1.Mechanisms of Bushen Tongluo Jiangzhuo Prescription in Improving Renal Fibrosis in Rats with Chronic Kidney Disease Based on PI3K/Akt/mTOR Signaling Pathway

ObjectiveTo investigate the mechanisms by which Bushen Tongluo Jiangzhuo prescription improves renal fibrosis in rats with chronic kidney disease (CKD) through the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR) signaling pathway. MethodsSeventy specific pathogen-free (SPF) Sprague-Dawley (SD) rats were randomly divided into a control group (n=15) and a modeling group (n=55). Rats in the modeling group were administered a 2.5% adenine suspension at a dose of 200 mg·kg-1·d-1 by gavage for 4 weeks to establish a CKD model. Successfully modeled rats were randomly divided into a model group, an irbesartan group (20.25 mg·kg-1·d-1), and Bushen Tongluo Jiangzhuo prescription low-, medium-, and high-dose groups (5.82, 11.64, and 23.28 g·kg-1·d-1, respectively), with 10 rats in each group. Each group was administered an equal volume of physiological saline, the corresponding concentration of irbesartan, or Bushen Tongluo Jiangzhuo prescription by gavage for 12 weeks. Body weight and renal function indices were dynamically monitored. Serum creatinine (SCr), blood urea nitrogen (BUN), urine albumin-to-creatinine ratio (ACR), 24-hour urinary total protein (24 hUTP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) levels were measured using an automatic biochemical analyzer. Renal histopathological changes were observed by hematoxylin-eosin (HE) and Masson staining. Immunohistochemistry (IHC) was used to detect the expression of PI3K, Akt, phosphorylated Akt (p-Akt), and mTOR in renal tissues. Western blot was performed to assess the protein expression of PI3K, p-Akt, Akt, phosphorylated mTOR (p-mTOR), and mTOR in renal tissues. Real-time quantitative polymerase chain reaction (Real-time PCR) was used to determine the mRNA expression levels of PI3K, Akt, and mTOR in renal tissues. ResultsCompared with the model group, rats in the irbesartan group and the low-, medium-, and high-dose Bushen Tongluo Jiangzhuo prescription groups showed significantly decreased levels of SCr, BUN, ACR, 24 hUTP, IL-1β, IL-6, and TNF-α (P<0.01). AST levels were significantly increased (P<0.01), while no significant difference was observed in ALT levels. Histopathological examination revealed that, compared with the model group, renal tubular epithelial cell edema and necrosis and Bowman's capsule dilation were alleviated, inflammatory cell infiltration was reduced, and interstitial and glomerular fibrosis was markedly improved in all treatment groups, with the most pronounced effect observed in the high-dose Bushen Tongluo Jiangzhuo prescription group. Real-time PCR results showed that mRNA expression levels of PI3K, Akt, and mTOR were significantly downregulated in the high-dose group (P<0.01). IHC results demonstrated that PI3K and p-Akt expression levels in renal tissues were significantly decreased in the high-dose group (P<0.01). Western blot analysis further confirmed that the expression levels of PI3K, p-Akt/Akt, and p-mTOR/mTOR were significantly reduced in the high-dose group (P<0.01). ConclusionBushen Tongluo Jiangzhuo prescription improves renal function indices in CKD rats, reduces collagen deposition in renal tissues, and decreases serum inflammatory factor levels. Its protective effect on renal function may be achieved by activating autophagy through downregulation of the PI3K/Akt/mTOR signaling pathway, thereby alleviating renal fibrosis.

Tuberculosis (TB) is a chronic infectious disease caused by Mycobacterium tuberculosis, which is primarily transmitted through the respiratory tract, and remains one of the diseases with the highest mortality rate of single-pathogen infections globally. The cell wall polysaccharides of M. tuberculosis are critical for maintaining bacterial structure, mediating pathogenesis, and enabling immune evasion. Lipoarabinomannan (LAM), a key polysaccharide component, has revolutionized non-invasive diagnostic technologies as a TB biomarker, while polysaccharide-based vaccines have emerged as innovative strategies for TB prevention. This review systematically examines the composition, subcellular distribution, and functional roles of M. tuberculosis cell wall polysaccharides in bacterial metabolism, drug resistance, and immune regulation. A particular emphasis is placed on recent advancements in LAM-based diagnostics and vaccine development. Future studies should utilize advanced technologies to precisely characterize the structural features of TB polysaccharides and explore their biological functions, providing a foundation for targeted diagnostic and therapeutic innovations. This article aims to provide reference for advancing both basic research and clinical applications related to M. tuberculosis.
3.Comparison of active constituent contents and their biological activities of Buzhong Yiqi Recipe with different dosage forms

AIM To compare total sugar,total protein,total phenol,total flavonoid,calycosin-7-O-β-D-glucoside,liquiritin,lobetyolin,quercetin,isoferulic acid,hesperidin,glycyrrhizic acid contents and their antioxidant activities,hypoglycemic activities of big honey pill,small honey pill,water pill,concentrated pill,granule,mixture and decoction of Buzhong Yiqi Recipe.METHODS Anthraquinone-sulfuric acid method,Coomassie brilliant blue method,Folin-phenol colorimetry method,sodium nitrite-aluminum nitrate method and HPLC were adopted in the content determination of total sugar,total protein,total phenol,total flavonoid and seven constituents,respectively,after which the scavenging capacities,reducing powers on DPPH·free radical,ABTS+free radical,hydroxyl free radical,and inhibition capacity on α-glucosidase activity were detected.Subsequently,correlation analysis was performed.RESULTS Total sugar,total protein,total phenol and total flavonoid contents demonstrated significant differences among different dosage forms(P<0.05,P<0.01).Calycosin-7-O-β-D-glucoside,glycyrrhizin,codonoside and quercetin displayed the highest contents in the decoction,while those of isoferulic acid,hesperidin and glycyrrhizin were observable in the mixture.The water pill exhibited the strongest antioxidant activity,while those of the concentrated pill and mixture were weak;the big honey pill exhibited the strongest hypoglycemic activity,while that of the decoction was the weakest.Total protein,total phenol,total flavonoid and liquiritin contents displayed significant positive correlations between antioxidant activity(P<0.05,P<0.01),while hesperidin content displayed significant negative correlation between the latter(P<0.05);total protein,calycosin-7-O-β-D-glucoside,codonoside and quercetin contents displayed significant negative correlations between hypoglycemic activity(P<0.05,P<0.01).CONCLUSION Active constituent contents and their biological activities of Buzhong Yiqi Recipe with different dosage forms exist differences,total sugar,total protein,total flavonoids,calycosin-7-O-β-D-glucoside,licorice glycoside,hesperidin,codonoside and quercetin can be taken as quality control indices for this prescription.
4.Efficacy of a self-locking zero-notch interbody fusion device for treating long-segment cervical spondylosis in elderly patients

Objective To compare the therapeutic efficacy of a self-locking zero-notch interbody fusion device for long-segment cervical spondylosis in elderly patients with traditional titanium plate combined with interbody fusion device.Methods From Jan 2019 to Jan 2021,elderly patients(>60 years)with 3-4 segments(C3-C7)radiculopathy,myelopathy,or mixed-type cervical spondylosis underwent anterior cervical discectomy and fusion(ACDF)using a zero-notch interbody fusion device(Group A,n=24)and ACDF using a titanium plate combined with an interbody fusion device(Group B,n=18).We recorded the surgery duration,blood loss,incision length and hospital stay,measure preoperative and postoperative intervertebral height,functional segment height and cervical lordosis,and also observe treatment outcomes and postoperative complications between the two groups.Results There were no statistically significant differences between the two groups in terms of gender,age,Japanese Orthopaedic Association(JOA)score,visual analogue scale(VAS)of upper limb,Neck Disability Index(NDI),preoperative intervertebral height,functional segment height and cervical lordosis.Blood loss,surgery time and hospital stay were similar in both groups,but Group A had shorter incision length(P<0.01)compared with Group B.There were no significant differences between the two groups in JOA scores,upper limb VAS and postoperative NDI,and even in postoperative intervertebral height,functional segment height and cervical lordosis recovery.Conclusion The zero-notch interbody fusion device is effective for treating long-segment cervical spondylosis.Compared with the traditional titanium plate combined with an interbody fusion device,it can avoid postoperative dysphagia with smaller incision and shorter surgery time,which makes it more suitable for elderly patients.
5.Centrifuge overload simulation method for post stall super maneuvering flights

Objective To propose a centrifuge overload simulation method for post stall super manueuvering flights to conduct the pilot centrifuge training during post stall super maneuvering flights.Methods Firstly,the independent active disturbance rejection control was designed in the longitudinal,transverse and heading channels to achieve closed-loop control of the aircraft at high angles of attack with thrust vector control technology.Secondly,a method for establishing control command for the three-axis centrifuge with the main arm rotation,cabin roll and cabin pitch was put forward based on the relationship between the pilot overload and the centrifuge control information.Finally,a simulation model was constructed based on the cobra maneuver to carry out digital simulation experiments.Results The method proposed effectively extracted overload information under super maneuvering flight conditions,then the centrifuge control commands could be determined accordingly to enable corresponding overload trainings with the centrifuge.Conclusion The proposed method facilitates the improvement of pilots'flight maneuvering performance in post stall super maneuvering flights under sustained high overload conditions.[Chinese Medical Equipment Journal,2025,46(7):21-26]
6.Construction of backpack-based field emergency medical rescue equipment system

Objective To establish a backpack-based field emergency medical rescue equipment system to enhance emergency rescue efficiency.Methods A backpack-based field emergency medical rescue equipment system was preliminarily constructed with the concept of medical treatment in echelons and prolonged field care(PFC)and the method of capability-based equipment need analysis;with the Delphi method 15 experts from relevant fields were invited to execute two rounds of questionnaire consultations,and the final equipment system was determined after the equipment varieties and quantities were revised based on the expert opinions.Results The response rate for the two rounds of expert questionnaire surveys was 100%.The experts'authority coefficients were 0.8734 and 0.87,respectively;Kendall coefficients were 0.232 and 0.345(both P<0.001),indicating statistically significant results.Ultimately,a backpack-based field emer-gency medical rescue equipment system comprising 15 backpacks and 158 individual pieces of equipment was established.Conclusion The established system demonstrates a certain degree of specificity and practicability,providing references for the equipment allocation and utilization of emergency medical rescue teams.[Chinese Medical Equipment Journal,2025,46(10):17-22]
7.Construction of a"trinity"collaborative training mechanism for postdoctoral fellows in university-af-filiated tertiary hospitals under the background of high-quality development:empirical research and institutional innovation based on a hospital in Guangzhou

Objective Postdoctoral fellows are a crucial force in hospital scientific research and serve as a"talent pool"for high-level young professionals.Exploring optimized pathways for postdoctoral training in university-affiliated tertiary hospitals under the high-quality development context is of great significance.Methods Taking a tertiary hospital in Guangzhou as the re-search subject,this study analyzed the hospital's postdoctoral management implementation measures,recruitment status,scientif-ic research output,and training outcomes over the past five years(2020-2024).Combined with policy and literature analysis,shortcomings in postdoctoral training were summarized,and a conceptual"Trinity"collaborative training mechanism involving the"hospital-department-postdoctoral fellow"was proposed.Results The retention rate of postdoctoral fellows increased to 42%,the number of provincial/ministerial-level and above research projects grew by 1.5 times,and the number of newly enrolled post-doctoral fellows in 2024 more than doubled compared to 2020.However,challenges remain,including insufficient high-quality candidates,inadequate guidance and cultivation by supervisors,lagging achievement transformation,imperfect assessment sys-tems,and incomplete incentive and support measures.Conclusion Postdoctoral fellows are a vital force in hospital scientific re-search.Constructing an effective collaborative training mechanism is essential.Through innovations in management and training models,departmental and supervisor team building,and diversified incentives and support mechanisms,multi-party collaboration and systematic integration should be pursued.Further exploration of the"Trinity"training mechanism will deepen the alignment of postdoctoral training with clinical needs and disciplinary development,thereby promoting high-quality hospital development.
8.Bufotaline Enhances the Sensitivity of Pancreatic Cancer Cells to Adriamycin Treatment by Inhibiting DNA Damage Repair

Pancreatic cancer has emerged as one of the most challenging malignancies worldwide,with its high resistance to chemotherapy being the primary cause of treatment failure.Therefore,enhancing the chemosensitivity of pancreatic cancer has become a major focus of current research.In this study,we in-vestigated how Bufotaline,a bufadienolide extracted from the traditional Chinese medicine toad venom,exhibits its antitumor activity.Specifically,we explored the potential of Bufotaline to enhance the chemo-sensitivity of pancreatic cancer cells to Adriamycin and elucidated its underlying molecular mechanisms.Using CCK-8 and colony formation assays,we demonstrated that Bufotaline enhances the inhibitory effect of Adriamycin on the survival of pancreatic cancer cell lines Patu-8988T,Aspc-1,and Patu-8988S.No-tably,Bufotaline treatment reduced the IC50 of Adriamycin in drug-resistant pancreatic cancer cells to lev-els comparable to those in non-resistant cells.Results from Western blot,immunofluorescence,comet as-say,and TUNEL assays revealed that Bufotaline promotes Adriamycin-induced DNA damage in pancreatic cancer cells.RNA-seq analysis of Patu-8988T cells treated with Adriamycin alone or in combination with Bufotaline showed significant changes in gene expression,and qRT-PCR analysis further confirmed that Bu-fotaline downregulates the expression of DNA damage repair proteins NBS1 and RAD50.Moreover,Western blot analysis revealed that Bufotaline reduces the levels of DNA damage response repair proteins,and Im-munofluorescence experiments indicated that Bufotaline inhibits the activation of the ATM/CHK2 signaling pathway.Finally,in a subcutaneous xenograft mouse model,the combination of Adriamycin and Bufotaline treatment significantly suppressed pancreatic cancer cell growth.In conclusion,Bufotaline enhances Adria-mycin-induced chemosensitivity in pancreatic cancer cells;the combination of Adriamycin and Bufotaline downregulates the expression of DNA damage response repair proteins NBS1 and RAD50,and inhibits the ATM/CHK2-mediated DDR signaling pathway,thereby delaying DNA damage repair.
9.Influencing factors of positivity in frailty patients with myocardial infarction after intervention and its mediating effect between illness-related shame and psychosocial adjustment

Objective:To explore the influencing factors of post-percutaneous coronary intervention(PCI)positivity in young and middle-aged patients with acute myocardial infarction(AMI)and simultaneously assess its mediating effect in the relationship between illness-related shame and psychosocial adjustment.Methods:A total of 178 patients,who admit-ted in Cangzhou People's Hospital between October 2021 and December 2022 for AMI,underwent PCI,and were subse-quently defined as frailty after PCI,were selected.General demographic information survey forms,Patient Activation Measure(PAM),Social Impact Scale(SIS),and Psychosocial Adjustment to Illness Scale Self-Report(PAIS-SR)were applied to investigate the patients.Clinical data characteristics of the patients were analyzed.Pearson correlation analysis was conducted to investigate the association among PAM score,SIS score and PAIS-SR score.Multivariate linear stepwise regression analysis was employed to identify the influencing factors of positivity.The Bootstrap procedure in the Process plugin was used to validate the mediating effect of positivity between illness-related shame and psychosocial adjustment.Results:In this study,PAM score was(57.87±7.02)points,SIS score was(52.07±12.66)points,and PAIS-SR score was(61.07±10.58)points.Pearson correlation analysis indicated that PAM score was significant negatively correlated with SIS score and PAIS-SR score(r=-0.439,-0.504,P<0.001 all),and SIS score was significant positively corre-lated with PAIS-SR score(r=0.722,P<0.001).Multivariate linear stepwise regression analysis indicated that male(B=2.226,P=0.020),educational level(B=2.194,P=0.005)and stigma associated with illness(B=-0.210,P<0.001)can influence the postoperative positivity of patients.Mediation effect analysis indicated that illness-related shame had a direct effect on psychosocial adjustment,accounting for 85.92%of the total effect,while positivity partially media-ted the relationship between illness-related shame and psychosocial adjustment with an effect size of 14.08%.Conclusion:Illness-related shame not only directly affects the psychosocial adjustment in frailty AMI patients after PCI,but also exerts an indirect influence on it through positivity.Medical staff should implement targeted strategies to enhance patients'positiv-ity,thereby reducing the negative impact of illness-related shame on psychosocial adjustment.
