1.Study on the correlation between urinary calcium levels and severity and prognosis of chronic kidney disease

Objective:To analyze the relationship between 24-hour urinary calcium (24 h UCa) level and the risk of end-stage kidney disease (ESKD), cardiovascular disease (CVD), and all-cause mortality.Methods:In the Chinese Cohort Study of Chronic Kidney Disease, we examined 3 375 patients aged 18-74 years with CKD stages 1-4. Kaplan-Meier survival and Cox proportional hazard regression models were used to test a time-to-event association between levels of 24 h UCa and incidence of ESKD, CVD, and all-cause mortality.Results:During a follow-up of 4.17 (3.37, 5.20) years, 179, 145, 104 and 38 ESKD events occurred in <0.60, 0.60-, 1.20-, ≥2.32 mmol 24 h UCa groups. Higher levels of 24 h UCa (1.20-,≥2.32 mmol) were independently associated with a lower incidence of ESKD events in patients with CKD, with HR (95% CI) of 0.71 (0.54-0.93) and 0.43 (0.29-0.64), respectively. No significant associations with CVD and all-cause mortality endpoints were detected. Conclusion:Among patients with CKD, levels of 24 h UCa displayed an association with the risk of ESKD among patients with CKD stages 1-4.
2.Study on the Mechanism of Yiqi Bushen Formula Regulating Circular RNA Circ_0051246 Against Proliferation and Migration of Gastric Cancer Stem Cells

[Objective]To investigate the mechanism action of Yiqi Bushen Formula on the proliferation and migration of gastric cancer stem cells(GCSCs).[Methods]SGC-7901 GCSCs with double positive markers of CD44 and epithelial cell adhesion molecule(EpCAM)were sorted by immunomagnetic beads.The selected CD44+EpCAM+SGC-7901 GCSCs were subcultured and stably transfected with circ_0051246 control,pcDNA circ_0051246 and si-circ_0051246 respectively.The above four groups of cells(including untransfected)were divided into three subgroups:blank control group,negative control group and Yiqi Bushen Formula group.The blank control group was normally subcultured,the negative control group was intervened with blank serum,and the Yiqi Bushen Formula group was intervened with Yiqi Bushen Formula-containing serum.The microstructure of SGC-7901 GCSCs was observed by laser confocal and transmission electron microscopy.Cell counting kit-8(CCK-8)and cell cloning experiments were used to detect the proliferation ability of SGC-7901 GCSCs.Flow cytometry was used to detect the positive rate of CD44 and EpCAM and the apoptosis rate.Cell scratch test and transwell were used to detect the migration and invasion ability of SGC-7901 GCSCs.The expression of circ_0051246 in each experimental group was detected by quantitative reverse transcription-polymerase chain reaction(qRT-PCR).[Results]Compared with respective negative control group,the number of mitochondrial cristae in each group of Yiqi Bushen Formula was significantly reduced,the endoplasmic reticulum was significantly swollen.Compared with respective blank control group,the expression of actin in the cytoskeleton structure of each group of Yiqi Bushen Formula was significantly decreased.Compared with respective negative control group,the cell proliferation ability of Yiqi Bushen Formula groups was weakened(P<0.05,P<0.01),the rate of double positive cells of CD44 and EpCAM antibodies was significantly decreased(P<0.01),the cloning ability was significantly decreased(P<0.01),the apoptosis rate was significantly increased(P<0.01),the migration rate was significantly decreased at 24 and 48 h(P<0.05,P<0.01),the number of cell migration and invasion was significantly reduced(P<0.01).The expression level of circ_0051246 in Yiqi Bushen Formula group was significantly decreased(P<0.01).[Conclusion]Yiqi Bushen Formula may play a role in anti-proliferation and migration of GCSCs by inhibiting the expression of circ_0051246 and reducing the expression of SGC-7901 CSCs surface markers CD44 and EpCAM,which provides a new idea for the treatment of gastric cancer.
3.Rabies epidemic surveillance analysis,China,2024

Objective:To analyze the epidemiological characteristics of rabies in China during 2024 to understand the patterns of disease transmission and provide a scientific basis for optimizing prevention strategies and advancing the rabies elimination process.Methods:Data on the national rabies epidemic in 2024 and case-specific information from national surveillance sites were collected from China's National Notifiable Disease Reporting System(NNDRS). Descriptive epidemiological methods were used to analyze the disease trends and epidemic characteristics.Results:In 2024,China reported 167 human rabies cases across 20 provincial-level administrative divisions(PLADs),resulting in 148 fatalities. This represents a 39.17% increase from 2023(120 cases)and the highest annual incidence recorded since 2019. The epidemic exhibited a distinct geographic clustering in central and southern China,with four provinces(Henan,Guangxi,Hunan,and Hubei)accounting for 62.87%(105/167)of nationally reported cases. October(20 cases)and November(20 cases)recorded the highest monthly incidence,while February had the lowest(2 cases). Farmers constituted the primary affected demographic(80.84%),with a marked male predominance(male-to-female ratio:2.15∶1)-higher than the 2023 ratio(1.86∶1). Cases spanned all age groups,predominantly affecting middle-aged and older adults. Analysis of 54 cases revealed dogs as the primary exposure source(93.88%),with domestic dogs constituting 75.00% of these.The laboratory diagnosis rate of rabies cases in 2024 was 43.71%(73/167).Conclusion:After 16 consecutive years of decline,China's rabies epidemic rebounded in 2024,primarily driven by deficiencies in domestic dog management and weaknesses in post-exposure prophylaxis. Key recommendations include enhancing canine vaccination coverage in high-risk areas,implementing targeted health education for farmers and middle-aged and older adults,improving standardized post-exposure treatment protocols,and optimizing integrated surveillance-alert-response systems,ultimately advancing the achievement of rabies elimination goals.
4.Monomorphic epitheliotropic intestinal T-cell lymphoma: a clinicopathological and genetic mutation characteristics analysis of forty-two cases

Objective:To investigate the clinicopathological and genetic characteristics of monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL).Methods:The forty-two MEITL cases diagnosed in the Department of Pathology, Ruijin Hospital affiliated to Shanghai Jiao Tong University School of Medicine, Shanghai, China from 2016 to 2022 was retrospectively analyzed. Clinical data were collected, and follow-up was performed. Morphological characteristics were observed. Immunohistochemistry, Epstein-Barr virus (EBV) in situ hybridization, clonal rearrangement analysis of T-cell receptor (TCR) genes, and targeted next-generation sequencing (NGS) were performed.Results:Among the 42 patients (male/female ratio of 2.8∶1.0), the age range was 32-77 years with a median age of 59.5 (52.0-65.0) years. Grossly, the tumors were presented as ulcerative or exophytic lesions, with a maximum diameter of 2-18 cm. There were 34 cases with a single lesion and 8 cases with more than 1 lesion. The tumor cells in all 42 cases were relatively monotonous in histology and small or medium in size. They had round or oval nuclei, moderately pale or clear cytoplasm, evenly distributed nuclear chromatin, inconspicuous nucleoli, and frequent mitotic figures. In one of the cases, there were moderately large cells, vacuolated nuclei, and clear nucleoli. Lymphoepithelial lesions were observed in 36 (85.7%) of the 42 cases, tumor necrosis in 4 (9.5%) cases, scattered eosinophils and/or plasma cell infiltration in the background in 9 (21.4%) cases, and a "starry sky" phenomenon in 1 (2.4%) case. The tumor cells in all cases exhibited high expression of CD3, CD2, CD7, CD8, CD56, TIA1, Granzyme B, and Perforin, while some also expressed CD4 (5/41, 12.2%), CD5 (3/41, 7.3%), CD20 (4/41, 11.9%), CD79α (2/37, 5.4%), and CD30 (1/34, 2.9%). The Ki-67 proliferation index ranged from 40% to 90%. EBER in situ hybridization tests were negative in all cases. TCR gene clonal rearrangement was detected in 96.4% (27/28) of the tested cases. Targeted NGS revealed commonly mutated genes including SETD2, STAT5B, JAK3, TP53, and CREBBP. The primary treatment was chemotherapy, with 2 cases undergoing autologous hematopoietic stem cell transplantation. Follow-up information was obtained for 29 cases, with a follow-up period of 1-73 months. The mortality was 93.1% (27/29).Conclusions:MEITL is a rare and highly aggressive peripheral T-cell lymphoma. Its clinical manifestations are diverse, and diagnosis primarily relies on a comprehensive assessment of pathological morphology, immunohistochemical profiles, and EBV infection status, supplemented by genetic testing if necessary. At present, there is no effective treatment, and its overall prognosis is poor.
5.Positive Effects of Programmed Nursing on Dry Eyes after the Phacoemulsification

Objective To evaluate the effect of programmed nursing on dry eye after the phacoemulsification for cataract.Methods A total of 100 cases undergoing the phacoemulsification for cataract in our hospital from January 2022 to January 2024 were divided randomly into the control group and the observation group based on nursing differences with 50 cases in each group.The control group received the routine nursing,while the observation group was given the programmed nursing.The incidence of postoperative dry eye,nursing satisfaction,and dry eye-related complications were compared between the two groups.Results The results showed that the corneal fluorescence staining,tear secretion test,and tear film rupture time in the observation group were(1.46±1.13)points,(17.18±1.45)mm,and(6.86±0.92)s,respectively,which were superior to the control group's(2.96±0.09)points,(11.04±2.79)mm,and(5.98±1.26)s(all P<0.05);The incidence of dry eye complications in the observation group at 45 days and 90 days after the surgery was 8.0%and 6.0%,respectively,lower than that in the control group at 18.0%and 16.0%(both P<0.05);The satisfaction rate of the observation group with nursing care was 98.0%,which was higher than the control group's 84.0%(P<0.05).The differences between the groups were statistically significant.Conclusion Programmed nursing for patients undergoing the phacoemulsification for cataract can reduce the incidence of dry eye,improve the eye condition of patients and increase the patient satisfaction.
6.Therapeutic potentials of natural products for post-traumatic stress disorder: A focus on epigenetics.

Post-traumatic stress disorder (PTSD) is a relatively common but complex mental illness with a range of diverse risk factors. Typical symptoms include the re-experience or avoidance of traumatic events, cognitive impairment, and hypervigilance. While the exact pathogenesis of PTSD is unclear, many studies indicate that epigenetic regulation plays a key role in its development. Specifically, numerous studies have indicated that the levels of histone acetylation and methylation, DNA methylation, and noncoding RNA are altered in PTSD patients. Further to this, natural products have been found to achieve epigenetic regulation of PTSD by regulating the expression of epigenetic enzymes, long noncoding RNA (lncRNA), and miRNA, thereby playing a role in improving PTSD symptoms. To date, however, no epigenetic regulation related drugs have been used in the treatment of PTSD. Furthermore, while natural products that can epigenetically regulate PTSD have received increasing levels of attention, there have not yet been any systematic reports on the topic. Here, we summarized the roles and mechanisms of natural products in the epigenetic regulation of PTSD, providing a novel and unique perspective that will help to guide the development and application of new PTSD treatments.
7.Research progress on the anti-aging mechanism of epigallocatechin gallate

Aging is a natural process in which tissue and organ function declines as organisms age. Aging is the inevitable outcome of the organism and is irreversible. The aging process is accompanied by degenerative changes in the morphological structure of multiple organs of the body, leading to an increase in the incidence of chronic diseases such as hypertension, cardiovascular and cerebrovascular diseases, hyperlipidemia, and diabetes year by year. In recent years, natural phytochemicals have attracted widespread attention from the public due to their advantages such as non-toxicity or low toxicity, low cost, and various biological activities. Epigallocatechin gallate (EGCG) is a dietary polyphenol extracted from green tea, which has a variety of biological functions including anti-oxidation, anti-inflammation, hypoglycemic, hypolipidemic and anti-aging activities. The EGCG-mediated anti-aging mechanism has been investigated in many studies with different aging models. This article reviews the research progress on rodents, nematodes, fruit flies, and cell aging models, focusing on summarizing the lifespan extension and physiological changes of rodents, nematodes and fruit flies after EGCG intervention from multiple angles, and exploring potential mechanism by which EGCG delays aging and extends lifespan. This review provides a theoretical reference for the study of the anti-aging mechanism of phytochemicals.
8.Heterotopic ossification: Current developments and emerging potential therapies.

This review aimed to provide a comprehensive analysis of the etiology, epidemiology, pathology, and conventional treatment of heterotopic ossification (HO), especially emerging potential therapies. HO is the process of ectopic bone formation at non-skeletal sites. HO can be subdivided into two major forms, acquired and hereditary, with acquired HO predominating. Hereditary HO is a rare and life-threatening genetic disorder, but both acquired and hereditary form can cause severe complications, such as peripheral nerve entrapment, pressure ulcers, and disability if joint ankylosis develops, which heavily contributes to a reduced quality of life. Modalities have been proposed to treat HO, but none have emerged as the gold standard. Surgical excision remains the only effective modality; however, the optimal timing is controversial and may cause HO recurrence. Recently, potential therapeutic strategies have emerged that focus on the signaling pathways involved in HO, and small molecule inhibitors have been shown to be promising. Moreover, additional specific targets, such as small interfering RNAs (siRNAs) and non-coding RNAs, could be used to effectively block HO or develop combinatorial therapies for HO.

9.Research on attention-enhanced networks for subtype classification of age-related macular degeneration in optical coherence tomography.

Subtype classification of age-related macular degeneration (AMD) based on optical coherence tomography (OCT) images serves as an effective auxiliary tool for clinicians in diagnosing disease progression and formulating treatment plans. To improve the classification accuracy of AMD subtypes, this study proposes a keypoint-based, attention-enhanced residual network (KPA-ResNet). The proposed architecture adopts a 50-layer residual network (ResNet-50) as the backbone, preceded by a keypoint localization module based on heatmap regression to outline critical lesion regions. A two-dimensional relative self-attention mechanism is incorporated into convolutional layers to enhance the representation of key lesion areas. Furthermore, the network depth is appropriately increased and an improved residual module, ConvNeXt, is introduced to enable comprehensive extraction of high-dimensional features and enrich the detail of lesion boundary contours, ultimately achieving higher classification accuracy of AMD subtypes. Experimental results demonstrate that KPA-ResNet achieves significant improvements in overall classification accuracy compared with conventional convolutional neural networks. Specifically, for the wet AMD subtypes, the classification accuracies for inactive choroidal neovascularization (CNV) and active CNV reach 92.8% and 95.2%, respectively, representing substantial improvement over ResNet-50. These findings validate the superior performance of KPA-ResNet in AMD subtype classification tasks. This work provides a high-accuracy, generalizable network architecture for OCT-based AMD subtype classification and offers new insights into integrating attention mechanisms with convolutional neural networks in ophthalmic image analysis.

10.The analysis of risk factors for death in patients with acute kidney injury undergoing continuous renal replacement therapy after cardiac surgery

Objective:This study intends to analyze the differences of indexes between the death group and the survival group of CSA-AKI patients undergoing CRRT before, during, and after heart surgery, and at the beginning and the end of CRRT, and to study the risk factors affecting the death of patients, so as to provide guidance for improving the prognosis.Methods:From January 2020 to October 2022, a total of 101 patients (59 in the mortality group and 42 in the survival group) were admitted to the Department of Cardiac Surgery at the First Affiliated Hospital of Zhengzhou University for cardiac surgery complicated by acute kidney injury necessitating postoperative continuous renal replacement therapy (CRRT). A retrospective analysis of clinical data was conducted to ascertain the risk factors influencing the 30-day mortality rate.Results:The postoperative complications of the two groups showed that the mortality group had a large proportion of MODS and LCOS ( P<0.05). Multivariate regression analysis showed that intraoperative mean arterial depression, high SOFA score at the beginning of CRRT, high APACHE Ⅱ score at the beginning of CRRT, and low platelet at the end of CRRT were independent risk factors for death( P<0.05). ROC curve analysis shows that SOFA score and APACHE Ⅱ score at the beginning of CRRT have predictive value for the 30-day death risk of CSA-AKI patients receiving CRRT. Conclusion:Low intraoperative MAP, high SOFA and APACHE Ⅱ scores at the beginning of CRRT, and low platelets at the end of CRRT were independent risk factors for 30-day mortality risk in patients with CSA-AKI undergoing CRRT.
