1.Research on type 2 diabetes prediction algorithm based on photoplethysmography.

To address the current issues of data imbalance and scarcity in photoplethysmography (PPG) data for type 2 diabetes mellitus (T2DM) prediction, this study proposes an improved conditional Wasserstein generative adversarial network with gradient penalty (CWGAN-GP). The algorithm integrated gated recurrent unit (GRU) networks and self-attention mechanisms to construct a generator, aiming to produce high-quality PPG signals. Various data augmentation methods, including the improved CWGAN-GP, were employed to expand the PPG dataset, and multiple classifiers were applied for T2DM prediction analysis. Experimental results showed that the model trained on data generated by the improved CWGAN-GP achieved the optimal prediction performance. The highest accuracy reached 0.895 0, and compared with other data enhancement methods, this approach exhibited significant advantages in terms of precision and F1-score. The generated data notably enhances the accuracy and generalization ability of T2DM prediction models, providing a more reliable technical basis for non-invasive early T2DM screening based on PPG signals.

2.Effect of behavior change wheel strategy based on information asymmetry in postoperative rehabilitation of patients with thoracolumbar fracture

Objective:To explore the effect of behavior change wheel strategy based on information asymmetry in the postoperative rehabilitation of patients with thoracolumbar fracture.Methods:From June 2020 to May 2021, 164 patients with thoracolumbar fracture in Spine Surgery who were admitted to the First Affiliated Hospital of Xinxiang Medical University were selected by convenience sampling as the research object. The patients were divided into observation group and control group according to random number table, 82 cases each. The control group adopted routine postoperative nursing management, while the observation group implemented the behavioral change wheel strategy based on nurse patient information asymmetry on the basis of the control group. After three months of intervention, the differences of kinesiophobia and recovery of lumbar and back function between the two groups were compared.Results:After intervention, the scores of kinesiophobia and recovery of lumbar and back function in the observation group at the end of the first, second and third months were lower than those in the control group, with statistically significant differences ( P<0.05) . Conclusions:The behavior change wheel strategy based on nurse patient information asymmetry can effectively alleviate postoperative kinesiophobia of patients undergoing thoracolumbar fracture surgery, and improve the recovery of lumbar and back functions after surgery.
3.Value of contrast-enhanced ultrasound in diagnosis of placental acerete

Objective To explore the value of contrast-enhanced ultrasound(CEUS) in diagnosis of placental accrete,placental increta and placental percreta. Methods Twenty suspected patients of placental accrete, placental increta and placental percreta were examined with CEUS after routine ultrasound examination. Sono Vue was injected intravenously as bolus and a real time CEUS was performed to observe the characteristics. Results Comparison of 15 CEUS cases with uterine curettage, 8 cases had a clear relationship between the residual disease of the uterine cavity and uterine wall,7 cases had a thin thickness (2-4 mm) between the residual disease of the uterine cavity and uterine wall. Five of the 20 cases performed cesarean cesion,3 cases of the uterine serosa of placenta affixing part were very thin,incomplete and rough, demonstrating placenta increta and placenta percreta, 1 case of uterine rupture showed non-invasiveness between uterine wall and uterine serosa,and 1 case of placenta increta pathologically proven, initially suspected gestational trophoblastic disease by the CEUS. Conclusions CEUS provides an important diagnostic message for placental accrete, placental increta and placental percreta by showing the different characteristics of the perfusion image.
4.Firebird stent for the treatment of patients with coronary heart disease: short-term clinical outcome

Objective The sirolimus-eluting stent (SES) has dramatically reduced the rate ofrestenosis in comparison to that with the bare-metal stent (BMS).This study aimed to evaluate the short-term efficacy and safety of Firebird stent implantation for patients with coronary heart disease (CHD). Methods From Apri12006 through July 2007, 155 patients (mean age 58.93~10.27 years) with CHD were implanted with Firebird stent or Cypher select stent at Daxing Hospital. Patients were followed up for one year. All-cause mortality, major adverse cardiac events (MACE, including cardiac death, myocardial infarction, recurrence of angina pectoris, heart failure, revascularization, and adverse arrhythmia) and stent thrombosis were compared between the 2 groups. Results Of the 155 consecutive patients, 147 patients were revascularized completely. Of these patients, 48 (with 59 lesions) were treated with Firebird stent, 59 patients (with 75 lesions) with Cypher select stent. The demographic characteristics were similar in the 2 groups. All the angiographic and procedural results were not significantly different between the 2 groups. All-cause mortality, myocardial infarction, recurrence of angina pectoris, MACE and stent thrombosis were almost identical between the 2 groups before discharge, at 6 months and at one year .Conclusion The short-term efficacy and safety of Firebird stent are similar to that of the cypher select stent for the treatment of patients with CHD.
5.Evaluation of the efficacy and safety of recombinant prourokinase in patients with acute myocardial infarction

Objective In this angiographic trial,we compared the efficacy and safety of Pro-UK with urokinase in Chinese patients with acute myocardial infarction.Methods We recruited patients with acute ST-elevation myocardial infarction presented within 6 hours in Beijing Tongren Hospital offiliated to Capital Vnirersity of Medical Sciences from Feb.2003 to Mar.2004.After giving informed consent,patients were assigned a bolus and infusion of Pro-UK or a infusion of urokinase.The primary efficacy end points and safety end points were observed.Results Overall 26 patients were enrolled in the trial,of whom 16 patients were assigned to receive Pro-UK(6 patients to 40mg,6 patients to 50 mg,4 patients to 60 mg),and 10 patients to urokinase.The rates of TIMI grade 3 flow were 56.25%(9/16)with Pro-UK and 70%(7/10) with urokinase(P=0.683),of whom 66.7%(4/6) with 50 mg Pro-UK,75%(3/4)with 60 mg Pro-UK. The rates of TIMI grade 2 or 3 were similar for patients treated with Pro-UK versus urokinase(56.25% and 80%,respectively,P=0.399). All safety end points were similar between the two groups. The level of fibrinogen in blood plasma was significantly higher in Pro-UK group than that in urokinase group,indicating that Pro-UK had higher fibrin-specificity.Conclusion The bolus and 30 minutes infusion of Pro-UK 50 mg and 60 mg was clinically safe and effective thrombolytic regimen,but need further study.
