1.A cohort study on the correlation between metabolic syndrome and cholecystolithiasis and gallbladder polyp in Uygur population in rural areas of southern Xinjiang

Objective:To investigate the correlation between metabolic syndrome (MS), its different components and the risk of cholecystolithiasis and gallbladder polyp in Uygur population in rural areas of southern Xinjiang.Methods:This study was a prospective cohort study. A baseline survey was conducted in August 2016. A typical sampling method was used to select 10 476 Uygur people in rural areas of southern Xinjiang as the research objects. Baseline clinical data were collected, including demographic data such as age, gender, and education level, and laboratory examination indicators such as blood glucose and triglyceride levels. According to the MS diagnostic criteria of the relevant guidelines, 10 476 subjects were divided into the MS group (3 475 cases) and the non-MS group (7 001 cases). The incidence of cholecystolithiasis and gallbladder polyp was followed up in 2019, 2021 and 2023, respectively. Cox regression was used to analyze the correlation between MS, its different components and the risk of cholecystolithiasis and gallbladder polyp. Chi-square test and independent sample t test were used for statistical analysis. Results:The median follow-up time was 6.43 years in 10 476 subjects, and the overall cumulative incidence of cholecystolithiasis and gallbladder polyp was 5.43% (569/10 476). The cumulative incidence of cholecystolithiasis and gallbladder polyp in the MS group was 10.73% (373/ 3 475), which was significantly higher than that in the non-MS group (2.80% (196/7 001)); χ2= 284.62, P<0.001). The results of multivariate Cox regression analysis showed that, 41 to 59 years old ( HR=1.26, 95% confidence interval (95% CI): 1.03 to 1.54, P=0.025), ≥60 years old ( HR=1.88, 95% CI: 1.45 to 2.45, P<0.001), female ( HR=1.34, 95% CI: 1.13 to 1.60, P=0.001), MS ( HR=2.19, 95% CI: 1.59 to 3.01, P<0.001), hypertriglyceridemia ( HR=1.47, 95% CI: 1.18 to 1.83, P=0.001), hypertension ( HR=1.30, 95% CI: 1.04 to 1.62, P=0.023), and hyperglycemia ( HR=1.24, 95% CI: 1.01 to 1.52, P=0.041) were independent risk factors for cholecystolithiasis and gallbladder polyp. After the adjustment of age and gender, MS ( HR=3.39, 95% CI: 2.82 to 4.07, P<0.001), hypertriglyceridemia ( HR=2.37, 95% CI: 2.00 to 2.81, P<0.001), hypertension ( HR=2.00, 95% CI: 1.66 to 2.41, P<0.001), and hyperglycemia ( HR=1.86, 95% CI: 1.55 to 2.23, P<0.001) were still correlated with cholecystolithiasis and gallbladder polyp, and there was the srtongest correlation between MS and cholecystolithiasis and gallbladder polyp. The results of univariate Cox regression analysis showed that along with the increase of accumulated of MS components, the risk of cholecystolithiasis and gallbladder polyp significantly increased (1 to 5 components corresponding HR (95% CI) were 1.92 (1.13 to 3.24), 2.21 (1.32 to 3.69), 6.91 (4.22 to 11.30), 8.56 (5.15 to 14.22), and 10.73 (5.66 to 20.33); P=0.015, =0.002, <0.001, <0.001, and <0.001); after age and gender were adjusted, this trend still existed (1 to 5 components corresponding HR (95% CI) were 1.81(1.07 to 3.06), 1.95(1.16 to 3.27), 5.64(3.42 to 9.32), 6.69(3.97 to 11.25), and 7.76(4.04 to 14.91); P=0.028, =0.012, <0.001, <0.001, and <0.001). Conclusion:MS and its components can increase the risk of cholecystolithiasis and gallbladder polyp, and the risk of cholecystolithiasis and gallbladder polyp significantly increases along with the increase of accumulated of MS components.
2.Effects of oxycodone on perioperative pain and cognitive function in elderly patients with colorectal cancer

Objective:To investigate the effects of oxycodone on perioperative pain and cognitive function in elderly patients with colorectal cancer.Methods:A prospective randomized-controlled trial was conducted. A total of 88 elderly patients with colorectal cancer who underwent elective laparoscopic surgery at Benxi Central Hospital from June 2022 to December 2023 were selected. According to random number table method, all patients were randomly divided to the experimental group and the control group, with 44 cases in each group. Anesthesia in the experimental group was induced with oxycodone 0.2-0.3 mg/kg, intraoperative oxycodone was maintained at 0.1-0.15 mg·kg -1·h -1; oxycodone was intravenously injected with analgesia pump after surgery. In the control group, anesthesia was induced with sufentanil 0.2-0.3 μg/kg, remifentanil was intraoperatively maintained at 0.1-0.3 μg·kg -1·min -1, sufentanil was intravenously injected with analgesia pump after surgery. Blood pressure and heart rate before anesthesia induction (T 0), at tracheal intubation (T 1), at skin incision (T 2), and at extubation (T 3) of both groups were compared; numerical rating scale (NRS) at 30 minutes, 6 h, 24 h, and 48 h after surgery were scored, the confusion assessment method (CAM) and the mini-mental state examination (MMSE) score at day 1, 3, 7 after surgery were used to assess the postoperative early cognitive decline and other adverse reactions. Results:Finally, 81 patients were included in the study. There were 41 cases in the experimental group including 20 males and 21 females with the age of (76±3) years, and 40 cases in the control group including 19 males and 21 females with the age of (75±2) years. There were no statistically significant differences in gender composition, age, body mass index, operative time, intraoperative blood loss (all P > 0.05). There were statistically differences in blood pressure [(91±8) mmHg (1 mmHg = 0.133 kPa) vs. (89±10) mmHg at T 0, (92±9) mmHg vs. (90±8) mmHg at T 2, (93±9) mmHg vs. (92±9) mmHg at T 3] and heart rate [(70± 15) times/min vs.(69±16) times/min at T 0, (68±12) times/min vs. (67±12) times /min at T 2, (70± 15) times/min vs. (69±14) times/min at T 3] between the experimental group and the control group (all P > 0.05). Blood pressure and heart rate [(101±9) mmHg, (83±15) times /min] at T 1 in the experimental group were higher than those in the control group [(93±11) mmHg, (70±17) times /min], and the differences were statistically significant ( t values were 3.73, 3.77; all P < 0.001). There were no statistically significant differences in NRS scores [(2.6±1.2) scores vs. (2.8±1.1) scores at 30 min, (2.8±1.6) scores vs. (2.9±1.3) scores at 6 h, (1.8±1.2) scores vs. (2.1±1.3) scores at 24 h, and (1.5±0.7) scores vs. (1.5±0.7) scores at 48 h after surgery] between the experimental group and the control group (all P > 0.05). The incidence of postoperative early cognitive decline [4.9% (2/41) vs. 22.5% (9/40)], nausea and vomiting after surgery [7.3% (3/41) vs. 25.0% (10/40)] in the experimental group was lower than that in the control group, and the difference was statistically significant ( χ2 values were 5.36, 4.70; P values were 0.021, 0.030, respectively). Conclusions:The application of oxycodone during laparoscopic surgery can meet the needs of perioperative analgesia and improve the postoperative early cognitive function of elderly patients with colorectal cancer.

5.Aging-Induced Endothelial Glycocalyx Alteration and Vascular Dysfunction

The endothelial glycocalyx(EG)is a polyglycoprotein complex present on the internal vascular surface,and its impairment is associated with the progression of multiple diseases,including atherosclerosis,stroke,sepsis,diabetes,kidney disease,hypertension,and lung edema.Therefore,glycocalyx health can be used as a biomarker to evaluate vascular health.Aging leads to dysfunctional changes in the glycocalyx;for example,its thickness decreases,and the genes of enzymes involved in its synthesis and digestion are dysregulated.As a natural barrier to the vascular system,age-related glycocalyx disruption is associated with vascular dysfunction,including impairment of vascular contraction and dilation,enhancement of permeability,dysregulation of inflammatory and immune reactions,and imbalance of anticoagulation and thrombin.From the perspective of'structure determines function'studies on the changing regularity of the thickness,components,microstructure,and mechanical properties of EG with aging and its relationship with vascular dysfunction are of great significance for the prevention,diagnosis,and treatment of atherosclerosis and other age-related cardiovascular diseases.
6.Establishment and practice of virtual simulation experimental teaching platform of Health Management

In order to solve the problems in traditional health management experimental teaching, such as high cost, safety and difficult repeatability, a virtual simulation experimental teaching platform for health management combined with Traditional Chinese Medicine (TCM) characteristics has been constructed by using virtual simulation, multimedia, and human-computer interaction technologies, which comprises TCM constitution identification, TCM health management and health management service process. Through the combination of virtual simulation and reality situation, the platform has formed an online and offline model of experimental teaching, which has improved the innovation and practice ability of students and enhanced the teaching quality.
7.Significance of PI3K/Akt/HIF-1α signaling pathway expression in nucleus pulposus cells at different oxygen concentrations in delaying intervertebral disc degeneration

BACKGROUND:Intervertebral disc degeneration is a condition caused by the combined effects of various factors,such as cellular apoptosis,oxidative stress,and inflammatory responses.Currently,it is believed that the core of this condition lies in the degeneration and apoptosis of nucleus pulposus cells(NPCs).However,the specific pathological mechanisms remain unclear. OBJECTIVE:By investigating the expression of the phosphatidylinositol-3 kinase(PI3K)/protein kinase B(Akt)/hypoxia-inducible factor 1-alpha(HIF-1α)signaling pathway and the apoptosis of NPCs under different oxygen concentrations,to clarify the biological characteristics of NPCs under varying oxygen levels,and to explore the mechanisms of intervertebral disc degeneration. METHODS:Normal and degenerated NPCs were collected.The second-generation cells were used for imaging identification.The third-generation cells were cultured in the following conditions:37℃,air,100%humidity.Leibovitz's medium containing 100 μmol/L CoCl2 and 10%fetal bovine serum medium containing 100 μmol/L H2O2 were used to create distinct oxygen concentration environments for the third-generation cells,allowing for the investigation of cellular responses and behaviors under normal,low oxygen,and oxidative stress conditions.The third-generation cells were divided into six groups:normal NPCs+hypoxia,normal NPCs+normoxia,normal NPCs+oxidative stress,degenerated NPCs+hypoxia,degenerated NPCs+normoxia,and degenerated NPCs+oxidative stress groups.Cell viability and proliferation were assessed using the cell counting kit-8 method.Cell apoptosis was detected using flow cytometry.RT-PCR and western blot assay were utilized to examine the protein and mRNA expression levels of PI3K,AKT,and HIF-1α. RESULTS AND CONCLUSION:At different oxygen concentrations,the cell proliferation rate of both normal and degenerated NPCs decreased as the oxygen concentration increased.Conversely,the apoptosis rate increased as the oxygen concentration rose(P<0.05).With the normal NPCs+hypoxia group as the control group,the effect of degenerated NPCs on the apoptosis rate was highly significant(P<0.001).Oxygen concentration had a highly significant impact on both NPC proliferation rate and apoptosis rate(P<0.001).The interaction between cell degeneration and oxygen concentration significantly affected both NPC proliferation and apoptosis rates(P<0.05).At different oxygen concentrations,the protein and mRNA expression levels of PI3K,AKT,and HIF-1α in normal NPCs were highest under low oxygen concentration,followed by oxidative stress environment,and lowest under normoxia.In degenerated NPCs,the protein and mRNA expression levels of PI3K,AKT,and HIF-1α decreasd as the oxygen concentration increased.The protein and mRNA expression levels of PI3K and Akt in normal NPCs were significantly higher than those in degenerated NPCs(P<0.05).With the normal NPCs+hypoxia group as the control group,the effects of normal or degenerated NPCs,as well as oxygen concentration,on the protein and mRNA expression of PI3K,AKT,and HIF-1α were highly significant.The interaction between cell degeneration and oxygen concentration also had an extremely significant effect on the protein and mRNA expression of PI3K,AKT,and HIF-1α(P<0.001).To conclude,there is a close correlation between the proliferation and apoptosis of NPCs and both oxygen concentration and the cellular functional state.The PI3K/Akt/HIF-1α signaling pathway exhibits antagonistic regulatory effects under various cellular functional states and different oxygen concentration environments.The mechanism may be associated with the activation of the PI3K/Akt signaling pathway,which consequently transcribes HIF-1α,thus maintaining the balance of reactive oxygen species metabolism.This pathway plays a significant role in inhibiting oxidative stress,antagonizing NPCs apoptosis,and consequently delaying intervertebral disc degeneration.
8. Effects and mechanism of combined exposure of di(2-ethylhexyl) phthalate and bisphenol A on glucose metabolism in female rats during gestational and lactational period

OBJECTIVE: To investigate the effects of combined exposure to di(2-ethylhexyl) phthalate(DEHP) and bisphenol A(BPA) on glucose metabolism in female rats during gestational and lactation periods, and its possible mechanism. METHODS: Twenty-four specific pathogen free pregnant SD rats were randomly divided into control group, DEHP group, BPA group, and combined exposure group, with 6 rats in each group. From the 5 th day of gestation to the 21 st day after birth of the offspring, the rats in the DEHP group were treated with DEHP 600 mg/kg body weight(bw); rats in BPA group were treated with 80 mg/kg bw BPA, and rats in combined exposure group were treated with 600 mg/kg bw DEHP and 80 mg/kg bw BPA by intragastric perfusion, while the rats in the control group were given the same amount of corn oil, once per day. After exposure, maternal rats were sacrificed immediately. The levels of glucose metabolism related indicators in liver tissues and serum were examined, and the mRNA and protein expression of phosphatidylinositol 3-kinase(PI3 K)/protein kinase B(AKT) signaling pathway related factors in liver tissues were detected by real-time fluorescence quantitative polymerase chain reaction and Western blot. RESULTS: Except for the activity of phosphoenolpyruvate carboxykinase(PEPCK) in BPA group, the levels of liver glycogen and serum high density lipoprotein cholesterol(HDL-C) in rats of the 3 exposure groups decreased(P<0.05), while the activity of serum PEPCK and the level of low density lipoprotein cholesterol(LDL-C) increased(P<0.05) compared with rats in the control group. The levels of liver glycogen and serum HDL-C in the combined exposure group were lower than that in the BPA group(P<0.05), while the level of serum LDL-C were lower than that in DEHP group and BPA group(P<0.05). The levels of serum glycosylated serum protein, total cholesterol and triglyceride in the 4 groups were not statistically different when compared with each other(P>0.05). Except for the PI3 K protein in DEHP group, the mRNA and protein expression of PI3 K, AKT, and glucose transporter 4 in liver tissues of rats in the 3 exposure groups decreased(P<0.05), and the mRNA expression of forkhead box protein 1(Foxo1) decreased(P<0.05), but the protein expression of FOXO1 increased(P<0.05) compared with the control group. CONCLUSION: Exposure to DEHP or BPA during pregnancy and lactation can cause glucose metabolism disorders in rats. The combined exposure of DEHP and BPA has certain synergistic effect. This process may be achieved through the PI3 K/AKT signaling pathway.

Objective To investigate the expression of E-cadherin,vimentin,β-catenin and transforming growth factor-β 1(TGF-β 1) in oral squamous cell carcinomas(OSCC).Methods Eighty-nine cases of OSCC and 20 cases of normal oral mucosa were collected.Then the 89 cases of OSCC were classified as grade Ⅰ,Ⅱ,Ⅲ.The semiquantitative method was used to calculated the positive intensity and positive rate.The reiationship between the OSCC differentiation and the four biomarkers was analyzed.Results The median of E-cadherin was 9.00 in the normal tissue,9.00,6.00 and 6.00 in OSCC Ⅰ,Ⅱ and Ⅲ,respectively.There was significant difference between the normal group and OSCC group(Z=-4.211,P=0.000).The median of vimentin was 0.00 in the normal tissue,0.00,0.00 and 4.00 in OSCC Ⅰ,Ⅱ and Ⅲ,respectively.There was significant difference between the normal group and OSCC group(Z=-3.675,P=0.000).The median of β-catenin was 9.00 in the normal tissue,3.00,4.00 and 3.00 in OSCC Ⅰ,Ⅱ and Ⅲ,respectively.There was significant difference between the normal group and OSCC group(Z=-6.300,P=0.000).The median of TGF-β1 was 2.00 in the normal tissue,3.00,4.00 and 6.00 in OSCC Ⅰ,Ⅱ and Ⅲ,respectively.There was significant difference between the normal group and OSCC group(Z=-3.329,P=0.000).E-cadherin expression was positively correlated to β-catenin expression(r=0.327,P=-O.002),negtively correlated to vimentin expression(r=-0.386,P=0.001) and positively correlated to TGF-β1 expression(r=-0.304,P=0.004).Vimentin expression was positively correlated to TGF-β 1 expression (r=0.401,P=0.000).Conclusions E-cadherin and β-catenin in OSCC had a down-regulated expression,while the vimentin has an up-regulated expression.
