P<0.05), as well as reduced expression of α-SMA in liver tissue ( P<0.05). Peipi Shugan Decoction medium- and high-dosage groups showed increased protein expression of p-STAT1/STAT1 in liver tissue ( P<0.05), while Peipi Shugan Decoction high-dosage group demonstrated decreased α-SMA protein expression ( P<0.05). Additionally, Peipi Shugan Decoction medium- and high-dosage groups exhibited reduced expressions of CD4 + and CD8 + ( P<0.05). Conclusions:Peipi Shugan Decoction has the characteristics of multi-component, multi-target and multi-pathway in the treatment of liver fibrosis. Its mechanism is mainly related to activating the JAK2/STAT1 signaling pathway, inhibiting cellular inflammatory responses and hindering the activation of hepatic stellate cells (HSC).

Liver transplant patients require lifelong immunotherapy, and if they are infected by SARS-Cov-2, their immune function will face double whammy. This report described the integrated approach of traditional Chinese and western medicine in the treatment of a liver transplant patient with severe COVID-19. The treatment was involved with western medicine such as antiviral, immunosuppressive, focusing on maintaining immune balance. Traditional Chinese medicine was given based on the differentiation of syndromes, targeting at the core pathogenesis and using methods such as promoting qi circulation, clearing heat and resolving dampness, draining lung and relieving panting. Following the treatment, the patient exhibited notable improvement in clinical symptoms and liver function, leading to the effective cessation of disease progression and a shortened recovery period.
3.Curved fenestration and drainage procedure for the treatment of 72 cases of nail root abscess

Objective:To investigate clinical efficacy of curved fenestration and drainage procedure for the treatment of nail root abscess.Methods:From June 2018 to October 2020, 72 patients with nail root abscess were included in Department of Dermatology, Henan Armed Police Corps Hospital. According to the size, location and severity of the abscess, minimally invasive curved fenestration and drainage procedure was performed under nerve block anesthesia to reduce the pressure and drain the pus. Dressing change was strengthened after surgery, and wound healing was monitored.Results:Among the 72 patients, there were 37 males and 35 females, and their ages varied from 39 to 80 years (average: 63 years) . They all received curved fenestration and drainage procedure. Fifteen days after surgery, 66 cases were cured, 6 obtained marked improvement, and the response rate was 100%. Lesions did not completely subside in 1 patient due to poor blood glucose control and small exposure area at the fenestration site, while the condition of other patients was effectively controlled. Telephone follow-up was conducted 3 months after surgery, and no obvious nail defects or deformed appearance was observed in any of the cases.Conclusion:Curved fenestration and drainage procedure is a simple approach for nail root abscess, with satisfactory efficacy.
4.Efficacy Comparison of Xiaoshi Lidan Capsules and Ursodeoxycholic Acid Capsules in the Treatment of Chronic Cholesterol Gallstone Cholecystitis

OBJECTIVE:To compare clinical efficacy of Xiaoshi lidan capsules and Ursodeoxycholic acid capsules in the treat-ment of chronic cholesterol gallstone cholecystitis. METHODS:120 patients with chronic cholesterol gallstone cholecystitis in our hospital were selected and divided into observation group and control group according to random number table,with 60 cases in each group. Observation group was given Xiaoshi lidan capsules 1.2 g,po,tid(after the meal);control group was given Ursodeoxycholic acid capsules 250 mg,po,qd(after dinner). Both group received treatment for 6 months. Effective rate of litholysis were observed in 2 groups as well as abdominal pain score [PRI,VAS score,present pain intensity(PPI)],the thickness of gallbladder wall before and after treatment. Clinical efficacy and the occurrence of ADR were compared between 2 groups during treatment. RESULTS:3 pa-tients withdrew from the observation group and 1 patient withdrew from the control group. Before treatment,there was no statistical significance in PRI,VAS score,PPI and the thickness of gallbladder wall between 2 groups(P>0.05). After treatment,above index-es of 2 groups were decreased significantly,while PRI,VAS score and PPI in observation group was significantly lower than in con-trol group,with statistical significance (P<0.05). The effective rate of litholysis (64.91%) in observation group was significantly lower than in control group (79.67%),with statistical significance (P<0.05). Total effective rate of observation group (57.89%) was slightly higher than that of control group(54.24%),without statistical significance(P>0.05). There was no statistical signifi-cance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Xiaoshi lidan capsules are similar to Ursodeoxycholic acid capsules in clinical efficacy for chronic cholesterol gallstone cholecystitis with good safety,and can be used as optional drug ex-cept for chronic cholesterol gallstone cholecystitis.
5.Stability of sodium nitroprusside for injection in two different dissolvants

Objective To study the stability of sodium nitroprusside for injection in 5% glucose injection and 0 .9%sodium chloride injection ,and provide reference for clinical rational drug use .Methods The content of sodium nitroprusside for injection in different dissolvants(5% glueose ,sodium chloride injection) within 26 h was determined by HPLC .The change of micro particle was measured by light blockage method in accordance with China pharmacopeia(2010 edition) ,and the appearance and UV visible absorption spectroscopy and pH ,the osmotic pressure of the mixtures were observed as well .Results The changes in appearnace ,UV visible absorption spectroscopy ,pH value ,content and microparticle were not significant .Conclusion Sodium nitroprusside for injection were stable within 26 h after mixing with different dissolvants (5% glueose ,sodium chloride injection) .
6.Optimization of Compatibility Plan for Sodium Nitroprusside and Infusions by Orthogonal Design

Objective:To investigate the best compatibility plan for nitroprusside sodium for injection. Methods: An L16 (43 × 26 ) orthogonal design was used with four factors:temperature, solvent, solvent volume and time. The content of sodium nitroprusside was detected by HPLC, the insoluble particles were observed by a GWF-5J type particle analyzer, pH was detected by a PHS-2C preci-sion acidity meter, and a comprehensive scoring method was used to optimize the compatibility plan. Results:The optimized compati-bility plan was as follows:50mg sodium nitroprusside was dissolved in 250ml sodium chloride injections, avoiding light and used up in 26h. Conclusion:The plan provides scientific basis for the clinical medication.

Objective:To evaluate the compatible stability of sodium nitroprusside for injection ( SNP) in two kinds of solutions (5% glucose and sodium chloride injections) . Methods:A, B, C and D solution of SNP was respectively prepared according to the concentration of clinical use. The stability of SNP solution was studied in different environment with different storing time, and the as-sessment indices included the appearance, insoluble particles, pH value, ultraviolet-visible absorption spectrum and SNP content. Re-sults:The SNP solutions in 5% glucose and sodium chloride injections were stable in 26 hours in photophobic condition at 20℃ and 35℃. Under an incandescent lamp with or without lightproof, the solutions were very unstable. Conclusion:Both sodium chloride and 5% glucose injections can be used as the solvent for SNP with 26h stability. SNP injections prepared and used in clinics should be pre-served away from light to ensure the safety and effectiveness.

9.Effect of regenerated silk fibroin film on transcription and expression of vascular endothelial growth factor gene.

As a biomaterial to be used for reparation in the case of trauma, the silk fibroin, particularly its effect on the transcription and expression of VEGF gene, is a concern. In this study, the ECV304 cell's growth shape and growth curve on the regenerated silk fibroin film were observed, and its VEGF secretion level was measured by ELISA test. It was found that the regenerated silk fibroin film did not interfere with ECV304 cell's growth and function. The L929 cell transfected with human VEGF gene grew on the regenerated silk fibroin film; the real-time quantitative RT-PCR method and ELISA test were used for detecting the transcription and expression of VEGF gene. The results showed the regenerated silk fibroin film did not interfere with the transcription and expression of VEGF gene. Therefore, the regenerated silk fibroin film is a safe biomaterial for inducing vascularization with no untoward effect on the reparation of trauma.

10.Influence of Anxiety and Depression in Digestive system Cancer Patients' Prognosis

Objective:To study the effect of anxiety and depression in 46 digestive system cancer patients' prognosis.Methods:The differences of SDS mark?SAS mark and ≥ 50 mark percentage were compared between patients and common mould in nationwide adopting SDS and SAS,moreover the prognosis was compared between varying degrees of anxiety and depression in patients.Results:The SDS mark?SAS mark and ≥ 50 mark percentage in patients were obviously higher than that in common mould in nationwide;the patients with anxiety and depression had bad prognosis.Conclusion:The results suggest that anxiety and deprssion is common feeling in digestive system cancer patients and seriously influence the patients' prognosis,psychotherapy must be conducted and it may contribute to improve the patient's prognosis.
