1.Differences in deltamethrin resistance and kdr gene mutation in Culex tritaeniorhynchus population in and outside the Yellow Sea wetland

Objective To gain insights into the biological characteristics of different populations of Culex tritaeniorhynchus within and around the Yellow Sea wetland from the perspective of the occurrence of resistance, we investigated the levels of resistance to deltamethrin and kdr gene mutation in the wetland and its peripheral areas. Methods Specimens were collected from Cx. tritaeniorhynchus populations at two monitoring sites in the Rare Bird National Nature Reserve and Tiaozi Ni Wetland Scenic Area, and also from two populations in Yancheng City and the Liuhe District of Nanjing, and the resistance of these mosquitoes to deltamethrin was determined using the CDC biotest bottle method. For each concentration of deltamethrin assessed, a random subset of exposed specimens was selected for amplification of the kdr gene fragment, followed by Sanger sequencing to identify and analyze resistance-associated mutations. Results The LC50 levels of deltamethrin among mosquitoes from the four populations in Luhe, Yancheng, the Rare Bird National Nature Reserve and the Tiaozi Ni Wetland Scenic Area were 2.048 5, 7.798 2, 3.473 3, and 17.695 5 mg/mL, respectively, with corresponding concentrations of deltamethrin ranging from 0.005 to 5.000,0.050 to 50.000,0.050 to 25.000 and 0.050 to 50.000 mg/mL, respectively. Furthermore, the ranges of the KT50 values were 11.76-107.43, 67.05-216.30,29.77-107.43 and 28.40-329.51 min; the 1-h knockdown rates were 34.58%-99.15%, 9.52%-43.80%, 55.09%-73.01%, and 10.09%-68.07%; and the 24-h mortality rates were 12.15%-67.52%,9.52%-79.56%,13.17%-82.21%, and 11.01%-78.99%, respectively. With respect to kdr gene mutation, we assayed a total of 63,70,59, and 57 mosquitoes for the four populations, for which we detected L1014F mutation frequencies of 14.29%, 35.00%, 20.34%, and 31.58%, respectively, with a majority of these mutations being heterozygous for resistance. In addition, five adult mosquitoes were identified has having synonymous mutations at site 1011[i. e. , AAT(asparagine)mutation to AAC(asparagine)]. Conclusions Our findings revealed the clear resistance of Cx. tritaeniorhynchus to deltamethrin in the Yancheng region of the Yellow Sea wetland, and the resistance phenotype and kdr frequency of Cx. tritaeniorhynchus in the wetland environment were comparable to those of Cx. tritaeniorhynchus in the wetland environment, thereby indicating that the resistance of different populations of Cx. tritaeniorhynchus was homogeneous under the pressure of different insecticide selection within and around the wetland. However, the underlying mechanisms need to be further studied.
2.Analysis of iodine nutritional status of key populations in Boxing County, Shandong Province in 2021

Objective:To investigate the distribution of iodine in drinking water and the iodine nutrition status of key populations in Boxing County of Shandong Province, and to provide a basis for continuous elimination of iodine deficiency disorders and scientific iodine supplementation.Methods:In 2021, in the historical iodine deficiency towns (districts, referred to as towns) in Boxing County, one water sample of centralized water supply terminal water of each administrative village (neighborhood committee) was collected for water iodine testing. Using stratified cluster sampling method, the historical iodine deficiency areas in Boxing County were divided into 5 areas according to east, west, south, north and center. One town was randomly selected from each area, 20 pregnant women and 40 non-boarding students aged 8 - 10 from a primary school were randomly selected from each town. Thirty pregnant women and 100 students were selected from each of the two historical high iodine towns, and random urine samples were collected from the above-mentioned population for urinary iodine detection and analysis.Results:A total of 329 water samples were collected, and the median water iodine was 10.9 μg/L. Of these, 267 samples (81.2%) were < 40 μg/L, 33 samples (10.0%) were among 40 - 100 μg/L, and 29 samples (8.8%) were > 100 μg/L. A total of 410 urine samples were collected from children, and the median urinary iodine was 162.0 μg/L. The median urinary iodine of children in different towns was compared, and the difference was statistically significant ( H = 86.34, P < 0.001). A total of 109 urine samples were collected from pregnant women, and the median urinary iodine was 57.4 μg/L. The median urinary iodine of pregnant women in different towns were compared, and the difference was statistically significant ( H = 38.35, P < 0.001). Conclusions:The distribution of iodine in drinking water in Boxing County is complex, with areas with high iodine, iodine deficiency, and suitable iodine sources coexisting. Children's iodine nutrition is at an appropriate level, but pregnant women's iodine nutrition is at a deficiency level. Relevant departments should strengthen health promotion, and each region should formulate iodized salt supply strategies based on actual conditions. At the same time, special attention should be paid to the iodine nutrition status of pregnant women.
3.The predictive value of endotracheal tube cuff pressure change in the outcome of extubation in mechanically ventilated patients with AECOPD

Objective To investigate the predictive value of endotracheal tube cuff pressure change(△Pcuff)on extubation outcome in mechanically ventilated patients with acute exacerbation of chronic obstructive pulmonary diseas(AECOPD).Methods A total of 93 AECOPD patients who underwent mechanical ventilation for at least 48 hours and required extubation through spontaneous breathing trial(SBT)from March 2023 to August 2024 in the Department of Critical Care Medicine of Deyang People's Hospital were selected as study subjects.According to the outcome of extubation,they were divided into successful extubation group and failed extubation group.General clinical data,laboratory results,△Pcuff at the start and at 30min of SBT were compared between two groups.Binary Logistic regression was used to analyze the risk factors affecting the outcome of extubation in AECOPD patients.The predictive value of △Pcuff for extubation outcome was evaluated by receiver operating characteristic(ROC)curve.Results Of the 93 patients,81 were successfully extubed and 12 were failed.Univariate analysis showed that △Pcuff(at the beginning of SBT),△Pcuff(SBT 30min),systolic blood pressure,pH,arterial partial pressure of oxygen,arterial partial pressure of carbon dioxide,and oxygenation index were all factors influencing the outcome of extubation(P<0.05).Binary Logistic regression analysis showed that △Pcuff(at the beginning of SBT)was an independent risk factor for extubation outcome(P<0.05).The ROC curve showed that △Pcuff(at the beginning of SBT)predicted extubation outcome with an area under the curve of 0.913,sensitivity of 84.0%,specificity of 83.3%,and Yoden index of 0.673,with an optimal cutoff of 34.5cmH2O(1cmH2O=0.098kPa).Conclusion The △Pcuff(at the beginning of SBT)has a good predictive value for the extubation outcome of mechanically ventilated patients with AECOPD,and the probability of successful extubation of a patient is higher when the △Pcuff is ≥34.5 cmH2O.
4.Research on the construction of evaluation index system for Huimin Insurance:Based on entropy weight TOPSIS method

Objective:To build an evaluation index system for Huimin Insurance and provide reference for promoting the steady and sustainable development of Huimin Insurance.Methods:The evaluation indicators were initially constructed through literature analysis method and further revised and improved.The entropy weight TOPSIS method was used to evaluate and analyze 84 Huimin Insurance products with complete evaluation indicator data in 21 provinces across the country.Results:The constructed evaluation index system for Huimin Insurance includes 4 first-level indicators,10 second-level indicators and 28 third-level indicators.The weights of the first-level indicators,from high to low,are the sustainability indicator of Huimin Insurance(0.335 0),the guarantee ability indicator(0.235 1),the fairness indicator of participation in insurance(0.229 9)and the guarantee level indicator(0.200 0).The evaluation results show that the top ranked products are mainly concentrated in Zhejiang and Guangdong.Conclusion:The evaluation results of the entropy weight TOPSIS method are suitable for the comprehensive evaluation of Huimin Insurance and are comprehensive and scientific.They can be used to evaluate the operation and development of Huimin Insurance products,laying a good foundation for further evaluation of the sustainability of Huimin Insurance.

6.Effect of 5E rehabilitation management mode combined with health education knowledge on rehabilita-tion and quality of life in elderly patients with AMI

Objective:To investigate clinical application effect of 5E rehabilitation management combined with health education knowledge in elderly patients with acute myocardial infarction(AMI).Methods:This randomized con-trolled study enrolled 98 elderly AMI patients admitted in Department of Cardiology,Hefei Eighth People's Hospital between July 2020 and February 2023.They were divided into control group(n=49,routine health education knowl-edge mode nursing)and intervention group(n=49,additional 5E rehabilitation management).Both groups were intervened for 2 months.Cardiac function indexes,mental state,scores of disease cognition,Pittsburgh Sleep Qual-ity Index(PSQI),Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)and incidence of com-plications were compared between two groups.Results:After 2-month intervention,compared with patients in control group,those in intervention group had significant higher left ventricular ejection fraction(LVEF)[(51.66±2.01)%vs.(45.88±1.51)%],scores of disease cognition[(89.43±2.38)points vs.(81.13±1.16)points]and SF-36[(645.58±26.00)points vs.(432.72±18.50)points],and significant lower left ventricular end-dias-tolic diameter(LVEDd)[(47.74±0.86)mm vs.(51.20±1.36)mm],creatine kinase(CK)[(140.89±5.15)U/L vs.(154.32±2.61)U/L],scores of Self-Rating Depression Scale(SDS)[(31.88±1.16)points vs.(41.06±1.22)points],Self-Rating Anxiety Scale(SAS)[(31.09±1.12)points vs.(40.47±0.87)points]and PSQI[(5.54±0.64)points vs.(7.70±0.85)points](P<0.001 all).Incidence of complications in intervention group was significantly lower than that of control group(4.08%vs.18.36%,P=0.025).Conclusion:5E rehabilitation management combined with health education knowledge mode could significantly improve cardiac function,negative emotions and sleep quality,improve disease cognition and quality of life,and reduce incidence of complications in elderly AMI patients.
7.Association of mental health literacy with advance care planning condition in elderly patients with coro-nary heart disease

Objective:To investigate mental health literacy and advance care planning(ACP)condition,their influencing fac-tors,and their correlation in elderly patients with coronary heart disease(CHD).Methods:General data of 500 elderly CHD patients admitted in the Second Affiliated Hospital of Chinese PLA Air Force Military Medical University between January 2020 and December 2021 were collected.Mental health literacy and ACP condition were assessed.Multivariate linear regres-sion was used to analyze their influencing factors,and Pearson method was used to analyze their association.Results:Total score of Multicomponent Mental Health Literacy(MHL)was(10.78±2.57)points,and total score of Advance Care Plan-ning Readiness Scale(ACPRS)was(71.41±4.84)points in the 500 elderly CHD patients.Multivariate linear regression a-nalysis indicated that level of disease uncertainty had negative impact on MHL score,presence of spouse and trust in medical care had positive impact on MHL score(P<0.05 or<0.01);number of complications had negative impact on ACPR score,while presence of spouse and family mean monthly income had positive impact on ACPRS score(P<0.05 or<0.01).Pearson correlation analysis indicated that MHL score was positively correlated with ACPRS score(r=0.476,P<0.001).Conclusion:Both mental health literacy and ACP status need to be improved in elderly CHD patients.They are posi-tively correlated with each other.Presence of spouse,number of complications and trust in medical care are the important influencing factors.Therefore,medical staff can take targeted measures to improve ACP readiness and mental health quality.
8.Rehabilitation effect of rehabilitation management mode based on Internet thinking on CHD patients after PCI

Objective:This study aims to explore the rehabilitation effect of continuous rehabilitation management based on Internet thinking on patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods:This randomized controlled study enrolled 100 CHD patients admitted in Second Affiliated Hospi-tal of Chinese PLA Air Force Military Medical University between October 2020 and December 2021.They were di-vided into control group(n=50)and intervention group(n=50).Patients in control group received routine reha-bilitation nursing mode,compared to those in intervention group applying"Internet+"continuous rehabilitation nursing intervention program,both groups were intervened for 3 months.Cardiac function,cardiopulmonary exer-cise test indexes,depression,medication compliance,quality of life and incidence of adverse cardiovascular events within 1-year follow-up were compared between two groups.Results:Compared with patients in control group after 3 months,those in intervention group had significant higher oxygen uptake at anaerobic threshold(VO2@AT)[(11.75±0.39)ml·kg-1·min-1 vs.(10.17±0.76)ml·kg-1·min-1],peak oxygen uptake(Peak VO2)[(17.87±0.72)ml·kg-1·min-1 vs.(16.66±0.24)ml·kg-1·min-1],left ventricular ejection fraction(LVEF)[(51.15±2.42)%vs.(44.09±1.94)%],scores of Morisky Medication Adherence Scale(MMAS-8)[(5.29±0.30)points vs.(4.11±0.27)points]and Seattle Angina Questionnaire(SAQ)[(85.50±4.37)points vs.(73.27±2.53)points],and significant lower left ventricular end-diastolic diameter(LVEDd)[(40.51±0.41)mm vs.(46.64±0.99)mm],score of Patient Health Questionnaire-9(PHQ-9)[(7.67±0.85)points vs.(9.66±1.43)points]and total incidence of adverse cardiovascular events within 1-year follow-up(4.00%vs.16.00%)(P<0.05 or<0.01).Conclusion:Internet continuous rehabilitation intervention could improve car-diopulmonary exercise capacity and cardiac function,help to improve their quality of life and relieve depression,and reduce the risk of adverse cardiovascular events in CHD patients after PCI.
9.A Novel Scorpion Toxin LmKTx13 Inhibits the Voltage-gated Potassium Channel Kv1.3

Kv1.3,a voltage-gated potassium channel,is highly expressed in T lymphocytes,the nervous system,and vascular smooth muscle cells.It plays a critical role in membrane excitability and electrical signal transduction,serving as an important target for studying T-cell function and providing a promising direction for developing therapeutics against autoimmune and inflammatory diseases.Therefore,the de-velopment of specific inhibitors of Kv1.3 channel has emerged as a novel therapeutic strategy for these disorders.In this study,we isolated and purified a novel Kv1.3-inhibitory peptide toxin,LmKTx13,from the venom of the scorpion Lychas mucronatus using reversed-phase high-performance liquid chroma-tography(RP-HPLC).LmKTx13 consists of 38 amino acid residues,including six cysteines that form three disulfide bonds.Whole-cell patch-clamp recordings revealed that LmKTx13 potently inhibited Kv1.3 with an IC50 of 7.92±3.0 nmol/L.Selectivity analysis showed that 2 μmol/L LmKTx13 also in-hibited Kv1.2 and Kv1.7,but exhibited no significant effects on other potassium channel subtypes or voltage-gated sodium channels.Further investigation into the mechanism demonstrated that LmKTx13 acts as a pore-blocking inhibitor of Kv1.3.By analyzing the effects of LmKTx13 on Kv1.3 channel gating ki-netics and performing sequence alignment of the pore regions of Kv1.3 and Kv1.5,we constructed site-directed mutants and identified the pore region of Kv1.3 as the critical binding site for LmKTx13.Key residues involved in the interaction included T425,G427,and H451.In summary,we discovered a no-vel pore-blocking Kv1.3 inhibitor,LmKTx13,from L.mucronatus venom,which exhibits high affinity and selectivity for Kv1.3.These findings highlight its potential as a potential lead molecule for developing Kv1.3-targeted therapeutics.
10.Research on the Standardization System of Mineral Medicines

Objective On the basis of systematically summarizing and organizing the existing standards of mineral drugs in traditional Chinese medicine,this study aims to construct the top-level framework of the mineral drug standard system and promote the standardization process of the mineral drug industry.Methods A comprehensive search and analysis of the standards for 87 kinds of mineral drugs were conducted on authoritative platforms such as the National Standard Information Public Service Platform,China Standard Service Network,and Traditional Chinese Medicine Standard and Guideline Information Service.By delving into the theoretical foundation and scientific methods,combined with practical research,this study uses analogy reference,systematic decomposition,and process analysis to design the top-level framework of the mineral drug standard system.Results Through comprehensive analysis and scientific design,this study summarizes the construction process and methods of the mineral drug standard system,clarifies the hierarchical structure and classification principles of the system,proposes the compilation path,and forms a standard system covering the entire industry chain of mineral drugs,including raw minerals,product categories,and circulation categories.Conclusion This study constructs a full industry chain process framework for the standard system of mineral drugs,providing a clear perspective for the research of mineral drugs.It can more comprehensively display the current state of development of mineral drug standards in our country,laying a foundation for the research on the standardization of mineral drugs.
