1.Association of the joint effect of body fat levels and dyslipidemia with cardiovascular structural changes among children

Objective:
To investigate the association of the joint effect of body fat levels and dyslipidemia with cardiovascular structural abnormalities in children, so as to provide a scientific reference for the early prevention of cardiovascular damage.
Methods:
Based on the data from the second follow up (October 2021 to January 2022) of the Huantai Children Cardiovascular Health Follow up Cohort, 1 308 children with complete data were included. The fat mass percentage (FMP), fat mass index (FMI), subcutaneous fat mass (SFM) and visceral fat mass (VFM), left ventricular mass index (LVMI), relative wall thickness (RWT), thickening of carotid intima-media thickening (cIMT) , left ventricular hypertrophy (LVH) and left ventricular geometric remodeling (LVG), triglyceride (TG), total cholesterol (TC) ,high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) were obtained. Multivariable Logistic regression model was used to analyze the associations of FMP, FMI, SFM and VFM with thickening of cIMT, LVH and LVG. The joint effects of these body fat indicators and dyslipidemia on the aforementioned cardiovascular outcomes were further explored. Restricted cubic spline model was used to examine the dose response relationships between body fat levels and cardiovascular structural abnormalities.
Results:
Elevated body fat levels were significantly associated with an increased risk of cardiovascular structural abnormalities, exhibiting J shaped dose response relationships (all P <0.05). Compared with the group with normal body fat and normolipidemia, the risks of thickening of cIMT, LVH, and LVG in the group with elevated FMP combined with dyslipidemia were higher[ OR (95% CI )=11.70 (6.49-21.27), 5.53 (2.97-10.17), 2.33 (1.30-4.05)]; in the group with elevated FMI combined with dyslipidemia, the corresponding risks were higher[ OR (95% CI )= 11.68 (6.43-21.38), 6.98 (3.73-12.92), 2.65 (1.50-4.61)]; in the group with elevated SFM combined with dyslipidemia, the corresponding risks were higher[ OR (95% CI )=10.55 (5.83-19.24), 5.11 (2.71-9.45), 1.99 (1.11- 3.46 )]; and in the group with elevated VFM combined with dyslipidemia, the corresponding risks were higher[ OR (95% CI )=12.44 (6.76-23.14), 6.17 ( 3.31 -11.38), 2.30 (1.30-3.99)] (all P <0.05). Sex stratified analyses showed that the risk of thickening of cIMT in the combined exposure group of all four body fat indicators and dyslipidemia was significantly higher in girls than in boys (all P <0.01).
Conclusions
Elevated body fat levels and dyslipidemia have a combined effect in children, collectively increasing the risk of cardiovascular structural abnormalities. Prevention of cardiovascular damage in children should focus on both adiposity management and blood lipid regulation.
2.CLIC5 Overexpression Suppresses Malignant Development of Lung Adenocarcinoma Cells

Objective To explore the role of chloride intracellular channel 5 (CLIC5) in the development of lung adenocarcinoma (LUAD). Methods The expression levels of CLIC5 in LUAD samples were determined using bioinformatics and qRT-PCR. The correlation between CLIC5 expression and LUAD progression was revealed through survival analysis and clinicopathological feature analysis. After the knockdown or overexpression of CLIC5 in the LUAD cells, cell proliferation was assessed through colony formation assay, cell apoptosis rate by flow cytometry, and cell migration and invasion by scratch and Transwell invasion assay. The protein levels of vimentin and Snail were detected by Western blot. Results Bioinformatics analysis results showed low CLIC5 expression levels in the LUAD samples. This finding was associated with the aggressive progression of LUAD and the poor survival of patients with LUAD. The qRT-PCR results confirmed that CLIC5 was mainly expressed in the cytoplasm of the LUAD cells. CLIC5 overexpression inhibited cell activity and proliferation and promoted the apoptosis of LUAD cells. The knockdown of CLIC5 exhibited the opposite effect. CLIC5 overexpression inhibited the migration and invasion of LUAD cells, downregulated Bcl-2, vimentin, and Snail and upregulated Bax, cleaved caspase-3 and E-cadherin in the LUAD cells. CLIC5 knockdown exhibited opposite effects. Conclusion CLIC5 overexpression promotes cell apoptosis and inhibits cell proliferation and invasion, thereby inhibiting the development of LUAD. Hence, CLIC5 is a novel biomarker of prognosis and therapeutic target for LUAD.
3.The Role of Gut Microbiota in Male Erectile Dysfunction of Rats

Purpose:
Erectile dysfunction (ED) is a common male sexual dysfunction. Gut microbiota plays an important role in various diseases. To investigate the effects and mechanisms of intestinal flora dysregulation induced by high-fat diet (HFD) on erectile function.
Materials and Methods:
Male Sprague–Dawley rats aged 8 weeks were randomly divided into the normal diet (ND) and HFD groups. After 24 weeks, a measurement of erectile function was performed. We performed 16S rRNA sequencing of stool samples. Then, we established fecal microbiota transplantation (FMT) rat models by transplanting fecal microbiota from rats of ND group and HFD group to two new groups of rats respectively. After 24 weeks, erectile function of the rats was evaluated and 16S rRNA sequencing was performed, and serum samples were collected for the untargeted metabolomics detection.
Results:
The erectile function of rats and the species diversity of intestinal microbiota in the HFD group was significantly lower, and the characteristics of the intestinal microbiota community structure were also significantly different between the two groups. The erectile function of rats in the HFD-FMT group was significantly lower than that of rats in the ND-FMT group. The characteristics of the intestinal microbiota community structure were significantly different. In the HFD-FMT group, 27 metabolites were significantly different and they were mainly involved in the several inflammation-related pathways.
Conclusions
Intestinal microbiota disorders induced by HFD can damage the intestinal barrier of rats, change the serum metabolic profile, induce low-grade inflammation and apoptosis in the corpus cavernosum of the penis, and lead to ED.
4.Clinical efficacy of donafenib combined with PD-1 inhibitor and vascular intervention therapy in the treatment of unresectable hepatocellular carcinoma

OBJECTIVE To observe the clinical efficacy of donafenib combined with programmed death-1 (PD-1) inhibitors and vascular intervention therapy in the treatment of unresectable hepatocellular carcinoma (HCC). METHODS This retrospective study included 165 patients with unresectable HCC who were treated at the Fourth and First Affiliated Hospitals of Soochow University between June 2022 and March 2023. Among them, 89 patients received PD-1 inhibitors (tislelizumab or sintilimab, similarly hereinafter) plus vascular intervention (control group) and 76 patients received donafenib in combination with PD-1 inhibitors and vascular intervention (observation group). Short-term efficacy (3 months after treatment), long-term efficacy (2 years after treatment), the levels of liver function indexes [serum alanine amino-transferase (ALT), aspartate transferase (AST), and total bilirubin (TBil)] and tumor biomarkers [alpha fetoprotein (AFP), carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and des-gamma-carboxy prothrombin (DCP)] before treatment and after 3 months of treatment, as well as the occurrence of adverse drug reaction (ADR) during treatment, were compared between the two groups. In addition, overall response rate (ORR) stratified by PD-1 inhibitor type was analyzed. RESULTS After treatment, the ORR was significantly higher in the observation group than in the control group (P<0.05); although the disease control rate was higher in the observation group compared to the control group, the difference was not statistically significant (P>0.05). The median overall survival of patients in the observation group was 16.9 months [95% confidence interval (CI): 14.2 to 19.1 months], which was significantly longer than that in the control group (12.4 months, 95%CI: 10.1 to 15.3 months) (P<0.05). Subgroup analysis result indicated that therapeutic advantage was consistent across both sintilimab and tislelizumab subgroups, with no significant heterogeneity (P>0.1, I 2<0.001%). Before treatment, there were no significant differences in liver function indexes or tumor marker levels between 2 groups (P>0.05). After treatment, both groups showed significant declines in these indicators compared with baseline (P<0.05), with greater reductions observed in the observation group (P<0.05). There were no statistically significant differences in overall incidence of ADR and grade ≥3 ADRs between the two groups (P>0.05). CONCLUSIONS For patients with unresectable HCC, the combination of donafenib, PD-1 inhibitors and vascular intervention therapy may achieve superior clinical outcomes without increasing the risk of treatment-related ADR.
5.Identification of the MYB transcription factor family involved in response to salt stress in Picea mongolica.

Picea mongolica, known for its remarkable tolerance to cold, drought, and salinity, is a key species for ecological restoration and urban greening in the "Three Norths" region of China. MYB transcription factors are involved in plant responses to abiotic stress and synthesis of secondary metabolites. However, studies are limited regarding the MYB transcription factors in P. mongolica and their roles in salt stress tolerance. In this study, 196 MYBs were identified based on the genome of Picea abies and the transcriptome of P. mongolica. Phylogenetic analysis classified the MYB transcription factors into seven subclasses. The R2R3-MYB subclass contained the maximum number of genes (84.77%), while the R-R and R1R2R3 subclasses each represented the smallest proportion, at about 0.51%. The MYB transcription factors within the same subclass were highly conserved, exhibiting similar motifs and gene structures. Experiments with varying salt stress gradients revealed that P. mongolica could tolerate the salt concentration up to 1 000 mmol/L. From the transcriptome data of P. mongolica exposed to salt stress (1 000 mmol/L) for 0, 3, 6, 12, and 24 h, a total of 34 differentially expressed MYBs were identified, which suggested that these MYBs played a key role in regulating the response to salt stress. The proteins encoded by these differentially expressed genes varied in length from 89 aa to 731 aa, with molecular weights ranging from 10.19 kDa to 79.73 kDa, isoelectric points between 4.80 and 9.91, and instability coefficients from 41.20 to 70.99. Subcellular localization analysis indicated that most proteins were localized in the nucleus, while three were found in the chloroplasts. Twelve MYBs were selected for quantitative real-time PCR (qRT-PCR), which showed that their expression patterns were consistent with the RNA-seq data. This study provides valuable data for further investigation into the functions and mechanisms of MYB family members in response to salt stress in P. mongolica.

6.Impact of gamified behavior intervention based on mobile health on patients with coronary heart disease after percutaneous coronary intervention

Objective:To explore the effects of gamified behavior intervention based on mobile health on compliance, psychological status, and physical activity behavior of patients with coronary heart disease after percutaneous coronary intervention (PCI), providing a guidance for effectively improving the intervention effect of patients after PCI for coronary heart disease.Methods:The research plan was designed using a randomized, prospective, single blind, single center clinical research method. The patients with coronary heart disease who received treatment after PCI in the Second Affiliated Hospital of Air Force Medical University from January 2021 to November 2023 were selected as the study subjects by convenience sampling method and divided into the control group and the intervention group by the random number table method. The control group received routine intervention, while the intervention group received gamified behavior intervention based on mobile health on the basis of the control group. Both groups were intervened for 3 months and followed up for 3 months. The compliance, psychological status, physical activity behavior, and quality of life before intervention and after 3 months of follow-up, as well as the occurrence of adverse cardiovascular events during intervention and follow-up between two groups were compared.Results:A total of 88 patients with coronary heart disease after PCI were included, with 44 cases in both the control group and the intervention group. There were 23 males and 21 females in the control group with an age of (64.54 ± 4.32) years old. There were 24 males and 20 females in the intervention group with an age of (65.01 ± 4.36) years old. The scores of exercise, medication, diet, lifestyle compliance scores, physical activity behavior levels, the scores of social function, emotional function, physical function, role function, cognitive function after a 3-month follow-up in the intervention group were (95.32 ± 1.54) points, (95.07 ± 1.60) points, (93.43 ± 1.76) points, (95.03 ± 1.43) points, (1511.32 ± 113.54) metabolic equivalent/week, (94.03 ± 1.02) points, (93.42 ± 1.09) points, (92.21 ± 1.32) points, (93.42 ± 1.15) points, (88.54 ± 3.03) points, respectively, which were higher than the control group (77.43 ± 2.54) points, (78.31 ± 1.71) points, (74.32 ± 2.43) points, (75.14 ± 1.98) points, (1 432.65 ± 107.65) metabolic equivalent/week, (87.22 ± 1.21) points, (86.74 ± 1.30) points, (85.65 ± 3.43) points, (88.76 ± 1.09) points, (73.76 ± 3.43) points, the differences were statistical significant( t values were 3.34-54.02, all P<0.05). The scores of the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) after a 3-month follow-up in the intervention group were (32.54 ± 1.85), (31.54 ± 0.87) points, respectively, which were lower than the control group (43.65 ± 2.03), (41.54 ± 1.23) points in the control group, the differences were statistical significant ( t=26.83, 44.03, both P<0.05). The incidence of adverse cardiovascular events during the intervention and follow-up period was 4.55% (2/44), which was lower than the control group 18.18% (8/44), the differences was statistical significant ( χ2=4.06, P<0.05). Conclusions:The nursing effect of gamified behavior intervention based on mobile health on patients with coronary heart disease after PCI was good, which could effectively improve compliance, promote the improvement of patients′ psychological state and physical activity behavior, reduce the occurrence of adverse cardiovascular events, further improve their quality of life.

Objective:To evaluate the value of ultrasound radiomics combined with machine learning for early diagnosis of seronegative Hashimoto’s thyroiditis (SN-HT) .Methods:This retrospective study included 164 patients from Liaoning Provincial People’s Hospital , Lixin County People’s Hospital, Linghai Dalinghe Hospital, Fengcheng Phoenix Hospital, who underwent thyroidectomy for solitary nodules with normal thyroid function between Nov. 2016 and Jan. 2024. Postoperative pathology confirmed Hashimoto’s thyroiditis (HT) in some cases, who were further categorized into antibody-positive and antibody-negative groups based on serum antibody status. Patients without Hashimoto’s thyroiditis served as the control group. A total of 298 ultrasound images were analyzed. Radiomics features were extracted from hypoechoic non-nodular areas within 0.5 cm surrounding the tumor. Two senior pathologists and two senior ultrasound physicians independently assessed lymphocytic infiltration, eosinophilic changes of follicular epithelium, and the proportion of hypoechoic areas in pathology and ultrasound images, respectively. A machine learning model, CCH-NET, was developed using linear regression and t-distributed stochastic neighbor embedding (t-SNE) techniques. The dataset was divided into a training set (80%) and a validation set (20%) to compare the diagnostic accuracy of CCH-NET with that of senior ultrasound physicians. Results:In internal validation, CCH-NET achieved a diagnostic accuracy of 88.89% for both antibody-positive and antibody-negative groups, significantly higher than the 66.67% accuracy of senior ultrasound physicians ( P<0.01). In external validation, CCH-NET achieved 75.00% and 66.67% accuracy for the two groups, compared to 50.00% by senior ultrasound physicians. For the control group, both methods achieved 93.33% accuracy. The AUC of CCH-NET was 0.848, outperforming senior ultrasound physicians (0.681) ,demonstrating superior diagnostic performance. Conclusion:The radiomics-based CCH-NET model, using non-nodular hypoechoic areas as a specific indicator, can accurately identify early SN-HT in euthyroid patients. It significantly outperforms senior ultrasound physicians, improving diagnostic accuracy and reducing missed diagnoses.
8.Thrombocytopenia in Liver Cirrhotic Patients Induced by Linezolid

Objective To investigate the risk factors and countermeasures of linezolid-induced thrombocytopenia.Methods Clinical pharmacists participated in the pharmacy practice of linezolid-induced thrombocytopenia in a patient with liver cirrhosis.They analyzed the risk factors,took timely countermeasures,and conducted pharmaceutical care.Results The liver cirrhosis,low creatinine clearance and basic platelet values were closely related to linezolid-related thrombocytopenia,platelet recovered after discontinuation.Linezolid(0.3 g,Ⅳ,q12h)was used again under serum drug concentration monitoring,and the infection was effectively controlled,no thrombocytopenia occurred.Conclusions Clinical pharmacists should conduct pharmaceutical care for patients,pay close attention to adverse reactions,and timely adjust the treatment plan to better ensure the effectiveness and safety of treatment.
9.Analysis of the effect of correcting of nasal septum deviation combined with temperature-controlled ablation radio-frequency in the treatment of chronic sinusitis with nasal septum deviation

Objective To explore and analyze the application value of correcting of nasal septum deviation under nasal endoscopy combined with temperature-controlled ablation radio-frequency in treatment of chronic sinusitis.Methods From June 2020 to June 2023,200 patients with chronic sinusitis and nasal septum deviation who underwent nasal endoscopic sinus surgery were randomly divided into control group(combined with nasal endoscopic sinus surgery)and observation group(combined with temperature-controlled ablation radio-frequency),100 cases in each group.The efficacy,nasal mucosal function,olfactory function,airway responsiveness and complications were compared between the two groups.Results The observation group has a better therapeutic effect than that in control group,the difference was statistically significant(P<0.05);After surgery,compared with the control group,the observation group had a shorter saccharin clearance time(SCT),faster mucociliary clearance transport(MCT),higher nasal mucociliary transport rate(MTR),the differences were statistically significant(P<0.05);After surgery,the observation group had better olfactory function grading than the control group,the difference was statistically significant(P<0.05);The airway resistance at 5 Hz(R5),airway resistance at 20 Hz(R20),and resonance frequency(Fres)of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05);There was no significant difference in complications between the two groups(P>0.05).Conclusion The combination of endoscopic nasal septum correction and temperature-controlled ablation radio-frequency for the treatment of chronic sinusitis has better clinical efficacy than endoscopic sinus surgery.It has significant advantages in improving nasal mucosal function,olfactory function,and reducing airway responsiveness,and is highly safe.It is worthy of clinical promotion and application.
10.Research progress on vascular inflammation in the pathogenesis of aortic dissection

At present,with the development of new diagnostic technologies,the detection rate of aortic dissection has been increasing year by year,but its mortality rate still remains high.Cardiovascular disease is a chronic inflammato-ry disease,and vascular inflammation plays a major role in the progression of aortic dissection.Therefore,this article sys-tematically describes the specific roles and mechanisms of inflammatory cells,inflammatory factors,and inflammasomes in the development of aortic dissection.
