Aim To explore the effects of nuciferine on cognitive behavior and the underlying mechanisms,white matter injury(WMI),neuroinflammation,and ferroptosis in mice with chronic ischemic WMI.Meth-ods Sixty C57BL/6 mice were divided into a control group,a bilateral common carotid artery stenosis(BCAS)model group,and low/high-dose nuciferine groups(20/40 mg·kg-1).A chronic ischemic WMI model was established using BCAS surgery.Following eight weeks of treatment,cognitive behavior(Y-maze,novel object recognition,Morris water maze),white matter integrity(LFB/MBP staining),microglial acti-vation(Iba-1 immunofluorescence),inflammatory cy-tokines(ELISA for TNF-α,IL-1β,IL-6),ferroptosis markers(Fe2+,ROS,MDA,GSH),mitochondrial ultrastructure(electron microscopy),and protein ex-pression of the PI3K/Akt and NRF2/xCT/GPX4 signa-ling pathways(Western blot)were evaluated.Results Compared with the control group,the BCAS group showed significant cognitive decline(P＜0.05),re-duced myelin density,elevated inflammatory cytokines and ferroptosis markers(Fe2+,ROS,MDA),shrunk-en mitochondria,and downregulated PI3K/Akt and NRF2/xCT/GPX4 pathway proteins(P＜0.05).Nu-ciferine intervention significantly ameliorated these in-juries in BCAS mice,with the high-dose group exhibi-ting superior effects(P＜0.05).Conclusions Nu-ciferine exerts protective effects against chronic ische-mic WMI and cognitive impairment by activating the PI3K/Akt and NRF2/xCT/GPX4 signaling pathways,thereby suppressing neuroinflammation and ferroptosis.

Objective To explore influencing factors in diagnosing stump vaginal lesions by comparing the clinical data of patients with vaginal wall resection after total hysterectomy.Methods This retrospective study included patients who underwent vaginectomy due to stump vaginal lesions[including high-grade intraepithelial lesion(HSIL)or cancer]in Obstetrics and Gynecology Hospital,Fudan University between Jan 1,2013 and Sep 30,2022.The diagnosis was confirmed based on pathology results after vaginal stump resection,then patients were categorized into HSIL group and vaginal cancer(VC)group.We analyzed the differences in clinical features between the groups and evaluated the diagnostic value of various indicators for vaginal cancer.Additionally,within the VC group,patients whose colposcopic biopsy showed HSIL while whose pathology upgraded to invasive cancer after stump colpotomy were classified as the high-grade pathologic invasive cancer group(HC group).We then compared the HSIL group and the HC group to assess the predictive value of each index for pathological escalation.Results Significant differences were observed between the HSIL and VC groups in terms of age,etiology of previous hysterectomy,proportion of clinical symptoms,proportion of abnormal physical examination findings,proportion of abnormal imaging findings,proportion of papillary lesions,proportion of vaginal roof lesions,and squamous cell carcinoma antigen(SCCA)(P<0.05).However,there were no significant differences in fertility,previous hysterectomy,or HPV infection rate.Abnormal physical examination and imaging findings demonstrated better diagnostic performance(AUC>0.7).The proportion of vaginal apex lesions was significantly higher in the HC group compared with the HSIL group(P<0.05).Conclusion Previous hysterectomy for cervical cancer,presence of clinical symptoms,abnormal physical findings,abnormal imaging manifestations,and lesions at the vaginal apex are significant influencing factors in the diagnosis of vaginal stump carcinoma.Although colposcopy remains the primary diagnostic method,vaginal resection should be considered to rule out invasive carcinoma when high-risk factors exist,even if colposcopic biopsy pathology suggests HSIL.

In July 2024,the Diagnosis Related Groups(DRG)2.0 is released based on the Notice from the National Healthcare Security Administration on Issuing the DRG 2.0 and Deepening the Relevant Work.Compared with DRG 1.1,version 2.0 was established based on a wider range of suggestions regarding the Adjacent Diagnosis Related Groups(ADRG),Major Comorbidity or Complication(MCC),and Comorbidity or Complication(CC)from various institutions.A list of disease diagnoses and surgical operations that are not used as grouping rules was compiled,and grouping efficacy was further improved by upgrading the algorithms for MCC and CC with the help of AI.Meanwhile,it is necessary to pay more attention to the number of cases of ADRG,the better methods to list the MCC/CC,the suggestions of various doctors and continuously standardize the data and update the grouping scheme of DRG.

Objective:To explore the coordinated development status of the medical service supply-economy-society trinity systems in Zhejiang Common Prosperity Demonstration Zone,and to offer references for formulating policies conducive to the efficient and coordinated development of these three systems.Methods:Based on the panel data from 2013 to 2022,this research was conducted on the 11 prefectural-level cities in Zhejiang Province.Firstly,an evaluation index system for the three systems was established,and the entropy method was employed to determine the weights of each index and calculate the comprehensive evaluation index.Secondly,a triangular model was introduced to delineate the relative relationships among the three systems,and the coupling coordination degree model was utilized to disclose the coordination degree of the three systems.Finally,the spatial autocorrelation analysis method was applied to investigate the spatial autocorrelation of the coupling coordination degree of the three systems.Results:(1)On the whole,the development status of the three systems has improved over time,yet the comprehensive development level remains to be improved.(2)Overall,each prefectural-level city has transited from a medical service supply-society-dominated development to a balanced and coordinated development of the medical service supply-economy-society trinity systems.The coupling coordination degree of the three systems has risen from 0.468(on the verge of imbalance)in 2013 to 0.609(primary coordination)in 2022,presenting an upward trend in general.However,the coordination level remains to be improved and there exist imbalances among regions.(3)The coupling coordination degree of the three systems in Zhejiang Province exhibits a significant positive spatial correlation,and the spatial distribution characteristics are relatively stable. Conclusion:The coupling coordination degree of the medical service supply-economy-society trinity systems in Zhejiang Province awaits further enhancement. At the level of Zhejiang Province,favorable policy support should be provided,the layout of medical resources should be rationally planned,and high-quality economic and social development should be promoted. Aim To examine the pathogenesis of disul-fide death gene in vascular dementia(VD)by bioin-formatics analysis of disulfide death differentially ex-pressed genes(DEGs)combined with experimental verification.Methods The death DEGs of disulfide were screened and their correlation was analyzed.The VD patients data in the data set were analyzed by clus-tering and typing and gene set variation.The clustering risk of DEGs was tested with a nomogram model,and the optimal learning model was predicted.After the es-tablishment of VD rat model,water maze test,HE stai-ning and RT-qPCR detection were performed to verify the results of health information.Results Four DEGs including SLC7A11 were obtained,which had antago-nistic or synergistic interaction with each other.The genetic data could be divided into two subtypes with significant differences.After typing,VD disulfide DEGs were mainly concentrated in GnRH signaling pathways.The accuracy of the nomogram prediction model was high.Generalized linear was the best ma-chine learning model.Compared with the sham opera-tion group,the escape latency of rats in the model group was prolonged,the number of crossing platforms decreased,the relative mRNA expression levels of Slc3a2 and Slc7a11 decreased,and LRPPRC in-creased.Conclusions SLC7A11 and other disulfide death DEGs and its related GnRH signaling pathway may be an important part of the pathogenesis of VD di-sulfide death.SLC3A2,LRPPRC and SLC7A11 can be used as characteristic genes in the regulation of VD by disulfide death,which may affect VD progression through the regulation of disulfide death.

Objective To observe the clinical efficacy and safety of different doses of poractant alfa injection combined with mechanical ventilation in the treatment of children with very low body mass and neonatal respiratory distress syndrome(NRDS).Methods According to cohort method,children with very low body mass and NRDS were divided into control group and treatment group.On basis of mechanical ventilation,control group were treated with 100 mg·kg-1 poractant alfa injection;treatment group were treated with 200 mg·kg-1 poractant alfa injection.The clinical curative effect,recovery related indexes,ventilation function,blood gas indexes[partial pressure of carbon dioxide(PaCO2),pH,partial pressure of oxygen(PaO2)],inflammatory factors[interleukin(IL)-6,C-reactive protein(CRP)]and complications in the two groups were compared,and the safety was evaluated.Results There were 43 cases in control group and 43 cases in treatment group.After treatment,there was significant difference in total response rate between treatment group and control group[93.02％(40 cases/43 cases)vs 76.74％(33 cases/43 cases),P＜0.05].After treatment,injection frequencies in treatment group and control group were(1.02±0.15)and(1.47±0.50)times,oxygen inhalation time was(90.45±7.31)and(116.72±10.24)h,mechanical ventilation time were(81.27±6.81)and(99.69±9.05)h,hospitalization time was(12.15±2.07)and(16.29±2.53)d,fraction of inspired oxygen were(36.39±4.07)％and(40.54±5.13)％,positive end-expiratory pressure were(3.64±0.53)and(4.05±0.69)cmH2O,peak inspiratory pressure were(20.12±1.45)and(22.69±1.62)cmH2O,PaCO2 were(31.45±4.21)and(37.72±5.06)mmHg,pH were 7.36±0.30 and 7.21±0.27,PaO2 were(83.46±6.45)and(78.58±5.93)mmHg,levels of serum IL-6 were(36.21±4.03)and(54.83±5.69)ng·L-1,CRP levels were(8.03±2.11)and(11.28±3.06)mg·L-1,and the differences were statistically significant(all P＜0.05).There was significant difference in incidence of complications between treatment group and control group[9.31％(4 cases/43 cases)vs 25.57％(11 cases/43 cases),P＜0.05].The adverse drug reactions were mainly on rash,gastrointestinal dysfunction and irritability in treatment group,while which in control group were rash and gastrointestinal dysfunction.There was no significant difference in total incidence of adverse drug reactions between treatment group and control group[9.30％(4 cases/43 cases)vs 6.98％(3 cases/43 cases),P＞0.05].Conclusion 200 mg·kg-1 poractant alfa injection combined with mechanical ventilation can promote the recovery of children with very low body mass and NRDS,improve ventilation function and blood gas indexes,alleviate inflammatory response and reduce complications,and the safety is good.

Objective:To explored the application value of dynamic susceptibility contrast-perfusion weighted imaging(DSC-PWI)combined with amide proton transfer(APT)imaging in brian glioma grading.Methods:Retrospective analysis of clinical data of 100 patients with brian glioma admitted to The Second Affiliated Hospital of Xinjiang Medical University from January 2022 to September 2024.They were divided into a low-grade group(WHO grades Ⅰ-Ⅱ,n=62)and high-grade group(WHO grades Ⅲ-Ⅳ,n=38)according to the World Health Organization(WHO)grading system for tumors.All patients underwent DSC-PWI and APT imaging preoperative examinations.Apparent diffusion coefficien(ADC),cerebral blood volume(CBV),cerebral blood flow(CBF),mean transit time(MTT),time to peak(TTP),maximum APT value(APTmax),minimum APT value(APTmin),the average APT value of the whole lesion(APTwhole)were recorded.The general datas and ADC,CBV,CBF,MTT,TTP,APTmax,APTmin,APTwhole of two group were compared.The influencing factors of high-grade brian glioma were analysed by Multivariate logistic regression.The diagnostic value of high-grade brian glioma was analysed of DSC-PWI combined with APT imaging by receiver operating characteristic(ROC)curve.Results:There was no statistically significant difference in age,gender,lesion location,basilar artery occlusion,necrosis and growth pattern between the two groups(P＞0.05),there was a statistically significant difference in the maximum diameter of lesions ≥2 cm and cystic lesions(P＜0.05).The ADC of the high-gradel group was lower than that of the low-grade group,while APTmax,APTmin,APTwhole,CBF,CBV,MTT and TTP were all higher than those of the low-grade group(P＜0.05).Elevated APTmax,APTmin,CBV,MTT,and TTP were risk factors for high-grade brian glioma(P＜0.05),while elevated ADC was a protective factor(P＜0.05).The results of ROC analysis showed that,the area under the curve(AUC)values of APTmax,APTmin,ADC,CBV,MTT,and TTP for diagnostic high-grade brian glioma were 0.830,0.868,0.852,0.843,0.803 and 0.827(all P＜0.05).The results of ROC analysis showed that,the AUC values of the logistic model for diagnostic high-grade brian glioma was 0.993.Conclusion:Single detection of DSC-PWI and APT imaging parameters has high diagnostic value in brian glioma grading,and combined detection of APTmax,APTmin,ADC,CBV,MTT and TTP imaging parameters can further improve diagnostic accuracy.
