1.Effect and mechanism of Tangzhi formula in improving glucose and lipid metabolism in type 2 diabetic rats

OBJECTIVE To investigate the effect and mechanism of Tangzhi formula(TZF) on improving glucose and lipid metabolism disorders in type 2 diabetes mellitus (T2DM) rats.METHODS Thirty SPF-grade SD rats were used to establish a T2DM rat model through high-fat diet feeding combined with intraperitoneal injection of streptozotocin. After successful modeling, the rats were divided into the model group, metformin group (positive control, 0.18 g/kg), and TZF low-, medium-, and high-dose groups (5, 10, and 20 g/kg), with 6 rats in each group. An additional 6 rats were selected as the blank group. The rats in each group were orally administered the corresponding volume of drug solution or saline, and continuous dosing was performed for 6 weeks, once daily. Fasting blood glucose (FBG), area under the curve of oral glucose tolerance test (OGTT-AUC), area under the curve of intraperitoneal insulin tolerance test (IPITT-AUC), serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) levels were measured. Hematoxylin-eosin (HE) staining and oil red O staining were used to observe hepatic pathological morphology and lipid deposition. Differentially expressed proteins in liver tissues were screened by proteomics analysis, and key proteins in signaling pathways were verified by Western blot and immunofluorescence staining.RESULTS Compared with the blank group, the levels of FBG, OGTT-AUC, IPITT-AUC, LDL-C, TG, and TC in the model group were significantly increased ( P <0.05), while HDL-C level was significantly decreased ( P <0.05); the liver tissues showed disordered hepatic cords, punctate necrosis of hepatocytes, and significant lipid deposition, the NAFLD activity score and positive area of lipid droplets were significantly increased( P <0.05). Compared with the model group, the above indicators in TZF low-, medium-, and high-dose groups and metformin group were significantly reversed ( P <0.05); hepatic pathological injury and lipid deposition were alleviated. Proteomics results showed that a total of 442 differentially expressed proteins were screened between the model group and the blank group, and 358 differentially expressed proteins were screened between the TZF high-dose group and the model group. Among these differentially expressed proteins, multiple proteins closely related to glucose and lipid metabolism showed a reversal trend after TZF intervention. Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis showed that the peroxisome proliferator-activated receptor (PPAR) signaling pathway was significantly enriched. The validation experiment results showed that TZF could significantly up-regulate the expression of PPARα, PPARγ, and apolipoprotein A1 (ApoA1) in rat liver tissues ( P <0.05), and down-regulate the expression of stearoyl-CoA desaturase 1 (SCD1) and perilipin 2 (PLIN2) ( P <0.05).CONCLUSIONS TZF can improve glucose and lipid metabolism disorders in T2DM rats; its mechanism may be related to the activation of the PPAR signaling pathway and the regulation of downstream lipid metabolism-related proteins SCD1, PLIN2, and ApoA1.
2.Comparison of the effect of reducing the diameter of back optical zone diameter of CRT and VST designed orthokeratology lens in controlling myopia progression

AIM:To compare and analyze the effectiveness and safety of reducing the diameter of the back optical zone diameter(BOZD)of orthokeratology lens designed by CRT and VST in controlling the progression of myopia in children and adolescents.METHODS:Retrospective study. The study subjects were 400 myopia patients aged 8-16 years who were admitted to the orthokeratology fitting center of our hospital from June 2019 to May 2022, with 400 eyes(including right eye data analysis). The subjects were divided into CRT-S group(BOZD<6.0 mm), CRT group(BOZD=6.0 mm), VST-S group(BOZD<6.2 mm), VST group(BOZD=6.2 mm)according to the brand of orthokeratology lens and BOZD group, with 100 cases in each group. Uncorrected visual acuity(UCVA), corneal flat K value, axial length, spherical equivalent, and incidence of corneal injury were collected and analyzed at 1 d, 1 wk, 1 and 6 mo, 1 and 2 a, respectively.RESULTS:After wearing lenses for 1 d, the UCVA of the VST-S group improved the fastest, but after 1 wk, all groups reached a good UCVA, and there was no significant difference between groups. The corneal flat K value of the CRT-S group decreased the most after wearing lenses for 6 mo, and there was no significant difference in the corneal flat K value of all groups after 1 year of lens wearing. At each time point, the axial length growth decreased significantly after reducing the BOZD of the same brand of orthokeratology lens. At 6 mo, there was no significant difference in the axial length growth and defocus ring diameter between the CRT-S group and the VST-S group, but at 1 and 2 a, the VST-S group had significantly lower axial length growth and defocus ring diameter than the CRT-S group. The growth of the diopter sphere and spherical equivalent(SE)was significantly reduced when the BOZD of the same brand of orthokeratology lens was reduced at 2 a follow-up. The VST-S group had the smallest changes in the degree of SE and had the best myopia control effect. There was no significant difference in the change value of the diopter cylinder and the incidence of corneal injury among the four groups.CONCLUSION:Reducing the BOZD of the orthokeratology lens can effectively control the growth of the axial length and the progression of myopia degree. The myopia control effect of the VST lens is better than that of the CRT lens after reducing the BOZD. Reducing the BOZD of the orthokeratology lens does not increase the risk of additional corneal injury.

4.Visceral fat-to-muscle ratio is associated with inflammatory ileus after small intestinal fistula resection in sarcopenic patients with severe abdominal adhesions

Objective:Postoperative inflammatory ileus(PII)is characterized by prolonged re-obstruction following temporary defecation after surgery.Small intestinal fistula resection(SIFE)in patients with severe abdominal adhesions may increase the risk of PII,particularly in those with oligomyosis.The ratio of visceral fat area(VFA)to total abdominal muscle area index(TAMAI)is thought to be associated with chronic inflammation and may negatively impact the recovery of intestinal motility after abdominal surgery.This study aims to evaluate the predictive value of VFA/TAMAI for postoperative PII in patients undergoing SIFE.Methods:Sarcopenic patients who underwent SIFE at Jinling Hospital between January 2016 and October 2022 were enrolled.Preoperative and intraoperative characteristics were analyzed.VFA/TAMAI was assessed as a potential risk factor for PII using regression analysis.Results:A total of 132 eligible patients(79 males[59.8%])with a median age of 52 years(interquartile range[IQR]:39~61 years)were included.Of these,18.2%(n=24)developed PII.Temporary defecation occurred 4 days after SIFE(IQR:2~5 days)and lasted for 2 days(IQR:1~2 days).Re-obstruction persisted for a median duration of 21 days(IQR:19~27 days)after temporary defecation.VFA/TAMAI was significantly associated with PII(OR=3.89,95%CI:1.88~7.92,P<0.001)and recovery of digestive tract function(HR=0.81,95%CI:0.65~0.99,P=0.04).Conclusion:VFA/TAMAI is associated with postoperative PII in sarcopenic patients with severe abdominal adhesions following SIFE.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
6.Survey on current status of"Perioperative Infection Control"initiative monitoring indicators in 138 medical institutions in Guizhou Province

OBJECTIVE To investigate the current status of monitoring indicators related to the"Perioperative In-fection Control"in medical institutions above the secondary level in Guizhou Province,and to delve into the imple-mentation of key measures for infection prevention and control during the perioperative period for patients under-going surgical operations.METHODS Based on the"Implementation Plan for the'Perioperative Infection Control'Initiative in Guizhou Province",a"Case Investigation Form on Key Measures for Infection Prevention and Control During the Perioperative Period for Patients Undergoing Surgical Operation"was developed.A total of 138 medi-cal institutions participated in the case investigation,and a total of 2 128 cases were investigated.RESULTS The overall monitoring indicators for the"Perioperative Infection Control"initiative in the 138 medical institutions a-bove the secondary level in Guizhou Province were at a relatively low level.The skin cleansing compliance rate was 80.32%,the hair removal compliance rate was 16.43%,the prophylactic antibacterial drug administration rate within 0.5-1 hour before surgery was 55.94%and the antibacterial drug discontinuation rate within 24 hours after prophylactic medication for type Ⅰ incision surgeries was 56.48%.The hair removal compliance rate was higher in tertiary medical institutions(19.21%)than in secondary medical institutions(14.34%)(P=0.039).The distri-bution of the four monitoring indicators varied in clinical departments and surgery types,with statistically signifi-cant differences(P<0.05).The preferred method for surgical site skin cleansing in medical institutions across the province was local wiping,primarily with clean water(57.21%).The primary method for hair removal was razors(68.82%),and hair removal on the day of surgery was most common(61.75%).CONCLUSIONS Conduc-ting a survey on the current status of"Perioperative Infection Control"initiative monitoring indicators in medi-cal institutions in Guizhou Province helps to understand the implementation of key measures for perioperative in-fection prevention and control and set intervention targets,thus providing references for establishing a dynam-ic monitoring indicator change mechanism.
7.Nobiletin,an active ingredient of Fructus Aurantii,attenuates Ang Ⅱ-in-duced cardiac hypertrophy by inhibiting cardiomyocyte pyroptosis

AIM:This study aimed to identify the key active components and signaling pathways in the tradi-tional Chinese medicine Fructus Aurantii that contribute to the prevention and treatment of cardiac hypertrophy,along with experimental validation.METHODS:H9C2 cardiomyocytes were pretreated with nobiletin(NOB)for 1 h and then ex-posed to 100 nmol/L angiotensin Ⅱ(Ang Ⅱ)for 24 h.RT-qPCR was used to quantify the mRNA expression of hypertrophy-related genes,including atrial natriuretic peptide(ANP),brain natriuretic peptide(BNP)and myosin heavy chain 7(MYH7).Immunofluorescence staining was employed to assess the surface area of cardiomyocytes.Additionally,a kit was utilized to measure levels of pyroptosis-related factors such as lactate dehydrogenase(LDH),interleukin-1β(IL-1β),IL-18 and caspase-1,while Western blot was performed to evaluate the expression of gasdermin D and caspase-1.RESULTS:Network pharmacology analyses indicated that NOB is the key active component in Fructus Aurantii that regu-lates cardiac hypertrophy,potentially through the pyroptosis pathway.Further molecular biology experiments confirmed that NOB inhibits Ang Ⅱ-induced cardiac hypertrophy and pyroptosis.Furthermore,the involvement of the pyroptosis pathway was highlighted in the protective effects of NOB against cardiac hypertrophy.CONCLUSION:The active compo-nent NOB in the traditional Chinese medicine Fructus Aurantii alleviates cardiac hypertrophy by inhibiting pyroptosis.
8.PEG-rhG-CSF for primary prevention of granulocytopenia in breast cancer chemotherapy

Objective:To explore the effect of Pegylated recombinant human granulocyte colony-stimulating factor (PEG-rhG-CSF) with chemotherapy on breast cancer patients who got agranulocytosis of 3 to 4 degree, agranulocytosis with fever (FN) and the influential factors of relative dose intensity (RDI) chemotherapy scheme. Meantime, the value of CD34 + and CD45 in peripheral blood on predicting agranulocytosis of 3-4 degree were investigated.Methods:A total of 104 women with breast cancer were treated at Huzhou Maternal and Child Health Hospital and Cancer Hospital of Zhejiang Province from Jan. 2022 to Sep. 2023. All subjects received primary prevention with PEG-rhG-CSF during chemotherapy. The clinical risk factors of agranulocytosis, FN and RDI were analyzed. The levels of CD34 + and CD45 in peripheral blood samples were analyzed by flow cytometry. Then the predictive value of the receiver characteristic curve (ROC) for Grade 3 to 4 agranulocytosis after primary prophylaxis with PEG-rhG-CSF for breast cancer chemotherapy was evaluated.Results:Among 104 breast cancer patients who received primary prevention of PEG-rhG-CSF during chemotherapy, 28 patients had agranulocytosis of 3 to 4 grade, 10 patients got FN, and 12 patients developed RDI<85%. The results of single factor analysis showed that CD34 +, CD45 and chemotherapy scheme were the influential factors of agranulocytosis of 3 to 4 degree, and low RDI of chemotherapy scheme ( OR=0.584, OR=0.999, OR=2.299, OR=0.100, OR=0.999, OR=3.088, P<0.05) . It also showed that CD34 + and chemotherapy scheme were the influential factors of FN ( OR=0.099, OR=2.667, P<0.05) . Multivariate Logistic regression analysis showed that CD34 +, CD45 and intensive chemotherapy were the independent risk factors of agranulocytosis of 3 to 4 degree after primary prevention with PEG-rhG-CSF ( OR=0.602, OR=0.999, OR=20.174, P<0.05) . CD34 + and intensive chemotherapy scheme were the independent influential factors of FN and RDI of chemotherapy scheme after primary prevention with PEG-RHG-CSF ( OR=0.072, OR=33.934, OR=0.086, OR=54.788, P<0.05) . The area under the curve (AUC) of CD34 + were 0.767 (95% CI:0.659-0.876) , AUC of CD45 were 0.743 (95% CI:0.644-0.842) , and the AUC of combined two indexes was 0.825 (95% CI:0.730-0.920) , which was higher than that of single index. So AUC of CD34 + and CD45 can be used for predicting agranulocytosis of grade 3 to 4 in breast cancer patients receiving primary prophylaxis with PEG-rhG-CSF. Conclusions:The levels of CD34 + and CD45 in peripheral blood of breast cancer patients with agranulocytosis of grade 3 to 4 receiving primary prevention with PEG-rhG-CSF during chemotherapy are lower. Combined detection of CD34 + and CD45 in peripheral blood can predict the occurrence of agranulocytosis of grade 3 to 4 in breast cancer patients after primary prevention with PEG-rhG-CSF. Also it can provide a reliable basis for assessing the risk of grade 3 to 4 agranulocytosis.
9.Identification of influencing factors for falls in hospitalized patients with cardiovascular diseases and construction of a prediction model based on machine learning technology

Objective:To assess the fall risk of hospitalized patients with cardiovascular diseases, analyze the related influencing factors, and construct a prediction model based on machine learning technology, so as to provide a basis for the fall management of hospitalized patients with cardiovascular diseases.Methods:This study was a retrospective cohort study. A total of 450 patients admitted to the Department of Cardiology, the First Affiliated Hospital of Nanjing Medical University from June 2017 to June 2024 were selected as the research objects by convenience sampling method. By reviewing electronic medical records, trained nurses extracted the patients' general information and Activities of Daily Living Scale (ADL) scores during hospitalization. Lasso regression was used to screen risk factors, and machine learning libraries were used to construct support vector machine (SVM), decision tree, XGBoost, and neural network models. Bootstrap resampling method and area under the curve (AUC) were used to verify the model performance.Results:Among the 450 patients, there were 261 males and 189 females, with a mean age of (66.0 ± 8.4) years. Among them, 90 patients fell during hospitalization and 360 patients did not fall. The results of Lasso regression showed that ADL score ≤60 points, use of hypnotics, hypokalemia, nighttime toilet visits≥2 times, use of antihypertensive drugs, no caregiver, and history of atrial fibrillation were all risk factors for falls in hospitalized patients with cardiovascular diseases (regression coefficients ranging from 0.61 to 1.20, all P<0.01). Among the machine learning models, XGBoost had the best comprehensive performance (AUC=0.98), which was better than decision tree (AUC=0.66), SVM (AUC=0.95), and neural network (AUC=0.87). Conclusions:The fall risk of hospitalized patients with cardiovascular diseases is jointly affected by physiological, medication and behavioral factors, and the XGBoost model can effectively identify high-risk groups. In actual clinical work, nursing strategies can be optimized in combination with risk factors, and the application of intelligent fall prediction and assessment tools can be promoted.
10.Comparison of temporomandibular joint images with different fields of view in cone beam CT

Objective:To observe and compare the application of different scanning fields of view (FOV) in cone beam CT (CBCT) imaging of the temporomandibular joint (TMJ), and to explore the advantages and disadvantages of different CBCT scanning FOV for TMJ imaging. The aim is to provide a reference for the rational selection of CBCT scanning FOV for TMJ imaging in clinical practice.Methods:A total of 46 patients with temporomandibular disorders [22 males, 24 females; median age 24.5 years (range from 12 to 84 years)] were enrolled from the Department of Stomatology,the First Medical Centre, Chinese PLA General Hospital, between January 2023 and January 2025. All patients underwent CBCT scanning of the temporomandibular joints with both large and small fields of view using the same CBCT device, Newtom 5G (QR S.r.l, Italy), including small-field-of-view (sFOV 6 cm×6 cm, 46 cases, 92 sides) and large-field-of-view (lFOV 15 cm×12 cm for 26 cases with 52 sides or 18 cm×16 cm for 20 cases with 40 sides). A self-matched design was used to compare imaging parameters (scan time, exposure time, reconstruction voxel, radiation dose, data volume), image quality, and lesion detection efficacy (articular surface flattening, surface erosion, osteophyte, subcortical sclerosis, subcortical cyst, intra-articular calcification). Additionally, differences in joint spaces, condylar head height, and condylar height were analyzed.Results:In the sFOV group, scan time (72 s vs. 24 s) and exposure time (14.6 s vs. 4.8 s) were significantly longer than those in the lFOV group, with smaller reconstruction voxels (0.15 mm vs. 0.3 mm). Radiation dose in the sFOV group [(199.94±5.52) mGy·cm] was significantly higher than that in the lFOV 15 cm×12 cm subgroup [(96.20±25.34) mGy·cm, t=-20.29, P<0.001] and the 18 cm×16 cm subgroup [(101.73±13.49) mGy·cm, t=-31.34, P<0.001]. In terms of data volume, the sFOV group [(274.18±1.74) MB] was larger than the lFOV 15 cm×12 cm subgroup [(208.83±20.13) MB, t=-16.75, P<0.001], while the lFOV 18 cm×16 cm subgroup [(386.39±1.63) MB] was significantly larger than the sFOV group [(274.83±1.78) MB, t=214.49, P<0.001]. sFOV images showed clearer anatomical margins, distinguishable trabecular bone, and fewer artifacts. The detection rate of subcortical cyst in the sFOV group (50.0%, 46/92) was significantly higher than that in the lFOV group (36.96%, 34/92, χ2=5.61, P=0.018), with a moderate agreement Kappa coefficient of 0.48. Detection rates of articular surface flattening (80.43%, 74/92) and osteophyte (55.43%, 51/92) were identical between groups, with a perfect agreement Kappa coefficient of 1. Detection rates of subcortical sclerosis (69.57% vs. 68.48%) and intra-articular calcification (5.43% vs. 4.34%) were slightly higher in sFOV but without statistical significance ( P>0.05), with high Kappa coefficients of 0.93 and 0.88, respectively. For subcortical erosion, sFOV detection rate (38.04%, 35/92) was higher than lFOV (26.09%, 24/92, Kappa=0.53, moderate agreement), with no significant difference in detection rate (χ2=2.97, P=0.085). There were no statistical differences in joint space or condylar head height measurements between groups ( P>0.05), but condylar height measurements in the sFOV group were significantly greater than those in the lFOV group ( t=4.52, P<0.001). Conclusions:Large FOV provides wide anatomical coverage, optimizes radiation dose and data processing efficiency, and is more convenient for quantitative TMJ measurements.Small FOV offers superior resolution for displaying subtle condylar structures and lesions.Clinically, the choice of scan field should be individualized based on examination objectives (large FOV for holistic assessment/quantitative analysis; small FOV for early bone lesion diagnosis), equipment performance, and patient characteristics.
