1.Study on the anti-hepatitis mechanism of Abrus pulchellus subsp. cantoniensis (Hance) Verdc. and Abrus pulchellus subsp. mollis (Hance) Verdc. based on serum pharmacochemistry and network pharmacology

OBJECTIVE To compare the anti-hepatitis mechanisms of Abrus pulchellus subsp. cantoniensis (Hance) Verdc. (AC) and Abrus pulchellus subsp. mollis(Hance) Verdc. (AM). METHODS SD rats were randomly divided into blank group, AC- treated group, and AM-treated group, with each group consisting of 10 rats. The rats’ orbital venous blood was collected at 5, 15, 30 minutes, and 1, 1.5, 2, 4, 6, 8, 12 hours after gavage administration of 24 g/kg of the corresponding drug (calculated by crude drug) or water, respectively. Ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry technology was utilized to identify the prototype components present in the serum. The network pharmacology method was adopted to predict the anti-hepatitis active components, key targets, and signaling pathways of AC and AM. Additionally, molecular docking technology was utilized to verify the binding activity of the core active components with key targets. RESULTS A total of 35 prototype components migrating to the blood of AC and AM were identified in the serum of administered rats, among which 24 were common components. The active components in AC, such as acetylanguidine, physcion, soyasaponin A3 and soyasaponin Ⅰ, as well as those in AM, including vicenin 3, acetylanguidine,soyasaponin Ⅰ and schaftoside, all acted on key targets such as steroid receptor coactivator, phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha, epidermal growth factor receptor (EGFR), and protein kinase B1(Akt1). These components modulated pathways in cancer, EGFR tyrosine kinase inhibitor resistance, and the phosphoinositide 3-kinase (PI3K) -Akt pathway, thereby exerting anti-hepatitis effects. Furthermore, the binding energies between these active components and their key targets were all less than -5 kJ/mol. CONCLUSIONS There are differences in the active components of AC and AM against hepatitis, but their mechanisms of action are similar. Both may exert their anti-hepatitis effects through pathways in cancer, EGFR tyrosine kinase inhibitor resistance, and the PI3K-Akt pathway.
2.Comparison of TightRope system versus plate-screw internal fixation for avulsion fractures of the posterior calcaneal tuberosity of Beavis type Ⅱ

Objective:To compare the effectiveness of TightRope system and plate-screw internal fixation in the treatment of avulsion fractures of the posterior calcaneal tuberosity of Beavis type Ⅱ.Methods:A retrospective study was conducted to analyze the clinical data of the 42 patients with avulsion fracture of the posterior calcaneal tuberosity of Beavis type Ⅱ who had undergone surgical treatment at Foot-ankle Surgery Center, The Fourth Hospital of Wuhan from January 2019 to January 2024. There were 25 males and 17 females, with an age of (64.4±10.1) years. The left side was affected in 23 cases and the right side in 19 cases. The patients were divided into 2 groups according to their different ways of internal fixation: a TR group of 22 cases subjected to open reduction and internal fixation with TightRope system and a PS group of 20 cases subjected to open reduction and plate-screw internal fixation. The operation time, bone healing time, complications, and American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score, MOS item short form health survey (SF-36) score and visual analogue scale (VAS) at 12 months after operation were compared between the 2 groups.Results:There were no significant differences in the preoperative clinical baseline data between the 2 groups, indicating comparability ( P>0.05). All patients were followed up for (19.9±3.1) months. In the TR group, the operation time [(59.3±7.1) min] was significantly shorter than that in the PS Group [(72.1±10.7) min], the incidence of complications [4.5% (1/22)] significantly lower than that in the PS group [35.0% (7/20)], and the AOFAS ankle-hindfoot score [(83.5±8.2) points] and SF-36 scale score [(88.2±5.3) points] were significantly higher than those in the PS Group [(64.4±10.8) points and (78.6±11.2) points] (all P<0.05). There was no significant difference in the bone healing time or VAS pain score at 12 months after operation between the 2 groups ( P<0.05). Conclusions:In the treatment of avulsion fractures of the posterior calcaneal tuberosity of Beavis type Ⅱ, compared with the plate-screw internal fixation, the TightRope system can shorten operation time, improve functional recovery, and reduce complications.
3.Guiding value of thromboelastography in blood transfusion therapy for patients undergoing liver rupture repair

Objective To explore the guiding value of thromboelastography(TEG)in blood transfusion therapy for patients undergoing liver rupture repair.Methods A total of 106 patients who underwent liver rupture repair and were admitted to the hospital from June 2021 to June 2024 were selected.According to the random number table method,the patients were divided into 53 patients who underwent liver rupture repair with TEG detection blood transfusion(study group)and 53 patients who underwent liver rupture repair with conventional coagulation function detection blood transfusion(control group).General information of patients was collected.The levels of coagulation function indicators,the dosage of blood products and intraoperative complications of patients in the study group and the control group before blood transfusion and 24 hours after blood transfusion were compared.Results 24 hours after blood transfusion,the fibrinogen level in the study group was higher than that in the control group,while the prothrombin time,activated partial thromboplastin time and thrombin time were shorter than those in the control group,and the differences were statistically sig-nificant(P<0.05).The dosages of red blood cells,frozen plasma,platelets and cryoprecipitate in the study group were all lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of intraoperative complications in the study group was lower than that in the con-trol group,and the difference was statistically significant(P<0.05).Conclusion TEG has significant guiding value in blood transfusion therapy for patients undergoing liver rupture repair.

To explore how to diagnose and treat brucellosis accurately and timely in patients with fever of unkown origin in non-pastoral areas.The epidemiological history,clinical symptoms,complete blood counts,procalcitonin and treatment efficacy of 7 patients with brucellosis were analyzed retrospectively.Some characteristic manifestations should be differentiated from tuberculosis.The clinical symptoms were relieved after combination of doxycycline,rifampicin,levofloxacin and amikacin for 6 weeks,only one patient with bone destruction needed orthopedic surgery.The overall response rate was 6/7.No relapse occurred during half year follow-up.
9.Analysis of COL1A1 gene mutation in an ethnic Han Chinese family from Henan affected with osteogenesis imperfecta.

OBJECTIVETo identify potential mutation of COL1A1 gene in an ethnic Han Chinese family from Henan affected with osteogenesis imperfecta (OI).
METHODSPeripheral blood samples were collected from all 11 members of the family and 50 healthy adults for the extraction of genomic DNA. All exons and introns of the COL1A1 gene were amplified by polymerase chain reaction and subjected to direct sequencing. Mutations found in the proband were analyzed through comparison with other members of the family, 50 healthy individuals and sequence from the GenBank.
RESULTSFifteen sequence variants were discovered, which included 1 missense mutation, 1 synonymous mutation and 13 intronic mutations. All of the 4 patients from the family were detected as having carried a novel heterozygous missense mutation (c.4193T>G, p.I1398S) in exon 50 of the COL1A1 gene. The father of the proband has carried the same mutation but had a normal phenotype. The same mutation was not found in other healthy members of the family.
CONCLUSIONThe OI type of this family may have been autosomal dominant with incomplete penetrance or autosomal recessive associated with COL1A1 gene mutations.

10.Cucurmosin combined with common chemotherapeutic drugs inhibited human pancreatic cancer cell line BxPC-3 in vitro

Objective To investigate the inhibitory effect of cucurmosin (CUS) combined with the commonly used chemotherapeutic drugs for pancreatic cancer in clinical practice including Gemcitabine (GEM),Fluorouracil (5-FU),Paclitaxel (PTX) and Cisplatin (DDP) on cell proliferation of human pancreatic cancer cell line BxPC-3.Mehtods Sulforhodamine B (SRB) assay was used to detect the inhibition on the cell proliferation of BxPC-3 cells in vitro after the treatment of CUS combined with GEM,5-FU,PTX and DDP,respectively.Colony formation assay was also conducted and Jin' s formula was used to assess the synergistic effect of these combinations.Results The inhibition rate of CUS combined with GEM,5-FU,PTX and DDP were all higher than those of each drug alone (q > 0.85),which became obvious in low concentrations.The colony formation inhibition rate of CUS combined with GEM,5-FU,PTX or DDP were all higher than each single drug treatment (q > 1.15).Conclusion CUS could enhance the cell growth inhibition of GEM,5-FU,PTX and DDP in BxPC-3 cells in vitro with a good synergistic effect.
