1.Clinical value of CT radiomics in predicting liver cancer grade and prognosis after hepatectomy

Objective To explore the clinical application value of CT radiomics in predicting the grade of liver cancer and the prognosis after hepatectomy.Methods A retrospective selection was conducted on 385 liver cancer patients who underwent preoperative CT scans,and they were divided into training group(265 cases)and testing group(120 cases).The CT radiomics features were extracted and logistic regression was used to construct radiomics markers.A retrospective analysis was performed on 60 patients with liver cancer who underwent hepatectomy and regular follow-up.Results The area under the curve(AUC)of the radiomics signature in the train-ing group was 0.889.The AUC in the testing group was 0.941.For predicting prognosis after hepatectomy,the radiomics features with filter parameter of Filter 1.0 achieved optimal performance,with Dif.Scale1.2 being a good independent predictor of overall survival(OS).Conclusion CT radiomics has certain application value in predicting the grade of liver cancer and the prognosis after hepatectomy.
2.The correlation between serum inflammatory markers and short-term prognosis in patients with single-segment lumbar intervertebral disc protrusion treated by transforaminal endoscopic discectomy

Objective To explore the correlation between serum inflammatory indicators and short-term prognosis in patients with single-segment lumbar intervertebral disc protrusion(LDH)treated by transforaminal endoscopic discectomy(TELD).Methods From January 2015 to June 2022,137 patients with single-segment LDH received TELD treatment.The levels of serum inflammatory factors such as C-reactive protein(CRP),white blood cell count(WBC),erythrocyte sedimentation rate(ESR),and procalcitonin(PCT)of the patients before the operation and 3 days after the operation,as well as surgical data such as anesthesia methods,operation time,and blood loss were collected.The time for patients to resume work was collected,and the pain relief and functional recovery of the patients were evaluated by using the Visual Analogue Scale(VAS)and the Oswestry Disability Index(ODI).Pearson or Spearman rank correlation analysis was used to explore the correlations between demographic characteristics,inflammatory indicators,surgical data and short-term prognosis.Results The average time for patients to return to work was(3.55±2.13)months.There were statistically significant differences in the pain and functional scores at each time point compared with those at the previous time point(P<0.05).The CRP,WBC and PCT of the patients after the operation were significantly increased compared with those before the operation,and the difference was statistically significant(P<0.05).The levels of CRP and PCT in patients 3 days after surgery were significantly positively correlated with VAS 6 months after surgery.The duration of symptoms and the CRP level 3 days after surgery were significantly positively correlated with ODI 6 months after surgery.BMI was significantly positively correlated with the time to return to work(P<0.05).Conclusion There is a certain correlation between the levels of serum inflammatory indicators in patients with single-segment LDH treated with TELD and the short-term prognosis.The increase of CRP and PCT 3 days after the operation may affect the rehabilitation process of the patients.
3.Yttrium-90 resin microsphere selective internal radiation therapy on giant hepatocellular carcinoma in right lobe of liver: a case report

Primary liver cancer is a common malignant tumor in China. The current main treatment methods include surgical resection, liver transplantation, interventional therapy, radiofrequency ablation, chemotherapy, targeted therapy, and immunotherapy. Yttrium-90 microsphere selective internal radiation therapy (90Y-SIRT) represents a critical treatment option for converting unresectable intermediate to advanced hepatocellular carcinoma. This report details a case of a patient with a giant hepatocellular carcinoma in the right lobe treated with 90Y resin microsphere at the Third Affiliated Hospital of Sun Yat-sen University. Post-treatment, the tumor showed significant necrosis and reduction in size, with a marked decrease in alpha fetoprotein (AFP) levels and notable improvement in clinical symptoms. The case highlights the substantial efficacy of 90Y-SIRT in controlling tumor progression and enhancing patient quality of life.
4.Traditional Chinese Medicine Regulates JAK/STAT Signaling Pathway to Treat Colorectal Cancer: A Review

Colorectal cancer (CRC) ranks as the second leading cause of cancer death worldwide. Although preventive colonoscopy screening has improved the survival rate of CRC patients in the past few years, there are still many patients diagnosed after symptoms appear. The surgery for CRC carries high risks and high recurrence, and ideal therapies remain to be developed. The Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway has become a focus of research due to its central role in cellular activities. As a classic oncogenic pathway, the JAK/STAT signaling pathway offers new possibilities for diagnosing and treating various malignancies, and it paves a new way for developing therapies for CRC. This pathway not only participates in basic cellular processes, such as proliferation, differentiation, and apoptosis but also plays a crucial role in immune responses and inflammation. Abnormal activation of the JAK/STAT signaling pathway is closely related to the occurrence and development of CRC. Studies have shown that the active components and compound prescriptions of traditional Chinese medicine (TCM) can inhibit the proliferation, invasion, migration, and angiogenesis while promoting the apoptosis and autophagy of CRC cells by interfering with the JAK/STAT signaling pathway. Furthermore, this pathway may also play a role in regulating the sensitivity of tumor cells to chemotherapy and radiotherapy, thus influencing the effectiveness of tumor treatment and impeding the progression of CRC. In recent years, research results have been updated rapidly, and previous literature summaries have failed to incorporate the latest findings, creating obstacles to accessing current literature. Therefore, this article supplements and summarizes information from the definition of the JAK/STAT pathway, association of this pathway with CRC, and TCM intervention of CRC. This review aims to provide references for future development of molecular biology regarding CRC and the research and development of new drugs.
5.Prioritization of potential drug targets for diabetic kidney disease using integrative omics data mining and causal inference.

Diabetic kidney disease (DKD) with increasing global prevalence lacks effective therapeutic targets to halt or reverse its progression. Therapeutic targets supported by causal genetic evidence are more likely to succeed in randomized clinical trials. In this study, we integrated large-scale plasma proteomics, genetic-driven causal inference, and experimental validation to identify prioritized targets for DKD using the UK Biobank (UKB) and FinnGen cohorts. Among 2844 diabetic patients (528 with DKD), we identified 37 targets significantly associated with incident DKD, supported by both observational and causal evidence. Of these, 22% (8/37) of the potential targets are currently under investigation for DKD or other diseases. Our prospective study confirmed that higher levels of three prioritized targets-insulin-like growth factor binding protein 4 (IGFBP4), family with sequence similarity 3 member C (FAM3C), and prostaglandin D2 synthase (PTGDS)-were associated with a 4.35, 3.51, and 3.57-fold increased likelihood of developing DKD, respectively. In addition, population-level protein-altering variants (PAVs) analysis and in vitro experiments cross-validated FAM3C and IGFBP4 as potential new target candidates for DKD, through the classic NLR family pyrin domain containing 3 (NLRP3)-caspase-1-gasdermin D (GSDMD) apoptotic axis. Our results demonstrate that integrating omics data mining with causal inference may be a promising strategy for prioritizing therapeutic targets.
6.Clinical characteristics and prognosis of patients with spontaneous coronary artery dissection

Objective To analyze the clinical characteristics and prognosis of patients with spon-taneous coronary artery dissection(SCAD).Methods A retrospective analysis was conducted in 40 patients with acute myocardial infarction(AMI).All patients were diagnosed with SCAD during hos-pitalization through coronary angiography(CAG)or intravascular ultrasound(IVUS).The clinical characteristics,CAG or IVUS findings,and treatment regimens of the patients were analyzed,and their prognosis was assessed through follow-up.Results Among 40 SCAD patients,there were 11 males and 29 females,with a mean age of(53.04±9.15)years.Twelve patients had identified causative factors,and seven patients presented with chest pain as the initial symptom.CAG or IVUS results showed that a total of 69 SCAD lesions were detected in 40 patients,with 15 patients having a single lesion.The primary treatment approach for the patients was medical therapy.The median fol-low-up duration was 34 months(ranging 2 to 48 months),during which 5 major adverse cardiovascu-lar and cerebrovascular events(MACCE)occurred.The patients were divided into MACCE group(5 patients)and non-MACCE group(35 patients)based on occurrence of MACCE.The mean age and the incidence of regional wall motion abnormalities on echocardiography were higher in the MACCE group than in the non-MACCE group(P<0.05).Conclusion SCAD has distinct imaging features on CAG,and IVUS can confirm the diagnosis.Most patients are treated with medical therapy and have a generally favorable prognosis.However,patients with older age or regional wall motion abnormali-ties on echocardiography have poorer prognosis.
7.Effect of post-transplant diabetes mellitus on left ventricular longitudinal function and its prognostic value in heart transplant recipients

Objective:To evaluate the left ventricular(LV)function in heart transplant(HTx)patients with post-transplant diabetes(PTDM),and to examine the relevance of PTDM and LV function to the patient's prognosis.Methods:Two hundred and thirteen adult HTx patients who underwent echocardiography at Union Hospital,Tongji Medical College,Huazhong University of Science and Technology between January 2018 and January 2022 were prospectively included. The patients were divided into PTDM group( n=86)and Non-PTDM group( n=127). LV function parameters were acquired using conventional and two-dimensional speckle-tracking echocardiography(2D-STE),and were compared between the two groups. The primary endpoints included all-cause mortality or transplant-related readmission. Results:Compared with Non-PTDM group,the LV mass of PTDM group was higher,the LV ejection fraction,LV global longitudinal strain(GLS),peak systolic global longitudinal strain rate,and early diastolic global longitudinal strain rate(dGLSr)were lower(all P<0.05). After a median follow-up period of 37.6(29.3)months,27 patients experienced clinical events. A multivariate analysis revealed that PTDM( HR=2.198,95% CI=1.018-4.743, P=0.045)and low GLS( HR=6.456,95% CI=2.889-14.426, P<0.001)were independent predictors of adverse clinical events after adjustment for dGLSr,body mass index and age. After subdividing the two groups into 4 subgroups by the cutoff value of GLS(16.5%),the prognosis was worst for HTx patients with PTDM and low GLS. Conclusions:HTx patients with PTDM have worse LV systolic and diastolic function than those without PTDM. Management of HTx patients with PTDM may be improved using GLS guidance.
8.Efficacy and safety of eculizumab on acute attacks in patients with aquaporin-4 antibody positive neuromyelitis optica spectrum disorder

Objective:To evaluate the efficacy and safety of eculizumab in treating acute attacks of aquaporin-4 antibody(AQP4-IgG)positive neuromyelitis optica spectrum disorder(NMOSD).Methods:Six patients with AQP4-IgG-positive NMOSD treated with eculizumab at the Affiliated Hospital of Qingdao University from August 2023 to April 2024 were included.The patients' clinical characteristics, changes in scores of neurological function, pain and spasm before and after eculizumab treatment, and adverse drug reactions during the treatment period were described, and the data were subjected to Friedman test using SPSS 25.0.Results:The median score of expanded disability status scale in the six patients decreased from 3.25 before treatment to 1.25 after treatment, and the difference was statistically significant ( F=44.77, P<0.01).The functional status score showed noticeable improvement as early as week 1, demonstrating greater sensitivity to treatment response.Three patients experienced moderate to severe pain before treatment, with a mean pain score of 6, which reduced to 4.67 and 1.33 at week 2 and 6, respectively( F=29.17, P<0.05).Two patients reported spasms before treatment, with a mean score of 3.00, which decreased to 2.00, 1.50, and 1.00 at week 2, 3, and 6 after treatment, respectively( F=18.00, P>0.05).No serious adverse reactions were reported during the treatment. Conclusion:Eculizumab can effectively improve the neurological function and alleviate pain in patients with AQP4-IgG-positive NMOSD during acute attacks, and demonstrate good safety.
