1.Correlation Analysis of MHR,NLR,PLR and Bone Density,Bone Metabolism Markers in Elderly Essential Hypertension with Osteoporosis Patients

Objective:To analyze the correlation of monocyte/high density lipoprotein ratio(MHR),neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and bone density,bone metabolism markers in elderly essential hypertension with osteoporosis(OP)patients.Methods:This study was a retrospective study,198 elderly essential hypertension patients who were admitted in our hospital from January 2022 to June 2024 were selected.According to the patient's bone mass test results,patients were divided into normal bone mass group(n=79),osteopenia group(n=68)and OP group(n=51).MHR,NLR,PLR,bone density(left hip,left femoral neck and lumbar spine L1-L4)and bone metabolism markers were compared among three groups,the correlation of MHR,NLR,PLR and bone density,bone metabolism markers were analyzed by Pearson correlation.Results:MHR,NLR,PLR in normal bone mass group,osteopenia group and OP group increased in turn(P<0.05).The left hip,left femoral neck and lumbar spine L1-L4 bone density in normal bone mass group,osteopenia group,OP group decreased in turn(P<0.05).25-hydroxyvitamin D[25(OH)D]in normal bone mass group,osteopenia group and OP group decreased in turn,alkaline phosphatase(ALP)and parathyroid hormone(PTH)increased in turn(P<0.05).MHR,NLR and PLR were negatively correlated with left hip bone density,left femoral neck bone density,lumbar spine L1-L4 bone density,25(OH)D and positively correlated with PTH and ALP(P<0.05).Conclusion:The increase of MHR,NLR and PLR in elderly essential hypertension with OP patients can affect bone mineral density and bone metabolism.
2.Comparison of the effects of remimazolam and propofol anesthesia on postoperative delirium in elderly patients with lung cancer undergoing thoracoscopic surgery

Objective To evaluate the effect of remimazolam on the incidence of postoperative delirium(POD)in elderly lung cancer patients undergoing thoracoscopic surgery.Methods A total of 114 elderly patients who underwent unilateral thoracoscopic lung surgery at the Affiliated Hospital of Xuzhou Medical University from October 2024 to April 2025 were recruited in this trial.Patients were randomly assigned to remimazolam group(group R)and propofol group(group P).Anesthesia induction and maintenance were performed with remimazolam and propofol,respectively.In R group,0.5 mg of flumazenil was intravenously injected at the end of the surgery for specific antagonism.The incidence of delirium and Quality of Recovery-15(QoR-15)scores on the first and third postoperative days were compared between the two groups.Intraoperative hemodynamic parameters,total opi-oid dosage,fluid balance,tracheal tube extubation time after surgery,and the incidence of adverse reactions were recorded.Results There were no statistically significant differences between the groups in the incidence of POD or in QoR-15 scores(P>0.05).Compared with group P,patients in group R had a shorter extubation time(P<0.05),more stable hemodynamics,lower incidences of intra-operative hypotension and bradycardia,and reduced requirement for vasoactive drugs(P<0.05).The consumption of rescue analgesics in the post-anesthesia care unit(PACU)was also lower in group R(P<0.05).No significant differences were observed between the groups in the amounts of sufentanil and remifentanil administered,fluid balance,bispectral index(BIS)values,or the inci-dence of nausea and vomiting(P>0.05).Conclusion In elderly patients undergoing elective thoracoscopic sur-gery,remimazolam-based induction and maintenance of anesthesia did not significantly alter the incidence of POD or compromise postoperative recovery quality compared with propofol.However,the remimazolam group required fewer vasoactive agents and exhibited a shorter tracheal extubation time.
3.Molecular epidemiological characteristics of Yersinia pestis in Marmota himalayana plague foci in Subei Mongolian Autonomous County,Gansu Province

This study was aimed at determining the genetic characteristics of Yersinia pestis in Subei County through differ-ential region(DFR),clustered regularly interspaced short palindromic repeats(CRISPR),and variable number tandem repeat(VNTR)analyses,to guide the tracing of plague outbreaks.The DNA of 89 Yersinia pestis strains isolated from various ani-mal in foci of Subei County from 1973 to 2017 was extracted.Primers for genotyping by DFR,CRISPR,and MLVA were used in PCR,and agarose electrophoresis was used to determine whether the amplified products were present.Genotypes were deter-mined through comparison against the DFR database of Yersinia pestis in China.The PCR products were sequenced and com-pared against the online CRISPR database for Yersinia pestis to determine the genotype.The number of VNTR repeats in each strain was calculated through capillary electrophoresis,and the minimum spanning tree was constructed with BioNumerics 7.6 according to the numbers of VNTR repeats from 89 Yersinia pestis strains in Subei County and 11 strains from a Marmota hi-malayana focus in Qinghai Province.The Yersinia pestis strains in Subei County were divided into six main genotypes by DFR:8,7,lb,5,32,and 44.The Yersinia pestis strains were divided into three gene clusters and three genotypes by CRISPR.Ca35'was the main gene cluster in Subei County;the genotype was 26';and the distribution was primarily in Dangchengwan Town,Yuerhong Township,and Shibaocheng Township.Ca7 and CaΔ5'comprised secondary gene clusters,with genotypes 22 and 24,and were distributed in Dangchengwan Town,Yuerhong Township,and Shibaocheng Township.The Yersinia pes-tis strains in Subei County were divided primarily into three clusters:the Dangchengwan Machang cluster,Yuerhong Township cluster and Dangchengwan Town cluster.Therefore,the Yersinia pestis strains in Subei County,divided into major and minor genotypes according to DFR,CRISPR and MLVA,showed different regional distribution characteristics,highly diverse geno-types,and complex population characteristics.These aspects are particularly important in tracing the sources of plague out-breaks.
4.In vivo pharmacokinetics of five effective constituents in Asteris Radix et Rhizoma in rats

AIM To investigate the in vivo pharmacokinetics of chlorogenic acid,isoquercetin,ferulic acid,isorhamnetin and shionone in Asteris Radix et Rhizoma in rats.METHODS Twelve rats were randomly assigned into 2 groups and given intragastric administration of conventional dose(0.63 g/kg)and large dose(3.3 g/kg)of Asteris Radix et Rhizoma extracts,respectively,after which blood collection was made at 0.083,0.167,0.33,0.5,0.75,1,1.5,2,4,6,8,12,24 h,UPLC-MS/MS was adopted in the plasma concentration determination of various effective constituents,and main pharmacokinetic parameters were calculated.RESULTS Various effective constituents in the large dose group demonstrated prolonged t1/2,MRT0-∞ as compared with those in the conventional dose group(P<0.05).After dose correction,Cmax of chlorogenic acid,isoquercetin,shionone in the large dose group displayed no obvious changes(P>0.05),while Cmax of ferulic acid,isorhamnetin,and AUC0-t,AUC0-∞ of various effective constituents were higher than those in the conventional dose group(P<0.05).CONCLUSION Various effective constituents in the high dose of Asteris Radix et Rhizoma can maintain high-concentration and long-time effects on rat bladder tissue.
5.Comparison of active constituent contents and their biological activities of Buzhong Yiqi Recipe with different dosage forms

AIM To compare total sugar,total protein,total phenol,total flavonoid,calycosin-7-O-β-D-glucoside,liquiritin,lobetyolin,quercetin,isoferulic acid,hesperidin,glycyrrhizic acid contents and their antioxidant activities,hypoglycemic activities of big honey pill,small honey pill,water pill,concentrated pill,granule,mixture and decoction of Buzhong Yiqi Recipe.METHODS Anthraquinone-sulfuric acid method,Coomassie brilliant blue method,Folin-phenol colorimetry method,sodium nitrite-aluminum nitrate method and HPLC were adopted in the content determination of total sugar,total protein,total phenol,total flavonoid and seven constituents,respectively,after which the scavenging capacities,reducing powers on DPPH·free radical,ABTS+free radical,hydroxyl free radical,and inhibition capacity on α-glucosidase activity were detected.Subsequently,correlation analysis was performed.RESULTS Total sugar,total protein,total phenol and total flavonoid contents demonstrated significant differences among different dosage forms(P<0.05,P<0.01).Calycosin-7-O-β-D-glucoside,glycyrrhizin,codonoside and quercetin displayed the highest contents in the decoction,while those of isoferulic acid,hesperidin and glycyrrhizin were observable in the mixture.The water pill exhibited the strongest antioxidant activity,while those of the concentrated pill and mixture were weak;the big honey pill exhibited the strongest hypoglycemic activity,while that of the decoction was the weakest.Total protein,total phenol,total flavonoid and liquiritin contents displayed significant positive correlations between antioxidant activity(P<0.05,P<0.01),while hesperidin content displayed significant negative correlation between the latter(P<0.05);total protein,calycosin-7-O-β-D-glucoside,codonoside and quercetin contents displayed significant negative correlations between hypoglycemic activity(P<0.05,P<0.01).CONCLUSION Active constituent contents and their biological activities of Buzhong Yiqi Recipe with different dosage forms exist differences,total sugar,total protein,total flavonoids,calycosin-7-O-β-D-glucoside,licorice glycoside,hesperidin,codonoside and quercetin can be taken as quality control indices for this prescription.
6.Epidemiological characteristics and risk factors for nosocomial infections in patients treated with ECMO in intensive care unit of a general hospital

OBJECTIVE To explore the current status,etiological characteristics and risk factors for nosocomial in-fections in the patients who are treated with extracorporeal membrane oxygenation(ECMO)so as to provide bases for treatment and prevention of nosocomial infections in the ICU patients treated with ECMO.METHODS The clinical data were retrospectively collected from the patients who were treated with ECMO in the ICU of Xinyang Central Hospital from Jan.2021 to Dec.2023.The patients were divided into the infection group and the non-infec-tion group according the status of nosocomial infection during the ECMO treatment period.The constituent ratios of pathogens isolated from the patients with infections were recorded,and multivariate logistic regression analysis was performed for independent risk factors for the ECMO-related nosocomial infections.RESULTS Of 86 patients who were finally enrolled in the study,33(38.37%)had nosocomial infections.Totally 54 strains of pathogens were isolated from the patients with the infections,43(79.63%)of which were gram-negative bacteria,7(12.97%)were gram-positive bacteria,and 4(7.41%)were fungi.There were 36(66.67%)strains of multi-drug-resistant organisms(MDROs)among the 54 strains of pathogens,and 27(81.82%)patients were detected with MDROs.Among the ECMO patients with postoperative nosocomial infections,21(63.64%)cases had pul-monary infections,8(24.24%)cases had bloodstream infection,and 4(12.12%)had urinary system infections.Multivariate logistic analysis showed that the high blood glucose level at the beginning of treatment with ECMO,long duration of ECMO treatment and long time of central venous catheter indwelling were the independent risk factors for the nosocomial infections in the patients treated with ECMO(P<0.05).CONCLUSION The isolation rate of gram-negative bacteria is relatively high among the pathogens isolated from the ECMO patients with post-operative nosocomial infections,and the drug resistance rates are high.The high blood glucose level,long duration of ECMO supporting treatment and long time of central venous catheter indwelling are the independent risk factors for the nosocomial infections in the patients treated with ECMO.

Aging is comprehensively influenced by multiple fac-tors such as internal genes,cellular metabolism,external envi-ronment,and lifestyle habits.Among them,epigenetic regula-tion plays a core role.Epigenetic modifications,including DNA methylation,histone modification,heterochromatin remodeling,and non-coding RNA regulation,act in concert with the three-di-mensional genome architecture to precisely regulate gene expres-sion.This review elaborates on the factors influencing epigenetic regulation,as well as the mechanisms of how epigenetics affects the occurrence of organismal aging and the corresponding inter-vention strategies,providing relevant insights for uncovering the mechanisms of aging and preventing/treating aging-related disea-ses.
8.Clinical observation of Huangkui capsule combined with cyclophosphamide and prednisone in the treatment of IgA nephropathy with renal insufficiency

OBJECTIVE To explore the clinical efficacy and safety of Huangkui capsule combined with cyclophosphamide and prednisone in the treatment of immunoglobulin A(IgA)nephropathy with renal insufficiency.METHODS A total of 117 patients with IgA nephropathy and renal insufficiency who were hospitalized in the department of nephrology of the Second Affiliated Hospital of Dalian Medical University from February 2021 to March 2024 were divided into prednisone group(n=38),cyclophosphamide group(n=39)and Huangkui group(n=40)according to the random number table method.On the basis of standardized basic treatment,the three groups were treated with prednisone,prednisone+cyclophosphamide,and prednisone+cyclophosphamide+Huangkui capsule,respectively,with a course of 6 months.The clinical efficacy,renal function indexes,immunoglobulin levels,inflammatory factor levels before and after treatment,and the incidence of adverse reactions during treatment were compared among the three groups.RESULTS Finally,107 patients completed the study(35 in prednisone group,37 in cyclophosphamide group,and 35 in Huangkui group).After 6 months of treatment,there was a statistically significant difference in the total effective rate among the three groups(P=0.028),and the total effective rate of the Huangkui group was significantly higher than that of the prednisone group(P=0.023).In terms of renal function,the levels of blood urea nitrogen(BUN),serum creatinine(Scr),and urinary microalbumin(Umalb)in the three groups after treatment were significantly lower than those before treatment,while the estimated glomerular filtration rate(eGFR)was significantly higher than that before treatment(P<0.05).Among them,the Huangkui group was superior to the other two groups in reducing BUN level(P<0.05),and both the Huangkui group and cyclophosphamide group were superior to the prednisone group in improving Scr,Umalb and eGFR(P<0.05).In terms of immunology,both the Huangkui group and cyclophosphamide group were superior to the prednisone group in increasing IgG level and decreasing IgA and IgM levels(P<0.05).In terms of inflammatory factors,the Huangkui group was superior to the prednisone group and cyclophosphamide group in reducing tumor necrosis factor-α level(P<0.05),and superior to the prednisone group in reducing interleukin-6 level(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions among the three groups(P>0.05).CONCLUSIONS Huangkui capsule combined with cyclophosphamide and prednisone has a good therapeutic effect on IgA nephropathy with renal insufficiency.It can further improve patients'renal function and immune function,regulate inflammatory status,and has good safety.
9.Exploring the regulatory mechanism of the peripheral nerves in osteoporosis through neuropeptides

Osteoporosis is highly prevalent and a leading cause of disability and death in the elderly. Bone metabolism homeostasis is maintained through the coupling of osteoblasts and osteoclasts. While current treatments can reduce bone loss, they may cause side effects such as gastrointestinal discomfort. Neuroboneology has gained attention as the nervous system, both directly and indirectly, influences bone metabolism, remodeling, and formation. Studies have also highlighted the role of peripheral nerves and blood vessels in bone health. Neuropeptides, small signaling molecules produced by neurons, regulate vascular function and bone homeostasis. These neuropeptides may offer new insights into the "peripheral nerve-angiogenesis-bone homeostasis" regulation theory. This study explores the impact of peripheral nerves on osteoporosis through neuropeptides.
10.Allergen Sensitization of 2610 Children in Qianwan New District of Ningbo City

Objective:To investigate distribution characteristics of allergen specific IgE of children with allergic diseases in Qianwan New District of Ningbo City,and provide guidance for clinical prophylaxis,cure and management of allergic diseases.Methods:This retrospective study enrolled pediatric patients with suspected allergic diseases presenting to the Department of Pediatrics at Ningbo Hangzhou Bay Hospital between January 2019 and October 2023.Allergen specific IgE and total IgE were detected by microarray enzyme-linked immunoassay.Results:52.9%of the investigated children are allergic to at least one allergen,39.8%had multiple sensitization.The main inhalation allergens were Dermatophagoides pteronyssinus(32.8%),D.farinae(32.3%),Blomia tropicalis(7.7%),Bermuda Grass(6.6%)and Timothy Grass(6.0%).The main food allergens were Egg White(23.1%),Milk(11.7%),Almond(4.9%)and Peanut(4.3%).Compared with girls(50.1%),boys(54.7%)are more likely to be sensitized to allergens.Infants and young children are predominantly affected by inhalation allergen;however,after three years old,there is a notable shift in the sensitization pattern,with inhalation allergens gradually becoming the predominant type of sensitization.Conclusions:It is common for children in Qianwan New District of Ningbo city to be sensitized to inhalation and food allergens.In addition to Mite,Egg White and Milk,Bermuda Grass and Timothy Grass are also important allergens.The distribution of allergens varies according to genders,age,and year.
