BACKGROUND:The quadriceps strength of patients with osteoporosis and sarcopenia is significantly reduced, which can further reduce the function of the knee joint, affect the function of the lower limbs and even lead to a decrease in whole-body coordination. It is speculated that a reasonable quadriceps training program and personalized guidance are beneficial to the recovery of knee joint function in patients with osteoporosis and sarcopenia.OBJECTIVE:To observe the effect of short-term moderate-intensity resistance rehabilitation training on the mass and function of the quadriceps and knee joint function in patients with osteoporosis and sarcopenia.METHODS:Using the integrated physical examination and rehabilitation model,375 patients with osteoporosis and sarcopenia were screened at the Health Management Center of Shanghai Public Health Clinical Center. They underwent 12 weeks of combined/comprehensive exercise rehabilitation based on resistance exercise, including quadriceps resistance isotonic and isometric contraction training twice a week(3-5 sets each time,10-15 minutes per set)and aerobic exercise/balance exercise two or three times a week(30 minutes each time).Assessments and data collection were performed before rehabilitation training,12 weeks after rehabilitation training, and at follow-up 12 weeks after stopping rehabilitation training, mainly including knee joint range of motion and proprioception, quadriceps muscle strength, and cross-sectional area(magnetic resonance imaging results),pain, knee joint function(Hospital for Special Surgery score)and walking function("up-and-go"time and 6 m pace test results)as well as the patient's psychological status assessment.RESULTS AND CONCLUSION:All 375 patients completed 12 weeks of rehabilitation training and 12 weeks of follow-up without any adverse events.(1)Compared with before training, the patients' gait speed and knee range of motion increased significantly after 12 weeks of rehabilitation training(P<0.01),the time of"stand-to-walk"decreased(P<0.01),and the proprioception of the knee joint and the strength of the quadriceps femoris were significantly improved(P<0.01);and at the follow-up visit 12 weeks after stopping training, the above indicators and functions of the patients were well maintained(P>0.05).(2)Magnetic resonance imaging results showed that the effective cross-sectional area of the quadriceps femoris did not improve significantly after 12 weeks of rehabilitation training(P>0.05);but the Hospital for Special Surgery score of knee joint function increased significantly(P<0.01),and the visual analog pain scale score decreased significantly(P<0.01),suggesting that this may be related to the improvement of quadriceps femoris quality by resistance rehabilitation training.(3)The results of the Hospital Anxiety and Depression Scale score showed that the anxiety and depression scores of the patients continued to decrease, both at 12 weeks of rehabilitation training and at 12 weeks after stopping training(P<0.01).It is suggested that resistance rehabilitation training of the quadriceps can help patients with osteoporosis and sarcopenia to restore quadriceps muscle strength, increase range of motion, improve proprioception and joint stability, thereby enhancing knee joint function, reducing pain, improving depression and anxiety, and to a certain extent promoting the coordinated recovery of the musculoskeletal system.

Oral squamous cell carcinoma (OSCC) is a common head and neck malignancy. Approximately 50% to 60% of patients with OSCC are diagnosed at a locally advanced stage (clinical staging III-IVa). Even with comprehensive and sequential treatment primarily based on surgery, the 5-year overall survival rate remains below 50%, and patients often suffer from postoperative functional impairments such as difficulties with speaking and swallowing. Programmed death receptor-1 (PD-1) inhibitors are increasingly used in the neoadjuvant treatment of locally advanced OSCC and have shown encouraging efficacy. However, clinical practice still faces key challenges, including the definition of indications, optimization of combination regimens, and standards for efficacy evaluation. Based on the latest research advances worldwide and the clinical experience of the expert group, this expert consensus systematically evaluates the application of PD-1 inhibitors in the neoadjuvant treatment of locally advanced OSCC, covering combination strategies, treatment cycles and surgical timing, efficacy assessment, use of biomarkers, management of special populations and immune related adverse events, principles for immunotherapy rechallenge, and function preservation strategies. After multiple rounds of panel discussion and through anonymous voting using the Delphi method, the following consensus statements have been formulated: 1) Neoadjuvant therapy with PD-1 inhibitors can be used preoperatively in patients with locally advanced OSCC. The preferred regimen is a PD-1 inhibitor combined with platinum based chemotherapy, administered for 2-3 cycles. 2) During the efficacy evaluation of neoadjuvant therapy, radiographic assessment should follow the dual criteria of Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 and immune RECIST (iRECIST). After surgery, systematic pathological evaluation of both the primary lesion and regional lymph nodes is required. For combination chemotherapy regimens, PD-L1 expression and combined positive score need not be used as mandatory inclusion or exclusion criteria. 3) For special populations such as the elderly (≥ 70 years), individuals with stable HIV viral load, and carriers of chronic HBV/HCV, PD-1 inhibitors may be used cautiously under the guidance of a multidisciplinary team (MDT), with close monitoring for adverse events. 4) For patients with a poor response to neoadjuvant therapy, continuation of the original treatment regimen is not recommended; the subsequent treatment plan should be adjusted promptly after MDT assessment. Organ transplant recipients and patients with active autoimmune diseases are not recommended to receive neoadjuvant PD-1 inhibitor therapy due to the high risk of immune related activation. Rechallenge is generally not advised for patients who have experienced high risk immune related adverse events such as immune mediated myocarditis, neurotoxicity, or pneumonitis. 5) For patients with a good pathological response, individualized de escalation surgery and function preservation strategies can be explored. This consensus aims to promote the standardized, safe, and precise application of neoadjuvant PD-1 inhibitor strategies in the management of locally advanced OSCC patients.

Objective:To investigate the changes of relevant subcortical regions in patients with dysphagia after stroke by diffusion tensor imaging. Methods:This study prospectively selected six patients with dysphagia early after stroke who were admitted to The Second Hospital of Jiaxing from January 2020 to March 2021 (stroke group). During the same period, 15 healthy volunteers were selected as a control group (volunteer group). Both groups underwent diffusion tensor imaging scans. The mean diffusivity, axial diffusivity, fractional anisotropy, and radial diffusivity were compared between the stroke group and the volunteer group, as well as between the affected and unaffected sides within the stroke group. Results:The mean diffusivity did not pass calibration, and no results were obtained. Axial diffusivity differed significantly between the two groups in fiber bundles located in the middle cerebellar peduncle, the transverse pontine tract, the splenium of the corpus callosum, the right corticospinal tract, the left superior cerebellar peduncle, the right cerebral peduncle, the right external capsule, and the right cingulate gyrus [(1.38 ± 0.06) ×10 -3 mm 2/s vs. (1.50 ± 0.05) × 10 -3 mm 2/s, (1.17 ± 0.05) × 10 -3 mm 2/s vs. (1.08 ± 0.04) × 10 -3 mm 2/s, (1.84 ± 0.04) × 10 -3 mm 2/s vs. (1.90 ± 0.06) ×10 -3 mm 2/s, (1.28 ± 0.04) × 10 -3 mm 2/s vs. (1.41 ± 0.03) ×10 -3 mm 2/s, (2.16 ± 0.06) × 10 -3 mm 2/s vs. (2.30 ± 0.07) × 10 -3 mm 2/s, (1.59 ± 0.04) × 10 -3 mm 2/s vs. (1.78 ± 0.05) × 10 -3 mm 2/s, (1.39 ± 0.05) × 10 -3 mm 2/s vs. (1.13 ± 0.04) × 10 -3 mm 2/s, (1.27 ± 0.04) × 10 -3 mm 2/s vs. (1.20 ± 0.03) × 10 -3 mm 2/s, t = 4.70, 4.34, 2.24, 8.17, 4.29, 8.26, 12.55, 4.40, all P < 0.05)]. Furthermore, significant differences in fractional anisotropy were observed in fiber bundles located in the knee of the corpus callosum, the right and left corticospinal tracts, the right superior cerebellar peduncle, the right and left cerebral peduncles, the left anterior limb of the internal capsule, the right and left posterior limbs of the internal capsule, the posterior portion of the right internal capsule around the lentiform nucleus, the right and left anterior corona radiata, the right and left superior corona radiata, the right and left posterior corona radiata, the right external capsule, and the left uncinate fasciculus ( t = 3.86, 7.54, 3.62, 3.77, 7.72, 3.77, 3.77, 10.86, 6.26, 5.17, 5.03, 3.77, 8.79, 5.03, 5.52, 3.62, 6.91, 4.14, all P < 0.05). Additionally, radial diffusivity differed significantly between the two groups in fiber bundles located in the middle cerebellar peduncle, the left medial lemniscus, the right and left anterior limbs of the internal capsule, the right and left posterior limbs of the internal capsule, the posterior part of the right internal capsule surrounding the lentiform nucleus, the right superior corona radiata, the left sagittal stratum, the right external capsule, and the left external capsule ( t = 4.34, 3.14, 15.71, 7.54, 19.49, 6.91, 12.07, 11.31, 8.69, 17.87, 10.68, all P < 0.05). Objective:To analyze and compare the interfractional setup errors between two body posi-tioning fixation methods(lithotomy position with carbon fiber full-body fixation frame vs.conventional carbon fiber body fixation frame combined with thermoplastic membrane)in radical radiotherapy for pros-tate cancer, and to calculate the clinical target volume(CTV)to planning target volume(PTV)margin(MPTV)for both methods to optimize immobilization techniques and radiotherapy workflows. Methods:A retrospective analysis was conducted on 37 consecutive patients who underwent radical prostate radio-therapy at Peking University First Hospital between August 2021 and March 2023.The patients were di-vided into two groups based on the immobilization method:Group A(18 patients,450 CBCT image sets)used a carbon fiber whole-body fixator in the lithotomy position, while Group B(19 patients,461 CBCT image sets)used a conventional carbon fiber fixator combined with a thermoplastic mask. All the patients underwent daily cone-beam computed tomography(CBCT)image guidance.Bone registration combined with manual registration was used to obtain the setup error data in the left-right(X),cranio-caudal(Y)and anterior-posterior(Z)directions.The positioning errors of the two groups were compared by using the independent sample t-test,the Mann-Whitney U test and the chi-square test. The average positioning error, systematic positioning error(Σ)and random positioning error(δ)were calculated, and the CTV-PTV extension distance was calculated by using the(MPTV=2.5Σ+0.7δ).Results:The analysis of the setup errors in the three-dimensional direction showed significant differences between the two groups(all P<0.01).Specifically,the median(quartile)absolute values of the errors in the X,Y,and Z di-rections of group A were[0.40(0.20,0.70)cm,0.50(0.30,0.80)cm,and 0.35(0.20,0.60)cm],respectively.In group B,the corresponding values were significantly reduced to[0.20(0.10,0.40)cm,0.40(0.20,0.70)cm and 0.20(0.10,0.40)cm].The results of Mann-Whitney U test showed that the differences in each direction were highly statistically significant(X:z=-6.86;Y:z=-2.76;Z:z=-5.71).The cumulative distribution ratio of the setup error displacement within 0.5 cm in the X,Y,and Z directions in group A and group B were 297(66.0%)and 408(88.5%)(P<0.01),250(55.6%)and 285(61.8%)(P=0.055),308(68.4%)and 391(84.8%)(P<0.01),re-spectively.The CTV-PTV margins in three directions were X 0.66 cm in group A and 0.35 cm in group B;Y0.67 cm and 0.45 cm;Z 0.54 cm and 0.42 cm.Conclusion:Conventional carbon fiber human body fixator combined with thermoplastic film can significantly reduce the setup error. However, the car-bon fiber whole-body fixator in the lithotomy position demonstrated larger setup errors in the X,Y,and Z directions compared with the conventional fixator combined with a thermoplastic mask, indicating the need for further workflow optimization.Given that the lithotomy position is essential for transperineal ultrasound-guided techniques, further research and improvements are required to achieve higher positioning accuracy.
