Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.
2.Distribution of pathogenic bacteria causing postoperative infections in children undergoing surgery for Hirschsprung's disease and clinical sig-nificance of serum sTREM-1 and HMGB1

Objective:To explore the distribution of pathogenic bacteria causing postoperative infections in chil-dren undergoing surgery for Hirschsprung's disease(HD),and the clinical significance of serum triggering receptor ex-pressed on myeloid cells-1(sTREM-1)and high mobility group box 1(HMGB1).Methods:98 children with HD admit-ted from January 2021 to January 2025 were divided into infection group(n=42)and non-infection group(n=56)accord-ing to whether the children were complicated with infection after operation.Automatic bacterial identification system was used to detect the distribution of pathogenic bacteria in secretion samples from infected sites of HD children after operation.ELISA was used to measure serum sTREM-1 and HMGB1.ROC was used to explore the diagnostic value of serum sTREM-1 and HMGB1 for severe infection in children with HD.Results:A total of 49 strains of pathogenic bac-teria were isolated from 42 children with HD in infection group,of which 63.27%(31/49)were gram-negative bacteria,accounting for the highest proportion,followed by Gram positive bacteria at 30.61%(15/49)and fungi at 6.12%(3/49).Among them,Pseudomonas aeruginosa had the highest proportion of 24.49%(12/49)among Gram negative bacteria.there were 15 cases of mild infection(15 strains of pathogens),16 cases of moderate infection(17 strains of pathogens)and 11 cases of severe infection(17 strains of pathogens).With the aggravation of the infection degree of HD children,the distribution pattern of pathogenic bacteria changed,and gradually changed to mainly gram-negative bacteria infec-tion.Compared with non infected group,the levels of serum sTREM-1 and HMGB1 in infected group were higher(P<0.05).The levels of serum sTREM-1 and HMGB1 in children with severe HD infection are higher than those in children with mild and moderate HD infection(P<0.05).The AUC of the combined diagnosis of serum sTREM-1 and HMGB1 for severe infection in HD children was the highest,at 0.985,which was superior to the separate diagnosis of serum sTREM-1 and HMGB1(Zcombination-sTREM-1=1.988,P=0.047;Zcombination-HMGB1=2.126,P=0.034).Conclusion:Serum sTREM-1 and HMGB1 are elevated in HD children with postoperative infection,and the pathogenic bacteria are mainly gram-negative bacteria.In addition,the combined detection of the two provides a reference for the clinical diagnosis of severe infection in HD children.
3.Study on the effectiveness and safety of a novel intravascular shock wave balloon for pre-treatment of severe coronary artery calcification lesions

Objective To evaluate the efficacy and safety of a novel intravascular lithotripsy(IVL)balloon—Vesscrack shockwave balloon—for vascular preparation before stent implantation in patients with severe coronary artery calcification(CAC).Methods This was a prospective,single-arm,multicenter study conducted in China from June 2022 to October 2022.Patients with severe CAC were treated with the Vesscrack shockwave balloon for lesion preparation,followed by drug-eluting stent(DES)implantation.Of these,33 patients underwent optical coherence tomography(OCT).The primary endpoint was procedural success,defined as successful stent implantation with residual stenosis≤30%and the absence of in-hospital major adverse events,including cardiac death,target vessel-related myocardial infarction,or target lesion revascularization.Results A total of 170 patients[mean age:(65.9±7.9)years,116 males]were enrolled.After treatment with IVL and DES,the minimum lumen diameter increased significantly compared to baseline[(2.34±0.40)mm vs.(0.95±0.33)mm,P<0.001],the degree of stenosis was significantly reduced[(13.24±6.60)%vs.(65.18±10.59)%,P<0.001].Procedural success was achieved in 100%of cases,and device success was 98.8%.The 30-day patient-related cardiovascular clinical composite endpoint(POCE)rate was 0.0,with no target lesion failure,no confirmed or potential thrombotic events were observed.The shockwave energy generator demonstrated excellent stability and ease of use.Among the 33 patients assessed with OCT,after IVL intervention,the maximum calcified area of the lumen[(3.51±1.51)mm2 vs.(2.85±1.80)mm2,P<0.001],and the minimum lumen area within the target lesion[(3.08±1.04)mm2 vs.(2.02±0.75)mm2,P<0.001],and after DES intervention,the luminal area of the largest calcified site[(6.59±1.64)mm2 vs.(2.85±1.80)mm2,P<0.001]and the minimum luminal area within the target lesion[(6.19±1.45)mm2 vs.(2.02±0.75)mm2,P<0.001]were significantly increased,and the differences were statistically significant.Conclusions The Vesscrack shockwave balloon is effective and safe for vascular preparation in patients with severe CAC prior to stent implantation.It achieves significant calcified plaque modification,high procedural success rates,and minimal complications.
4.Study on Risk Factors and Etiology of Urinary Tract Infection Caused by Urinary Calculi in Eastern Fujian Region

Objective:To explore the risk factors and etiological characteristics of urinary tract infection caused by urinary calculi in eastern Fujian region,in order to attract clinical attention and improve the prevention and treatment of urinary tract infection caused by urinary calculi.Methods:A total of 154 patients with urinary calculi admitted to Ningde People's Hospital(n=80)and Ningde Mindong Hospital(n=74)from November 2022 to October 2023 were selected as the main research objects.According to whether the patients had urinary tract infection,they were divided into infection group and non-infection group.The baseline data of the two groups were analyzed in detail,and the risk factors and pathogen distribution of urinary tract infection in patients with urinary calculi were analyzed.Results:There were 33 cases of urinary tract infection in 154 patients with urinary calculi,accounting for 21.43%.Univariate analysis showed that the urinary white blood cell count in the infection group was higher than that in the non-infection group,and the proportion of patients with effusion,urinary tract obstruction,calculi in the upper urinary tract,staghorn calculi,smoking history,diabetes,and urinary nitrite positive was higher than that in the uninfected group(P<0.05).The results of binary logistic regression analysis showed that effusion,urinary tract obstruction,staghorn calculi,smoking history,diabetes,high urine white blood cell count and positive urine nitrite were independent risk factors for urinary tract infection in patients with urinary calculi(OR>1,P<0.05).A total of 33 strains of pathogenic bacteria were isolated from 33 patients in the infection group.Among them,23 strains(69.70%)were gram-negative bacteria,8 strains(24.24%)were gram-positive bacteria,and 2 strains(6.06%)were fungi.Among gram-negative bacteria,escherichia coli accounted for the highest proportion(48.48%),followed by klebsiella pneumoniae(9.09%).Among gram-positive bacteria,enterococcus faecalis accounted for the highest proportion(12.12%),followed by enterococcus faecium(6.06%).Candida and candida tropicalis in fungi was the same,accounted for 3.03%.Conclusion:The risk of urinary tract infection in patients with urinary calculi in eastern Fujian region is high.Effusion,urinary tract obstruction,staghorn calculi,smoking history,diabetes,high urine white blood cell count and positive urine nitrite are independent risk factors for urinary tract infection in patients with urinary calculi.The main urinary tract pathogens are gram-negative bacteria.

9.Risk factors of colostomy site incisional hernia after colorectal cancer and the construction and validation of the nomogram model

Objective:To explore the risk factors of colostomy site incisional hernia associated with colorectal can-cer and to construct and validate a nomogram model.Methods:A retrospective case-control study was conducted to collect and analyze the clinical data of patients who underwent stoma reversal after colorectal cancer surgery in the Gen-eral Surgery Department of Linyi People's Hospital from January 2019 to January 2023.A total of 371 patients were en-rolled and divided into SSIH group and non-SSIH group according to whether SSIH occurred.SPSS 25.0 software was used to perform univariate analysis on 22 related factors of patients,and multivariate analysis was performed on statisti-cally significant risk factors to screen out the independent influencing factors of SSIH related to CRC surgery.According to the results of multivariate analysis,the R language version 4.3.1 was used to establish and verify the nomogram risk prediction model.Results:SSIH occurred in 52 of 371 patients(14.02%).The results of univariate analysis showed that BMI,coronary heart disease,incision infection,stoma location,parastomal hernia,and closure time were statistically sig-nificant(P<0.05).Multivariate analysis showed that coronary heart disease,closure time and parastomal hernia were in-dependent risk factors for SSIH after CRC(P<0.05).Based on the above results,the nomogram prediction model was successfully drawn and verified.Conclusions:Coronary heart disease,closure time and parastomal hernia are inde-pendent risk factors for SSIH after CRC surgery.The nomogram constructed based on these factors has a high predictive value for SSIH,which can provide a basis for clinicians to prevent and treat SSIH,and help reduce the incidence of SSIH.
