1.Current status of advanced study personnel engaging in hospital-acquired infection control in a three-A hospital

OBJECTIVE To investigate and analyze the current situation and problems of the advanced personnel en-gaging in the hospital-acquired infection control during their training period and explore the existing countermeas-ures and future development.METHODS The literatures regarding to the advanced study in China were retrieved from databases,the subjects of the literatures covered infection control-related advanced study practice,discipline construction,position competence,talent cultivation,scientific research innovation,professional title evaluation,laws,regulations and development plans.From Aug.2024 to Nov.2024,a questionnaire survey and face-to-face interviews were conducted among 36 advanced study personnel from 9 provinces of China who engaged in hos-pital-acquired infection control in the First Medical Center of Chinese PLA General Hospital.Eventually,36 ques-tionnaires were retrieved,all of which were valid with a questionnaire recovery rate of 100.00%.RESULTS Among the 36 advanced study personnel of hospital-acquired infection control,58.33%were medium-grade professional ti-tle;preventive medicine(41.67%),clinical medicine(25.00%)and nursing(16.67%)ranked the top 3 majors.The personnel engaged in the infection control for more than 6 years,and the duration of the advanced study was generally 3 or 6 months.In reality,the personnel faced the choices in terms of the purposes of further education,learning approaches and learning contents.The advanced study personnel also encountered the problems of challenges from promotion,improvement of position competency,integration with clinical training,supervision and practice,as well as physiological,psychological and family pressure.CONCLUSION Aiming at the problems that the advanced study personnel are generally concerned about,such as how to scientifically and effectively carry out hospital-acquired infection control advanced study and preset and solve the problems that may encounter,it is necessary to formulate targeted training programmes so as to provide bases and enlightenment for establishment of a long-term mechanism for advanced study of infection control in China.
2.Current status and influencing factors for water contamination in humidification tanks of ventilators of invasive mechanical ventilation patients

OBJECTIVE To observe the current status and influencing factors for the water contamination of humid-ification tanks of ventilators of the patients undergoing invasive mechanical ventilation.METHODS Totally 139 hu-midified water samples were collected from the patients who received invasive mechanical ventilation in The First Medical Center of Chinese PLA General Hospital from Jul.2024 to Nov.2024.The humidified water samples were collected,cultured and identified on Day 1,3,5 and 7 after humidification tanks replacement.RESULTS Among the 139 samples,25 were tested positive for pathogens,with the positive test rate 17.99%;the positive test rate was highest(33.33%,8/24)in respiratory intensive care unit.The positive test rate was 12.90%on Day 1,18.42%on Day 3,15.00%on Day 5 and 26.67%on Day 7,showing an upward trend.The result of bacterial culture showed that the percentage of isolated Burkholderia cepacia was highest(36.00%).The result of analysis of the related factors for positive test of humidified water samples showed that the sample adding pattern was as-sociated with the isolation rate(P<0.05).Only 1 case was diagnosed with ventilator-associated pneumonia(VAP)after the humidified water was detected with pathogens and varied in the pathogens.CONCLUSIONS There is an association between the status of water contamination of the ventilator humidification tanks and usage duration of humidification tank.It is necessary to attach great importance to the contamination of humidification tanks,carry out standard management,and implement the prevention and control measures in an all-round way.

4.SIRT3 protects endometrial receptivity in patients with polycystic ovary syndrome.

BACKGROUND:
The sirtuin family is well recognized for its crucial involvement in various cellular processes. Nevertheless, studies on its role in the human endometrium are limited. This study aimed to explore the expression and localization of the sirtuin family in the human endometrium, focusing on sirtuin 3 (SIRT3) and its potential role in the oxidative imbalance of the endometrium in polycystic ovary syndrome (PCOS).
METHODS:
Endometrial specimens were collected from both patients with PCOS and controls undergoing hysteroscopy at the Center for Reproductive Medicine, Peking University Third Hospital, from July to August 2015 and used for cell culture. The protective effects of SIRT3 were investigated, and the mechanism of SIRT3 in improving endometrial receptivity of patients with PCOS was determined using various techniques, including cellular bioenergetic analysis, small interfering ribonucleic acid (siRNA) silencing, real-time quantitative polymerase chain reaction, Western blot, immunofluorescence, immunohistochemistry, and flow cytometry analysis.
RESULTS:
The sirtuin family was widely expressed in the human endometrium, with SIRT3 showing a significant increase in expression in patients with PCOS compared with controls ( P <0.05), as confirmed by protein and gene assays. Concurrently, endometrial antioxidant levels were elevated, while mitochondrial respiratory capacity was reduced, in patients with PCOS ( P <0.05). An endometrial oxidative stress (OS) model revealed that the downregulation of SIRT3 impaired the growth and proliferation status of endometrial cells and reduced their receptivity to day 4 mouse embryos. The results suggested that SIRT3 might be crucial in maintaining normal cellular state by regulating antioxidants, cell proliferation, and apoptosis, thereby contributing to enhanced endometrial receptivity.
CONCLUSIONS
Our findings proposed a significant role of SIRT3 in improving endometrial receptivity in patients with PCOS by alleviating OS and regulating the balance between cell proliferation and apoptosis. Therefore, SIRT3 could be a promising target for predicting and improving endometrial receptivity in this patient population.

6.Impact of female reproductive tract microbiota and cytokine disorders on recurrent implantation failure

Objective:To explore the changes in the functional composition of genital tract microbiota in patients with recurrent implantation failure (RIF) and its relationship with cytokines related to immune regulation.Methods:This was a case-control study. The study included patients from the Reproductive Medicine Center of Peking University Third Hospital, enrolled from August 2020 to January 2021. The enrolled patients were divided into control group ( n=20) and the RIF group ( n=15). The study examined the expression of flora and cytokines in vaginal and uterine lavage fluid by 16S rRNA sequencing and MSD electrochemiluminescence. Spearman analysis was used to observe the correlation between flora and differently expressional cytokines. Results:Compared with control group, there were statistically significant differences in the Shannon index of vaginal microbiota and uterine lavage fluid microbiota in the RIF group ( P=0.032, P=0.015). There was a statistically significant difference in β diversity of the bacterial community in uterine lavage fluid ( P=0.020). The expression level of monocyte chemotactic protein-4 (MCP-4) in the RIF group [(3.39±1.23) ng/L] was significantly higher than that in control group [(1.07±0.31) ng/L, P=0.044], while the expression level of interleukin (IL)-17A [(1.12±0.29) ng/L] was significantly lower than that in control group [(2.70±0.52) ng/L, P=0.040]. The expression levels of interferon (IFN)-γ [(4.70±2.10) ng/L], IL-10 [(0.20±0.10) ng/L], tumor necrosis factor-α [(0.84±0.13) ng/L], and IL-5 [(0.35±0.05) ng/L] in the RIF group in the uterine lavage were significantly higher than those in control group [(0.97±0.30) ng/L, P=0.049; (0.05±0.01) ng/L, P=0.009; (0.56±0.07) ng/L, P=0.045; (0.23±0.03) ng/L, P=0.029]. Correlation analysis revealed significant correlations between vaginal flora and MCP-4 ( r=-0.42, P=0.025), and in the microbial community of uterine cavity lavage fluid, there was a significant positive correlation between the Muribaculaceae genus and IL-5 ( r=0.51, P=0.017). Conversely, the Vulcaniibacterium genus, Cupriavidus genus, and Anoxybacillus genus exhibited a significantly negative correlation with IL-5 ( r=-0.55, P=0.010; r=-0.62, P=0.003; r=-0.45, P=0.041). Additionally, the genera Anoxybacillus, Geobacillus, Thermus, and Meiothermus showed a significantly negative correlation with IFN-γ ( r=-0.43, P=0.015; r=-0.38, P=0.035; r=-0.39, P=0.029; r=-0.38, P=0.035). Conclusion:The reproductive tract microbiota and corresponding cytokines in RIF patients are disrupted, and there is a correlation between the microbiota and cytokines.

8.Development and reliability and validity of the Exercise Knowledge and Behavior Questionnaire for Osteoporosis Patients

Objective:To develop the Exercise Knowledge and Behavior Questionnaire for Osteoporosis Patients suitable for China, and to test its reliability and validity.Methods:Guided by the theory of knowledge, attitude and practice, a questionnaire framework was constructed, and the first draft of the Exercise Knowledge and Behavior Questionnaire for Osteoporosis Patients was formed through literature review, group discussion, semi-structured interview, Delphi expert consultation and pre-investigation. From June to August 2020, convenience sampling was used to select 120 osteoporosis patients admitted to the First Affiliated Hospital of Soochow University as the research object to conduct a questionnaire survey to test the reliability and validity of the questionnaire. A total of 120 questionnaires were distributed, 110 valid questionnaires were recovered, and the valid recovery rate was 91.67% (110/120) .Results:Among 2 rounds of Delphi expert consultation, the expert authority coefficients were 0.880 and 0.900, respectively, and the Kendall harmony coefficients were 0.308 and 0.332, respectively, with statistically significant differences ( P<0.01) . The Exercise Knowledge and Behavior Questionnaire for Osteoporosis Patients included two sub-questionnaire of knowledge and behavior, with a total of 19 items, including 10 items in knowledge and 9 items in behavior. Exploratory factor analysis showed that 2 common factors were extracted from the exercise knowledge sub-questionnaire, and the cumulative variance contribution rate was 60.712%. A total of one common factor was extracted for the exercise behavior sub-questionnaire, and the cumulative variance contribution rate was 72.075%. The content validity index of the total questionnaire and each sub-questionnaire were 0.925, 0.921, and 0.929, respectively, and the item-level content validity index of was 0.857 to 1.000. The Cronbach's α coefficients of the total questionnaire and each sub-questionnaire t were 0.935, 0.830, and 0.953, respectively, and the split-half reliability coefficients of the total questionnaire and each sub-questionnaire were 0.979, 0.898, and 0.974, respectively, and the test-retest reliability coefficients of the total questionnaire and each sub-questionnaire were 0.878, 0.813, 0.860, respectively. Conclusions:The Exercise Knowledge and Behavior Questionnaire for Osteoporosis Patients has good reliability and validity, and can be used to evaluate exercise knowledge and behavior in patients with osteoporosis.
9.Phenomenological study of effective communication between doctors and nurses in emergency treatment of gynecology patients

Objective To explore the main factors of the medical communication between physicians and nurses in gynecological emergency treatments. To evaluate the function of SBAR communication mode in enhancing communication skills and to explore the countermeasures of promotion. Methods Totally 18 gynecological physicians and nurses were selected as the subjects in this study,and semi-structured interviews were conducted. After the establishment of coding and archiving system, the collected information was extracted and analyzed. Results The factors that affect gynecologic emergency medical staff effective communication included assessment of the patients′ condition, the limitation of communication conditions,the different concerns between doctors and nurses,the differences of medical communication training degree,seniority,experience and knowledge reserve,the medical team consciousness. Conclusion The effective communication of gynecologic emergency care can be promoted by improving professional levels of medical care personnel,strengthening the cultivation of the health care team,using communication skills,to ensure the accurate transfer of key information.
10.Effect comparison of two types of continuing care on rehabilitation efficacy for women with genital malignancies

Objective To explore the influence of two continuing care methods on the rehabilitation in female genital malignant tumor patients.Methods A total of 30 patients from June through December 2016 were included in control group which given continuing nursing care,another 30 patients from January through June 2017 were included in the experimental group which given continuing nursing care by WeChat.The urinary tract infection and quality of life of the two groups after chemotherapy were observed.Results The physiological status,social/family status,emotional status,quality of life score and treatment compliance of experimental group were higher than that in the control group,the incidence rate of urinary tract infection outside hospital of experimental group was lower than the control group (P < 0.05).Conclusion The continuing nursing mode of WeChat can better improve the life quality of patients with malignant tumors,improve patients'compliance,reduce the incidence of urinary tract infection,and has the advantages of simplicity,economy,and lower visit lose.
