1.Xuefu Zhuyutang Ameliorates Metabolic-associated Fatty Liver Disease via AMPK Signaling Pathway

ObjectiveTo investigate the therapeutic mechanism of Xuefu Zhuyutang (XFZYT) for metabolic-associated fatty liver disease (MAFLD) through integrated network pharmacology and animal experiments. MethodsNetwork pharmacology was utilized to predict the core components, key therapeutic targets, and signaling pathways of XFZYT in the treatment of MAFLD. For animal experiments, a rat model of MAFLD was established by feeding a high-cholesterol diet for 4 weeks. Intervention was then administered with low-dose (2 g·kg-1) and high-dose (4 g·kg-1) XFZYT for 2 weeks. Biochemical assays were performed to measure the serum levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), total cholesterol (TC), triglycerides (TG), high-density lipoprotein (HDL), and low-density lipoprotein (LDL). In addition, the activities of superoxide dismutase (SOD) and catalase (CAT) and levels of malondialdehyde (MDA) and glutathione (GSH) in the serum were measured. The same way was adopted to measure the levels of TC and TG in the liver tissue. Enzyme-linked immunosorbent assay (ELISA) was employed to quantify the serum levels of interleukin (IL)-6, IL-1β, and tumor necrosis factor-alpha (TNF-α). Histopathological evaluations included hematoxylin and eosin (HE) staining for liver tissue morphology, Oil Red O staining for lipid deposition, and dihydroethidium (DHE) probe staining for reactive oxygen species (ROS) levels. Western blot analysis was conducted to assess the protein levels of AMP-activated protein kinase (AMPK), phosphorylated (p)-AMPK, nuclear factor erythroid 2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), nuclear factor-kappa B (NF-κB), and p-NF-κB in the liver tissue. Untargeted metabolomics analysis of the serum was performed by liquid chromatography-tandem mass spectrometry (LC-MS/MS). ResultsNetwork pharmacology analysis predicted 155 potential targets of XFZYT for MAFLD treatment, with core targets including signal transducer and activator of transcription 3 (STAT3), protein kinase B1 (Akt1), TNF, and IL-6. Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment primarily implicated the AMPK signaling pathway. Animal experiments demonstrated that compared with the normal group, the model group exhibited dyslipidemia, hepatic function impairment, pronounced hepatic lipid deposition, and inflammatory manifestations, with elevated serum levels of AST, ALT, TC, TG, LDL, and MDA (P<0.05), reduced HDL and GSH levels plus decreased SOD and CAT activities (P<0.05), downregulated protein levels of Nrf2, HO-1, and p-AMPK (P<0.05), and upregulated protein level of p-NF-κB (P<0.05) in the liver tissue. Compared with the model group, XFZYT intervention groups showed significant amelioration of dyslipidemia and hepatic function impairment, markedly reduced hepatic lipid deposition and inflammatory cell infiltration, decreased serum levels of AST, ALT, TC, TG, LDL, and MDA (P<0.05), increased HDL and GSH levels plus enhanced SOD and CAT activities (P<0.05), upregulated protein levels of Nrf2, HO-1, and p-AMPK (P<0.05), and downregulated protein level of p-NF-κB (P<0.05). Serum metabolomics revealed 511 differentially expressed metabolites (231 upregulated and 280 downregulated) between normal and model groups, while XFZYT groups versus model group showed 94 differential metabolites (51 upregulated and 43 downregulated). Among them, 11 metabolites displayed the most significant alterations, with enriched pathways including glycerolipid metabolism, cholesterol metabolism, and insulin resistance, multiple of which demonstrated AMPK association. ConclusionXFZYT alleviates MAFLD by regulating the AMPK signaling pathway and associated metabolic networks.

The prescription of Qidong Yixin oral liquid is derived from the experience of national medical master Ren Jixue in treating viral myocarditis (VMC). It has the functions of tonifying Qi, nourishing the heart,calming the mind, and relieving palpitations. It is used to treat VMC and angina pectoris of coronary heart disease caused by deficiency of both Qi and Yin. However,the understanding of its efficacy evidence, advantageous aspects, dosage and administration, and medication safety remains insufficient in clinical practice. Therefore,the development of the Expert Consensus on the Clinical Application of Qidong Yixin Oral Liquid (hereinafter referred to as consensus) was initiated. Consensus strictly followed the process and methods of the expert consensus on the clinical application of Chinese patent medicines of the China Association of Chinese Medicine,successively completing multiple tasks such as the consensus project initiation,determination of clinical problems,evidence search and evaluation,formation of recommendation opinions and consensus suggestions,solicitation of opinions,peer review, submission for review and release, and so on. Consensus formed a total of 10 recommendation opinions and 12 consensus suggestions,clarifying the clinical positioning,efficacy advantages,syndrome differentiation,dosage and administration,combination therapy,timing of medication,adverse reactions,contraindications, and precautions of Qidong Yixin oral liquid,indicating that it has good clinical advantages and safety in the treatment of VMC and angina pectoris of coronary heart disease,providing norms and references for physicians to safely and rationally apply Qidong Yixin oral liquid. Consensus was reviewed and approved for release by the Standardization Office of the China Association of Chinese Medicine on December 23, 2024. Standard number:GSCACM-376-2024.
3.Sclera Vessel Segmentation Based on Fusion Filtering and Reflection Suppression

ObjectiveIn traditional Chinese medicine (TCM), the foundational doctrine that the eyes reflect the essence of the internal viscera establishes ocular observation as a cornerstone of diagnostic practice. Specifically, the morphological characteristics and coloration variations of the scleral microvasculature serve as critical clinical indicators for assessing the dynamic balance of Qi and Blood, as well as the pathological status of internal organs. Historically, however, TCM eye diagnosis has relied predominantly on the subjective clinical experience and visual acuity of individual practitioners, leading to inherent challenges in standardization and reproducibility. While automated computer-aided diagnostic systems offer a promising solution, existing vessel segmentation algorithms encounter significant domain-specific bottlenecks when applied to scleral imagery. These challenges primarily stem from the highly reflective and moist nature of the ocular surface, which generates severe reflective interference. Furthermore, the inherent low contrast of fine capillary networks against complex background textures, compounded by non-uniform illumination, frequently results in high false-positive rates, misdetections, and severe vessel fragmentation. To address these critical limitations and advance the objective quantification of TCM diagnostics, this paper proposes a novel, highly robust sclera vessel segmentation framework that innovatively integrates Frangi-Sato dual-filter adaptive enhancement with pixel-level reflection detection. MethodsThe proposed methodology systematically addresses the segmentation pipeline through three synergistic stages. First, to overcome the structural limitations of single-filter approaches, a multi-scale weighted fusion strategy is meticulously designed to harness the complementary extraction capabilities of both Frangi and Sato filters. This adaptive enhancement optimally balances the preservation of main vessel trunk continuity with the heightened sensitivity required for delineating delicate, low-contrast peripheral capillaries. Second, to tackle the persistent issue of reflective highlights, a sophisticated multi-feature synergistic reflection detection module is introduced. By jointly analyzing local information entropy, gradient field variations, and intensity statistical distributions, this module achieves precise, pixel-level identification and elimination of reflective artifacts without compromising the underlying vascular structures. Finally, a dual-level adaptive thresholding strategy, featuring an innovative “core protection” mechanism, is implemented. This critical step effectively suppresses complex background noise while rigorously preserving the structural and topological integrity of the intricate vessel network, preventing the structural breaks often seen in conventional binarization methods. ResultsThe efficacy of the proposed framework was rigorously evaluated using both self-constructed clinical datasets specifically acquired for TCM research and standardized public datasets. Extensive experimental results demonstrate that the proposed method consistently outperforms state-of-the-art traditional approaches and contemporary deep learning models. Specifically, the proposed method achieves a Dice similarity coefficient of approximately 0.71 on the private clinical dataset, and secures the best performance across the majority of quantitative metrics on both datasets. Notably, the framework exhibits exceptional robustness and generalization capabilities in highly challenging scenarios characterized by intense reflective interference, low signal-to-noise ratios, and cross-domain image variations. ConclusionThis study successfully realizes the high-integrity, automated segmentation of scleral vessel networks under complex clinical imaging conditions. By overcoming the fundamental algorithmic challenges of reflection interference and micro-vessel loss, the proposed methodology provides potential support for the digitization, objective standardization, and intelligent advancement of modern TCM eye diagnosis systems.

5.Hypotension prediction index in the prediction of better outcomes: a systematic review and meta-analysis

Background:
The hypotension prediction index (HPI) is an algorithm designed to predict hypotension. Some studies have reported that HPI-guided hemodynamic management strategies decrease intraoperative hypotension and complications; however, the effect of HPI on reducing perioperative complications is controversial. This meta-analysis aimed to assess the efficacy of the HPI in reducing major complications and intraoperative hypotension.
Methods:
We conducted this meta-analysis according to the PRISMA statement and Cochrane Handbook guidelines. A comprehensive literature review was conducted to identify studies focusing on the efficacy of HPI-guided management in reducing intraoperative hypotension and postoperative complications. The PubMed, Embase, Scopus, and Web of Science databases were searched, and the resulting data were combined to calculate the pooled mean differences or risk ratios (RRs) with 95% CIs of both randomized controlled trials (RCTs) and retrospective studies, as appropriate. Heterogeneity and potential publication bias were also assessed.
Results:
Nineteen articles (12 RCTs and 7 retrospective studies) with 2570 recruited patients were included in this meta-analysis. The critical evaluation of the study quality revealed a low risk of bias in the included RCTs. Among the non-randomized trials, one was rated 7, two were rated 8, and the remaining four were rated 9 on the Newcastle-Ottawa Scale, indicating high quality and a low risk of bias. HPI-guided management significantly reduced intraoperative hypotension and associated major complications (RR = 0.79, 95% CI [0.69–0.90], I2 = 0, P < 0.001). Blood loss and length of hospital stay were comparable between the groups.
Conclusions
HPI-guided management significantly reduced intraoperative hypotension and major complications.

Sinusoidal obstruction syndrome/veno-occlusive disease (SOS/VOD) after hematopoietic stem cell transplantation (HSCT) in children has high incidence and mortality rates, and missed diagnosis of jaundice-free cases is common. In addition, there are still controversies over the efficacy of prophylactic defibrotide (DF) use, and the European Medicines Agency does not recommend the prophylactic use of DF, whereas clinical needs remain urgent. In 2026, by integrating controversial research evidence and focusing on specific prophylaxis for high-risk children, the European Society for Blood and Marrow Transplantation established a weighted cumulative scoring system to quantify risks and accurately identify the populations who may benefit from DF prophylaxis, in order to provide evidence-based support for the prophylactic use of DF, reduce the incidence and mortality rates of SOS/VOD in children, and improve the prognosis of patients. This article gives an excerpt of the key points in the consensus guidelines.
7.Jiebiao Qingli Decoction Regulates TLR7/MAPK/NF-κB Pathway to Prevent and Treat Pneumonia Induced by IAV Infection

ObjectiveTo explore the mechanism of Jiebiao Qingli decoction (JQD) in treating pneumonia caused by influenza A virus (IAV) infection. MethodsA total of 132 Balb/c mice were randomly assigned into normal control (NC), model control (IAV), oseltamivir (OSV, 37.5 mg·kg-1), and high-, medium-, low-dose JQD (H-, M-, and L-JQD: 6.05, 3.02, and 1.51 g·kg-1, respectively) groups. The NC group was treated with normal saline nasal drops, and the other groups were intranasally inoculated with A/Brisbane/02/2018 (H1N1) [pdm09-like virus (H1N1)] for the modeling of IAV infection. Two hours post-modeling, the NC and IAV groups were administrated with normal saline by gavage, while other groups received corresponding drugs for 7 d. The body mass, survival status, and deaths of mice were recorded daily during the administration of the drugs. On days 3 and 7, the lung index was measured for mice in each group. Pathological changes in the lung tissue were observed via hematoxylin-eosin staining. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was conducted to measure the viral load (IAV-M) and the mRNA levels of Toll-like receptor 7 (TLR7), p38 mitogen-activated protein kinase (p38 MAPK), and nuclear factor-kappa B (NF-κB) in the lung tissue. Western blot was employed to measure the protein levels of p38 MAPK and NF-κB. Enzyme-linked immunosorbent assay was used to quantify serum levels of interleukin-2 (IL-2), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-α). ResultsCompared with the NC group, the IAV group showed reduced survival quality and survival days (P<0.01), lung congestion, inflammatory cell infiltration, elevated lung index (P<0.01), increased viral load (P<0.01), upregulated TLR7, p38 MAPK, and NF-κB levels (P<0.05, P<0.01), decreased IL-2 level (P<0.01), and elevated IL-6 and TNF-α levels (P<0.01). Compared with the IAV group, H-JQD prolonged survival days (P<0.05). All JQD groups alleviated pathological changes in the lung tissue and reduced the lung index (P<0.01). M-JQD and H-JQD decreased the viral load (P<0.01). H-JQD downregulated the mRNA levels of TLR7, p38 MAPK, and NF-κB (P<0.05, P<0.01) and the protein levels of p38 MAPK and NF-κB (P<0.01), increased the serum IL-2 level (P<0.01), and lowered the IL-6 and TNF-α levels (P<0.05, P<0.01). M-JQD downregulated the mRNA level of NF-κB (P<0.01) and the protein level of p38 MAPK (P<0.05), elevated the IL-2 level (P<0.01), and lowered the TNF-α level (P<0.01). ConclusionM- and H-JQD can prevent and control IAV infection-induced pneumonia dose-dependently by inhibiting the TLR7/MAPK/NF-κB signaling pathway, increasing IL-2, and reducing excessive secretion of IL-6 and TNF-α.
8.Efficacy and safety of interventional treatment for refractory bleeding caused by head,neck,and maxillofacial trauma

Objective:To investigate the efficacy and safety of transcatheter arterial embolization(TAE)in the treatment of refractory bleeding caused by head,neck,and maxillofacial trauma.Methods:The clinical data of 26 patients with refractory bleeding caused by head,neck and maxillofacial trauma who were treated with TAE in The First Affiliated Hospital of Zhengzhou University and The Third People's Hospital of Henan Province were analyzed retrospectively,including 16 males and 10 females,with an average age of 57.96±15.52 years.All patients were treated with TAE because of the poor effect of medication,packing,and other measures and repeated bleeding.The clinical data,operation success rate,effective hemostasis rate,30-day clinical success rate,and postoperative complica-tions were analyzed.The patients were followed up for 3 to 6 months.Results:The operation success rate of TAE was 100%.The bleed-ing symptoms of all patients were effectively controlled,and the effective hemostasis rate was 100%.The hemoglobin level was in-creased from(96.21±12.42)g/L before operation to(111.38±7.70)g/L at 3 days after operation(P<0.001).After operation,3 pa-tients had slight swelling,pain,or skin color changes in the head,neck,and maxillofacial region,which were effectively relieved or dis-appeared after symptomatic treatment;2 patients died of severe traumatic brain injury and cerebral edema.No serious complications such as cerebral infarction occurred in any patients.There was no rebleeding in any patients within 3 to 6 months of post-discharge follow-up.Conclusion:TAE has definite curative effect and good safety for refractory bleeding caused by head,neck,and maxillofa-cial trauma,and has high clinical application value.
9.Paeoniflorin alleviates sepsis-associated acute kidney injury in mice by inhibiting aerobic glycolysis through the β-catenin/c-Myc pathway

Objective To investigate the role and mechanism of paeoniflorin(PF)in sepsis-associated acute kidney injury(SA-AKI)in mice.Methods Mouse SA-AKI model was constructed by intraperitoneal injection of 15 mg/kg LPS.Twenty-four male C57BL/6J mice(6~8 weeks old,weighing 20~25 g)were randomly divided into Control group,model group,PF group(intraperitoneally injected with 50 mg/kg PF 30 min before LPS administration),and β-catenin specific agonist BML284 group(10 mg/kg BML284 by intraperitoneal injection after 50 mg/kg PF administration).The renal histopathological changes were observed by HE staining and Paller scoring.ELISA was used to determine the contents of serum creatinine(Scr),neutrophil gelatinase-associated lipocalin(NGAL)and lactate,and renal contents of hexokinase 2(HK2),lactate dehydrogenase A(LDHA),IL-1β and IL-18.Western blotting was performed to detect the expression of total β-catenin,p-β-cateninY654,nucleus β-catenin and c-Myc.Results Compared with the Control group,the LPS group showed obviously damaged renal tissue,higher Paller score(P<0.05),increased serum Scr and NGAL levels(P<0.05),elevated renal contents of aerobic glycolytic indexes such as HK2,LDHA and serum lactate,as well as contents of IL-1β and IL-18(P<0.05),and enhanced expression of total β-catenin,p-β-cateninY654,nucleus β-catenin and c-Myc in the renal tissue(P<0.05).PF treatment attenuated the renal tissue damage,decreased Paller score(P<0.05),reduced serum Scr and NGAL levels(P<0.05),HK2,LDHA and serum lactate levels,and contents of IL-1 β and IL-18 in renal tissues(P<0.05),and down-regulated the renal expression of total β-catenin,p-β-cateninY654,nucleusβ-catenin and c-Myc when compared with the levels in the model group(P<0.05).While,addition of BML284 reversed above effects of PF treatment with significant differences(P<0.05).Conclusion PF can alleviate SA-AKI,and its mechanism may be through its inhibiting the β-catenin/c-Myc pathway,thus reducing the aerobic glycolysis level and inflammatory response in renal tissue.

OBJECTIVE To investigate the expression of long non-coding RNA(lncRNA)HOXC13-AS in head and neck squamous cell carcinoma tissues and its effects on tumor cell proliferation and migration in head and neck tumor cell lines.METHODS Clinical samples from 26 patients with laryngeal squamous cell carcinoma and 4 patients with hypopharyngeal squamous cell carcinoma who were treated surgically in the Second Hospital of Jilin University from March 2024 to December 2024 were collected as research subjects.Head and neck squamous cell carcinoma tissue specimens and corresponding adjacent tissue specimens were selected.The expression of HOXC13-AS in 30 cases of head and neck squamous cell carcinoma tissues and corresponding adjacent tissues was detected by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Clinical data were collected to analyze the correlation between HOXC13-AS expression and various clinical pathological factors.siRNA technology was used to interfere with the expression of HOXC13-AS in laryngeal cancer cell line TU212 and hypopharyngeal squamous cell line FaDu.Cell Counting Kit-8(CCK-8)assay was used to detect cell proliferation ability,and cell scratch assay was used to detect cell migration ability.RESULTS The expression level of lncRNA HOXC13-AS in head and neck squamous cell carcinoma tissues(12.60±11.26)was significantly higher than that in corresponding adjacent tissues(1.40±0.61),with a statistically significant difference(t=5.485,P<0.001).The expression of HOXC13-AS in head and neck squamous cell carcinoma tissues was closely related to T stage(P=0.004),lymph node metastasis(P=0.006),and clinical stage(P=0.007)of patients.CCK-8 results showed that the absorbance values of TU212 cell line groups at 24,48,and 72 h were si-NC group(0.373±0.010,0.738±0.026,1.003±0.124),si-HOXC13-AS-1 group(0.365±0.015,0.686±0.019,0.935±0.028),and si-HOXC13-AS-2 group(0.364±0.024,0.700±0.026,0.943±0.053),with statistically significant differences(F24 h=0.484,F48 h=7.893,F72 h=1.345,P<0.01);the absorbance values of FaDu cell line groups at 24,48,and 72 h were si-NC group(0.727±0.054,0.834±0.072,1.224±0.127),si-HOXC13-AS-1 group(0.532±0.005,0.650±0.079,1.021±0.044),and si-HOXC13-AS-2 group(0.647±0.088,0.687±0.025,1.074±0.055),with statistically significant differences(F24 h=16.143,F48 h=14.259,F72 h=9.409,P<0.001).Cell scratch assay showed that the migration rates of TU212 cell line groups at 12 and 24 h were si-NC group(38.971%±3.824%,69.185%±0.469%),si-HOXC13-AS-1 group(18.182%±2.580%,33.378%±2.302%),and si-HOXC13-AS-2 group(25.017%±0.288%,48.413%±0.805%),with statistically significant differences(F12 h=47.295,F24 h=471.745,P<0.0001);The migration rates of FaDu cell lines in each group at 12 and 24 hours were as follows:si-NC group(39.067%±3.196%,58.222%±0.448%),si-HOXC13-AS-1 group(13.689%±0.132%,39.358%±3.985%),and si-HOXC13-AS-2 group(23.335%±0.680%,35.526%±0.758%),with statistically significant differences(F12 h=138.1,F24 h=101.749,P<0.0001).CONCLUSION HOXC13-AS is significantly upregulated in head and neck squamous cell carcinoma tissues and cells,and is associated with the clinical pathological characteristics of patients.Silencing HOXC13-AS can significantly inhibit the proliferation and migration of head and neck tumor cells.
