1.Early visual quality and stereopsis after implantation of trifocal intraocular lenses

AIM:To investigate the visual quality and stereopsis after the implantation of PanOptix trifocal intraocular lens(TFNT00).METHODS: A prospective clinical study was conducted. A total of 36 cataract patients(50 eyes)who underwent phacoemulsification combined with TFNT00 implantation in Jinan Mingshui Eye Hospital from November 2022 to April 2024 were selected. They were followed up until 3 mo after the operation. The uncorrected distance visual acuity(UCDVA), uncorrected intermediate visual acuity(UCIVA), uncorrected near visual acuity(UCNVA), objective scatter index(OSI), modulation transfer function cut off(MTF-cut-off), Strehl ratio(SR)and 100%, 20%, 9% contrast visual acuity(CVA)were observed. The binoptometer was used to collect the patients' far and near stereopsis acuities. The defocus curve was drawn after the operation; the Chinese version of the VF-14 Visual Function Index Scale was used to evaluate the visual quality and satisfaction after the operation.RESULTS: There were statistically significant differences in the UCDVA, UCIVA and UCNVA of the patients at different time after the operation(all P<0.05). The transition of the defocus curve was gentle between +0.5--3.0 D after the operation. The OSI value at 3 mo postoperatively after the operation was significantly lower than that before the operation(P<0.01), and the MTF-cut-off, SR, 100% CVA, 20% CVA and 9% CVA were significantly improved than those before operation(all P<0.01). The far and near stereopsis acuities of 34 patients were abnormal before the operation. The far stereopsis acuities of the patients who underwent bilateral eye surgeries were all normal after the operation, and the near stereopsis acuity of 12 patients was normal. Among the patients who underwent unilateral eye surgeries, the far stereopsis acuities of 13 patients were normal, and the near stereopsis acuities of 11 patients were normal. The far and near stereopsis acuities of the patients who underwent bilateral eye surgeries were significantly better than those patients who underwent unilateral eye surgeries.CONCLUSION: The PanOptix trifocal intraocular lens can provide patients with good full-range visual acuity. The stereopsis of the patients with bilateral implants is better than that of the patients with unilateral implants. The overall visual quality and satisfaction of the patients after the operation are relatively high.
2.Clinical features and gene variation analysis of aminoacylase-1 deficiency patients caused by ACY1 gene mutation

Objective:To investigate the clinical characteristics and genetic variations of patients with aminoacylase-1 deficiency (ACY1D) caused by ACY1 gene mutations, in order to enhance clinicians′ understanding of this rare disease. Methods:Clinical and genetic data of a child with ACY1D admitted to Sir Run Run Hospital, Nanjing Medical University in December 2021 were collected. Using "aminoacylase-1 deficiency" "aminoacylase-1 gene" " ACY1" and "ACY1D" as keywords, relevant cases of ACY1 gene mutations were searched in CNKI, Wanfang Data Knowledge Service Platform, OMIM, and PubMed databases until February 2025. The clinical characteristics and types of genetic variations of previously reported ACY1D patients were summarized and analyzed. Results:The patient was an 8-year and 4-month-old boy. Clinical manifestations included growth retardation, ataxia, and focal epileptic seizures. Increased excretion of various N-acetylamino acids was observed in the urine. Cranial magnetic resonance imaging showed cerebellar atrophy. Whole-exome sequencing results showed a compound heterozygous mutation in the ACY1 gene: c.1063-1G>A (IVS14-1G>A) and c.170G>A (p.G57D) (reference transcript NM_000666.2), with c.170G>A (p.G57D) being a novel mutation. Family validation results showed that the c.1063-1G>A (IVS14-1G>A) mutation originated from his mother, and the c.170G>A (p.G57D) mutation originated from his father. By literature review 11 English articles were retrieved reporting 18 ACY1D patients, along with the child in this study, totaling 19 cases, with an onset age ranging from 1 week to 4 years and 6 months. Among them, 13/19 patients showed growth retardation, 9/19 patients had language disorders, 8/19 patients had intellectual disabilities, 7/19 patients had ataxia and low muscle tone, 6/19 patients had epilepsy and febrile convulsions, and 3/19 patients had irritability, autism, and muscle weakness. Genetic testing results indicated various types of mutations in the ACY1 gene, including missense, splicing, and frameshift mutations. Conclusions:ACY1D is an autosomal recessive genetic disease caused by ACY1 gene mutations, which is relatively rare in China. The main clinical manifestations include growth retardation, intellectual and language disorders. The c.170G>A heterozygous mutation is a newly discovered variant site, expanding the mutation spectrum of the ACY1 gene. Screening for ACY1 gene variations can aid in achieving a definitive diagnosis..
3.Efficacy of tranexamic acid in reducing perioperative blood loss in craniomaxillofacial plastic and cosmetic surgery: a systematic review and meta-regression analysis

Objective: To comprehensively evaluate the clinical efficacy of a single dose of tranexamic acid (TXA) in reducing perioperative blood loss in patients undergoing craniomaxillofacial plastic and cosmetic surgery through meta-regression analysis. Methods: Embase, PubMed, Wanfang Data, VIP database, China National Knowledge Infrastructure (CNKI), the Chinese Clinical Trial Registry (ChiCTR) and Cochrane Central Register of Controlled Trials (CENTRAL) were electronically retrieved to collect clinical studies evaluating efficacy of perioperative TXA administration in patients undergoing craniomaxillofacial plastic and cosmetic surgery, from inception to August 2024. Quality assessment of randomized controlled trials (RCTs) was performed using Cochrane Collaboration's Risk of Bias Tool. Based on the results of methodological heterogeneity, corresponding meta-analyses were conducted using either random-effects or fixed-effects models in R programming software. Results: Thirty-one articles were included, involving 2 072 patients who underwent craniomaxillofacial plastic and cosmetic surgeries. Among these patients, 1 051 were in the TXA treatment group, and 1 021 were in the control group. The paired meta-analysis showed that compared with the control group, the use of TXA significantly reduced bleeding volume in perioperative patients [standardized mean difference (SMD)=-1.13; 95%CI (-1.47, -0.80), P<0.001]. Subgroup analysis revealed that TXA significantly reduced intraoperative bleeding volume in patients across different surgeries, with the order of efficacy as follows: orthognathic surgery [SMD=-1.44; 95%CI (-2.07, -0.80), P<0.001], cleft palate repair [SMD=-1.32; 95%CI (-2.14, -0.50), P<0.001], rhinoplasty [SMD=-0.97; 95%CI (-1.63, -0.30), P<0.001], and craniosynostosis [SMD=-0.96; 95%CI (-1.40, -0.53), P=0.040]. The result of the meta regression showed there was no significant difference in the hemostatic effect of TXA on patients with increasing doses (5, 10, 15, 20, 25 mg/kg) (P=0.650). Sensitivity analysis verified that the pooled values were stable and reliable. The Egger's test indicated a certain degree of publication bias (Z=-3.40, P<0.001). Conclusion: Existing evidence suggests that TXA effectively reduces perioperative blood loss in patients undergoing craniofacial plastic surgery, regardless of its dosage administered.
4.Comparison of the accuracy of intraocular lens calculation formulas based on different types of corneal refractive power

AIM: To compare the accuracy of intraocular lens(IOL)calculation formulas based on different corneal refractive power in calculating IOL diopters of cataract patients with a history of corneal refractive surgery.METHODS: A prospective clinical study was conducted with a cohort of 32 cataract patients(42 eyes)who had previously undergone myopic laser corneal surgery at Jinan Mingshui Eye Hospital between February 2022 and August 2024. The study employed several IOL calculation formulas, including the Haigis-L formula, the Barrett True K formula based on simulated keratometry(SimK), the Haigis formula based on total keratometry(TK), the Potvin-Hill Pentacam(PVP)formula based on corneal true net power(TNP), and the OCT formula based on net corneal power(NCP). These formulas were used to calculate IOL power and predict postoperative refractive outcomes. At 1 mo postoperatively, subjective refraction was performed, and the prediction error(PE), mean absolute prediction error(MAE), median absolute prediction error(MedAE), and the percentage of prediction errors within the ranges of ±0.25, ±0.50, ±0.75, and ±1.0 D were determined.RESULTS: The intraclass correlation coefficient for the four types of corneal refractive power was 0.986(P<0.001). There was no significant difference between TNP and NCP(P=0.491), and there were differences between the other two groups(all P<0.001). Statistically significant differences were observed between PE and 0 for the Haigis-L(K)and Haigis(TK)formulas(all P<0.001). In contrast, no statistically significant differences were noted between PE and 0 for the PVP, OCT, and Barrett True K formulas(all P>0.05). The MedAE value of Barrett True K was the smallest 0.32(0.19, 0.71)D among the five formulas, and there was no significant difference in MedAE among the five formulas(P=0.870). The proportion of eyes with PE within ±0.25 and ±1.0 D in Barrett True K formula was 38%(16/42)and 95%(40/42), respectively. The proportion of eyes within ±0.50 D in PVP formula was 71%(30/42); and the proportion of eyes with PE within ±0.75 D in Haigis(TK)formula was 83%(35/42).CONCLUSION: After corneal refractive surgery, there are differences between different types of corneal refractive power. When calculating IOL, the accuracy of TK combined with Haigis formula is better than that of Haigis-L(K)formula, and Barrett True K formula shows good accuracy.
5.Establishment and application of TaqMan fluorescent quantitative PCR detection method of Nocardia in dairy cows

Nocardia is a Gram-positive pathogen responsible for causing dairy mastitis,which leads to purulent granulomatous lesions in mammary tissue and can significantly impact the dairy indus-try,resulting in substantial economic losses.To develop a rapid and accurate diagnostic method for detecting Nocardia of bovine origin,a conserved sequence of the 16S rRNA gene from Nocardia was selected from the NCBI database.Based on this sequence,a pair of primers and a TaqMan fluo-rescent quantitative probe were designed.The validation of the TaqMan fluorescence quantitative PCR(qPCR)method found in this study showed that the Ctvalue had a good linear relationship with recombinant plasmid concentrations ranging from 1×1010 to 1×102 copies/μL,with a regres-sion equation of y=-3.536x+43.78,a correlation coefficient(R2)of 0.997 5,a slope of-3.536,and an amplification efficiency(E)of 91%(where 90%<E<110%).The specificity was strong,with no cross-reactions with other pathogens.The standard curve had a high sensitivity with a low-er detection limit of 1 × 102 copies/μL,it was 100-fold higher than conventional PCR.The repeatability of the standard curve was also good.Both intra-and inter-group coefficients of varia-tion were below 2%.Using this method,234 milk samples and 80 environmental samples were tested using this method,respectively,with a positive detection rate of 27.07%,whereas conven-tional PCR had a positive detection rate of 19.43%,indicating that this method was more sensitive compared to conventional PCR.The fluorescent quantitative PCR detection method established in this study provides an effective means for the clinical detection of Nocardia in dairy cows.

7.Progress in mutual regulation of lncRNA and METTL3 in tumors

Epigenetic modifications of RNA play a crucial role in the initiation and progression of tumors.Among these modifications,N6-methyladenosine(m6A)modification catalyzed by the METTL3 methyltransferase is one of the most common RNA modifications.It is essential for regulating RNA transcription,stability,and translation.At the same time,long non-coding RNAs(lncRNAs)also serve as important regulatory molecules and play a significant role in the development and progression of tumors.However,the reciprocal interaction between lncRNAs and METTL3,as well as their functional mechanisms within tumors,are not fully understood.This paper aims to provide a comprehensive over-view of the mutual regulation between lncRNAs and METTL3 in tumors.The goal is to offer new insights and theoretical foundations for a better understanding of tumor pathogenesis.
8.Constructing the medical insurance version of the Long's scale using the Delphi method:A research study

Objective:To transform the categorical Long's Activities of Daily Living Assessment Scale(referred to as the Long's Scale)into a medical insurance version(continuous scoring)by assigning scores,enabling a more intu-itive reflection of functional changes in individuals with disabilities during the rehabilitation process.Method:The research team,through in-depth discussions and comprehensive literature review,revised the scoring and assessment methods of the Long's Scale(0 points,1 point,2 points),and proposed two im-proved scoring schemes for the Long's Scale(+7 points,+9 points).The Delphi expert consultation method was employed to consult experts and determine the Medical Insurance Long's Scale.Retrospective analysis was used to validate the reliability and validity of this scale.Result:A total of 24 experts were invited to participate in the consultation,with an expert consultation re-sponse rate of 87.5%(21/24).The average authority coefficient of the experts was 0.836,with 66.7%(14/21)of experts supporting the+7 points improved scoring scheme for the Long's scale.The Medical Insurance Long's Scale demonstrated good internal consistency and correlation with the modified Barthel index(MBI),consistent with the initial Long's Scale,demonstrating showing excellent reliability and validity.Conclusion:The+7 points scoring scheme in this study transformed the Long's Scale into a continuous scor-ing scale,enabling the measurement of functional improvement,thereby providing a new basis for value-based medical insurance payments in rehabilitation.

10.Early Closed Reduction and Percutaneous Kirschner Wire Pinning for Pediatric Supracondylar Humerus Fractures

Objective To investigate the effect of early closed reduction and percutaneous Kirschner wire pinning for Gartland type Ⅱ and Ⅲ pediatric supracondylar humerus fractures.Methods Clinical data of 123 children with Gartland type Ⅱ and Ⅲsupracondylar humerus fractures admitted to our hospital from January 2021 to December 2022 were retrospectively analyzed.According to the time from injury to operation,they were divided into early group(≤ 24 h)and delayed group(>24 h).The perioperative conditions,postoperative complications,fracture healing,and elbow joint functional recovery were compared between the two groups.Results Compared to the delayed group,the early group had less operation time[(22.9±11.3)min vs.(40.3±17.5)min,t=-6.116,P=0.000],less number of intraoperative fluoroscopy[(10.6±4.5)times vs.(17.4±5.5)times,t=-7.471,P=0.000],shorter hospital stay[(3.3±1.0)d vs.(4.2±1.3)d,t=-3.866,P=0.000],and less hospitalization cost[(10 555.0±971.2)yuan vs.(10 972.4±938.9)yuan,t=-2.345,P=0.021].The rate of anatomical reduction in the early group was higher than that in the delayed group[79.0%(60/76)vs.59.6%(28/47),x2=5.354,P=0.021].No serious complications such as vascular and nerve injury,compartment syndrome,ischemic myosclerosis,myositis ossificans,or cubitus varus and valgus occurred in both group after surgery.The incidences of tension blisters[6.6%(5/76)vs.23.4%(11/47),x2=7.265,P=0.007],plaster loosening[22.4%(17/76)vs.40.4%(19/47),x2=4.574,P=0.032],and pin tract infection[2.6%(2/76)vs.14.9%(7/47),x2=4.757,P=0.029]in the early group were lower than those in the delayed group.The fracture union time,and the time to start active and full weight-bearing activities in the early group were earlier than those in the delayed group(P<0.05).The Flynn elbow joint scores at 3 and 6 months after surgery in the early group were better than those in the delayed group(Z=-3.469,P=0.001;Z=-2.961,P=0.003).Conclusion Early closed reduction and percutaneous Kirschner wire pinning is effective in the treatment of pediatric supracondylar humerus fractures,which can shorten the operation time and hospital stay,reduce the hospitalization cost,lower the incidence of complications,and restore elbow joint function in early stage.
