1.Mechanism of Qingre antai decoction in improving pregnancy outcomes of threatened abortion rats with blood heat syndrome based on JAK2/STAT3 and PI3K/AKT dual signaling pathways

OBJECTIVE To explore the mechanism by which Qingre antai decoction improves pregnancy outcomes of threatened abortion rats with blood heat syndrome. METHODS The pregnant rats were randomly divided into normal group, model group, dydrogesterone group (0.002 g/kg), and Qingre antai decoction group (44.1 g/kg), with 13 rats in each group. Except for normal group, other groups were given warming-yang Chinese medicine and corresponding drugs intragastrically, once a day, for 12 consecutive days. On the 13th day of pregnancy, a single intragastric administration of mifepristone (5 mg/kg) was performed to establish a model of threatened abortion with blood heat syndrome. On the 14th day of pregnancy, the abortion rate and uterine coefficient were calculated; the pathological morphology of pregnant uterine was observed; the serum levels of 3,5,3′-triiodothyronine (T3), thyroid hormone (T4), thyroid stimulating hormone (TSH), as well as the levels of vascular endothelial growth factor (VEGF) and nitric oxide (NO) in the pregnant uterus were all determined; the expressions of mRNA and protein related to Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (AKT) pathways were detected. RESULTS Compared with normal group, the model group exhibited endometrial tissue damage, a reduced number of decidual cells, and a significant presence of blood stasis within the uterus; abortion rate, the serum levels of T3, T4 and TSH, the mRNA expressions of JAK2, STAT3 and suppressor of cytokine signaling 3 (SOCS3) as well as protein expressions of p-JAK2, p-STAT3 and SOCS3 in the pregnant uterus were increased significantly ( P <0.05); uterine coefficient, the levels of VEGF and NO in pregnant uterus, mRNA expressions of VEGFR2, PI3K, AKT and endothelial nitric oxide synthase(eNOS), protein expressions of VEGFR2, PI3K and eNOS as well as phosphorylation level of AKT in the pregnant uterus were significantly reduced ( P <0.05). Compared with model group, the endometrial tissue damage and congestion in the Qingre antai decoction group were significantly improved, and the levels of the aforementioned quantitative indicators were significantly reversed ( P <0.05). CONCLUSIONS Qingre antai decoction can improve the pregnancy outcomes in rats with threatened abortion of blood heat syndrome, the mechanism of which may be associated with inhibiting JAK2/STAT3 pathway and activating PI3K/AKT pathway.
2.Hearing loss prevalence and burden of disease in China: Findings from provincial-level analysis.

BACKGROUND:
Without timely and effective rehabilitation, hearing loss may profoundly affect human life quality. China has a large population of hearing-impaired individuals, which imposes a heavy health burden on society. Moreover, this population is projected to increase rapidly owing to China's aging society.
METHODS:
We used data from a population-representative epidemiological investigation of hearing loss and ear diseases in four Chinese provinces. We estimated the national prevalence using multiple linear regression of the age-group proportions and prevalence in 31 provinces with clustering analysis. We used years lived with disability (YLDs) to analyze the disease burden and forecasted the prevalence of hearing loss by 2060 in China.
RESULTS:
An estimated 115 million people had moderate-to-complete hearing loss in 2015 across the 31 provinces of China (8.4% of 1.37 billion people). Of these, 85.7% were older than age 50 years (99 million people) and 2.4% were younger than 20 years old (2.8 million people). Of all YLDs attributable to hearing loss, 68.9% were attributable to moderate-to-complete cases. By 2060, a projected 242 million people in China will have moderate-to-complete hearing loss, a 110.0% increase from 2015.
CONCLUSIONS
The hearing loss prevalence in China is high. Population aging and socioeconomic factors substantially affect the prevalence and severity of hearing loss and the disease burden. The prevalence and severity of hearing loss are unevenly distributed across different provinces. Future public health policies should take these trends and regional variations into account.

3.Improvement mechanism of Pangshi antai zhixue decoction on spontaneous abortion with heat syndrome by regulating NLRP3 inflammasome

OBJECTIVE To investigate the mechanism of Pangshi antai zhixue decoction in the improvement of spontaneous abortion with heat syndrome by regulating the NOD-like receptor protein 3 (NLRP3) inflammasome.METHODS The binding activities of 13 main components in Pangshi antai zhixue decoction with NLRP3,apoptosis-associated speck-like protein containing a CARD (ASC),and caspase-1 precursor (pro-caspase-1) were predicted by molecular docking.Sixty 1-day-old pregnant rats were randomly divided into normal group,model group,dexamethasone group (0.002 g/kg),and Pangshi antai zhixue decoction low-,medium-,and high-dose groups (11.025,22.05,44.10 g/kg),with 10 rats in each group.Each group was given distilled water/corresponding medicinal solution intragastrically,once a day,for 12 consecutive days.Except for normal group,other groups were given traditional Chinese medicine for warming yang and mifepristone to establish a model of spontaneous abortion with heat syndrome.24 h after the last medication,serum levels of triiodothyronine (T3),thyroxine (T4),interleukin-2 (IL-2),IL-4,IL-6,IL-10,and interferon-γ (IFN-γ) were all detected;the abortion rate and uterine coefficient were calculated;the pathological morphology of the pregnant uterus was observed;protein expressions of NLRP3,ASC and caspase-1 were detected.RESULTS The molecular docking results showed that the binding energies of 13 main components of Pangshi antai zhixue decoction with NLRP3,ASC,and pro-caspase-1 were all less than-5 kJ/moL.The animal experiment results showed that compared with normal group,the uterine coefficient and serum levels of IL-4,IL-6 and IL-10 were decreased significantly in model group (P<0.05);the abortion rate and serum levels of T3,T4,IL-2 and IFN-γ as well as protein expressions of NLRP3,ASC and caspase-1 were increased significantly (P<0.05);there were abortion lesions in the pregnant endometrium.Compared with the model group,most of the quantitative indicators mentioned above were significantly reversed in Pangshi antai zhixue decoction groups (P<0.05),and the endometrial miscarriage lesions in pregnancy were improved to varying degrees.CONCLUSIONS Pangshi antai zhixue decoction influences the immune balance between mother and fetus by regulating the formation of NLRP3 inflammasome,down-regulating pro-inflammatory cytokines such as IFN-γ and IL-2,and up-regulating anti-inflammatory cytokines such as IL-4,IL-6 and IL-10,thereby improving spontaneous abortion with heat syndrome.
4.Improvement effect of arbutin on myocardial fibrosis model rats and its mechanism

OBJECTIVE To study the improvement effects of arbutin on myocardial fibrosis (MF) model rats and its mechanism. METHODS The network pharmacology was used to predict the potential target of arbutin in improving MF and molecular docking was used to validated. Totally 50 SD rats were given isoprenaline subcutaneously (5 mg/kg, once a day, for 14 consecutive days) to induce the MF model. Modeled rats were randomly divided into model group, captopril group (9 mg/kg), arbutin low-dose, medium-dose and high-dose groups (50, 100, 200 mg/kg), with 10 rats in each group. Another 10 healthy rats were included as normal group. Each group was given the corresponding drugs, once a day, for 28 consecutive days. Twenty-four hours after the final administration, electrocardiograms and heart-related indexes [heart weight index (HWI), left ventricular weight index (LVWI)] of rats were detected; the levels of creatine kinase (CK), lactate dehydrogenase (LDH), N-terminal pro-brain natriuretic peptide (NT-proBNP) and type Ⅰ collagen (Col Ⅰ) and Col Ⅲ were detected in myocardial tissue of rats; the pathological changes of myocardial tissue were observed, and protein and mRNA expressions of adenosine deaminase (ADA) and adenosine kinase (ADK) were detected in the myocardial tissue of rats. RESULTS The results of network pharmacology showed that the main targets of arbutin improving MF were ADA and ADK. The results of molecular docking showed that arbutin bind stably with ADA and ADK. The results of experimental verification showed that compared with model group, the amplitude of ST and T waves in electrocardiogram were improved in administration groups, and the symptoms of atrial flutter were alleviated; HWI (except for arbutin medium-dose group), LVWI, the levels of CK, LDH, NT-proBNP, Col Ⅰ and Col Ⅲ in the myocardial tissue of rats were decreased significantly (P<0.05); the degree of myocardial fibrosis in rats decreased; protein and mRNA expressions of ADA and ADK in the myocardial tissue were significantly increased (P<0.05). CONCLUSIONS Arbutin can improve cardiac fibrosis and cardiac function of MF model rats, the mechanism of which may be associated with up-regulating protein and mRNA expressions of ADA and ADK,influencing the nucleotide metabolism and collagen generation. zhangminghao@hactcm.edu.cn
5.Effect of flurbiprofen axetil on perioperative body temperature and shivering in patients undergoing radical gastrectomy

Objective To investigate the effect of flurbiprofen axetil on perioperative body temperature and shivering in the patients undergoing radical gastrectomy.Methods A total of 120 patients with ASA grade Ⅱ or Ⅲ who underwent radical gastrectomy and entered the recovery room after operation from January to August 2022 were selected.The patients were divided into the flurbiprofen axetil group (F group) and con-trol group (C group) by the random number table method,60 cases in each group.The group F was intrave-nously infused by flurbiprofen axetil 100 mg (10 mL) after anesthesia induction,and the group C was intrave-nously infused after anesthesia induction.The general data of the patients were collected.The blood pressure,heart rate,mean arterial pressure and eardrum temperature of the patients were records before operation (T0),entering the recovery room (T1) and leaving the recovery room (T2).The number,intensity grade and duration of chills after entering the recovery room were evaluated.The VAS scores and adverse reactions such as nausea and vomiting were recorded at 10 min (T3 ) and 30 min (T4) after extubation.Results Compared with at T0,the eardrum temperature of the two groups at T1 and T2 was significantly decreased (P<0.05). There was no significant difference in ear temperature at T1 and T2 between the two groups (P>0.05).Com-pared with the control group,the incidence of shivering was significantly improved and the intensity was light-er in the F group (P<0.05).However,there was no statistically significant difference in the duration of shiv-ering (P>0.05).The VAS scores at T3 and T4 in the group F were significantly lower than those in the group C (P<0.05).There was no statistically significant difference in the incidence rate of postoperative nausea and vomiting between the two groups (P>0.05).Conclusion Flurbiprofen axetil could improve the occurrence of postoperative pain and shivering in the patients with radical gastrectomy,but does not affect the occurrence of perioperative hypothermia.
6.Diagnosis and management of surgical complications and dysplasia related to the ileal pouch

At present, total proctocolectomy (TPC) and ileal pouch-anal anastomosis (IPAA) is the standard surgical treatment for patients with medically refractory ulcerative colitis (UC) , UC combined with carcinoma, and familial adenomatous polyposis. However, there may be a series of surgical complications related to the pouch after IPAA, such as anastomotic leakage, pouch stenosis and pouch fistula formation. There are specificities in clinical diagnosis and treatment for these complications. The wrong diagnosis and improper treatment can lead to the failure of pouch and the switch to the permanent stoma. The dysplasia of pouch is a high-risk factor for carcinoma. And it may become carcinoma, if not detected or treated in time. Therefore, based on complete understanding of the surgical complications and dysplasia related to pouch, a clear diagnosis and timely treatment will help to increase the success rate of surgery and improve the prognosis of patients. This article summarizes and discusses the current progress in the diagnosis and treatment of surgical complications and dysplasia related to pouch for reference.

8.Historical evolution and current concepts of surgical treatment for inflammatory bowel diseases.

Patients with inflammatory bowel disease (IBD) may require surgical intervention for refractory disease or complications. Prompt surgery and appropriate surgical procedures are critical when surgery is indicatedd. With continuous optimization and innovation of surgical procedures, there have been significant changes in the concepts and operations of IBD in the past century. Learning the evolution of surgical treatment for IBD could help us understand the rationale, indications, and pertinent techniques of surgical procedures. Innovations are emerging in IBD management including the advent of biological agents, laparoscopy, and multi-disciplinary team approach, it is imperative for IBD specialist to learn the state-of-the-art knowledge.

9.Experimental study of adipose-derived mesenchymal stem cells in the treatment of Crohn's disease.

OBJECTIVETo explore the efficacy of adipose-derived mesenchymal stem cells (ADMSCs) in a murine model of inflammatory bowel disease, and its potential mechanism.
METHODSMurine colitis mouse model of Crohn's disease(CD) was created by trinitrobenzene sulfonic acid(TNBS)-induced colitis. Seventy-five 6-8 weeks female BALB/c mice were randomly divided into 3 groups: control group, TNBS group and ADMSC group. To verify the therapeutic effect of ADMSC, real-time PCR and immunohistochemical staining were performed to measure inflammatory cytokines levels in colon tissues. The 10-day survival statuses were recorded after the infusion of ADMSCs.
RESULTSIntraperitoneal injection of ADMSCs alleviated the clinical and histopathologic severity of intestinal inflammation, and increased survival(60% vs. 30%, P<0.05) in the TNBS-induced mouse model of CD. Compared with TNBS group, proinflammatory cytokines, including TNF-α, IL-12 and VEGF of ADMSC group were significantly reduced, with significant increase of IL-10 expression.
CONCLUSIONADMSCs can effectively repair the injury of colonitis through down-regulation of proinflammatory cytokines TNF-α, IL-12 and VEGF expression, and up-regulation of anti-inflammatory cytokine IL-10 expression, which may be a potential new alternative of cell-based therapy for CD.

10.Application of k-carrageenan in valsartan immediate-release pellets by extrusion-spheronization

The aim of the study was to select a suitable pelletisation aid of valsartan immediate-release pellets in the extrusion process and optimized the formulation. The properties of the pellets with five excipients which were microcrystalline cellulose(MCC), low-substituted hydroxypropyl cellulose(L-HPC), crospovidone(PVPP), pregelatinized starch(PCS)and k-carrageenan were evaluated and compared by the single factor test. And the pelletisation aids were chosen preliminary. The properties of the pellets with MCC, L-HPC, k-carrageenan respectively were evaluated and compared and k-carrageenan was determined as the most appropriate pelletisation aid. The Box-Behnken design was employed to optimize the formulation. The optimised formulation was k-carrageenan 16. 98 g, HPMC-E5 2. 03 g, SLS 0. 26 g. The yield and aspect ratio of pellets was 91. 23% and 1. 14, respectively. And there was no significant difference between observed and predictive responses. The results showed k-carrageenan pellets owned properties of a high yield, acceptable sphericity and fast drug release.
