Journal of Neurogastroenterology and Motility 2025;31(1):75-85
Background/Aims:
Anti-reflux mucosal ablation (ARMA) is a promising endoscopic intervention for proton pump inhibitor (PPI)-dependent gastroesophageal reflux disease (GERD). However, the effect of ARMA on esophageal motility remains unclear.
Methods:
Twenty patients with PPI-dependent GERD receiving ARMA were prospectively enrolled. Comprehensive self-report symptom questionnaires, endoscopy, 24-hour impedance-pH monitoring, and high-resolution impedance manometry were performed and analyzed before and 3 months after ARMA.
Results:
All ARMA procedures were performed successfully. Symptom scores, including GerdQ (11.16 ± 2.67 to 9.11 ± 2.64, P = 0.026) and reflux symptom index (11.63 ± 5.62 to 6.11 ± 3.86, P = 0.001), improved significantly, while 13 patients (65%) reported discontinuation of PPI. Total acid exposure time (5.84 ± 4.63% to 2.83 ± 3.41%, P = 0.024) and number of reflux episodes (73.05 ± 19.34 to 37.55 ± 22.71, P < 0.001) decreased significantly after ARMA. Improved esophagogastric junction (EGJ) barrier function, including increased lower esophageal sphincter resting pressure (13.89 ± 10.78 mmHg to 21.68 ± 11.5 mmHg, P = 0.034), 4-second integrated relaxation pressure (5.75 ± 6.42 mmHg to 9.99 ± 5.89 mmHg, P = 0.020), and EGJ-contractile integral(16.42 ± 16.93 mmHg · cm to 31.95 ± 21.25 mmHg · cm, P = 0.016), were observed. Esophageal body contractility also increased significantly (distal contractile integral, 966.85 ± 845.84 mmHg · s · cm to 1198.8 ± 811.74 mmHg · s · cm, P = 0.023). Patients with symptom improvement had better pre-AMRA esophageal body contractility.
Conclusions
ARMA effectively improves symptoms and reflux burden, EGJ barrier function, and esophageal body contractility in patients with PPIdependent GERD during short-term evaluation. Longer follow-up to clarify the sustainability of ARMA is needed.

Objective To evaluate the efficacy and safety of systemic thrombolysis(ST)and standard anticoagulation(SA)in the treatment of lower extremity fracture complicated with distal deep vein thrombosis(DDVT).Methods We retrospectively analyzed the clinical data of 60 patients with lower extremity fracture complicated with DDVT treated from January 2021 to December 2023.When the lower limb venography indicated a calf thrombus burden score ≥3 points,a retrievable inferior vena cava filter(IVCF)was successfully placed in the healthy femoral vein before orthopedic surgery. The patients who received further anticoagulant or thrombolytic therapy after surgery were allocated into a ST group(n=30,urokinase ST and SA)and a SA group(n=30,only SA).The two groups were compared in terms of calf thrombus burden score, thrombus dissolution rate,IVCF placement time,IVCF retrieval rate, intercepted thrombi, hemoglobin level, platelet count,D-dimer level, and complications. Results There was no statistically significant difference in the calf thrombus burden score between the two groups before treatment(P=0.431).However,after treatment, the scores in both groups decreased(both P<0.001),with the ST group showing lower score than the SA group(P=0.002).The thrombus dissolution rate in the ST group was higher than that in the SA group(P<0.001).There was no statistically significant difference in the IVCF placement time between the two groups(P=0.359),and the IVCF retrieval rate was 100% in both groups. The ST group had fewer intercepted thrombi than the SA group(P=0.002).There was no statistically significant difference in hemoglobin level(P=0.238),platelet count(P=0.914),or D-dimer level(P=0.756)between the two groups before treatment. However, after treatment, both groups showed an increase in platelet count(both P<0.001)and a decrease in D-dimer level(both P<0.001).There was no statistically significant difference in the occurrence of complications between the two groups(P=0.704).Conclusions Both SA and ST demonstrate safety and efficacy in the treatment of lower extremity fractures complicated with DDVT,serving as valuable options for clinical application.Compared with SA,ST not only enhances the thrombus dissolution in the calf but also mitigates the risk of thrombosis associated with IVCF.

Digital technologies have become an integral part of complete denture restoration. With advancement in computer-aided design and computer-aided manufacturing (CAD/CAM), tools such as intraoral scanning, facial scanning, 3D printing, and numerical control machining are reshaping the workflow of complete denture restoration. Unlike conventional methods that rely heavily on clinical experience and manual techniques, digital technologies offer greater precision, predictability, and efficacy. They also streamline the process by reducing the number of patient visits and improving overall comfort. Despite these improvements, the clinical application of digital complete denture restoration still faces challenges that require further standardization. The major issues include appropriate case selection, establishing consistent digital workflows, and evaluating long-term outcomes. To address these challenges and provide clinical guidance for practitioners, this expert consensus outlines the principles, advantages, and limitations of digital complete denture technology. The aim of this review was to offer practical recommendations on indications, clinical procedures and precautions, evaluation metrics, and outcome assessment to support digital restoration of complete denture in clinical practice.

Objective:To understand the characteristics of new infections in the newly reported HIV-1 infected population in Luzhou, to find out the characteristics of the high-risk population, which may provide a basis for developing precise prevention and control measures locally.Methods:HIV-1 LAg Avidity EIA test was applied for newly reported cases in Luzhou from 2018 to 2021. The chi-square test was used for univariate analysis, logistic regression model for multivariate analysis, and spatial autocorrelation and hotspot analysis were applied to explore the spatial distribution characteristics of new infections in Luzhou city. Results:A total of 4 494 cases reported in the odd-numbered months were selected for testing, with 673 newly infected cases and the proportion of newly infected cases was 14.98%. Newly infected cases were predominantly male (472/673, 70.13%), married (335/673, 49.78%), heterosexual transmission (621/673, 92.27%), age>50 years (520/673, 77.27%), and junior high school or lower education (599/673, 89.00%). The proportion of new infections in each year were 11.74% (129/1 099), 17.11% (247/1 444), 13.57% (154/1 134) and 17.50% (143/817), respectively, with a statistically significant difference ( χ2=20.024, P<0.001) and an upward trend ( χ2=5.997, P=0.014). There were statistically significant differences in different transmission routes, gender, education level, occupation, sample source and residence (all P<0.05). Logistic regression analysis showed that gender, marital status, transmission route, current address, and sample source were all influencing factors for new infections. There are spatial autocorrelation characteristics of new case incidence in 2018 and 2021, hotspot areas gradually shifted from dispersed to relatively concentrated, with the number fluctuating with reported year. Conclusions:The proportion of new HIV-1 infections in Luzhou is relatively low throughout the province, but it is on the rise. The proportion of new infections is higher among youth, students, homosexual transmission, and retired persons. Attention should be focused on hotspot areas and routine surveillance and testing of key populations in hotspot areas should be strengthened.

Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.

Chinese Journal of Organ Transplantation 2024;45(12):878-884
Objective:To compare the efficacy of single kidney transplantation (KT) from pediatric donors between donors aged <2 and 2- 18 years. Methods:Between August 2016 and May 2023, 127 single pediatric kidney transplantations involving pediatric donors were conducted. They were assigned into two subgroups based upon age of small pediatric donors (n=22, SPD, donors aged <2 years) and normal pediatric donors (n=105, NPD, donors aged 2-18 years). A retrospective analysis was performed to compare recipient/donor baseline characteristics, postoperative complications and recipient/graft survival rates between two groups. Results:Significant inter-group differences existed in donor age[11.0 (10.0, 15.0) vs 121.0 (74.0, 166.0) month], donor weight[8.3 (8.0, 9.4) vs 30.0 (20.0, 50.0) kg]and graft-to-recipient weight ratio[0.3 (0.2, 0.5) vs 1.0 (0.6, 1.5) ] ( P<0.001). Conversely, no significant inter-group differences existed in donor gender/type, warm/cold ischemic time, human leukocyte antigen mismatch number, estimated glomerular filtration rate, recipient gender/age/weight, number of transplants, preoperative dialysis, preoperative induction therapy, panel-reactive antibody or primary disease ( P>0.05). The incidence of vascular thrombosis was 9.1% (2/22) and 0 in SPD and NPD groups with statistically significant differences ( P=0.029) ; the incidence of post-transplant hemorrhage was 13.6% (3/22) and 1.9% (2/105) with statistically significant difference ( P=0.036). However, no statistically significant inter-group differences existed in recurrent renopathy, delayed graft function or 1-year cumulative incidence of acute rejection ( P>0.05). Six recipients (27.3%) in SPD group lost allografts due to recurrent or primary nonfunction (n=1), vascular thrombosis (n=2), post-transplant hemorrhage (n=2) and thrombotic microangiopathy (n=1). In comparison, three recipients (2.9%) in NPD group lost allografts due to rejection (n=2) and infectious rupture of transplanted renal artery (n=1). Three-year recipient survival rates were 100% and 99.0% in SPD and NPD groups with no statistically significant differences ( P=0.600). And 3-year death-censored graft survival was significantly lower in SPD group than that in NPD groups (77.3% vs 91.5%) ( P<0.001, HR=8.3, 95% CI: 2.0-34.2) . Conclusions:Early postoperative vascular complications after single pediatric KT from pediatric donors aged under 2 years are frequent and predispose to graft loss.
