1.Effect of periacetabular osteotomy on pelvic sagittal tilt in developmental dysplasia of hip

BACKGROUND:Developmental dysplasia of hip refers to the deformity of acetabulum and proximal femur,which can lead to hip instability and hip osteoarthritis. Periacetabular osteotomy is the main treatment for non-terminal developmental dysplasia of hip in adolescents and adults. Pelvic inclination plays an important role in biomechanics of hip joint,which determines the stress direction of hip joint. In hip joint diseases,pelvic compensatory tilt can occur to minimize the abnormal stress of hip joint.OBJECTIVE:To observe whether periacetabular osteotomy changes pelvic sagittal tilt in patients with developmental dysplasia of hip,and to investigate the effect of acetabular covering on pelvic sagittal tilt.METHODS:A retrospective analysis was performed on 29 patients who underwent periacetabular osteotomy due to developmental dysplasia of hip in West China Hospital of Sichuan University from August 2019 to June 2022. Anteroposterior X-rays of the standing pelvis were collected before and 6 months after surgery. The pelvic tilt parameters,including the lateral center-edge angle,pubic symphysis to sacroiliac joint distance,pubic symphysis to sacroiliac joint midline distance,sacrofemoral-pubic angle,and pelvic tilt,were measured. Hip functions of all patients were assessed by hip disability and osteoarthritis score and Harris score before and 6 months after surgery.RESULTS AND CONCLUSION:(1) The lateral center-edge angle,pubic symphysis to sacroiliac joint midline distance,and sacrofemoral-pubic angle were significantly increased 6 months postoperatively compared to preoperative values (P<0.05). Tonnis angle and pelvic tilt were significantly decreased (P<0.05). There was no significant difference in pubic symphysis to sacroiliac joint distance between preoperative and postoperative measurements (P>0.05). (2) The hip disability and osteoarthritis scores and Harris scores were significantly increased 6 months after operation (P<0.01). (3) It is indicated that periacetabular osteotomy surgery can significantly improve acetabular coverage and the pelvis showed significant anteversion changes in patients with developmental dysplasia of hip.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
4.The effects of hip arthroscopy or periacetabular osteotomy in the treatment of borderline developmental dysplasia of the hip

Objective:To explore the clinical efficacy of hip arthroscopy (HA) and periacetabular osteotomy (PAO) in the treatment of borderline developmental dysplasia of the hip (BDDH).Methods:The clinical data of 57 patients with BDDH treated with HA or PAO from July 2016 to June 2020 were retrospectively studied. According to surgical methods, they were divided into HA group and PAO group. There were 32 patients (6 males and 26 females) in HA group, with an average age of 34.5±6.4 years, ranging from 19 to 47 years. There were 25 patients (5 males and 20 females) in the PAO group, with an average age of 37.2±6.9 years, ranging from 21 to 50 years. Patients were followed up till May 2023. Imaging data of patients before the surgery and during follow-up were collected including lateral center edge angle (LCEA), anterior center edge angle (ACEA), extrusion index, T?nnis angle, femoral neck anteversion angle, and T?nnis grade. The modified Harris hip score (mHSS), non-arthritic hip score (NAHS) and hip outcome score activity of daily living scale (HOS-ADL) were evaluated at 6 months, 12 months and the final follow-up.Results:All patients were followed up for an average of 54.2±8.2 months in HA group and 55.5±9.4 months in PAO group separately. Before surgery, there were no significant differences in LCEA, ACEA, T?nnis angle, extrusion index, femoral neck anteversion angle and T?nnis grade between the two groups ( P>0.05). α angle of HA was 60.8°±5.8°, which was significant different from α angle of PAO 53.0°±5.9° ( t=5.07, P<0.001). At the final follow-up, in HA group, LCEA, ACEA, T?nnis angle, extrusion index were 20.0°±2.1°, 20.0°±2.6°, 30.6%±2.3%, 10.8°±1.6°, 39.8°±5.4, which were all significantly different from pre-operation data ( P<0.05). While there was no significant difference between pre- and post-operation in T?nnis grade (χ 2=5.72, P=0.057). In PAO group, LCEA, ACEA, T?nnis angle, extrusion index were 33.0°±3.0°, 33.3°±4.0°, 15.0%±4.0%, 2.8°±1.7° which were all significantly different from pre-operation data ( P<0.05). While there were no significant differences between pre- and post-operation in α angle and Tonnis grade ( P>0.05). Between HA group and PAO group, there were significant differences in LCEA, ACEA, T?nnis angle, extrusion index and α angle ( P<0.05), while no significant difference was found in T?nnis grade (χ 2=3.024, P=0.996). There were no significant differences between the two groups in preoperative mHHS, NAHS and HOS-ADL ( P>0.05). At 6 months,12 months and the final follow-up after operation, the mHHS, NAHS and HOS-ADL were significantly improved compared to pre-operation data ( P<0.05). At 6 months and 12 months after operation, the three hip function scores in HA group were significantly better than those in PAO group ( P<0.05). At the final follow-up, there was no significant difference in three hip scores between the HA group and the PAO group ( P>0.05). Conclusion:Both HA and PAO can achieve good short- and mid-term clinical efficacy in the treatment of BDDH. Specifically, HA is superior to PAO in the early clinical efficacy, and the mid-term clinical efficacy is similar. PAO is able to improve the acetabular radiographic parameters of BDDH (except α angle) while HA can only improve α angle. However, they all can effectively delay the imaging progression of hip osteoarthritis.

【Objective】 4-like protein with down-regulated expression and development in neural precursor cells (NEDD4L) plays an important role in blood pressure (BP) regulation and sodium homeostasis by regulating epithelial sodium channel protein. In this study, we aimed to explore the relationship of NEDD4L gene polymorphisms with BP responses to sodium and potassium intake. 【Methods】 In 2004, 514 subjects from 124 families in Meixian County, Shaanxi Province, were recruited to establish a salt-sensitive hypertension study cohort. All the subjects received a 3-day baseline survey, a 7-day low-salt diet, a 7-day high-salt diet, and finally a 7-day high-salt and potassium supplementation. Their BP was measured and peripheral blood samples were collected at different intervention periods. The 14 gene polymorphisms of NEDD4L gene were genotyped and analyzed by MassARRAY platform. 【Results】 BP decreased on a low-salt diet, and significantly increased on a high-salt diet, and decreased again after potassium supplementation. NEDD4L SNPs rs74408486 were significantly associated with systolic BP, diastolic BP and mean arterial pressure responses to the low-salt diet. SNPs rs292449 and rs2288775 were significantly associated with pulse pressure response to the high-salt diet. In addition, SNPs rs563283 and rs292449 were significantly associated with diastolic BP, mean arterial pressure, and pulse pressure responses to high-salt and potassium supplementation diet. 【Conclusion】 NEDD4L gene polymorphisms were significantly associated with BP responses to sodium and potassium intake, suggesting that NEDD4L gene may be involved in the development of salt sensitivity and potassium sensitivity.
6.The study of smoking impact on autophagy in alveolar macrophages of human silicosis

Objective:To investigate the effect of smoking on autophagy in alveolar macrophages (AMs) of silicosis patients.Methods:In December 2019, a random sampling method was used to select 42 male patients with silicosis (19 cases of stage II and 23 cases of stage III) who were treated with large volume whole lung lavage from August to December 2017 in the Beidaihe sanatorium. According to the different smoking index of the study subjects (smoking index=smoking cigarette consumptions per day×years of smoking) , we divided them into high (Smoking index>400) , medium (200≤smoking index≤400) , low (smoking index <200) and non-smoking group. The levels of autophagy related proteins LC3, Beclin1, p62 and apoptosis related protein Cleaved Caspase-3 were detected by Western blot. The effects of smoking on autophagy activity of AMs in silicosis were analyzed.Results:The ratio of autophagy related protein LC3 II/LC3 I, the expression of Beclin1, p62, and apoptosis related protein Cleaved Caspase-3 in the high smoking group were significantly higher than that of the middle, low smoking group and the non-smoking group ( P<0.05) . Conclusion:Smoking can aggravate the dysfunction of autophagic degradation in silicosis patients' AMs, which may accelerate the progress of silicosis through increasing apoptosis in AMs.

8.Lymph nodes sorting based on nanometer carbon staining and supplying artery distribution in right-sided colon carcinoma

Objective To evaluate a novel lymph node (LN) sorting method on surgical resected sample guided by nanometer carbon staining and principle following the supplying artery tributory in rightsided colon carcinoma.Methods From May 2015 to June 2016,51 patients were randomly divided into two groups adopting traditional LN sorting method and that of a combination of nanometer carbon and artery guided.The final LN status were compared between the 2 groups.Results The total LN number and the positive LN in novel method group were higher than control group (437 vs.349,70 vs.54).The dissection time used,the number of harvested positive LN that was < 5 mm were significantly different [(13.1 ± 3.2) minvs.(17.8 ±3.8)min,t=4.75,P=0.000;1.0±l.0vs.0.2 ±0.6,t=3.51,P=0.000].The number of patients with harvested LN less than 12,the rate of positive lymph nodes,the rate of metastasis were not significantly different (all P > 0.05) between the two groups.Conclusions Use of nanometer carbon development combined with artery approach facilitates LN sorting,yielding more positive LNs,and increating the accuracy of pathological staging in right-sided colon cancer.
9.Influence of expressing IrrE from Deinococcus radiodurans on osmotic stress tolerance of succinate-producing Escherichia coli.

Hyper-osmotic stress is one of the key factors that decrease the efficiency of biological succinic acid production. To increase the osmotic stress tolerance of succinate-producing Escherichia coli, we studied the influence of IrrE, an exogenous global regulator, on cell osmotic stress resistance. Fermentation results showed that cell growth and succinic acid production by the recombinant increased under different Na+ concentrations. Meanwhile, the maximum dry cell mass, glucose consumption and succinic acid concentration increased 15.6%, 22% and 23%, respectively, when fermented in a 5-L bioreactor. Expressing IrrE improved cell resistance to hyper-osmotic stress. Further comparison of intracellular osmoprotectants (trehalose and glycerol) concentrations showed that trehalose and glycerol concentrations in the recombinant increased. This suggested that introduction of IrrE could enhance intracellular osmoprotectants accumulation which conferred cell with improved resistance to osmotic stress.
10.Succinic acid production from sucrose and sugarcane molasses by metabolically engineered Escherichia coli.

Sugarcane molasses containing large amounts of sucrose is an economical substrate for succinic acid production. However, Escherichia coli AFP111 cannot metabolize sucrose although it is a promising candidate for succinic acid production. To achieve sucrose utilizing ability, we cloned and expressed cscBKA genes encoding sucrose permease, fructokinase and invertase of non-PTS sucrose-utilization system from E. coli W in E. coli AFP111 to generate a recombinant strain AFP111/pMD19T-cscBKA. After 72 h of anaerobic fermentation of the recombinant in serum bottles, 20 g/L sucrose was consumed and 12 g/L succinic acid was produced. During dual-phase fermentation comprised of initial aerobic growth phase followed by anaerobic fermentation phase, the concentration of succinic acid from sucrose and sugarcane molasses was 34 g/L and 30 g/L, respectively, at 30 h of anaerobic phase in a 3 L fermentor. The results show that the introduction of non-PTS sucrose-utilization system has sucrose-metabolizing capability for cell growth and succinic acid production, and can use cheap sugarcane molasses to produce succinic acid.
