1.Influence of perceived stress on sleep quality among resident physicians: the chain mediating role of self-control and anxiety emotions

BackgroundResident physicians represent a high-risk group for sleep disorders, exhibiting a significantly higher prevalence of such conditions compared with the general population, which severely impairs their physical and mental health. It is hypothesized that perceived stress negatively impacts sleep quality through psychological mechanisms, such as depleting self-control resources and triggering anxiety. However, this pathway warrants empirical validation. ObjectiveTo explore the mediating role of self-control and anxiety emotions in the association between perceived stress and sleep quality among resident physicians, and to elucidate the underlying psychological mechanisms, aiming at providing theoretical basis for developing targeted psychological interventions. MethodsA cross-sectional survey was conducted in April 2025. First- to third- year resident physicians at a hospital in Fuyang City were recruited as participants (n=372). The Chinese Perceived Stress Scales (CPSS), the Chinese version of the Dual-Mode of Self-Control Scale (DMSC-S), the Pittsburgh Sleep Quality Index (PSQI), and the Generalized Anxiety Disorder Scale-7 item (GAD-7) were used for group testing. The model 6 of the Process macro version 4.1 was ultilized to examine the mediating pathway of self-control and anxiety emotions between perceived stress and sleep quality. ResultsA total of 322 valid questionnaires were collected, yielding an effective responsive rate of 86.56%. Among the respondents, 146 (45.34%) reported poor sleep quality. The CPSS score and GAD-7 score of resident physicians were positively correlated with the PSQI score (r=0.727, 0.784, P<0.01), while the DMSC-S score was negatively correlated with the PSQI score (r=-0.615, P<0.01). Perceived stress directly and positively predicted poor sleep quality (B=0.124, P<0.01), with the direct effect accounting for 31.39% of the total effect. Furthermore, perceived stress indirectly affected sleep quality through the independent mediating effects of self-control and anxiety emotions. The indirect effect values of 0.053 (95% CI: 0.019 - 0.091) and 0.192 (95% CI: 0.141 - 0.249), accounting for 13.42% and 48.61% of the total effect, respectively. Perceived stress also impact sleep quality through the serial mediating effect of self-control and anxiety, with the indirect effect value of 0.026 (95% CI: 0.005 - 0.049), accounting for 6.58% of the total effect. ConclusionThe perceived stress of resident physicians can influence sleep quality by impairing self-control, exacerbating anxiety, and through the serial mediation of both factors.
2.Development and application of a plasma ablation device

Although low temperature plasma ablation technology has significant advantages in clinical applications,its poor stability,high power consumption and thermal injuries to the tissues are still key factors limiting its widespread use.To address these issues,a device that uses bipolar pulses to excite plasma is developed,and it can flexibly adjust its output voltage,frequency and duty cycle,effectively reducing breakdown voltage and power consumption.By conducting simulation and saltwater discharge experiments,the excitation process of plasma is elaborated in detail.Meanwhile,the effects of two different pulse excitation modes on plasma excitation are discussed and compared.The results show that the bipolar pulse excitation mode is advantageous in reducing discharge current and breakdown voltage,and shortening the formation time of the vapor layer,which effectively relieves thermal injuries to the tissues.Finally,the broad application potential of the plasma ablation device is demonstrated in plasma ablation experiments which are conducted at different voltages,with pork as the experimental material.The study provides a new theoretical basis and practical guidance for the application of plasma ablation technology in minimally invasive surgery.
3.Application of quantitative electroencephalography in digital screening for mild cognitive impairment

Objective To explore the quantitative electroencephalography(qEEG)characteristics of the prefrontal cortex in patients with mild cognitive impairment(MCI)during digital screening tasks for MCI screening.Methods A total of 592 MCI patients(MCI group)and 317 normal cognitively elderly individuals(control group)were recruited from 40 communities in Nanjing,Jiangsu Province,from July to August,2024.All participants were as-sessed using Montreal Cognitive Assessment-Beijing Version(MoCA-BJ).Prefrontal EEG data were collected using a portable EEG device,and power spectral analysis was performed via Fast Fourier Transform.An XG-Boost algorithm was employed to construct an MCI identification model based on qEEG power features,and the model's performance was evaluated using receiver operating characteristic(ROC)curve.Results Compared with the control group,prefrontal δ,α,and β band power increased during screening tasks in MCI group(P<0.05);δ power was negatively correlated with MoCA-BJ total scores,and visuospatial/executive func-tion,attention and delayed recall scores(r=-0.269,-0.169,-0.133,-0.171,P<0.001);α power was negative-ly correlated with MoCA-BJ total scores,attention and delayed recall scores(r=-0.113,-0.075,-0.091,P<0.05).The XGBoost model based on δ and α power was excellent in MCI identification,with an area under the curve of 0.91,accuracy of 0.81,precision of 0.89,F1 score of 0.84,recall of 0.80,and specificity of 0.81.Conclusion MCI patients exhibit increased power in the prefrontal δ and α frequency bands during digital screening tasks,which is associated with cognitive decline.An XGBoost model based on qEEG power features can enable early prediction of MCI.

Objective: To systematically analyze the revisions content and technological development trends of microbiological standards in the Chinese Pharmacopoeia (ChP) 2025 Edition, and explore its novel requirements in risk-based pharmaceutical product lifecycle management.
Methods: A comprehensive review was conducted on 26 microbiological-related standards to summarize the revision directions and scientific implications from perspectives including the revision overview, international harmonization of microbiological standards, risk-based quality management system, and novel tools and methods with Chinese characteristics.
Results: The ChP 2025 edition demonstrates three prominent features in microbiological-related standards: enhanced international harmonization, introduced emerging molecular biological technologies, and established a risk-based microbiological quality control system.
Conclusion: The new edition of the Pharmacopoeia has systematically constructed a microbiological standard system, which significantly improves the scientificity, standardization and applicability of the standards, providing a crucial support for advancing the microbiological quality control in pharmaceutical industries of China.

The microbial limit standards in the Chinese Pharmacopoeia 2020 edition primarily focus on total aerobic microbial count and specified objectionable microorganisms,which are insufficient for comprehensively assessing the potential risks posed by microbial contamination to drug efficacy and patient safety.With the increasing com-plexity of rising regulatory requirements,there is an urgent need to establish a scientific and systematic microbial risk assessment and control framework.In response,the Chinese Pharmacopoeia 2025 edition,introduces a new general guideline 9212 Risk Assessment and Control of Unacceptable Microorganisms for Non-sterile Products.This chapter systematically constructs a framework for the identification and control of risks associated with unacceptable microorganisms,filling a gap in the relevant field within international pharmacopoeias.This article systematically elaborates on its key elements based on its core framework and content.It covers the development background,core concepts,testing and identification strategies,evaluation of risk characterization factors,formulation of risk control measures,and implementation pathways.The aim is to provide pharmaceutical enterprises and regulatory agencies with systematic and clear practical guidance.

8.Evaluation of a cut-off value for the plasma aldosterone-to-renin ratio in screening for primary aldosteronism in different age and gender groups

Objective:To explore the optimal cut-off value of aldosterone-to-renin ratio (ARR) for primary aldosteronism screening in hypertensive populations stratified by sex and age.Methods:This study was a cross-sectional study. Patients who were diagnosed with hypertension from November 2016 to December 2023 at the First Affiliated Hospital of Dalian Medical University were included. Upright direct renin concentration (DRC) and plasma aldosterone concentration (PAC) were measured using chemiluminescence, and the ARR was calculated as PAC/DRC. Patients were divided into primary aldosteronism and primary hypertension groups based on the results of comprehensive screening tests and confirmatory tests (saline infusion test and/or captopril challenge test). Spearman correlation analysis was used to explore the correlation between ARR and age. Patients were stratified by age (≤40, >40 to 50, >50 to 60, and >60 years) and sex. The efficacy of ARR for primary aldosteronism screening was assessed by drawing the receiver operating characteristic curve and calculating the area under curve ( AUC), and to explore the optimal cut-off values for different ages and sexes. Results:A total of 1 282 hypertensive patients were enrolled, aged 46.0 (37.0, 56.0) years with 746 males. ARR showed a positive correlation with age in both primary aldosteronism ( r=0.168, P=0.007) and primary hypertension patients ( r=0.327, P<0.001). In the general population, male patients, and female patients, the AUC values of ARR screening for primary aldosteronism were 0.975 (95% CI 0.967-0.982), 0.989 (95% CI 0.983-0.995), 0.957 (95% CI 0.942-0.972), respectively. In the four groups of patients ≤40, >40 to 50, >50 to 60, and >60 years, the AUC values of ARR screening for primary aldosteronism were 0.990 (95% CI 0.983-0.997), 0.973 (95% CI 0.958-0.988), 0.965 (95% CI 0.947-0.982), 0.958 (95% CI 0.933-0.984), respectively. In the four groups of patients aged ≤40, >40 to 50, >50 to 60, and >60 years, the optimal ARR cut-off values for primary aldosteronism screening were 2.31, 2.67, 2.94, and 3.68 (ng·dl -1)/(mU·L -1)(1 ng/dl=27.7 pmol/L), respectively. The optimal ARR cut-off values for primary aldosteronism screening were 2.37 and 2.94 (ng·dl -1)/(mU·L -1) in male and female patients, respectively. Conclusion:The optimal cut-off value of ARR in the screening of primary aldosteronism increases with age, and the optimal cut-off value of ARR in female patients is higher than that in male patients. The ARR cut-off value should be selected individually based on the patient′s characteristics in clinical practice.
