OBJECTIVE To compare the anti-hepatitis mechanisms of Abrus pulchellus subsp. cantoniensis （Hance） Verdc. （AC） and Abrus pulchellus subsp. mollis（Hance） Verdc. （AM）. METHODS SD rats were randomly divided into blank group， AC- treated group， and AM-treated group， with each group consisting of 10 rats. The rats’ orbital venous blood was collected at 5， 15， 30 minutes， and 1， 1.5， 2， 4， 6， 8， 12 hours after gavage administration of 24 g/kg of the corresponding drug （calculated by crude drug） or water， respectively. Ultra-performance liquid chromatography-quadrupole time-of-flight mass spectrometry technology was utilized to identify the prototype components present in the serum. The network pharmacology method was adopted to predict the anti-hepatitis active components， key targets， and signaling pathways of AC and AM. Additionally， molecular docking technology was utilized to verify the binding activity of the core active components with key targets. RESULTS A total of 35 prototype components migrating to the blood of AC and AM were identified in the serum of administered rats， among which 24 were common components. The active components in AC， such as acetylanguidine， physcion， soyasaponin A3 and soyasaponin Ⅰ， as well as those in AM， including vicenin 3， acetylanguidine，soyasaponin Ⅰ and schaftoside， all acted on key targets such as steroid receptor coactivator， phosphatidylinositol-4，5-bisphosphate 3-kinase catalytic subunit alpha， epidermal growth factor receptor （EGFR）， and protein kinase B1（Akt1）. These components modulated pathways in cancer， EGFR tyrosine kinase inhibitor resistance， and the phosphoinositide 3-kinase （PI3K） -Akt pathway， thereby exerting anti-hepatitis effects. Furthermore， the binding energies between these active components and their key targets were all less than －5 kJ/mol. CONCLUSIONS There are differences in the active components of AC and AM against hepatitis， but their mechanisms of action are similar. Both may exert their anti-hepatitis effects through pathways in cancer， EGFR tyrosine kinase inhibitor resistance， and the PI3K-Akt pathway.

Objective To explore the guiding value of thromboelastography(TEG)in blood transfusion therapy for patients undergoing liver rupture repair.Methods A total of 106 patients who underwent liver rupture repair and were admitted to the hospital from June 2021 to June 2024 were selected.According to the random number table method,the patients were divided into 53 patients who underwent liver rupture repair with TEG detection blood transfusion(study group)and 53 patients who underwent liver rupture repair with conventional coagulation function detection blood transfusion(control group).General information of patients was collected.The levels of coagulation function indicators,the dosage of blood products and intraoperative complications of patients in the study group and the control group before blood transfusion and 24 hours after blood transfusion were compared.Results 24 hours after blood transfusion,the fibrinogen level in the study group was higher than that in the control group,while the prothrombin time,activated partial thromboplastin time and thrombin time were shorter than those in the control group,and the differences were statistically sig-nificant(P<0.05).The dosages of red blood cells,frozen plasma,platelets and cryoprecipitate in the study group were all lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of intraoperative complications in the study group was lower than that in the con-trol group,and the difference was statistically significant(P<0.05).Conclusion TEG has significant guiding value in blood transfusion therapy for patients undergoing liver rupture repair.

Objective: To investigate the feasibility of one-step coronary and carotid-cerebrovascular computed tomography angiography (CTA) using high-pitch Double Turbo Flash mode and to analyze the image quality and radiation dose in patients with heart rate variability using multi-slice detector dual-source CT. Methods: A total of 79 patients with heart rate variability higher than 3 beat/min (bpm) were retrospectively analyzed. They were grouped by the scanning methods. Group A (n=40) were performed double turbo flash mode for one-step coronary and carotid-cerebrovascular arteriesinjected with one injection of contrast agent.further divided into Single A (only the phase 1 image)and Double A(combined images of phases 1 and 2). Group B (n=39) were performed separately with twice injection of contrast agent. Subjective scoring was performed on the image quality of the group Single A, Double A and B using a 4-point. Then the objective parameters of image quality, CT attenuations, image noise, signal-to-noise (SNR), and contrast-to-noise (CNR), were evaluated. Then subjective scores, objective evaluation indicators, and radiation dose were compared with one-way ANOVA analysis among the three groups.Contrast agent were compared by using t test in group A and B. Results: No significant difference was found (F=2.093, P=0.128) for the subjective scores of the head and neck CTA among the three groups with (3.47±0.51), (3.53±0.51), (3.69±0.47). Significant difference was found (F=50.955, P<0.01) for the subjective scores of coronary CTA among the three groups with (2.70±0.76), (3.35±0.66), (3.58±0.50), and there was no statistically significant difference between Double A and B groups (P=0.104), met the requirements for diagnosis. The objective evaluation of the three groups compared the head and neck CTA images were statistically significant (P<0.05), of which the difference between the Single A and the Double A was not statistically significant (P>0.05), met the requirements for diagnosis; the differences in SNR and CNR of coronary CTA images are statistically significant (F=12.991, 12.236, P<0.01), and there was no statistically significant difference between Double A and B (P>0.05), met the requirements for diagnosis. Radiation dose in the group Double A was lower than group B (decreased by 46.15%,P<0.01).The amount of the contrast agent in the group Double A was lower than group B (decreased by 44.13%, t=-45.455;P<0.01). Conclusion The double turbo flash mode is feasibility for one-step coronary and carotid-cerebrovascular CTA in patients with the heart rate variability using multi-slice dual-source CT.This scan mode can maintain the diagnostic image quality with low contrast agent and radiation dose.

To explore how to diagnose and treat brucellosis accurately and timely in patients with fever of unkown origin in non-pastoral areas.The epidemiological history,clinical symptoms,complete blood counts,procalcitonin and treatment efficacy of 7 patients with brucellosis were analyzed retrospectively.Some characteristic manifestations should be differentiated from tuberculosis.The clinical symptoms were relieved after combination of doxycycline,rifampicin,levofloxacin and amikacin for 6 weeks,only one patient with bone destruction needed orthopedic surgery.The overall response rate was 6/7.No relapse occurred during half year follow-up.

OBJECTIVETo identify potential mutation of COL1A1 gene in an ethnic Han Chinese family from Henan affected with osteogenesis imperfecta (OI).

METHODSPeripheral blood samples were collected from all 11 members of the family and 50 healthy adults for the extraction of genomic DNA. All exons and introns of the COL1A1 gene were amplified by polymerase chain reaction and subjected to direct sequencing. Mutations found in the proband were analyzed through comparison with other members of the family, 50 healthy individuals and sequence from the GenBank.

RESULTSFifteen sequence variants were discovered, which included 1 missense mutation, 1 synonymous mutation and 13 intronic mutations. All of the 4 patients from the family were detected as having carried a novel heterozygous missense mutation (c.4193T>G, p.I1398S) in exon 50 of the COL1A1 gene. The father of the proband has carried the same mutation but had a normal phenotype. The same mutation was not found in other healthy members of the family.

CONCLUSIONThe OI type of this family may have been autosomal dominant with incomplete penetrance or autosomal recessive associated with COL1A1 gene mutations.

Objective To investigate the inhibitory effect of cucurmosin (CUS) combined with the commonly used chemotherapeutic drugs for pancreatic cancer in clinical practice including Gemcitabine (GEM),Fluorouracil (5-FU),Paclitaxel (PTX) and Cisplatin (DDP) on cell proliferation of human pancreatic cancer cell line BxPC-3.Mehtods Sulforhodamine B (SRB) assay was used to detect the inhibition on the cell proliferation of BxPC-3 cells in vitro after the treatment of CUS combined with GEM,5-FU,PTX and DDP,respectively.Colony formation assay was also conducted and Jin' s formula was used to assess the synergistic effect of these combinations.Results The inhibition rate of CUS combined with GEM,5-FU,PTX and DDP were all higher than those of each drug alone (q ＞ 0.85),which became obvious in low concentrations.The colony formation inhibition rate of CUS combined with GEM,5-FU,PTX or DDP were all higher than each single drug treatment (q ＞ 1.15).Conclusion CUS could enhance the cell growth inhibition of GEM,5-FU,PTX and DDP in BxPC-3 cells in vitro with a good synergistic effect.

Objective To report the clinical experience of extracorporeal membrane oxygenation (ECMO)in the emergency management of fulminant myocarditis.Methods The patient,an 11 year-old boy,had fever for 4 day and abdominal pain,vomiting for 1 day and anuria for 12 hours.Electrocardiogram showed Ⅲ atrial-ventricular block,multifocal ventricular rhythm,bundle branch block,convulsivum multifocal ventricular tachycardia,extensive low voltage,ST-T elevation at lead Ⅰ,AVR,V1,V2,V3;and there were ST-T intrusion,T wave inversion at lead Ⅱ,Ⅲ,AVR,AVF,V4～5.Blood creatine kinase was 2 161 U/L,reatine kinase isoenzyme 109 U/L,α-hydroxybutyrate dehydrogenase 612 U/L,lactate dehydrogenase 696 U/L,troponin Ⅰ 22.1 U/L.Echocardiogram showed right atrium 4.4 mm,right ventricular 2.3 mm,severe tricuspid valve regurgitation,left ejection fraction 33％,left ventricular fractional shortening 15％,ventricular wall motion dyssynchrony.Blood lactate was 4.0 mmol/L.The patient's condition was still unstable after using dobutamin,dopamine,milrinone,furosemide,large dose methylprednisolone,intravenous human immunoglobin,phosphocreatine and so on.ECMO was used for cardio-pulmonary support.It is necessary to monitoring the consciousness,temperature,heart rate,respiration,blood pression,SaO2,urinary volume,ariterial blood gas,blood electrolytes,blood lactate,blood glucose,liver function,renal function,blood routine,activated clotting time(ACT),lower extremity blood supply and so on.ACT was maintained at 160～200 s.Heparin was used persistently[5～10 U/(kg·min)].Results ECMO system had been successfully used for 7 days.The cardiac function of the patient was improved significantly.There was no complication,such as hemorrhage,infection,and embolism.Heart arrest in the patient occurred three times,ventricular fibrillation and ventricular flutter occurred one time respectively during ECMO.The rhythm was recovered by electric defibrillation and antiarrhythmic drugs.On day 20,the patient was discharged.At the time of hospital discharge,the patient demonstrated good activity,with normal myocardial enzymes.The echocardiogram showed the size of the cardiac chambers and the contractile function of the myocardia were normal.Electrocardiogram showed Ⅰ degree atrial-ventricular block,complete right bundle branch block.Two weeks later,the electrocardiogram demonstrated complete right bundle branch block.Echocardiogram showed septal thickening(0.9 cm).Two months later,the electrocardiogram was just as that of two weeks before.Echocardiogram showed septal thickening(0.7 cm).The children had no symptom after he was discharged and acted without limitation.Conclusion ECMO is a kind of effective treatment for fulminant myocarditis.The key to desirable therapeutic effect is the timing of its application.

Objective The aim of this study is to investigate the correlation between small airway disease and airway hyper-responsiveness,and explore the predicting value of small airway diseases for asthma.Methods Pulmonary function tests and bronchial provocation tests were performed in 249 patients with chronic cough from Sep. 2004 to Sep. 2006.The incidence of small airway disease and airway hyper-responsiveness were observed.Results There were 91 patients with small airway disease,and 103 patients with positive tests for bronchial provocation in total 249 chronic cough patients.The incidence of positive tests for bronchial provocation in 91 patients(73.63%)with small airway disease was significantly higher than that in 158 patients(22.78%)without it,P

PurposeTo evaluate the advantages and limits of different diagnostic imaging modalities for adrenal neuroblastoma diagnosis in children. MethodsImaging manifestations of 26 pathologically proved cases of adrenal neuroblastoma were retrospectively reviewed. Results37.5% of the cases were revealed by IVP, 82% were found by US, while 90% of the cases were demonstrated by CT. ConclusionUS should be considered as the first choice for diagnosing the adrenal neuroblastoma, while CT seems superior to US.

Objective To investigate the CT manifestations of recto-endometriosis. Methods Three pathologically proved cases of recto-endometriosis were reported, the CT features and clinical manifestations were investigated retrospectively with review of literatures. Results The patients were all menstruating females (28, 34, and 45 years old, respectively), and anal pain emerged during menses in one case. Rectal examination revealed stiff immobile mass along the rectal wall in all three cases. All the cases showed solid masses on CT scan with moderate enhancement after contrast administration. Two of them confined to the rectal walls, another involved the rectum and peri-rectal aspect. One case was misdiagnosed as well-differentiated adenocarcinoma of rectum in frozen section by biopsy specimen.Conclusion A solid mass was always found in recto-endometriosis. No characteristic CT feature was manifested. Attention should be paid to the differentiation from rectal tumors when making the diagnosis.
