1.Effect of icariin-containing serum on lipopolysaccharide-induced inflammatory damage in human chondrocytes

BACKGROUND:Previous studies of the research group have found that icariin-containing serum can delay the progression of knee osteoarthritis,promote chondrocyte proliferation and stem cell cartilage differentiation in rats,but there is still a lack of sufficient basis for clinical application.OBJECTIVE:To investigate the repair effect of icariin-containing serum on lipopolysaccharide-induced inflammatory damage of human chondrocytes.METHODS:The effect of icariin-containing serum on chondrocyte viability was detected by cell counting kit-8 method,and the optimal volume fraction and time of icariin-containing serum were screened.The cells were then divided into blank group,lipopolysaccharide group,icariin-containing serum group and lipopolysaccharide+icariin-containing serum group.After grouping,immunofluorescence was used to detect the secretion of type Ⅱ collagen in chondrocytes in each group.Real-time PCR was used to detect the expression of related genes.RESULTS AND CONCLUSION:(1)Icariin-containing serum showed good biosafety characteristics for human chondrocytes,and the cell viability reached the highest level after 48 hours of intervention when the icariin-containing serum volume fraction was 15%,and the follow-up experiments were carried out according to the conditions.(2)Compared with the blank group,lipopolysaccharide significantly inhibited the expression of type Ⅱ collagen in chondrocytes,while icariin-containing serum showed a positive mobilization effect,which could effectively promote the secretion of type Ⅱ collagen in chondrocytes under normal and inflammatory conditions.(3)The mRNA expression of type Ⅱ collagen and Aggrecan in chondrocytes decreased significantly under lipopolysaccharide stimulation,and the mRNA expression levels of matrix metalloproteinase 13 and a disintegrin-like and metalloproteinase with thrombospondin motifs-5 increased significantly.Icariin-containing serum promoted the mRNA expression of type Ⅱ collagen in human chondrocytes under inflammatory conditions and reduced the mRNA expression of matrix metalloproteinase 13 and a disintegrin-like and metalloproteinase with thrombospondin motifs-5 in chondrocytes after lipopolysaccharide intervention.Therefore,these findings indicate that the icariin-containing serum has good safety for human chondrocytes and plays an important role in maintaining the normal physiological functions of chondrocytes,promoting the synthesis of extracellular matrix,and inhibiting the secretion of inflammatory factors.

3.Constructing a risk prediction model for the failure of arteriovenous fistula formation in patients with CKD stage 5

Objective To explore the construction of a risk prediction model for arteriovenous fistula(AVF)formation failure in patients with chronic kidney disease(CKD)stage 5 based on multiple factor analysis.Methods A retrospective analysis was conducted on the data of 278 CKD stage 5 patients who underwent hemodialysis at Fuzhou First General Hospital from July 2022 to July 2024.According to the occurrence of AVF failure,they were divided into surgical success group(n=251)and surgical failure group(n=27).Compare two sets of baseline data,analyze the risk factors affecting the failure of AVF formation,and construct a risk prediction column chart model based on the relevant factors obtained.Results By comparing the data of two groups,it was found that the arterial diameter,venous diameter,serum albumin,and hemoglobin in surgical success group were all higher than those in surgical failure group(P<0.05);while the mean platelet volume(MPV)in surgical success group was lower than that in surgical failure group(P<0.05).Constructing Logistic regression model showed arterial diameter,venous diameter,serum albumin,and MPV were all influencing factors for surgical failure in CKD stage 5 patients(P<0.05).Based on the influencing factors obtained from regression analysis,a column chart risk prediction model was constructed,and the Bootstrap method was used for internal validation of the model.The fit between the internal calibration curve and the ideal curve was good,and the receiver operating characteristic curve showed that the overall predicted area under the curve of the model was 0.887,indicating good accuracy in model evaluation.Conclusion The vascular inner diameter,serum albumin,and MPV are influencing factors for the risk of AVF failure in CKD stage 5 patients.The column chart model established based on this has a good predictive effect on the risk of surgical failure in patients.
4.Correlation between plasma ADAMTS13 level,disease severity and prognosis in patients with endometriosis

Objective To investigate the relationship between plasma von willebrand factory-lytic protease (ADAMTS13) level and the severity of the disease and its prognostic value in patients with endometriosis (EMS). Methods 130 patients with EMS treated in the hospital from January 2019 to December 2022 were retrospectively selected. Plasma ADAMTS13 level was detected by enzyme-linked immunosorbent assay (ELISA),and its correla-tion with American Fertility Society (r-AFS) stage and endometriosis Health Status Scale (EHP-30) score was analyzed. The 1-year prognosis was recorded,and the risk factors of poor prognosis were analyzed by logistic regression. Receiver operating characteristic curve (ROC) was plotted to evaluate the predictive value of plasma ADAMTS13 levels in patients with EMS. Results The plasma ADAMTS13 level of EMS patients decreased signifi-cantly with the increase of r-AFS stage,and was negatively correlated with the score of EHP-30 (P<0.001). Multiple factors showed that low plasma ADAMTS13 level,bilateral location of pelvic lesions and high r-AFS stage were independent risk factors for poor prognosis (all P<0.05). ROC analysis showed that the area under the curve (AUC) of plasma ADAMTS13 was 0.889 (cut-off point 13.315,sensitivity 0.931,specificity 0.690). Conclusion Plasma ADAMTS13 level is negatively correlated with the severity of EMS,and is an independent risk factor for poor prognosis of EMS patients,and has a certain predictive value for prognosis of EMS patients.
5.Ten-year real-world data analysis of clinical characteristics in treatment-naive patients with highly suspected prostate cancer and PSA level ≥20 ng/mL

[Objective] To analyze the clinicopathological characteristics of treatment-naive patients with highly suspected prostate cancer (PCa) with prostate-specific antigen (PSA) level ≥20 ng/mL, to provide reference for promoting early screening of PCa. [Methods] A retrospective analysis was conducted on the clinical data of treatment-naive patients with PSA level ≥20 ng/mL, undergoing prostate biopsy for highly suspected PCa at the Department of Urology, Tianjin Medical University Second Hospital during Jan.2013 and Jun.2023. The correlation between patients' age, body mass index (BMI), PSA, prostate volume (PV), prostate cancer-specific antigen density (PSAD), prostate imaging reporting and data system (PI-RADS) score, and International Society of Urological Pathology (ISUP) grade with highly suspected PCa metastasis and PSA stratification were analyzed. [Results] A total of 1778 suspected patients were enrolled. Pathological findings confirmed PCa in 1465 cases (82.4%), with 487(33.2%) diagnosed as metastatic PCa. Over the past decade, the number of patients undergoing prostate biopsy for highly suspected PCa and being confirmed has been increasing annually, with the proportion of metastatic cases remaining at around 30%. Compared with those with PSA level being 20-50 ng/mL, patients with PSA level >50 ng/mL had older age, lower BMI, higher PSAD, higher PI-RADS, higher ISUP, more diverse pathological types, and a higher incidence of metastasis (P<0.05) with lower proportion of urban residents. Additionally, analysis of metastatic PCa cases showed that 46.8%(228/487) had oligometastasis (≤5 metastatic lesions), including 99.0% bone metastasis, 4.1% extraregional lymph node metastasis, and 4.3% other organ metastasis. [Conclusion] Over the past 10 years, there has been a continuous increase in the number of treatment-naive biopsied cases and newly diagnosed cases of highly suspicious PCa with PSA level ≥20 ng/mL, while the proportion of metastatic cases remains high. Therefore, proactive efforts should be made to promote early screening of high-risk suspected cases.
6.Preparation and osteogenic properties of liquid crystal display light-cured polylactic acid scaffold loaded with nano-tantalum

BACKGROUND:Polylactic acid(PLA)has good biocompatibility and a controllable degradation rate and is currently widely used in biomedical engineering.However,PLA has shortcomings such as low mechanical strength and insufficient biological activity,which limits its further application in bone tissue engineering. OBJECTIVE:To construct polylactic acid/polydopamine/tantalum(PLA/PDA/Ta)bone tissue engineering scaffolds,and explore their biosafety and in vitro osteogenesis. METHODS:A PLA scaffold with a porous structure was prepared through liquid crystal display light-curing technology.PLA/PDA scaffolds and PLA/PDA/Ta scaffolds were prepared by soaking PLA scaffolds in dopamine solution and dopamine-tantalum nanoparticle solution,respectively.The microstructure and water contact angle of scaffolds were characterized.MC3T3-E1 cells were co-cultured with PLA,PLA/PDA,and PLA/PDA/Ta scaffolds,respectively,and CCK-8 assay and live/dead cell staining were performed.After osteogenic differentiation,alkaline phosphatase,alizarin red staining,and osteogenic gene detection were performed. RESULTS AND CONCLUSION:(1)The scanning electron microscope results exhibited that the three kinds of prepared scaffolds had an interconnected porous three-dimensional structure,and the average pore diameter was 200 μm.The water contact angle of PLA/PDA/Ta scaffolds was lower than that of PLA and PLA/PDA scaffolds(P<0.05).(2)CCK-8 assay showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could promote cell proliferation(P<0.05).Live/dead cell staining showed good cell proliferation in the three groups.(3)Alkaline phosphatase and alizarin red staining showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could promote the expression of alkaline phosphatase and the formation of mineralized nodules.RT-qPCR showed that compared with PLA and PLA/PDA scaffolds,PLA/PDA/Ta scaffolds could enhance the mRNA expression of cell bone morphogenetic protein,Runx-2,and type I collagen(P<0.05,P<0.01).(4)The results showed that the PLA/PDA/Ta scaffold had excellent osteogenic activity and the ability to promote cell proliferation.
7.Study on the mechanism of Xiaoqinglong decoction in intervening in airway inflammation of asthma with syndrome of cold retention accumulation in lung

OBJECTIVE To investigate the effects and potential mechanisms of Xiaoqinglong decoction on airway inflammation in asthma with syndrome of cold retention accumulation in lung based on the metastasis-associated lung adenocarcinoma transcript 1(MALAT1).METHODS Forty Wistar rats were randomly divided into blank group,model group,dexamethasone group(positive control,1 mg/kg),and Xiaoqinglong decoction group(2.72 g/kg),with 10 rats in each group.A rat model of asthma with syndrome of cold retention accumulation in lung was established,and the corresponding drugs were administered once daily starting from the second day of modeling for 21 consecutive days.Lung histopathological changes and lung function were evaluated.The levels of superoxide dismutase(SOD),malondialdehyde(MDA),interferron-γ(IFN-γ),tumor necrosis factor α(TNF-α),and interleukin-13(IL-13)in serum were measured,and the mRNA expression levels of MALAT1,TNF-α,IL-13,IFN-γ,and transient receptor potential melastatin 2(TRPM2)in lung tissue were determined.Twenty C57BL/6J wild-type mice and twenty C57BL/6J MALAT1(-/-)mice were randomly divided into wild-type model group,wild-type Xiaoqinglong decoction group,MALAT1(-/-)model group,and MALAT1(-/-)Xiaoqinglong decoction group,with 10 mice in each group.The same asthma model was established,and Xiaoqinglong decoction was administered once daily for 21 days starting from the second day of modeling.The serum levels of SOD,MDA,IFN-γ,TNF-α and IL-13 were measured,along with the mRNA and protein expression levels of TRPM2 in lung tissue.RESULTS The results of the rat experiment showed that,compared with model group,the airway resistance,functional residual capacity,the serum levels of IL-13,TNF-α and MDA as well as inflammatory infiltration and collagen fiber deposition in lung tissue,and the expressions of IL-13,TNF-α and TRPM2 in lung tissue were all significantly decreased in the treatment group(P<0.05 or P<0.01).The peak expiratory flow,forced expiratory flow at 50%of forced vital capacity,the serum levels of SOD and IFN-γ,and the expression levels of IFN-γ and MALAT1 in lung tissue were significantly increased(P<0.05 or P<0.01).The results of the mice experiment demonstrated that,compared with the wild-type model group,serum levels of IL-13,TNF-α and MDA in wild-type xiaoqinglong decoction group were significantly reduced(P<0.05 or P<0.01),while serum IFN-γ levels and SOD activity were significantly increased(P<0.01).Compared with the wild-type Xiaoqinglong decoction group,the MALAT1(-/-)Xiaoqinglong decoction group showed significantly decreased serum IFN-γ levels and SOD activity(P<0.01),along with significantly increased levels of IL-13,TNF-α and MDA(P<0.05 or P<0.01),as well as significantly elevated TRPM2 mRNA and protein expression in lung tissue(P<0.05).CONCLUSIONS Xiaoqinglong decoction may alleviate airway inflammation by regulating the expression of MALAT1,modulating oxidative stress,inhibiting TRPM2 activation,and reducing the release of pro-inflammatory cytokines.

9.Application value of needle-knife accurate fistulotomy for difficult biliary cannulation during endoscopic retrograde cholangiopancreatography

Objective:To investigate the application value of needle-knife accurate fistulo-tomy (NKAF) for difficult biliary cannulation during endoscopic retrograde cholangiopancreato-graphy (ERCP).Methods:The retrospective and descriptive study was conducted. The clinicopatho-logical data of 137 patients with difficult biliary cannulation during ERCP at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine between January 2021 and December 2022 were collected. There were 51 males and 86 females, aged (69±13)years. All 137 patients received NKAF for cannulation during ERCP. Observation indicators: (1) surgical situations; (2) complications. Measurement data with normal distribution were represented as Mean± SD. Count data were repre-sented as absolute numbers. Results:(1) Surgical situations. Of 137 patients, 136 cases had succe-ssful cannulation, 1 case had failed cannulation with NKAF following unsuccessful double-guidewire technique. In the 136 successful cases, the endoscope was straightened in 42 cases, left in a long position in 37 cases, and maintained in the standard position in 57 cases. The cannulation time was (90±8)s. (2) Complications. The serum amylase at postoperative 6 hours in the 136 successful cases was (163±23)U/L. No patient developed post-ERCP pancreatitis. Of the 136 patients with successful cannulation, one case experienced post-sphincterotomy bleeding, which was observed oozing from the papillary orifice on emergency gastroscopy. The patient was successfully controlled with endoscopic clips.Conclusion:NKAF is safe and effective for difficult biliary cannulation during ERCP.
10.Study on the short- and mid-term clinical efficacy of covered stents in the treatment of transplanted renal artery stenosis

Objective:To explore the short- and medium-term clinical outcomes of covered stent implantation in the treatment of transplant renal artery stenosis (TRAS).Methods:A retrospective analysis was conducted on the clinical data of 12 consecutive patients with TRAS (transplant renal artery stenosis) who underwent covered stent implantation in Henan Provincial People′s Hospital from January 2021 to December 2024. The changes in indicators such as serum creatinine (Cr), blood urea nitrogen (BUN), mean arterial pressure (MAP), peak systolic velocity (PSV), intrarenal resistance index (RI), and the diameter of the stenotic site were analyzed before the operation, one week after the operation, six months after the operation, and 12 months after the operation. Repeated measures analysis of variance was used to investigate the changes of each observation index over time.Results:The surgery was successfully performed on all 12 patients, with a technical success rate of 100%. Among them, 8 cases used self-expanding covered stents, and 4 cases used balloon-expandable covered stents. One week, six months and twelve months after the surgery, the levels of Cr, BUN, PSV and MAP were all lower than those before the surgery. The RI and the diameter of the stenotic site were significantly increased compared with those before the surgery, the difference was statistically significant ( P<0.05). During the perioperative period and the postoperative follow-up period, no surgery-related complications were found. Conclusion:The implantation of the covered stent can effectively relieve the stenosis of the transplant renal artery, significantly improve renal function, and reduce blood pressure levels in TRAS patients, while maintaining excellent short-to medium-term clinical outcomes.
