1.Modified Lemaire procedure in anterior cruciate ligament reconstruction with highly positive pivot shift test.

OBJECTIVE:
To explore the effectiveness of the modified Lemaire procedure in anterior cruciate ligament reconstruction (ACLR) in patients with a highly positive pivot shift test.
METHODS:
The clinical data of 18 patients with anterior cruciate ligament (ACL) rupture and highly positive pivot shift test between April 2020 and September 2022 were retrospectively analyzed. There were 13 males and 5 females with an average age of 28.3 years (range, 17-41 years). Causes of injury included 11 cases of direct violence injury, including 6 cases of traffic accident injury, 4 cases of sports injury, 1 case of falling injury; 7 cases of indirect violence injury, all sports injury. All patients had complete ACL rupture, including 15 acute injuries and 3 old injuries. The preoperative pivot shift test was grade Ⅱ in 9 cases and grade Ⅲ in 9 cases. All patients were treated with ACLR combined with modified Lemaire procedure. The International Knee Documentation Committee (IKDC) score and Lysholm score were used to evaluate the effectiveness before operation and at 3, 6, 12 months after operation. KT-2000 arthrometer was used to measure the anterior stability of the knee joint, and the difference between the healthy and affected sides was recorded. Pivot shift test was used to evaluate the rotational stability of the knee joint. During the follow-up, X-ray films were taken to observe the bone tunnel and internal fixation, and MRI was used to examine the healing of ACL, anterolateral collateral ligament and fibular collateral ligament grafts.
RESULTS:
All patients completed the operation successfully without complications such as knee joint infection, vascular and nerve injury. All patients were followed up 12-19 months (mean, 13.2 months). After operation, the rotational stability of the knee joint recovered satisfactorily, and there was no adverse symptom such as knee instability and locking at last follow-up. X-ray film and MRI showed that the bone tunnel was anatomically located and healed well, the internal fixation was in good position, and the reconstructed ACL and iliotibial band were continuous and in good tension. The IKDC score, Lysholm score, and the difference of KT-2000 between the healthy and the affected sides significantly improved at 3, 6, and 12 months after operation ( P<0.05). All the indicators further improved with time after operation, except that there was no significant difference in IKDC score between 3 and 6 months after operation and in the difference of KT-2000 between 3 months and 6, 12 months after operation ( P>0.05), and there were significant differences in other indicators between different time points ( P<0.05). Pivot shift test was negative immediately after operation and at last follow-up.
CONCLUSION
In ACL injuries with a highly positive pivot shift test, ACLR combined with the modified Lemaire procedure can effectively restore anterolateral knee stability, leading to satisfactory knee stability and function in the early postoperative period.

2.Probucol combined with tirofiban for angina pectoris in patients with coronary heart disease after interventional surgery

Objective To discuss the effect of probucol combined with tirofiban treatment on serum lipid peroxide levels and vascular endothelial function in patients with angina pectoris after interventional surgery of coronary heart disease(CHD).Methods A total of 120 patients with angina pectoris occurring after interventional surgery from January 2018 to December 2022 at the People's Hospital of Inner Mongolia Autonomous Region of China were enrolled in this study.Using random digital table method,the patients were divided into observation group(n=60)and control group(n=60).The patients of the control group received tirofiban,while the patients of the observation group received tirofiban plus probucol.All the preoperative and postoperative 7-day relevant indicators were recorded.The main observation indicator was the overall effective rate,and the secondary observation indicators included the frequency and duration of angina attacks,Seattle Angina Questionnaire(SAQ)score,cardiac function,myocardial injury markers,serum lipid peroxide levels,endothelial function and adverse reactions.Results The total effective rate in the observation group was 91.67%(55/60),which was higher than 78.33%(47/60)in the control group,the difference was statistically significant(P<0.05).After treatment,the improvement degree of angina pectoris,SAQ score,LVEF,serum superoxide dismutase(SOD),nitric oxide(NO)in the observation group were higher than those in the control group,while the LVESD,LVEDD,serum creatine kinase isozyme(CK-MB),lactate dehydrogenase(LDH),creatine kinase(CK),malondialdehyde(MDA),endothelin-1(ET-1)in the observation group were lower than those in the control group,the differences in the above indexes were statistically significant(all P<0.05).Conclusion For patients with angina pectoris occurring after interventional surgery of CHD,probucol combined with tirofiban has exact efficacy,which can promote the recovery of cardiac function,reduce myocardial injury,regulate serum lipid peroxide levels,and improve vascular endothelial function.
3.Correlation between plasma ADAMTS13 level,disease severity and prognosis in patients with endometriosis

Objective To investigate the relationship between plasma von willebrand factory-lytic protease (ADAMTS13) level and the severity of the disease and its prognostic value in patients with endometriosis (EMS). Methods 130 patients with EMS treated in the hospital from January 2019 to December 2022 were retrospectively selected. Plasma ADAMTS13 level was detected by enzyme-linked immunosorbent assay (ELISA),and its correla-tion with American Fertility Society (r-AFS) stage and endometriosis Health Status Scale (EHP-30) score was analyzed. The 1-year prognosis was recorded,and the risk factors of poor prognosis were analyzed by logistic regression. Receiver operating characteristic curve (ROC) was plotted to evaluate the predictive value of plasma ADAMTS13 levels in patients with EMS. Results The plasma ADAMTS13 level of EMS patients decreased signifi-cantly with the increase of r-AFS stage,and was negatively correlated with the score of EHP-30 (P<0.001). Multiple factors showed that low plasma ADAMTS13 level,bilateral location of pelvic lesions and high r-AFS stage were independent risk factors for poor prognosis (all P<0.05). ROC analysis showed that the area under the curve (AUC) of plasma ADAMTS13 was 0.889 (cut-off point 13.315,sensitivity 0.931,specificity 0.690). Conclusion Plasma ADAMTS13 level is negatively correlated with the severity of EMS,and is an independent risk factor for poor prognosis of EMS patients,and has a certain predictive value for prognosis of EMS patients.
4.Changes in Cardiac Function and Prediction of Late Recurrence Risk Factors in Atrial Fibrillation Patients undergoing One-stop Procedure

Objective To investigate the impact of catheter ablation(CA)combined with left atrial appendage closure(one-stop procedure)on cardiac function in atrial fibrillation(AF)patients,and analyze the risk factors for late AF recurrence after one-stop pro-cedure.Methods A total of 225 patients who visited the Inner Mongolia Autonomous Region People's Hospital for AF from January 2020 to December 2023 were included,and they were divided into the one-stop procedure group(n=93)and the CA group(n=132)ac-cording to the procedure,and 55 patients with matching clinical baseline characteristics were respectively screened out at a ratio of 1:1 by the method of propensity score matching,namely the CA group(n=55)and the one-stop procedure group(n=55),and the changes in cardiac function between the two groups were compared between preoperative and 1-year postoperative periods.Risk factors for late post-operative recurrence of AF and their predictive value in the one-stop procedure group were assessed using multivariate Logistic regression analysis and receiver operating characteristic(ROC)curve.Results At 1 year postoperatively,N-terminal B-type natriuretic peptide(NT-proBNP),left atrial internal diameter(LAD),and left ventricular end-diastolic diameter(LVEDD)were significantly lower and left ventricular ejection fraction(LVEF)was significantly higher in both groups compared with the preoperative period(P<0.05),but there was no statistically significant difference in the improvement degree of cardiac function between the two groups at 1 year postopera-tively(P>0.05).The results of the univariate analysis showed that there were statistically significant differences in NT-proBNP,LAD,and left ventricular end-systolic diameter(LVESD)in the one-stop procedure late postoperative recurrence group compared with late postoperative non-recurrence group(P<0.05),and the results of multivariate Logistic regression analysis showed that the preoperative NT-proBNP(OR=1.075,95%CI:1.029-1.124,P=0.001)and preoperative LAD(OR=1.222,95%CI:1.094-1.365,P<0.001)were the risk factors for late recurrence of AF after one-stop procedure.The results of the ROC curve analysis showed that the area under the curve(AUC)of NT-proBNP,LAD,and the combination of the two in predicting late recurrence of AF after one-stop procedure were 0.788,0.772 and 0.846,respectively,with a sensitivity of 84.4%,68.8%and 87.5%,and a specificity of 68.9%,78.7%and 77.0%,respectively.Conclusion Both one-stop procedure and CA can significantly improved cardiac function in patients with AF with comparable results.Preoperative LAD enlargement and NT-proBNP elevation are independent risk factors for late recur-rence of AF after one-stop procedure,and the combination of the two can effectively improve the efficacy of AF recurrence risk assess-ment.

7.Association between the 24-hour movement behaviors and executive function in preschoolers

Objective:To explore the characteristics of 24-hour movement in preschoolers and its relationship with executive function.Methods:The study subjects consisted of 555 preschoolers in Zhuhai City. An objective measurement approach was employed, physical activity level was assessed by utilizing accelerometers and executive function was evaluated by computerized behavioral tests.The characteristics of 24-hour movement in children were described.Data were analyzed using SPSS 26.0 software, independent samples t-test was used to compare differences in 24-hour activity behaviors across genders, and multivariate linear regression was used to explore the effect of preschoolers' achievement of activity behaviors on executive functioning. Results:According to the physical activity guideline for Chinese preschoolers aged 3-6 years recommendations for children's daily moderate to vigorous-intensity physical activity, sedentary screen time, and sleep duration, among the study participants, the compliance rate of moderate to vigorous-intensity physical activity (≥60 min/d) was 33.9 %(188/555), sedentary screen time (≤1 h/d) was 43.4%(241/555), and sleep duration (10-13 h/d) was 83.4 %(463/555). Only 13.3 %(74/555) of the participants met the recommended guidelines for all three behaviors. The correct score of the working memory test was (60.6±20.3) and the reaction time of the inhibitory control test was (695.1±135.8) ms. In male preschoolers, moderate to vigorous-intensity physical activity (≥60 min/d) positively predicted working memory( β=0.121, P=0.037), while sedentary screen time (≤1 h/d) negatively predicted inhibitory control ( β=-0.122, P=0.036). Conclusions:The daily compliance rate of moderate to vigorous-intensity physical activity and sedentary screen time is low among preschooler. The former is positively correlated with working memory in preschool boys, while the latter is negatively correlated with inhibitory control.
8.Analysis of neutron beam quality control test items in boron neutron capture therapy (BNCT) equipment

Objective:To provide reference for establishing the testing method for quality control of neutron beam in boron neutron capture therapy (BNCT) equipment in China by testing the radiation characteristic parameters and dosimetry characteristic parameters of epithermal neutron beam in hospital neutron irradiator (IHNI).Methods:By comparing the uncertainties in the result of various test items with the deviation values recommended by the European Joint Research Center (EC-JRC), the feasibility of the relevant of testing method was analyzed and evaluated.Results:The uncertainty in epithermal neutron fluence rate was 2.7%. The uncertainty in ratio of thermal to epithermal neutron fluence rate was 3.1%. The uncertainty in ratio of fast neutron air kerma to epithermal neutron fluence rate was 9.3%. The uncertainty in ratio of gamma air kerma to epithermal neutron fluence rate was 8.7%. The uncertainty in spatial distribution of neutron fluence rate was 2.7%. The uncertainty in thermal neutron fluence rate in phantom was 1.8%. The uncertainty in neutron and gamma-ray dose rate in phantom was 17.1% and 4.0%, respectively.Conclusions:The uncertainty in neutron dose rate measurement result in phantom is higher, and further research is needed to improve the accuracy of the testing method. The uncertainty in the measurement result of other test items is lower, and the accuracy of the test result is expected to meet the allowable deviation value recommended by the European Joint Research Center, and the test method is feasible.
9.Mechanism of "Sanyang" combined therapy of traditional Chinese medicine in alleviating colonic injury in mice induced by influenza virus based on transcriptome sequencing technique

Objective:To explore the mechanism of Ma-Xing Shi-Gan decoction combined with Xiao-Chai-Hu decoction (hereinafter referred to as " Sanyang combined treatment" ) in alleviating colon injury in mice infected with influenza virus by transcriptome sequencing technique.Methods:The mouse model of colonic injury caused by influenza virus was induced by intranasal drip of influenza A virus H1N1 suspension. The mice were divided into Control group, Model group, and Sanyang combined treatment (SCT) group. Model group and SCT group were fed with PBS and Ma-Xing Shi-Gan decoction combined with Xiao-Chai-Hu decoction respectively. Seven days later, the colon tissues of each group were taken, the colon length and pathological damage were observed, and the transcriptome was sequenced to screen the significantly different genes between the SCT group and model group for Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) and Gene Set Enrichment Analysis(GSEA).Results:After the therapy with SCT, the length of the colon of mice was significantly improved and the pathological injury of the colon was reduced. There are 92 differentially expressed genes between the SCT group and the model group. GO analysis indicated that the differential genes were enriched in biological processes such as regulation of cytokine and chemokine production, inflammatory response, defense response, immune response, regulation of NF-κB inducing kinase(NIK)/Nuclear factor-κB(NF-κB) signal and Mitogen-activated protein kinase(MAPK) cascade, as well as cell components related to intestinal barrier such as brush border membrane, brush border and microvilli. KEGG analysis indicated that the differential genes were enriched in Toll-like receptor signaling pathway, intestinal immune network for IgA production, complement and coagulation cascade, and Peroxisome proliferator-activated receptor(PPAR) signaling pathway. GSEA indicated that the intestinal immune network for IgA production, PPAR signaling pathway, propionic acid metabolism and butyrate metabolism were significantly up-regulated after the intervention with SCT, while apoptosis and MAPK signaling pathway were significantly down-regulated.Conclusions:Sanyang combined therapy can protect the intestinal tract of mice infected with influenza virus mainly through immunity, inflammation and metabolism pathways.
10.Application of transmesenteric vein extrahepatic portosystemic shunt to treat extrahepatic portal vein obstructive disease

Objective:To explore the feasibility, safety and effectivity of applying transmesenteric vein extrahepatic portosystemic shunt (TEPS) to treat extrahepatic portal vein obstructive disease (EHPVOD).Methods:From December 2020 to April 2021, 12 patients with EHPVOD in the Vascular Surgery Department of Zhengzhou University People′s Hospital were prospectively enrolled in the study. The infra-umbilical median longitudinal minilaparotomy was performed to expose the branch of superior mesenteric vein (SMV). RUPS-100 was introduced into the trunk of SMV. A balloon with a diameter of 20 mm was introduced through right internal jugular vein (RIJV) into inferior vena cava (IVC). Under fluoroscopy, RUPS-100 was used to puncture the balloon in IVC. A stiff guide wire was used to establish the pathway between RIJV and SMV. Finally the portosystemic shunt between IVC and SMV was established with a covered stent-graft. The total operative time, the time of establishing portosystemic shunt alone, the dosage of contrast agent, the preoperative and postoperative pressure of SMV were recorded. Paired t test was used to compare the preoperative and postoperative pressure of SMV. Results:All 12 patients were successfully performed TEPS. The total operative time was (113±32) min, the time of establishing portosystemic shunt alone was (31±5) min, the dosage of contrast agent was (129±48) ml. The postoperative pressure of SMV [(14.3±2.1) mmHg] decreased significantly ( t=20.125, P<0.01) compared to baseline [(27.8±2.7) mmHg]. All portal hypertension symptoms released after the operations.There was 1 case of delayed incision healing, 1 case of bacteremia and 1 case of slight hepatic encephalopathy, but all of them were cured. There was no death case. Postoperative CT showed all portosystemic shunts were patent. Conclusion:TEPS is a new, safe, effective and feasible treatment method for patients of acute and chronic EHPVOD.
