1.A study of a comprehensive nutrition education program in patients undergoing pancreaticoduodenectomy

Objective Based on the intervention map to develop a comprehensive nutrition education program for pancreaticoduodenectomy patients and to explore the effect of its clinical application,aiming at providing references for clinical nursing practice.Methods A convenience sampling method was used to select 76 patients who were to undergo pancreaticoduodenectomy in the department of hepatobiliary and pancreatic surgery of a tertiary hospital in Anhui Province as the study subjects.The 38 patients admitted from October 2021 to September 2022 were in a control group,and the 38 patients admitted from October 2022 to July 2023 were in an experimental group.The experimental group received the comprehensive nutritional education programme constructed in this study,and the control group used conventional nutrition health education measures,and the length of intervention for both groups was from pre-hospitalization to discharge for 6 months.Nutrition-related indicators,postoperative complications,hospitalisation time,hospitalisation costs and satisfaction were compared between the 2 groups.Results A total of 64 patients completed this study,with 33 in the experimental group and 31 in the control group.Repeated measurement analysis of variance showed that the interaction effects of BMI,total serum protein and serum preprotein were statistically significant(P<0.05).The incidence of complications,hospitalization days and hospitalization costs of the experimental group were lower than those of the control group(P<0.05).The scores of nutrition education-related satisfaction in the experimental group were higher than those in the control group(P<0.001).Conclusion A comprehensive nutritional education program based on intervention map can improve the nutritional status of pancreaticoduodenectomy patients to a certain extent,reduce the occurrence of complications,and promote patients'recovery.
2.A Comparitive Study Between Laparoscopic Assisted Ileostomy Closure and Open Surgery

Objective To explore the application value of laparoscopic assisted ileostomy closure after prophylactic ileostomy.Methods A retrospective analysis was conducted on 63 cases of middle and low rectal cancer who received ileostomy closure after prophylactic ileostomy in natural orifice specimen extraction surgery(NOSES)from September 2017 to May 2023.Among them,31 cases underwent laparoscopic assisted ileostomy closure(observation group),and 32 cases underwent conventional open ileostomy closure(control group).The operative time,intraoperative blood loss,time to first ambulation,time to first flatus,time to first liquid diet,postoperative pain score,postoperative hospital stay time,and postoperative complications were compared between the two groups.Results All the 63 cases successfully underwent ileostomy closure.The observation group showed significantly better outcomes than the control group in operative time[(63.2±5.7)min vs.(93.5±4.7)min,t=-23.109,P=0.000],intraoperative blood loss[7.0(6.0,8.0)ml vs.22.5(21.0,24.0)ml,Z=-6.853,P=0.000],time to first ambulation[1.0(1.0,1.0)d vs.2.0(2.0,2.0)d,Z=-5.653,P=0.000],time to first flatus[1.0(1.0,2.0)d vs.2.0(2.0,2.0)d,Z=-5.304,P=0.000],time to first liquid diet[2.0(2.0,3.0)d vs.3.0(2.0,3.0)d,Z=-3.000,P=0.003],postoperative pain score[24 h:3.0(3.0,4.0)vs.4.0(3.0,4.0),Z=-4.501,P=0.000;48 h:2.0(2.0,2.0)vs.3.0(2.0,3.0),Z=-3.750,P=0.000;72 h:1.0(1.0,2.0)vs.2.0(2.0,2.0),Z=-2.996,P=0.003],and postoperative hospital stay[(6.8±1.6)dvs.(8.5±1.5)d,t=-4.297,P=0.000].The observation group had a lower postoperative incision infection rate than the control group[3.2%(1/31)vs.34.4%(11/32),x2=9.908,P=0.002],while no significant differences were observed in incision dehiscence,intestinal obstruction,or abdominal hemorrhage(P>0.05).Conclusions For patients with middle and low rectal cancer who undergoing ileostomy closure after prophylactic ileostomy in NOSES,laparoscopic assisted ileostomy closure is safe and feasible.Compared with open surgery,it reduces incision infection rate,alleviates postoperative pain,shortens hospital stay,and promotes recovery.
3.Free inferior gluteal perforator flap for immediate breast reconstruction: a case report and literature review

Objective:To explore the possibility of using a inferior gluteal artery perforator flap (IGAPF) for breast reconstruction in the patient who did not have suitable donor site in back and abdomen.Methods:In November 2024, a 25-year-old unmarried and childless woman with right breast cancer received immediate right breast reconstruction by a right free IGAPF after modified right mastectomy in the Department of Breast and Thyroid Surgery, Second Affiliated Hospital of Hainan Medical University. The locations of perforators were confirmed by both Multi-detector computed tomography angiography (MDCTA) and portable Doppler blood flow detector before surgery. The IGAPF was designed to take the inferior gluteal wrinkle as the lower edge, the axis of the flap was parallel to the inferior gluteal wrinkle, and the width of the flap was estimated where the incision could be directly closed. The size of right IGAPF was 6.0 cm×19.0 cm. Sharp dissection was performed between the sarcolemma and muscle fibres of gluteus, then the perforators were dissected along the direction of muscle fibres of gluteus. The vascular pedicle was kept at about 8.0 cm in length. The diameter of artery was about 2.0 mm and that for the veins was about 1.5 mm. End-to-end anastomoses with the right thoracodorsal artery and vein were successfully carried out. The donor site was directly closed, and it was hidden in the inferior gluteal wrinkle. Postoperative outpatient clinical review was made.Results:Pathological examination reported: an invasive carcinoma of right breast, axillary lymph node metastasis (2/10). The patient recovered well and the flap survived without any complication, i.e. ischemic necrosis, infection and haematoma. The patient was off-bed at 3 days and discharged at 13 days after surgery. At the 40 days of postoperative follow-up, the patient achieved a good recovery and the lower limb activity was not affected by the surgery. The patient was satisfied with the reconstructed breast and donor site recovery. The patient followed with scheduled chemotherapy and subsequent radiotherapy. The volume of reconstructed breast was smaller than the other breast, of which the patient was fully informed before the surgery.Conclusion:A free IGAPF provides an alternative donor sites for achieving a breast reconstruction due to the reliable pedicle vessels and invisible donor scars.

7.A review of human amniotic mesenchymal stem cells in the treatment of acute lung injury

BACKGROUND:In recent years, with the in-depth research on thecharacteristics and functions of stem cells,stem cell research has become a hot research targeting thetreatment of a variety of diseases.OBJECTIVE:To summarize the research progress of human amnioticmesenchymal stem cells in the treatment of acute lung injury.METHODS:The PubMed and CNKI databases were searched by computerfor relevant literatures published from January 2000 to January 2017. The key words were mesenchymal stem cells, acute lung injury in Chinese and English,respectively.RESULTS AND CONCLUSION:As an important member of adult stem cells, human amniotic mesenchymal stem cells not only have the advantages and characteristics of most other stem cells, but also have unique advantages, such as these cells can be extracted from a vast of sources, require only non-invasive operations, and involve no violation of ethics. Human amniotic mesenchymal stem cells can be driven by inflammation reaction and homing to the damaged tissue, and be activated by inflammatory factors to secrete a variety of cytokines in a paracrine fashion, which inhibit neutrophils, macrophages and other immune cell migration and inhibit dendritic cell maturity, and reduce the proliferation of NK cells, B lymphocytes and T lymphocytes, and up-regulate anti-inflammatory cytokines and down-regulate proinflammatory cytokines, and alleviate alveolar inflammatory injury through anti-oxidation and anti-apoptotic processes. Human amniotic mesenchymal stem cells can also assist in the repair or homing to the damaged tissue to directionally differentiate to replace the apoptotic and necrotic cells. All of these characteristics of human amniotic mesenchymal stem cells provide new insight into the treatment of acute lung injury. As acute lung injury is caused by many pathogenic factors and its pathophysiological process is very complicated, human amniotic mesenchymal stem cell transplantation in the treatment of acute lung injury has many issues to be resolved.

High‐speed economic development and progress of medical technologies in China have significantly benefitted people′s health conditions.Significantly higher life expectancy in average ,as a result ,also contributed to population aging process.How to deal with the contradictions between population aging and rapid growth of medical costs is challenging the healthcare sector of the country.Analysis of medical costs at three tertiary hospitals in Liaoning Province discovered problems in out‐of‐pocket medical expenses of elderlies ,and proposed on measures to effectively ease their burden in medical expenditure .
9.Comparison of Effect on Antioxidative Stress Between Benezepril and Tranilast in Diabetic Nephropathy Rat

Objective To investigate ACEI ( benazepril ) and tranilast exert renoprotective properties in diabetic nephropathy( DN) through the inhibition of thioredoxin( Trx) . Methods Forty male SD rats were randomly divided into normal control group,model control group,tranilast group and benazepril group (n=10 each).Normal control group was fed with normal diet. Other groups were fed with high-glucose high-fat diet to make DN models. Rats in model control, tranilast, and benazepril groups were fed with normal diet,400 mg??kg-1??d-1 tranilast plus normal diet,and 10 mg??kg-1??d-1 benazepril plus normal diet,respectively,via oral gavage for 12 weeks.The 24-hour proteinuria,blood glucose(BG),blood urea nitrogen (BUN),serum creatinine ( Scr) and renal pathology changes were detected. Expression of Trx was measured by Western-blot. Results The 24 h urine protein, BG, BUN, Scr, kidney/body weight, and glomerular sclerosis index were significantly decreased in tranilast group and benazepril group,as compaired with model control group ( P<0.05) ,but there was no statistical difference between the two drug groups (P>0.05).Both tranilast and benazepril can reduce renal pathological changes,and can increase the expression of Trx of DN rats, but benazepril had a more significant effect on increasing Trx expression. Conclusion Both tranilast and benazepril have renoprotective function in DN, and benazepril is more effective than tranilast in delaying the progression of diabetic nephropathy by increasing Trx expression and decreasomg oxidative stress.
10.The investigation of exercise-induced fatigue on pesticide spraying operation.

OBJECTIVETo evaluate the exercise-induced fatigue in operators during pesticide spraying based on surface electromyography (sEMC) signals and heart rate.
METHODSThe sEMG signals were recorded from the right trapezius, biceps brachii, extensor digitorum, and flexor digitorum of 19 operators by portable electromyography units during pesticide spraying using 3WS-16 manual sprayers. Meanwhile, the heart rate of operators was measured by the ambulatory blood pressure monitoring system. The changes in median frequency and heart rate were analyzed.
RESULTSPesticide spraying was defined as low-intensity labor. The median frequency of the right trapezius, biceps brachii, extensor digitorum, and flexor digitorum decreased significantly as pesticide spraying went on (P < 0.05). The body remained in a state of slight fatigue from 8 min after pesticide spraying began to the end of the operation.
CONCLUSIONThe degree of exercise-induced fatigue of operators can be evaluated based on sEMG signals and heart rate during pesticide spraying.
