1.Prevalence and determinants of dry eye syndrome among civil aviation flight attendants

Objective:To investigate the prevalence and determinants of dry eye syndrome among civil aviation flight attendants.Methods:Data from 1 201 civil aviation flight attendants across 6 airlines, along with their eye examination and general health records from the previous year, were collected and analyzed between January and March 2024. A self-designed questionnaire was designed to assess risk factors to dry eye syndrome. The severity of dry eye syndrome of these civil aviation flight attendants was assessed using the Ocular Surface Disease Index (OSDI) scale before they were grouped based on OSDI scores. The Fatigue Assessment Scale and Pittsburgh Sleep Quality Index were used to evaluate levels of fatigue and sleep quality. Multivariable Logistic regression was employed to identify factors associated with the severity of dry eye syndrome.Results:A total of 1 201 flight attendants were enrolled in this study. Questionnaires were distributed to all the participants, and 1 021 valid responses were collected, yielding a response rate of 85.0%. All flight attendants experienced dry eye syndrome. Among these cases, 657 cases were classified as moderate and 364 as severe. Logistic regression analysis showed that flying hours in the previous year ( OR=1.001, 95% CI: 1.001-1.002), a history of eyelid disease ( OR=2.059, 95% CI: 1.311-3.232), fatigue ( OR=2.438, 95% CI: 1.633-3.640), use of preservative-containing eye drops ( OR=3.034, 95% CI: 1.800-5.113), use of artificial tears ( OR=2.431, 95% CI: 1.544-3.827), use of contact lenses ( OR=2.095, 95% CI: 1.381-3.179), refractive progression of <-0.50 D ( OR=3.102, 95% CI: 1.955-4.920) and -0.50 to <-1.00 D ( OR=3.846, 95% CI: 1.845-8.018), sleep scale scores of 6-10 ( OR=1.940, 95% CI: 1.258-2.993), 11-15 ( OR=2.730, 95% CI: 1.564-4.768), and 16-21 ( OR=6.010, 95% CI: 1.752-20.614) were determinants of the severity of dry eye syndrome. Conclusions:Dry eye symptoms are generally severe among civil aviation flight attendants. Priority should be given to monitoring sleep quality and fatigue levels, with particular emphasis on individuals exceeding 713 flying hours in the previous year for early detection of dry eye syndrome.

3.Compatibility Principle and Efficacy Characteristics of Fuzi Shanzhuyutang from Perspective of Tangye Jingfa Tu

Tangye Jingfa Tu is an important content in the ancient book Fuxingjue from Dunhuang, implying the fundamental principles of formula compatibility in traditional Chinese medicine (TCM). Our research group has delved into nearly 200 formulas (both classical and contemporary formulas) recorded in the Fangjixue under the theoretical framework of the deficiency or excess syndrome of five Zang-organs together with the reinforcing and reducing effects of Chinese medicinal materials of five flavors. We have initially elucidated the essential principles of the correspondence between formulas and syndromes, revealing the deep-level logic of medicinal material selection and compatibility, thus enriching the understanding about the core characteristics and essence of the diseases and syndromes targeted by formulas. The lunar year of 2024 is Jia Chen year. The formula recorded in Sanyin Jiyi Bingzheng Fanglun for treating the epidemic diseases characterized by excessive earth, prevalent dampness and wetness, and invasion of pathogenic factors into the kidney water in Jia Chen year is Fuzi Shanzhuyutang. Therefore, elucidating the compatibility principle of Fuzi Shanzhuyutang is of great significance for clinical prescription and medication modification in Jia Chen year. According to the Tangye Jingfa Tu theory on the deficiency or excess of syndrome of five Zang-organs and the reinforcing and reducing effects of Chinese medicinal materials of five flavors, this article dissects Fuzi Shanzhuyutang regarding the etiology and pathogenesis of the main indications, as well as the five-element properties and efficacy characteristics of Chinese medicinal materials constituting this formula. It explains the compatibility principles of Fuzi Shanzhuyutang and puts forward suggestions for modifying the formula to address different indications, providing a reference for guiding clinical syndrome differentiation and treatment.

5.Research on the Compatibility Rules of Medicinal Ingredients in Classical Cough Treatment Formulas Based on Guidelines/Textbooks

Objective Under the theory guidance of the Fuxingjue and the Tang-Ye-Jing-Fa map,this study aims to explore the common patterns and unique characteristics of five-flavor combinations in formulas for treating different syndromes of cough.Methods Chinese herbal formulas for cough treatment were collected from traditional Chinese medicine(TCM)clinical practice guidelines,consensuses,and textbooks,and their flavor combinations and distribution were clarified.Based on the theory of the Tang-Ye-Jing-Fa map regarding the five flavors for lung diseases(sour for tonifying,salty for purging,and pungent for transforming),the dominant flavors in each formula were statistically analyzed.Further classification was conducted according to the cold-heat properties and the amount of phlegm,and the characteristics and five-flavor combination patterns of formulas for different syndromes were analyzed.Results A total of 21 formulas for treating different syndromes of cough were statistically analyzed,among which 85.7%of the formulas had dominant flavors of"sour,salty,and pungent".For hot cough(9 formulas),sour was mainly used to tonify the lung and bitter to purge the heart(e.g.,Sangxing Tang and Qingjin Huatan Tang);for cold cough(4 formulas),pungent was mainly used to tonify the liver and warm yang(e.g.,San'ao Tang combined with Zhike San and Xiaoqinglong Tang);for cough with abundant phlegm(7 formulas),the focus was on using pungent to purge the spleen and eliminate dampness(e.g.,Erchen Tang combined with Sanzi Yangqin Tang);for cough with little or no phlegm(6 formulas),sour was used as the core to tonify the lung and nourish yin(e.g.,Sha Shen Mai Dong Tang).Conclusion From the perspective of the Tang-Ye-Jing-Fa map,the flavors of"sour,salty,and pungent"play an important role in the composition of formulas for treating cough.Although the five-flavor combinations of formulas for different syndromes of cough vary,they share common characteristics when classified according to cold-heat properties and phlegm amount.These findings provide a theoretical basis for optimizing the combination of Chinese herbal formulas for cough treatment by adjusting the proportion of the five flavors in clinical practice.

8.Study on the relationship between supervisors' guidance and the personal ability of postgraduates in the Professional Master's Program in Clinical Medicine: role of learning engagement and learning burnout

Objective:To analyze the role of learning engagement and learning burnout in the relationship between supervisors' guidance and the personal ability of postgraduates in the Professional Master's Program in Clinical Medicine.Methods:A total of 4 016 postgraduates in the Professional Master's Program in Clinical Medicine from 61 colleges were surveyed in 2020. Common method bias was assessed using the Harman's single factor test and total score was determined by the entropy weight method. Causality and mediation effect were analyzed by linear regression, and mediation effect was tested by Bootstrapping.Results:The average scores of supervisors' guidance, learning burnout, learning engagement and personal ability were (4.13±0.87), (2.49±1.11), (3.88±0.83), and (3.71±0.78), respectively. Supervisors' guidance significantly and positively impacted learning engagement ( β=0.689, P<0.001) and personal ability ( β=0.504, P<0.001). Learning engagement played a partial mediating role (89.30% of mediation effect) between supervisors' guidance and personal ability. Learning burnout (interaction term β=0.078, P<0.001) positively regulated the relationship between supervisors' guidance and learning engagement. Conclusions:Learning engagement can mediate the effect of supervisors' guidance on the personal ability of postgraduates in the Professional Master's Program in Clinical Medicine, and learning burnout positively regulates the effect of supervisors' guidance on learning engagement. Strengthened supervisors' guidance, increased attention to students' learning burnout, and enhanced learning engagement can further improve the personal abilities of postgraduates in the Professional Master's Program in Clinical Medicine.
9.The "burnout-engagement continuum" status of postgraduate supervisors of the Professional Master's Program in Clinical Medicine

Objective:To investigate the "burnout-engagement continuum" status of postgraduate supervisors of the Professional Master's Program in Clinical Medicine and to identify key populations.Methods:From October to November 2023, an anonymous online questionnaire survey was conducted to determine the "burnout-engagement continuum" status of postgraduate supervisors of the Professional Master's Program in Clinical Medicine. The scores of different dimensions of job burnout are expressed as mean±standard deviation, and the distribution of supervisors of different status is presented in frequency and percentage. All data analyses were performed in R 4.3.1. Distribution of the five profiles among supervisors of different status was compared using the chi-square test and compared pairwise using the Bonferroni method.Results:A total of 2 664 valid questionnaires were collected. The scores of emotional exhaustion, cynicism and personal accomplishment were(2.46±0.95),(1.74±0.89), and(4.23±0.90), respectively. The prevalence of job engagement, inefficacy, overexertion, disengagement, and burnout among the supervisors were 42.76%(1 139/2 664), 43.54%(1 160/2 664), 33.03%(880/2 664), 14.71%(392/2 664), and 13.66%(364/2 664), respectively. In terms of demographics, the distribution of the "burnout-engagement continuum" was significantly different by age, sex, marital status, and highest education level( P<0.05). In terms of work-related characteristics, the distribution of the "burnout-engagement continuum" was significantly different by income, professional title, administrative position, and working hours on weekdays and holidays( P<0.05). The distribution of engagement, inefficacy, and overexertion was significantly different between the eastern and western regions( P<0.05). Conclusions:There is a high percentage of individuals with burnout among postgraduate supervisors of the Professional Master's Program in Clinical Medicine, and inefficacy is a common negative state. Supervisors who work in the western regions, are under 45 years of age, are male, are married, have a doctorate degree, hold associate senior professional title, have lower relative income, are not holding administrative duties, and work more than 10 hours on workdays and more than 4 hours on holidays are key populations that require increased attention. The psychological state of supervisors can be improved by providing them with more resources, work benefits, and incentives.
10.Research on the enrollment status of students in Master's Degree Program in Pediatrics in China and the countermeasures

Objective:To investigate the enrollment scale and distribution of Master's Degree in Pediatrics programs in China, and to provide a reference for promoting pediatric education and disciplinary development.Methods:Data on colleges and universities authorized to award Master's Degree in Pediatrics in 2023 were collected, sorted, and analyzed for the number, structure, distribution, and enrollment scale and direction of these institutions using descriptive statistics.Results:Among the 117 clinical medicine academic master's degree programs in China, 72 enroll pediatric academic master's degree candidates, with an enrollment of 260 students. Among the 120 master's degree programs in clinical medicine, 104 enroll professional master's degree candidates, enrolling 1 195 students. Enrollment is mainly concentrated in East China, "non-double first-class" colleges and universities, medical colleges and universities with subject level B, and enrollment is carried out in the direction of secondary disciplines.Conclusions:The number of colleges and universities authorized to award Master's Degree in Pediatrics was small, and the distribution of these colleges and universities was unbalanced. The enrollment scale was small and the orientation of Professional Master's Degree was not reasonable. Some colleges and universities were authorized to award Master's Degree in Pediatrics, but did not enroll any students. It is suggested to increase the number of colleges and universities authorized to award Master's Degree in Pediatrics and strengthen the staffing of pediatric departments. The aim is to expand the enrollment scale of candidates for Master's Degree in Pediatrics, improving the differential training of candidates for Academic Master's Degree and Professional Master's Degree, and attach importance to the construction of pediatrics.
